So, to me, the only right answer in this kind of mistake is, "Wow, sorry to hear that, no idea how that could have happened, how can we make it right?" And I'd have suggested give my dad SOME amount of refund. I don't even much care how much, $10, $20, just make an effort, you know? But no. No adjustment. Empower your receptionist to offer SOMEthing. So, thumbs-up for a having a warranty, but thumbs-down for not fessing up to your mistake.
