They called us whenever there was any change in her condition, or anything at all going on with her. We were, and still are, impressed with how very well they kept us informed, all along the way. There were just a couple of things that didn't work well for my sister, and once the staff was told, adjustments were made in her care, making my sister more comfortable, both emotionally and physically. Once, when my cousin was at the nurses station, discussing a change that needed to be made, a doctor interjected some unpleasant comments from the sidelines. It was an isolated, one time occurrence. His behavior was completely out of character to every other person we encountered there. The nurse still listened, and the appropriate changes were made, to assure that my sister got care that was best for her needs. She was well cared for and never afraid there. On her final day, they treated the family with kindness and respect, and showed genuine compassion and caring at her passing. That was a week ago, and in that time, they've reached out to her son, with sympathy and compassion. We are abundantly thankful that we chose their facility for her to spend her final four months. I thank her care staff for helping to make a very difficult time as easy as it could have been. She had her favorite staff members, who she would joke back and forth with. I won't mention any names, for fear I might leave someone out, but one of the favorites or not, they were all kind to her. I give them my heartfelt thanks. Knowing what we know now, if we were back at the beginning of this four months, we would again choose Diamond Ridge.

The nursing team communicated daily with their counterparts (i.e., patient needs/preferences) when shifts were ending/starting. My CNA's were also wonderful. Employees always had a smile on their face, no matter what their areas of responsibility were, a testament to the positive work environment. Cleanliness was a top priority. Every team member took his/her job seriously and with pride. The positive team dynamics I experienced are a direct reflection of the leadership skills of Judith Gasmin, Director of Nursing. Both she & Herna Mellizo, Assistant Director of Nursing, appeared in my doorway daily to check in and be sure everything was going smoothly for me. I appreciated the diversity of the therapy (PT & OT) teams as well. They tried to accommodate me into their a.m. schedule whenever possible. Shadelands has continued to follow up with me. I received a phone call the day after I was discharged with customer satisfaction questions. They said they'd follow up with me again. Stellar customer service!

Excellent care by staff at Delta View Post Acute…read more I was a patient for 20 days in August of 2026, recovering from abdominal surgury. I required frequent care to empty my illeostomy pouch and general help. The care provided by the following personnel to accomodate my needs was greatly appriciated. CNA's - Ashley Aragon, Patricia, Paula & Saleem - During their shift assigned to me, were attentive to my needs, checked on me regularly and made me comfortable. Acctivities Coordinators - Maylon & Ana were very helpful and provided the enjoyable experience. Physical/Ocupational Therapist - Lampert, Jesus, Darius, Delaney were forcused on my needs and attended to my recovery. I had a comfortable recovery period and was able to walk with a walker when I left the facility.

Warned by the Kaiser placement person that I should not ask to be moved to another…read morerehab/post-hospital facility because "they're all the same," I was glad that, at least, Delta View was IN Antioch (where I live). That made visits from my husband easier on him, at least. I had a roommate who snored loudly, watched loud TV all night, even when asleep, it seemed, and there was nothing that anyone could do. I get that. The actual rehab staff was great; they really did help me to literally get back on my feet. Most of the nurses, LPN and CNAs were good, kind, caring, and patient. But a few bad apples, as they say. The TWO who, when called to help with a bedpan overnight, said, just pee in your diaper appalled me. It could take forever to get someone to answer a call button, and they let you know that, pretty much, they would not respond to buttons during meal times. Not sure if the placement coordinator from Kaiser was right, but I would not choose to go there again.

When we moved to Antioch four years ago, she had lost her interest in watching television, reading, and spent most of the day sleeping in a chair. I feared that she was about to become bedridden, and that I would have no choice but put her in a nursing home. This changed thanks to the splendid adult daycare provided by the Bedford Center, where she has been going for the last two years. My mother enjoys riding the bus that takes her there, loves being with other senior citizens, the activities that take place in the Center, and is always saying how good the staff is to her. I must also point out that the advice that the nurse and the dietitian have given us has been invaluable. Thanks to the Bedford Center, my mother now has a schedule and something to look forward to five days per week. Although she requires a great deal of maintenance, they have taken excellent care of her. They have improved my mother's quality of life considerably, and the chances are that, if it were not for them, she would now be living in a nursing home. In a nutshell, there is no question that the Bedford Center constitutes an important asset to the city of Antioch.

We are still early in her stay, but at this point our family is very pleased with the care she is receiving and will continue to watch the experience over time.

Thanks to the entire staff's exceptional care, I am walking again! The PT and OT folks (Nikki, Deandre and Akashi) patiently gave me the support and confidence to not only walk, but to climb stairs! I have never had PT before and was amazed how their simple instructions and encouragement helped heal my body so that I could return home. There are too many other names to mention here, but the following are a few. I'd like to give a shout out to the nursing staff, particularly Dahlia. The nurses were so kind and patient, helping me with every need, including cleansing. I hope that I was kind and respectful, but I noticed that some patients could be quite difficult. The nurses and CNAs consistently maintained a calm, caring and professional demeanor. They catered to every need and clearly cared about the patients' well-being. Claire, the Activities Director delivered daily menus and a list of scheduled activities. What a sweet individual, dropping by each morning with a smile and a little conversation. Nicole of the Housekeeping Staff was lovely. I engaged her in conversation at every opportunity, as she made sure that each room was clean and in proper order for each patient. My case worker, Mary, was instrumental in working with doctors and staff to secure my release date. She also recommended an appt. with my PCP and referred me to out-patient PT care. Another shout out is to the kitchen staff. The meals were healthy and delicious and tailored to each patient's needs. Mistakes were sometimes made and were quickly rectified. One small concern is that breakfasts always arrived cold. Having worked in catering, I know it is difficult to serve a hot meal to 50 people at once, but it can be done. Icing on the cake: Greta, an optometrist visited some of the patients for eye exams and glaucoma tests. I did not expect this extra service and appreciate it so much! In additional, the facility has followed up with me to check on my healing progress. I hope none of you require such intense care, but if you do, I highly recommend PH Post-Acute Care to administer to both physical and emotional needs.

As a family member, seeing someone provide that level of kindness and respect meant so much to me. I visited about two weeks ago, and her kindness really stayed with me.
