Truly awful medical care. The nurses--to whom you'll inevitably be routed and they gatekeep like crazy to keep you away from their actual providers--are borderline incompetent. The internal workings of seeing a provider, seeing a specialist, communicating with your provider and not some random person in their system, getting tests ordered ... it's all a massive mess. You never know whom you're getting care from and if it will be in any way decent. There's no way to meaningfully ask your provider for help. And everything--any task at all from the most profoundly medical to the most simply administrative--takes an obscene amount of time, that would be comical in a dark nihilist way if it wasn't your health on the line. Wonderful physical health care!!! I am trans masc and have complex PTSD regarding health care. Prism Health has been so healing for my nervous system - I really hope more support for expansion of centers like this (gender-affirming, trauma-informed holistic care) start appearing in the coming years. I feel Prism Health has created a safe enough environment for my nervous system that for the first time in my life I have actually felt comfortable going to the doctor. So much so that I no longer just go when my body is in urgent need of care, but I am actually starting to consider my health pro-actively because it feels like their team / doctors truly care. They take the time to consider all aspects of your experience & hardships that may be impacting your physical health. Unfortunately the support staff is completely inept, losing forms, failing to correctly send referrals for weeks and weeks. I don't recommend! They have absolutely no knowledge of HRT for menopausal women. I wanted to take an oral troche of testosterone, which I've done for years. It's a very low dose, but they refuse to prescribe it, saying it will affect my liver. Well, not anymore than the oral progesterone they already prescribe me. I take such a low dose, it wouldn't affect me, and the pharmacist even said he would be happy to explain it to them. They seem to only to want to help those taking HRT for gender care and not for women who need the help of hormones. Very disappointing. I will find somewhere else to go. read more

If I wanted easy access to HRT and solid gender-affirming care: Prism would be my first choice. Even if I -did- seek gender-affirming care from them, I would not use them for any other aspect of my medical situation. As far as actual day-to-day medicine and routine care? I'm going to assert a big ole nope. Dr. Sokoloff asserted that modafinil had greater abuse potential than adderall, which told me all I needed to know about his skills and set off alllll my alarm bells, but I stuck with them because I like supporting LGBTQ+ affirming businesses. It wasn't worth it. Both me and my husband were patients of Rian's but some of Dr. Sokoloff's wrong thinking ended up corrupting them and we ended up both in a very bad way. There was ONE good actual MD in the practice that went to bat for me and that I'd trust, I think it was Dr. Wiser. Unfortunately, this practice is an inmates running the asylum situation where being pro queer representation and affirming folks is more important than actual qualification and delivery of quality, timely, and careful care. I wouldn't send anyone I liked to Prism Health. Ultimately they were so bad that I changed my CareOregon provider to Kaiser to get access to things like "people with actual MDs" as well as the ever-controversial "people who actually know about the things they prescribe" and the earth shattering "timely referrals to specialist services". I know that's all crazy talk, but do yourself a favor. You can do better than Prism Health. I have two huge regrets about leaving them behind: First regret is: The pharmacy that's in their building, who as near as I can tell is in no way actually affiliated with Prism, they're just there for convenience, is AMAZING. The folks who work there were the apples of my eye, and brought me great joy and I appreciated that they always looked out for me in ways my actual doctors did not. Second regret is: I didn't contact a malpractice attorney or raise my care issues with OHP. They shouldn't be receiving public funds for their efforts. read more

Really sick of the terrible front desk communication. Make an appointment with a doctor which includes vaccination and don't get to see the doc when it was clearly requested. Ask to put my husband on waitlist so we can see same provider... nope completely ignored and told at least 4 months before waitlist opens again. They don't listen. Let me start by saying that I am not an easy patient and I am aware of that. I have to say I truly like Prism Health and my PCP. I've seen several of the medical practitioners here and all are great. I appreciate that my PCP (Dr. Sokoloff) is professional, calm, and practices evidence based medicine. I went through some health issues and was convinced I needed surgery and luckily I did not. Before I changed medical insurance I made sure my doctor was accepted by the plan. The peeps that work here are friendly, respectful and great at what they do. The office is super clean. The MA's rock at giving vaccines (didn't even feel my last 2 vaccines here!) The phlebotomist is a complete ROCK star and always finds a vein and I'm not an easy blood draw. Overall A+ and so grateful I found this place. Thanks Prism! read more

The staff and providers at Prism are the best I've ever seen. I go there for my primary visits, STI screening, and to get prescriptions at Avita pharmacy. The care is great, the staff is knowledgeable and professional, I don't have to wait around to see my provider, and I never feel rushed once I'm in the appointment. Keep up the good work! read more

I really just want to say how thankful I am that CAP and Prism exist. This is really more of a review of CAP as they have been so lucky to use Prism facilities since their move from downtown but the staff from both are A-mazing and so thoughtful and helpful. I hope all will consider donating to both causes if they find themselves in a position to do so after being fortunate enough to utilize their services when not able to support the cause like myself. I'm sorry to say, I can't give any kind of positive review for this place. Maybe like 1/10 stars. When I started going to Prism, I was told that I was in-network and so I booked several appointments, and went over the course of a couple months. (I pay out-of-pocket for Marketplace coverage and that's already a big expense). Then I received a large, unexpected bill. My insurance provider informed me that Prism had essentially lied to me, I was never covered for services there. Prism never informed me of that when I came for appointments, and at no time did they ever call me to let me know there had been a mistake. I've been asking for follow-up on this situation for over a week now, and so far, I haven't even been contacted, and this hasn't been handled, despite me following up with several phone calls. Also, several appointments I booked were canceled on me, by Prism, because my provider wasn't actually available, despite them saying they were. I work as a teacher and we use similar scheduling systems as medical providers, and it's NOT HARD to block out times when a provider is unavailable. So yeah from what I've experienced and what other queer friends of mine have experienced, so far it's pretty bad. I won't be going back. This is an unacceptable way to treat clients, especially queer folx who are seeking mental health treatment and other kinds of medical help. I would say steer clear unless you absolutely need to go here, or you already have a provider here who is working for you. Do go for free testing, sure, but don't give them your money. read more

Everything is controversial these days--even medicine. It shouldn't be. But it is. I spent years working as a firefighter and paramedic...and now I'm a lawyer. What I've learned is that not all "lawyers" or "doctors" are created the same--and that picking either--is a very personal decision for everyone to make. I picked Prism for a number of reasons. First, it's only a few blocks from home. Second, while I'm not a member of the LGBTQ community, I have nothing but love and support in my heart for the community and the organizations who are stepping up on my behalf. Finally, coming from an HMO PPO type healthcare setting, I was eager to try and find a practice where I felt like I could get to know my doctors and care providers -- and feel like they took an interest in getting to know me. The whole "person" not a "number." Healthcare these days IS a number game, its inescapable, and the costs of providing solid care are astronomical. I needed to only look at the numbers and sizes of the donors on the wall in the lobby at Prism to realize just how much (money and support) goes into anything 'healthcare.' Alas, the answer is yes--I have a patient number, I can't just call and walk in, and a lot of the care and correspondence is done using "My Chart." Looking from one perspective I could see how some might not feel the place is as 'personal' as labeled. But I would disagree. Here's why: First, I started with the first provider, Josh Oppenheim, who spent nearly an hour in an appointment with me. I didn't feel rushed, I felt completely comfortable sharing the highs and lows of my medical history, and 100% of the conversation felt that everything I was sharing wasn't being judged...just processed with an eye on making sure my care fit. Then unfortunately Josh left the practice. Second, I met with Dr. Sokoloff...and unlike other Doc's I've been 'transferred' to in other systems--I could tell the first time we met wasn't the first time Doctor S, had reviewed my chart. He'd clearly taken time BEFORE we met to make sure when we met...we could get to know one another. Third, as nice as it might be to have a doctor who is a 'yes' person...i.e. you say: I'm suffering X, I want Y, and they say...how many? I wanted something else, a doctor who would look at my history and truly assess my needs and make solid recommendations. This doesn't mean everything has always worked...but it means we tried the lesser of two evils always first, and it also means my doctor is thinking about more than turning a room, writing a script, and billing my insurance. Finally, I've at times been frustrated by what I felt wasn't "progress" like I wanted. Instead of pushing back, the staff have always been sensitive to how I feel and what I've shared and said to them. They've always been punctual, compassionate, non-judgmental, and just plain comfortable to be around. In my experience--this is an uncommon luxury. I get the sense that any patient who visits this place in good faith--will find a staff and doctors who they can trust...and more importantly...staff and doctors who trust their patients. I don't give five stars easily. And when it comes to medicine--I've seen the brilliant outcomes of good docs and the dismal tragedies that come from docs who don't take a personal interest in their patients or their work. For all of these reasons...I'm thankful I found Prism, and I'm glad to be a part of their family. read more

Incompetent in every regard. I've had asthma since childhood, but the doctor decided I had long covid. Getting maintenance medications refilled every month was a massive hassle, as they would only write a one month prescription, or didn't know how to put more than one month at a time in the system. Every month my pharmacy had to call, often causing a return of my asthma symptoms and missed work when I missed a dose because it took days to straighten out. I made an appointment when I had the flu, but they scheduled me a month later, so I canceled and went to an urgent care. However, they didn't take me off the schedule; and found out when I got an email letting me know that while they're aware "I missed the appointment because of a domestic violence situation" (W.T.F), their time is important and if it happens again I will have to find a new doctor. Which is fine. My health is important and I'd rather have a doctor whose treatments and assumptions are based on reality rather than whatever was in their facebook feed this morning. read more

I was looking forward to finding a doctor that was LGBTQ+ inclusive and close to home. I found Prism Health and decided to give them a call. I am a new patient so I was informed on the voicemail to leave my information and someone would call back. It also said to wait a month and if I didn't hear back to reach out again. I made this call in November and never heard anything. I saw they were maybe doing renovations or closed for the holiday time so I waited till early January to email them. It is now end of January and I haven't received a call or an email back. I'd love to have a better review for them but if this is how they are with the beginning stages then I have no trust with my medical stuff and decided to find a different primary. read more

I've been going there just shy of two years and really like the clinic. They are my primary care and speciality care physician. I've been going to Prism Health for almost two years now. I'm fortunate enough to have insurance that covers both Prism and other primary and specialists physicians throughout the area. I mainly go to Prism for my primary care and I have never been disappointed in the care I have received from everyone involved from the front desk to the assistants to the primary care giver, Joshua. Joshua is the kindest, most thorough P.A. I have ever encountered in the same vein as the specialists I see at Legacy and other hospitals. He gives a damn, follows up and always remembers your history and your needs. Anthony, the assistant, always makes you comfortable and is always professional and friendly. Great location in S.E. Two things first - I got very nice care from them, medically speaking. I work in healthcare as well and the staff was courteous and friendly. I got what I came in for, which was to try and establish Prism as the first primary care I've had in years. That is the only reason I am giving ANY stars, as my billing experience has been such an ongoing nightmare that I would never, ever recommend someone go there unless they 1) Only have one insurance plan that explicitly contracts with Prism and 2) Have already paid their deductible. Because it's obvious to me that their billing department is at best clueless and at worst down right negligent. I have two insurance plans, and Prism only took one of them (my secondary insurance). After billing my primary insurance and getting the expected denial, I was contacted by the billing department to inquire about how I was going to pay back my ~$200 primary care visit. I had to insist that they try to bill my secondary. "Okay, we will, but they're probably going to deny it" is what I was told by Prism. I talked with the insurance specialist *at my own work, which is another health clinic* and they were perplexed by this response. That isn't how secondary insurance works, they said. This is a preventative care visit, covered by your insurance plan, at a clinic that says they contract with that insurance provider. It should work out. Months go by. I hear nothing, so I figure it went through. Then all of a sudden I get a call from Prism saying they mailed me a bill (to an old address - by this point it was 9 months since that one appointment and I had moved). I was expected to pay in full and "your insurance denied it, I can't really see why but they did. You need to pay us". I am fuming, and bewildered, and certain this means my visit was not coded correctly so that it was billed to my insurance as a preventative, but in desperation to get this solved I agree to a payment plan. I state $30 a month to be billed on the 12th of each month is what I will do as I fight this with my insurance. The first month, they don't bill me. They call me and say I gave them the wrong card info. We go over it together and alas, they had just typed the numbers of my debit card in incorrectly. Silly them. Second month, I get a call. "You can't pay us with a Health Savings Account, you need to pay us with a debit or credit card". I explain that I can 1) See that my $30 payment had already gone through with them and 2) Said payment came from my checking account connected to a visa debit card that I already double confirmed with them last month. Fast forward 15 minutes as I fight my way to speak to the billing manager, who then admits to me that this was a mistake on their part and that there was in fact no issue with my payment. She promised me that "there shouldn't be any issues in the future". This is the third month. It is the 21st. I have not been billed yet, despite the 12th being my requested payment day. I am waiting for the phone to ring so I can be bugged with this again. I am past angry, I am furious. It has almost been a full year since my one visit and I am still dealing with this - I would never in my life go back there simply because of how outreagous it has been to try to bill my insurance through them, much less how it seems to be impossible to get credit for trying to pay them in the mean time. The products were of good quality. I received 30 Alginate dressing that I couldn't use. I never ordered any of this stuff. It was ordered by my wound doctor. My insurance paid all but $54.09. When I asked about returning the 30 bandages unopened for a credit of $54.09 I was told I missed the dead line by 2 days. Now I have some bandages that are new and someone could use but this company is interested in nothing but money. The insurance has paid them $123.36 but because of greed they will not help you. Not a good company and you have no choice because of their hook up with the doctors. read more

Huge plus for trendy interior design, and the staff is friendly and helpful enough in person. However in working with them over the phone, or over the MyChart app, is a complete nightmare. All the times I've tried to call during normal business hours, no one picks up the phone so I leave a voicemail. They only reply to voicemails at around 4pm while I'm at work so by time I can call them back at 4:15-4:30, it goes to voicemail even though they're supposed to be open until 5, and then the cycle repeats. Even my pharmacists, and offsite lab techs have problems getting in touch with them during normal business hours. I can't even get appointments without a minimum of 6-8 week wait. I have a medical condition that they supposedly specialize in that requires lab results or else the doctor and I would have nothing to talk about. When I messaged through the app to enquirer about getting labs scheduled, the staff person said I had no lab orders so I'll just have to come in for a completely unknown reason. I replied that without lab results that a trip in would be a complete waste of my copay, and her reply was to take that up with my doctor on my visit about having too many visits. At that point, I was done being nice, and after waiting a couple weeks, I finally replied having to lecture her, a medical professional with access to my medical records, about my condition before she would even communicate with my doctor. Of course my doctor replied stating she forgot to put the order in for my lab tests, which I'm not mad about, but it shouldn't have taken me having to lecture them for the staff person to ask my doctor a question. For boutique premium health care, I expect my experience to not be so profoundly inconvenient. I'm sad because I really liked them to begin with, and the only reason I haven't changed providers just yet is because the Portland area is extremely under served for my condition. read more

I have only recently had the pleasure of visiting Prism Health at their Belmont clinic, but they have earned a lifelong client and fan. From Michael, the Health Services Coordinator and the reception team to Miguel Carreon, one of the wonderful NPs, the staff creates a warm, positive environment in a setting where the standard is almost always the opposite. My (many) questions were always answered promptly and completely with a smile and good humor while not affecting the staff's professionalism. The service itself was great and the staff was friendly. They coded my visit incorrectly, then didn't bill me for six months. So when I went to resubmit the claim it was denied because it was too far in the past. I have to now pay hundreds of dollars for their mistake.
