I want to give a big thank you to the team at Align who are working so hard to ensure that trans people get the care they need. This is a tough job - fighting with insurance companies, hospitals, and all the other people who want to deny us care. Not a lot of people have been willing to go to bat for me in my journey, but Align certainly has. In particular, I want to shoutout Priscilla, the surgery coordinator/patient navigator for Dr. Mittermiller. Priscilla is on the front line dealing with the hospitals and surgery centers that are making it impossible to get surgery dates scheduled. If not for her, no dates would be scheduled at all. She has taken the time to speak with me and reassure me about the process and I know she is doing all she can for every patient. After speaking with Priscilla, I felt more assured that my surgery would happen than I had felt at any other point. Priscilla is a star and so are all the lovely people she works with. The team at Align have a tough job of delivering bad news to patients, but they are far more responsible for the good news patients receive, even if those patients don't know it. This is a team that truly cares about the trans community and, despite how tough the day-to-day might get, they never give up. I hope they see this review to know that they are very appreciated. Dr Mittermiller is wonderful. While I haven't had a full "FFS" yet, I'm already in love with my new side profile. Dr Mittermiller recommended correcting my bite with orthognathic(jaw) surgery before having the typical FFS procedures I was initially interested in. I'm now 7 months post op Jaw Surgery and 1+ year post op Forehead+Orbital Contour, Browlifting, Adams Apple reduction and I'm so thankful he was my surgeon. I had hoped to leave a positive review, but my recent experience was largely negative due to what I perceived as a chaotic and disorganized environment, coupled with concerning interactions with Dr M's staff and an unsettling consultation. The disorganization, particularly involving staff members Priscilla and Tiffany, was a significant issue. I found myself receiving contradictory information and being placed on hold, during which I occasionally overheard staff conversations where an unprofessional attitude was apparent. While a front desk employee, Adelina, seemed kind, my interactions with her were minimal. I find it concerning that patients are subjected to incorrect information, repeated requests for approval letters, and a general lack of clarity. My wife, a registered nurse, accompanied me to the consultation to help me better understand the surgical procedures, given my background as an IT engineer. Although Dr M's initial demeanor seemed pleasant, this quickly appeared insincere. During the consultation, when I was asked to move to another room, the instructions were unclear. When my wife asked if she should wait in the original consultation room, we overheard what sounded like snide remarks and observed Dr M rolling his eyes. Considering that doctors often use multiple consultation rooms, I believe my wife's question was reasonable. Beyond the issues with staff and communication, I was particularly troubled by what appeared to be a limited understanding of facial anatomy. Dr M seemed uncomfortable discussing procedures outside a very narrow scope of knowledge, and I noted a lack of interest in exploring feminizing surgical procedures beyond FFS. As a craniofacial surgeon, I expected a broader expertise in this area, but my wife and I observed that his knowledge appeared limited to a few specific feminizing procedures. My overall impression is that the practice's disorganization may be intentional, possibly to dissuade certain patients or to gauge their reactions under pressure. Furthermore, my research indicates that some patients have encountered significant issues stemming from what they described as medical missteps, which reveals a different side of thepractice. I also felt that Dr M's ego overshadowed his willingness to consider alternative approaches, suggesting a reluctance to to improve. Do not expect to have FFS and end up with a completely passable face, though, and this isn't Dr M's fault because it all boils down to our starting point, but Dr M doesn't make this very clear. Whenever his patients have expressed their concerns due to the fact that their faces are still read as male, Dr M doesn't do a good job at explaining that there is only so much that FFS can do, but he instead gaslights his patients. read more

I recentlytly got breast augmentation done by Dr. Mittermiller and I could not be more pleased with him and everyone on staff involved in the process. Everything leading up to my procedure simple and comfortable. All I basically had to do was show up and be ready on my surgery date. I couldn't have been treated any better. Dr. Mittermiller was absolutely amazing!! I was seeking out the best option to remove several skin tags on my face, I had hesitated for so long because of fear of scaring. Dr. Mittermiller examined me during a consultation and assured me that he could easily remove the skin tags from my face as well as others he found on my neck and body with little to no scaring, he did not pressure me or over sell the procedure. i received ffs from paul. he was unprofessional, disrespectful and dehumanizing to me. he botched my very simply nose job so i couldn't breath. he made inappropriate comments about my appearance all the time and any discomfort i expressed was met with aggression and personal insults. he made me deeply uncomfortable. he waited till right before the surgery to change and withhold approved procedures for personal reasons. I've never heard of an ffs surgeon behaving in the ways he behaved. he seemed so nice in the consult and i had even liked him at first but the mask dropped as the surgery approached and his attitude towards me as a trans woman felt more and now like he thought he was a parent or a preschool teacher. he is a cis man 10 years younger than me and he talked to me like i was a dumb child who he was saving or something weird. i can't imagine a cut person being talked to the way he behaved toward me. getting ffs from him was the worst choice of my life. partially because the work itself was not what we had agreed to and barely noticable, partially because he personally abused his position to, among other things, trquire a series of zoom sessions at the very last minute in which he insisted i stroke his ego. he kept pretending he was a shrink and trying to talk to me about my feelings but he's not a qualified shrink, he's a surgeon who's weird toward trans women. the personal stuff he required me to talk about with him in order for him not to cancel the surgery with no reason given was mostly about just stroking his ego and i did it and played his weird little game but i feel disgusting and abused that an unqualified surgeon would make me go through a process no cis woman would ever have to just to get a nosejob. i went through with the unconventional and uncomfortable hoops he would make me jump through because i had been on a wait list for over a year and needed the revision of his botched work to be able to breath. he knew it would be over a year to find sometime else and i had to choose between letting him behave in a way that was highly unprofessional or live with a collapsed inner nose for a long time, so i put my head down and let him treat me like less than a person. it was the most dehumanizing experience of my life. all the procedures he suggested and that we agreed on a year in advance were re evaluated and some withdrawn weeks before the procedure with no reason given other than that he legally had the prerogative to change his mind at the last minute with no reason given and he would remind me that his idea of what i looked like was the only thing that mattered and would accuse me of being dysphoric or otherwise mentally unable able to make decisions about how i look because i was transgender when i tried to ask for the procedures we had agreed to, that insurance had agreed to cover and that he had initially suggested. it was 100% him just deciding he didn't like me and flexing on me. it was the most transphobic experience of my life. i got the surgery because i needed to breath and the system would have taken forever to have to switch after waiting a year to get him booked. he used the fact that he collapsed my inner nose to treat me like less than a person. he told me multiple times exactly what he thought of me and i don't think a cis person would even believe what trans people have to go through sometimes with guys like this, when you don't have great health insurance and you have to go with the bottom of the list options. if you get stuck with him for whatever reason, make sure to constantly stroke his ego because the second you don't treat him like your personal savior or try to talk to him as an equal, that was when he changed gears on me and his attitude on me as a trans woman became clear and ugly. good luck out there, ladies. read more

I recently had a septoplasty x rhinoplasty x turbinate reduction and Dr. Mittermiller was incredible throughout the entire process! I was nervous for years to get my damaged septum/front of my nose fixed. Dr. Mittermiller made sure I was comfortable from the first appointment until after the procedure was done. First and foremost, Dr Mittermiller is incredible!!! Where do I begin...from the very beginning of my consultation appointment, he has always been SO attentive and passionate about his work. I had a rhinoplasty in December 2021. I was so well educated from him on the procedure and what we will accomplish. I say WE because he is so involving with his patients. He really takes his time and and sits with you to come up with a plan until you are confident and happy. We had maybe 3 visits before my surgery just going over different reshaping ideas with photoshop. He also spent a lot of time educating me on my bone structure and how my nose works, and showed me how NORMAL BREATHING should feel like! It was a miracle even way before. Surgery day was SO MUCH FUN! The nurses and staff are SO kind and friendly and made me so comfortable. I get anxiety in hospitals and with needles, and I was nothing but excited and relaxed the entire pre-op. I know Dr Mittermiller took longer with the surgery than was needed, because he is that careful and detail oriented. Fast forward to NOW! I AM SO HAPPY with the outcome of my nose and breathing! He communicated to me so well of the healing process and stayed in touch the 1 week post-op to make sure all is good. Not to mention, the recovery was the easiest and painless experience for me. Now, Dr M and I do minor botox and filler to enhance my features. He is so conservative with it all and is really honest and realistic to my face structure, scale, and what will look the most natural. Nobody suspects I had work done on my face because it is so minimal and so true to my actual self. As a bonus, he is so personable, has side conversation with me, and asks about the latest and greatest of my life. I am about 11 months post-op and so so so pleased with him. Dr Miller helped me with an abscess that I had under my chin. After further examination he found the root to the problem, the root of one of my teeth was causing the legion. Once I had the tooth removed the abscess cleared and is now healing. The dentist, the dermatologist could not find the cause of the problem. I'm glad I had turned to an ENT which referred me to him. I had previously formed keloids with a past injury and knew I needed to be seen by expert hands.
