This expiration policy was never stated anywhere on the gift certificate. There is no expiration date listed and nothing indicating it would become invalid. It was extremely disappointing to be told I couldn't use the gift certificate especially when the service was fully paid for and clearly specified on the certificate. This feels misleading and unfair to customers who receive gift certificates in good faith. Based on this experience, I would not recommend purchasing or gifting certificates from CaloSpa unless all expiration terms are clearly disclosed in writing.

My skin was looking dry and dull and just plain tired with a combination of dry flakiness and mild breakouts. Nothing I'd tried was helping much, so I reached out for professional help and I'm so glad I did. Lindsey was right on time, very friendly and professional, and it was clear from the start that I was in excellent hands. She explained the processes, devices and products she would be using and encouraged me to ask questions if I wanted as we went along. The entire experience was very relaxing and pampering, and best of all, effective. My skin has never felt so thoroughly, yet gently deep-cleaned and nourished, somehow calm and perky at the same time. In the week since my HydraFacial, there's a translucence, and yes, glow, that I hadn't seen in years. I can't wait for my next visit!

My family gave me a gift card to the center so I went through their website on Friday to request an appointment as the website directs. Once you have requested an appointment, their website tells you someone will be in contact with you to confirm. After not hearing from anyone by end of day Monday (they appear to be closed on the weekend), I called the office and left a voicemail asking for a return call. I made a second attempt to call on Tuesday during business hours and after again not hearing from them, I texted the number provided on the answering machine (they tell you you can text) and several days later still no response. I even went by the office in person and although I could hear someone in a room receiving some kind of service, there was no one at the front desk or anywhere else in the building. At this point, I am not interested in becoming a client here, rather am frustrated that my family member wasted $200.00 on a gift card. With so many amazing med spas/wellness centers in the southern Indiana/Louisville area, please spare yourself and book with one that is reputable and responsive! My hope is that as a good business practice, Azure would want to rectify this situation with a refund, but I won't hold my breath on a return call. Update 10/2/23: Sherri, this seems to be the only way to communicate with you! I have not received a text from you and returned your missed call same day (Friday 9/29) at 1547. I left you another voicemail asking about a refund. Please call me tomorrow to confirm you've seen this and to discuss how to proceed. 10/6/23: still no response.

    This is the strangest review I have ever written and it will probably hold that title for a while…read more I don't know if people would use Yelp to look for surgeon, but hey, it is one other platform to share information. I am giving 5 stars on my overall experience with the office staff, doctors, pricing, and results. At this practice, I went to see Dr. Geoffrey Durham-Smith for a breast reduction (non-cancer/illness related). I was referred by a friend who had a surgery done by him in the past and was satisfied with the results. I went for a consult (sometimes these are "free" meaning they may be covered by your insurance plan, CHECK WITH YOUR INSURANCE FIRST). At the consult, they will take your general information (height, weigh, blah blah) and it may include taking pictures of your breasts if that is what your surgery is for (it's weird, but just a heads up). Then the doctor will come in and briefly speak with you, ask you a few questions, and give you more information. If you want them to, they will submit a request for medical review/prior authorization to your insurance company to see, if you decide to have the surgery, if it is a covered benefit. Once they receive a response from your insurance company, they will get in touch with you. After the initial anger of my insurance company refusing to even review it for medical necessity (because it was NOT cancer related), I was able to talk with them about the self pay option. When it comes to self pay, they can give a pretty accurate quote (including the doctors fee, anesthesia, operating room, and hospital fees). Make sure to ask what fees may come up NOT included in the quote. I had a few fees come up that they did not say, which led to some anger and heated discussions with them, but everything was sorted out. The office staff was, 99% of the time, helpful and patient with all my questions and paperwork requests (needing receipts, FMLA paperwork for me and a caregiver, disability paperwork for work). The other 1% was after I received an unexpected bill and someone who didn't do the billing/cost information tried to help me and did not have all the information when I was already mad (if you already paid everything for you were quoted and you get another bill when you have already paid everything out of pocket, you would be mad too). Dr. Durham-Smith is a very professional and to the point doctor, he did not chit-chat and shoot the breeze. His bedside manner was all business, which when surgery and pain scare you and make you feel like a child, may not be the best, but the nurses at the office and at the hospital, help with that. I am very happy with my results from this surgery and experience with this office/doctor. If there are any women who are even THINKING about getting breast reduction, I greatly encourage you to at least get a consult, and I would recommend this office. Just make sure you are asking questions of your insurance company and to the office itself when it comes to money and coverage.

    I've been coming here for quite a few years due to my skin and the amount of moles/freckles I have…read more The skin check appointment is normally about 10 minutes and they check every part of the body (as long as you are comfortable with it). I've had several moles removed and a precancerous spot frozen off. While not the most comfortable, it's quick and relatively painless. The freezing isn't the most pleasant thing but I'd rather be safe than sorry. They are often booked fairly far out so I try to make my next appointment when leaving the current appointment. The staff, including the doctors, are all pleasant and friendly. Taking care of my skin is becoming more a bit of a priority to me and I'll continue using Dermatology Associates for my yearly skin check to make sure there aren't any areas to be concerned about.

I paid the money as drove over 50 miles just to get there. I am scared I have lost my money. NEVER EVER go there. I have not ever been asked to pay up front. After calling Medicare I found that yes they can legally do this but the absolute majority of Doctors don't. I will not ever go back. Medicare assured me that they would see to it that I will get a refund, if there is one, after billing. The receptionist was beyond nasty and unfriendly. Now to add to my stupidity I found them at the Kentucky State Fair. I would bet anything that they are losing patients left and right. In short...DON'T GO IF YOU ARE ON SOCIAL SECURITY.
