Do not send your family or loved ones to this unskilled non-nursing facility. I have never seen anything like this. The place is disgusting and the people working there are incompetent, overworked, understaffed and because they have to hurry they can't take care of your loved one. There is no doctor on site, new patients don't get seen for over a week. When my loved one passed away in this facility after a week staying there, someone called in incomprehensibly English to tell me that she "expired" When I came to pick up her belongings, they didn't say anything, not even condolences. They just gave me a plastic garbage bag with her clothes, glasses and phone... It was one of the most shocking experiences in my whole life and I am a nurse who has seen many rehabilitation places, assisted living facilities and nursing homes. I'm surprised that place didn't get shut down and that people allow the elderly to be treated like that. read more

This place needs a system of taking bedridden patients outside at least 3 times a week. - even if it's for 10-15 minutes. It's not dirty in appearance unless you're spending a good deal of time there on your visits. For example, I opened my family member's drawer and found roaches. The staff didn't seem embarrassed, just told me to alert maintenance as if I worked there. Conveniently, maintenance doesn't work weekends. It was very hurtful and inconsiderate.When they saw that I was getting ready to take photos, they acted on getting rid on the nastiness. read more

We went to attend to a patient and were horrified to hear the staff cursing, refusing to help their patients (one of whom was naked in the hallway). I live right next door to this facility, and I know they don't treat their patients right. I hear the patients scream and moan all throughout the night and it's so hard to hear. They sound like they are in pain/ scared, and they will do this for hours nonstop. It's so so disheartening to hear this knowing the nurses are not even trying to care for them as they should. Late August 2024. I had a horrible case of Vertigo. I had it two years ago...was treated and it never resurfaced until two weeks ago. I had the same treatment as two years ago but the dizziness didn't subside. It was somewhat better but really too debilitating to live a normal life. I finally went to Cedars Marina Del Rey Emergency. After doing many tests they admitted me to the hospital. My assigned doctor was John Joseph Pack MD. I did have the Epley Maneuver done at the hospital and I was getting some relief but after five days Dr. Pack wanted me to come here to build up my strength to enable me to go home. I was quite hesitant to come here because of the reviews I had read. But as soon as I was wheeled in (on a gurney) I felt comfortable. There was a pleasant scent to this facility. It was spotlessly clean and bright. I was put in a room and felt very comfortable being here. Over the next few days I met so many of their staff...RN's, CNA's, dietician's, a pharmacist, PT therapists, executive staff members and others. Every single one of them made me feel welcome and happy to be here. I have been pampered by so many wonderful people here. They truly are lovely carrying individuals. And it's quiet at night. The food here is adequate. I don't have much of an appetite any more and this facility does a good job with the three meals being served daily and snacks also. REVIEW UPDATED Sept 16, 2024: I have now been here close to three weeks and I am changing my opinion of the food. NOT ADEQUATE. Something is always missing from the tray. It could be a knife. Or the printed menu shows a roll and butter. Sometimes no butter sometimes both are missing. The main dishes are described beautifully but that's not what you get. A hoagie Philly style with cheese, onions and peppers. Only no onions and peppers. And it was on a slice of white bread. Or a tuna sandwich with a plate of lettuce and tomato. No lettuce or tomato. Toast with eggs in the morning with butter and jelly. Sometimes you will get the butter other times nothing. A side of coleslaw never came, the cooked meats are cremated...dried out. One dish was turkey with mashed potatoes and gravy. No gravy. I keep asking for packets of ketchup and mustard so I can make a sauce myself. With that being said I'm keeping the rating at five stars. And that's because of the people working here. They are incredible. And I needed to lose a few pounds anyway. So put your worries to rest. This place is amazing. I live right next to door to this facility, i have never been inside nor do i have anyone i know in there. BUT, let me tell you, the amount of times that patients SCREAM for help, so much so that the whole building next door can hear, for HOURS ON END, is so much. People scream, at the top of their lungs for help, for hours. Not sure what kind of care they're getting. Can't be much. Cops have been called multiple times on this center in late hours of the night and early hours of the morning because patients are not getting help. read more

My mother is currently getting rehabilitation in this facility. The staff sit on their phones and do not tend to the patients. They bathe the patients once a week. They told my mother that she has to urinate a few times before cleaning or changing supplies. Though this facility was recommended, the quality of the care is a disaster. In addition, case management and doctors at this facility do not communicate patient care or return calls. The staff "lost" my mother's personal belongings. If you can help it, please don't send your loved one to this facility. The only reason I gave two stars is because the physical therapist seem to be diligent in working with the patients, that is, when they show up. read more

I never write negative reviews, but this place needs to be shut down. The staff is horrible, and the patient conditions are atrocious. My 88-year-old father was at this facility after a broken hip and was severely neglected by the doctors and nursing staff. They did not take blood tests on time, they did not dispense medication properly, they let him sit in a soiled diaper for 30-60 minutes after he called for the nurse, they first discovered a large pressure sore on the bottom of his foot at the FULLY black stage (see photo), and they did not adhere to his dietary needs. In addition, my dad lost 12 pounds during 4 weeks in their care, and being very thin to begin with, he ended up skin-and-bones. They did not even inform us that he was not eating. We showed pictures to Darren, the guy who runs the place and his response was, "What exactly are you showing me? All I see is a man without a shirt."(see photo). Rose and Pam, the other two people in charge are completely incompetent as well. I left numerous messages for Darren, Rose, and Pam to discuss my Dad's case, but none of them returned my calls. They did not inform our family about any of the above-mentioned issues, but I found out either from my father directly via phone or upon visiting him in person. They had the audacity to blame our family for their mis-steps for the nonsensical reason that "we lived out of the area" (my father lived close to the facility and my sister and I flew there on weekends from Northern California to visit him). During the day, patients are placed in hallways sitting in their wheelchairs staring at the walls or screaming out for help, and the staff ignores them. The facility itself looks nothing like the pictures online as it is way more run-down in person, and it has a bad odor, usually smelling of urine and feces. The first night my father was there, we asked them to put up the guard rails on the sides of his bed, but they refused saying that studies show that patients don't fall out of bed. Well, that first night, my father fell out of bed and the only reason why we found out was because my dad's roommate called me to let me know. Only then, did the office staff call to inform me, and they said they were upset with the roommate as he should not have called me due to HIPPA violations. If the roommate hadn't called, the staff would have never let us know. Upon one visit, I found my father laying diagonally in his bed, his legs dangling off the bed, in a soiled diaper, and the nurse button was behind his bed, completely out of his reach. They also removed the emergency phone numbers we taped to his bedside table, which included huge letters that said, "DO NOT REMOVE". My father was unable to call us since he couldn't remember our phone numbers. His personal hygiene was also neglected as they barely showered him and did not give him a haircut in the 4 weeks he was there despite multiple requests for one. Needless to say, we moved him to a different facility and I will be pursuing further action with Playa Del Rey Center. Darren and Pam should enjoy their careers while they have them. Lastly, they did not catch my dad's pneumonia. We told the nursing staff on numerous occasions about a cough my dad was experiencing and asked them to send the doctor in to check him. They never did and it turned out to be pneumonia. We found out because when he was admitted into the new facility, they did a sputum culture upon admittance and it tested positive for pneumonia. On a positive note, there was one CNA named Chi who treated my dad very nicely, and the PT staff seemed fairly competent. read more

Do not place your loved one at this facility. They do not properly care for the patient. When you call to check on status, you are placed on hold for more than 15 minutes, then they hung up on you. They don't return calls. The nurses are incompetent. They don't know how to properly care for diabetic people. The only reason I selected one star is because of a few nurses that do care like JB and Charmaine, otherwise I would have selected zero star if that choice was available. But that is only because I call a lot and know who to talk to. But if you don't call and don't micromanage the staff, who knows what kind of care the patient would get? And you can forget about the other nurse Gina. She has zero people skills. Very rude. Don't be fooled. Most of the other 5 star reviews that you see here are from the staff. Normally, I'd make sure the facilities staff was on top of my loved ones care. Covid has restricted this and caused my father more pain than he should have. Staff blame each other or do run arounds. Never a good communication with family. My dads doctors office was on top of having translators yet they didn't bother to do that. Didn't bother to open a gift sent to him either just left it sitting in the room till it went bad and someone must have tossed it out since it was an edible arrangement. Takes them 8+plus to get to patients which is the reason many are probably taken to the Er for dehydration. Do better. LA public health department shit this place down. Do something. read more

With the exception of a few caring nursing assistants, this place is horrible. My mom stayed here for 3 weeks after a stay in the hospital and it was awful. The staff was so rude and never wanted to deal with family. It was impossible to reach anyone and they don't return your phone calls. There were times when my mom pushed the button for help and would wait 30 minutes and no one would come such that she would call me and I would constantly have to call the front desk to get someone to go help her. The worst was Nester the case manager. He never returns phone calls and he dropped the ball on important paperwork which delayed my mom's discharge and still can not get anyone to follow up on her home health care which was supposed to be in place. read more

First, you must know that more than half of the nursing staff at Playa were absolute heroes during the height of the Covid-19 pandemic when the ward swelled to to a whole floor of patients--me being one of them. Many could deeply empathize with me, having survived the virus themselves and returned to work, comforted me, took total care of me when I couldn't, and saved my life. I have had a lifetime of serious health problems and have been in many different hospitals and long term care and Playa Del Rey Center stands out as one of the very best not only because of the generally high level of individual caregiver qualifications but because of the vision and mission to be the best--set and maintained by Ryan, the Executive Director, and his team. If there are any short falls or problems they are quickly rectified on a personal basis by his staff with a big shout-out to Janey, the Patient Advocate and Social Services Director, who had my back at every difficult turn as I recovered literally step-by-step (shout-out to Rahim and the persistent PT staff!) until I walked out on my own. G-d bless them all! As with anything having value in life--you have to work hard to earn it. If you have a burning desire to get better and need that trained, experienced, and inspired hand to help you--this is the place where angels dwell. read more

I do not recommend this place for your family members. They will lie to you to get you in the doors. Marge, who must be in charge of sales, originally told us we'd be able to visit my dad in the outdoor garden area for 45 minutes a day. I didn't believe that'd be the case, so when I asked for confirmation in writing, how ironic is it that all of a sudden the health department just so happened to come that day and all of a sudden those rules are no longer applicable and that we can't see my dad for 28 days? When I called the center to check on their COVID restrictions and expressed concern that my father may pass while he's in their facility, RN Rosemarie laughed about it on the phone....I asked to speak w her boss and she came back on the phone pretending to be her manager. When I told her what happened, she didn't say anything. I thought she didn't hear me, but then she responded with "that's just how Rosemarie is." As an alternative, they said they would provide him with a bed next to the window so we can make visits. We just went today & that was certainly not the case. I have another employee blaming another member Evelyn that she was the one that assigned the bed to my father yet when I spoke with her today, she said that was not true.... I was also informed that there are two Korean nurses here and upon arrival to visit my dad today, that was also not the case. The speech therapist here cannot comprehend basic instructions. Just overall not a great first impression of the center. The only reason I give this place 2 stars instead of just one star is because of Chavon. She is the best thing we've encountered thus far. I was told we'd receive a call from Dr. Feld later today and Chavon made sure that happened. Also, I ended up receiving a call from Andy later in the day and he was able to accommodate my dad by placing him near the window. read more

My elderly father was placed her in October of 2020 after having Covid. I can't go into everything because it is too painful, but he was physically abused and nearly died here. We came every single day at the window, and he was still severely neglected and abused. They say all of the right things, and individual people certainly care, but overall it was a very dangerous place. We finally broke him out and hired our own ambulance to bring him home. He rehabbed quickly after getting care at home. We would wait forever to get through on the phone. He had PTSD from the people screaming for help all night and the night worker who hurt him. My dad was a patient here for about 3 months. He just passed away in the hospital March 12, 2020. However, praises to all the staff, nurses and physical therapy. Dad had 3 strokes, was paralyzed, unable to speak, but they never gave up on him. He had physical therapy, showers, haircut, lots of medical AND personal attention. Place was clean, other patients said food was good. My dad was on a gtube and they checked it like clockwork. They caught pneumonia, had him taken to the hospital, and he recovered after 6 days. Unfortunately, he did pass later, but there again, staff was on top of it when his vitals were off, and sent him to the hospital. For a nursing home, it's one of the best. Thank you for the great care, and never making me feel I was in the way while there. read more

It is with much disappointment that I am writing this review. There were no previous reviews on Yelp and only a two star rating via the Nursing Care facility website (link below). My hope was to get him the help he needed to aid in his recovery. Unfortunately, during our father's stay at Playa Del Rey Rehab and Nursing facility we had no choice but to micromanage absolutely everything. This wasn't the plan, but after missed prescriptions and not informing us of important changes in his rehab and other crucial information in his recovery we had to constantly try and reach someone for answers. There were many red flags that I documented during his stay and it was rare to receive any accountability from those in charge. I was never at ease knowing that mistakes could happen and affect my father. In conclusion, if you have other options for care I would definitely consider those over this facility. I wish my rating was different, but this was our experience and one I witnessed others go through as well. I hope my review is taken as constructive and that change in some areas is considered. Nursing homes are important and it's sad that these places have such a bad rep. I hope this facility can turn this around. read more

My mom had a hip replacement surgery in June 2018, and we were given the option to have her go to a rehab center after surgery so she could get full time nursing care and physical therapy for her hip. We thought it was a no brainer for her to get help post-surgery and decided on this facility because it was close to where I live and work. I went and visited the facility before we agreed to it. It appeared nice enough from a quick walk-through. What a mistake this was!!! Pros: 1. Conveniently located to me. Cons: 1. Had to constantly micro-manage and ended up going there 2-3 times per day when the whole point was for her to get good care and me to not have to worry. 2. Food was bad. I ended up making her smoothies regularly and bringing them over. 3. Most of the staff were not nice and it took them forever to come and help when she pushed her button. She had to wait forever to get help to use the restroom on several occasions. 4. They claimed she was going to get more physical therapy than what she received. Her doctor wanted her up and moving a lot, but they only got her up once every morning and she spent the rest of the day in bed because the nursing staff was not allowed to walk her because it was a liability. Say what?!?! The main purpose of her being there was to get moving and have someone help her do that. And they wouldn't leave her a walker in the room!!! That was a liability too. Only the therapists could get her up for walks and only during their scheduled time in the morning. What a joke. 5. She got bad bed sores because she was kept in bed so much! 6. Someone stole her pain meds which we had picked up at CVS prior to her being transferred from the hospital. They were in her bag which was brought over from the hospital and they were nowhere to be found once she got there. We asked the staff and no one seemed to know. 7. There were many patients/residents there with dementia and other mental conditions. She had to room with someone who was constantly blasting her TV. And she got no sleep because she could hear everyone else's TVs blasting at all hours of the night and people screaming down the hall. I guess we were under the impression she would be at a facility with patients who had undergone similar surgery - like knee replacement, hip replacement, etc. 8. The facility itself at a closer glance is very old. Equipment, rooms, TVs, etc. all appeared very old. Her TV would work intermittently, which sounds like first world problems, but that TV was her only saving grace and even that didn't work. We ultimately pulled her out of there after one week and brought her home. Best decision we could have made. But it was not easy to get her out of there even though both her primary and ortho signed off on it and demanded it since they heard she was not getting the movement she needed. Of course NOW it was all hands on deck when they heard we wanted her discharged. I had a meeting with several of the staff there and they tried to talk me into keeping her there. I mean without this bed filled, they would lose money right? I asked my mom what she wanted, and she wanted the heck out of there. We got her home and set her up with everything she needed and it went smoothly from there. The staff also claimed they never saw me there and told my mom's doctors this - security footage would show that I was there multiple times per day. I am sure I will think of more things and add to this post when I can. I do not believe all facilities are like this. I heard that Fireside in Santa Monica is really good. All I can say is think twice before sending your loved one to this place or any skilled nursing facility. If you can get them set up comfortably at home and get them physical therapy, either at home or by taking them somewhere a few times a week, then go that route for sure! I should have taken the other two yelp reviews on this place seriously before sending my mom here. I didn't want to base my decision off of two bad experiences at this place, but their reviews could not be more on point! Don't make the same mistake we did! read more

Called for a reference check for a nurse who worked there in the past and was redirected and hung up on. Called again talked to someone and then hung up (female). Called again talked to someone and they hung up (male). Called for the 4th time thinking it might be my phone and was immediately hung up on. This is very unprofessional on the front desk and would not recommend anyone calling for any type of care because they may just hang up on you mid sentence. This place is so abysmally horrible I don't know where to begin. They don't deserve even one star. I was transferred there for rehab after a total knee replacement. The room was occupied by two other patients, one of whom suffered from advanced dementia. I woke several times in the middle of the night to find her staring at me confused about why I was in her bedroom. My bedrails, on a bed at least 25-30 years old was coated with crusted on human body fluids and blood. The "professional staff" with the exception of Seema, an RN, and the director of nursing and assistant director of nursing were some of the most incompetent and uncaring staff I've ever seen. And I say that as a retired RM/nurse practitioner of 42 years. Feel like waiting 30" for a bedpan because the RN, "Jude," feels that handling a bed pan is below his station so he makes you wait until the CNA returns from her dinner break to assist you, or who casually takes his own break before giving you a pain pill, then this is the place for you. How about an MD who only sees you once and never orders blood work, even though you arrived st the facility with a critically low hemoglobin. I demanded my blood be checked and when it came back at 7, his plan was to recheck it in two days and start me on iron pills. He apparently forgot that he already had me on iron pills. I insisted on being taken to the ER, where a competent physician transfused me with a pint of blood. This angered Dr Egg on His Face who responded by admitting me as an Observational Patient, meaning I would be stuck with the hospital bill. When I asked why he did that, his unbelievable response was, "You're the one who wanted to come over here." I couldn't believe any doctor would be so callous as well as so incompetent. Oh, did I mention that this "doctor" has privileges at both the rehab center and the hospital, which is a pretty sweet deal, as well as a conflict of interest. When I refused to return to that hell hole, he became visibly perturbed. By his reaction, you might think my refusal was somehow hurting him financially. I don't know if that's the case, but one can only wonder. I was supposed to begin therapy immediately upon arrival at this place, but because they lacked proper equipment, my leg remained immobilized for seven days. Later I was discovered to have developed two clots in my operative leg. I could have died had they not been discovered by the excellent staff at Cedars Sinai, where I was admitted for treatment related to medical complications due to horrific treatment at this place. The CNA's were the saving grace here. All were responsive, caring, and competent. Also, the DON, and assistant DON were caring and competent, which are qualities in a health care provider that allows you to sleep at night, assured you are in good hands. Their Medicare rating is an abysmal 2 out of a possible 5, which I didn't know until checking while there. How can Marina Del Rey Hospital send their TKR patients to such an awful place for rehab? They should have a sign at the entrance: "Beware all who enter here. Enter at your own peril" read more

This is the filthiest facility I've ever seen. I spent 12 brutal days in this sh**hole recovering from Covid. The staff is rude and unresponsive. I waited two hours one night when I requested a Tylenol. That happened numerous times as I would awake in the middle of the night with a headache. I've seen restrooms in NYC subways that are cleaner than this dump. The main doctor (Pack) is totally useless. He refused to answer questions from my personal physician. Over the 12 days, he spent less than 2 minutes total checking on me. The meals are cold and inedible. I lost 30 pounds while I was caught in this crappy place. It should be shut down by the health department. The toilet in the bathroom was surrounded by rotting floor and ready to fall through. The "nurse" told me not to use it because it kept overflowing. She said I had to call for a bed pan or urinal if I had to go. I ended up sneaking across the hall to use the clean "staff only" restroom. I was later moved to the "general population" and given a bed where the previous tenant had died that morning. His belongings were still in the night stand and closet. There were no sheets or blankets on the plastic covered mattress. After waiting two hours while sitting in a wheel chair, I asked again for bed linens. They were brought in and tossed on the bed. I made the bed myself! I was supposed to be receiving physical therapy as I had lost muscle control from Covid. I was given less than an hour total trying to regain my strength. I ended up sitting on the side of my bed doing exercises myself because I planned to walk the 17 miles home even in my hospital gown to get the hell out of this trash receptacle. They're sending me a bill even though I paid it. Someone there cashed my check as it cleared my bank account. Now, they're threatening to turn me over to a collection agency! Again, the health department should close this waste sight down! I feel very sorry for those who are sent there because they're likely not coming out. read more

There is only one thing to say about this place and that's why do people go there for rehab when they could have better care from their home. When a person goes to this place for rehab the personnel are so attentive and helpful after admission but after that the patients are left alone to fend for themselves as it was for my mother who has many health problems which were not addressed. I finally had her checked out of there and it was not too soon. She called for someone to help her to the bathroom many times and had to wait for over 20 minutes each time which is unacceptable for anyone especially someone who is in a rehab center.
