Subacute at Autumn Lake is no exception. We discovered this in 2025 after a family member experienced a traumatic injury requiring residential PT. The opulent ambiance and crystal chandeliers? Only there to lure your business. The attractive dietary menus? You'll take that tiny grey hot dog for your Sunday dinner and you'll damn well like it. Daily PT? Not in this lifetime. Lucky if twice weekly. Clean sheets? Only if you complain loud enough. Caring experienced nurses? Better get used to non-English speaking "assistants" assigned to ten patients at a time supervised by an invisible administrative nurse. The "Family Meeting" to address grievances involving abuse or lapses in care? Pre-scripted and choreographed to shut you down. Recommend you take a professional advocate along. If you find yourself in need of serious post-hospitalization PT or convalescing care, I would direct you to only the facilities associated with established, wealthy retirement communities that still offer the pre-Covid levels of care no longer available at former standalone facilities. Places like Subacute Autumn Lake have been converted into storage bed facilities for impoverished dying elderly Medicaid patients who've overstayed their hospital authorizations. If you have good private insurance, stay way clear of this place if you care at all about your loved one.

When I got into the Lobby and the young lady was pleasant but the older woman had major issues with me. Not to mention, I was juggling my mom and two service animals and to have more of a Hard time, was disheartening. My animals are registered and fully vaccinated and have their vests and proof but they wanted vet records. I asked for a card or Fax number and she wouldn't give it to me. She insisted on me physically bringing the paperwork to them. I'm about 35-40 minutes away and it is a BIG inconvenience. Do better and stop Being Rude and giving me a Hard time when I wanted to cheer up my good friend Who is Your patient there.... Disgusted and frustrated and Aggravated. If I ever Need Rehab. Power back or Promedica Won't Be My choice.

    This facility was a horror show and this is putting it lightly. My mom was mistakenly placed in…read morethis facility recently from the hospital when no other rehab had an opening. Unfortunately since we needed a rehab and there was no where to place her, she was placed here. Huge mistake on our part. She was there approximately two weeks ago for only two days total and it was two days too many. The building looks like converted Section 8 and that is an insult to Section 8. It's clear that it is run by the slum lords of nursing care. The minute you step in the vestibule there are random old boxes and packages thrown about. The supposed refreshed lobby looks rundown, dim, and not secure. I'm not sure I saw one security person in the multiple trips I made nor an administrative figurehead. The elevators look like something out of a horror movie and do not appear to have ever been wiped in the entirety of their operation. My mother, who is immune compromised, was on the 5th floor, which contained their COVID unit, which consists of a plastic sheeting separating one side of the floor from another and that's it. But more on that later. Upon stepping off the elevator you are smacked with a smell of rancid urine and feces, which only gets stronger when you enter a patient's room. The walls are an off white as they haven't been painted and minimally cleaned. The floors look dirty. The day room looks sad and depressing. There is NOTHING for the residents to do; the furniture is falling apart; and the scant books clearly have never been cycled out. I never saw anyone come in to clean the rooms. Only someone to collect the tiny wastebasket that is in each patients' room, which are not private. Btw, there are no biohazard large closed lid baskets to be found in any of the patients' room or out on the floors so it was concerning to understand what they were doing with the medical waste. The rooms looked disgusting. My moms sidetable still had the food contents inside from the prior patient. The baseboards had all kinds of disgusting buildup. Regarding care, it was subpar to nonexistent. My mom was wheeled in from the hospital and the sheeting and padding from the hospital, including diaper was still there the next day (nearly over 24 hours). That means they left her sitting in her own mess for that length of time. Mind you my mom was hospitalized with a urinary tract infection and now they were about to create a new one. We had to complain that they hadn't check her since intake and they still didn't come in for over another hour and a half from our complaint. In that timeframe my sister and I change our mother ourselves. Multiple times we asked to speak to the doctor but couldn't because he wasn't there. It seems the facility only has one doctor for the entire facility and he may not even be there sometimes. Similarly they only have one RN so good luck getting any seasoned care. Each floor has appears to be two LPNs or someone with less training and they are handling a significant number of patients each. We we complain that our mother had be changed the one LPN told us they were short staffed as if that's an excuse for neglecting patients. That's a administrative issue and it needs to be addressed as patients cannot be expected to get sick, injured or die because the administration has determined to cut corners. My moms condition continued to deteriorate in the two days there as we could not get the facility to get the doctor in on the weekend to look at her meds and care so she wound up with another infection and we unilaterally called the ambulance and thankfully got her out of that hell, back in the hospital and working on a better placement. If you love your family, do not put them in this facility and, if they are there, do everything in your power to get them out. No good can come of them being warehoused in this place.

They don't just plop her in a chair and pretend to be helping her. They the physical therapist coming for real. They have a great team to how to get her back home to us. She absolutely loves it there. They are patient with me as well. I know I can be a bulldog when it comes to my mom. But I have not had to be that way. I have never had such a wonderful experience in rehabilitation facilities till now. I highly recommend this lovely place. The only con is the parking. But as for care it is top notch.

I also worked here after to see the care they give & there is NO CARE besides the receptionist & maintenance they are nice (Dawn & Russ) I don't know how that place is still open!!! Poor care & poor quality workers!!!! Ask any of the seniors there they will say the same thing!!! PLEASE DO NOT LEAVE YOUR FAMILY HERE!!
