Me and my beautiful fiance .Tina who is the reason I moved from Oregon to Southern California..After a 9 year online relationship

At first it was a rocky start. But as time went on it turned out to be a very nice place All the nurses and CNAs were helpful for whatever I needed. The food was top notch healthy meals .I gained the weight I lost before I got there The social services people were spot on.. Got me transportation to my appointments when needed . I'm gonna miss the people there . Home away from home I want to say thank you to all employees for a comfortable stay And wish them all a very happy holidays

My father had been at this facility since early November and it's been nothing but problems after problems. The first week being their his new clothes (with tags and worth hundreds of dollars) were stolen and administration has yet to reimburse since his property was never recovered. That's just the start of my laundry list of problems I have. I set up a care meeting regarding the neglect of not having a call light and personal belongings that were taken and never returned. I feel like all staff members at that meeting just appeased our complaints and our concerns were never addressed. My father has a service dog and they have been denying us visits because proof of service paperwork for the dog needs to be provided. Even with proper paperwork that's discrimination. Today January 26th 2023 at 2:13pm pst I received the most terrifying phone call from a staff member saying my father escaped and is missing. MISSING! After the "proper" amount of time pasted and all people on his emergency contact list were notified then they proceeded to call the police. My father managed to walk all the way to Foothill Blvd. in 20-30 mph winds, Foothill is 2 miles from the nursing center! Thankfully the police found him and my husband was able to inform me that he was found safe and returned before any staff member called me to inform me that he had been found. As it stands we are currently waiting on a call back from DHS. Just look at his smile. He's happiest with his family! I hate that he even needs to be in skilled nursing. Please consider other facilities to take your family members to. This center isn't the one!

Catching some sun on a walk around the center. Nice grounds in back!

My mom has been in CNC for a little over a month now and our experience so far has been very good. I believe the success of a patient is based on the attitude of all involved and so far I have the highest regard for all of the staff from leadership down to the CNA's in my mom's care. She's pretty stubborn and if they can make her comfortable and reasonably happy, we're good. Obviously, this is not a 5 star hotel so comments such as poor food, cleanliness, treatment have to be taken with a grain of salt but again, I'm pleased with what's been happening so far. Thank you Citrus Nursing Center team for doing what you do well.

A bag of urine emptied from a urinal by a staff and left on the shared bathroom floor for hours!

As in most cases my brothers and I had to figure out in a short time where to put Mom. We searched around the Inland Empire as our most convenient location since my brothers and sisters are scattered throughout the western United States. We looked around and happened upon Villas. From day one everyone at Villas went above and beyond helping answer the myriad of questions that we all had about moms care. We spoke to the staff at Villas and they put in simple terms how Mom would be taken care of how much the cost would be for the different levels of care that were available. The food service and also her medicines and how they would be handled. Needless to say we all feel very much at ease now knowing that mom is there at Villas. The place is immaculate the staff is professional and very gentle with mom. There are a lot of activities for all the guest and there are different levels of care that can accommodate any situation. We are very thankful for Villas and highly recommend this place to anyone that finds themselves in this difficult spot. Tommi and Gladys who helped us out with Mom are angels. My moms caregiver Dulce takes care of mom as we would if we could. This is the place for your mom or dad.

Other than those things she does all her own dressing, personal care, eating and is active in the community. The owner was Dan and the director was Tommi when she moved in. It was wonderful then. It was sold to Oxford Health Corp and the current director of the facility acts like he cares but then is unkind. The Director acts good to the residents when corporate comes to visit. He is very rude, disrespectful and condescending to my mother. The Villas is a less desirable place to live now compared to how it was. Making money and cutting programs, staff and the quality of the food is now the Corporations #1 priority, not the residents. The Receptionists, Activity Directors, Med Techs, direct care givers and Admin Assistant are wonderful people and really care about the residents. Although many of the caregivers are under trained and often short staffed so answering call buttons takes a long time. But that is the fault of their directors. Most of them are very kind, just not properly instructed. The food is not gourmet as they claim. They say they offer transportation but the bus is often broke down. The building, rooms and grounds are nice but if you have to share a room, due to financial reasons, be prepared for sharing a bathroom, refrigerator and to have competing noise with TVs, tablets, phone conversations and visitor conversations. The cost of the room is reasonable for all the amenities but if you need any kind of care the cost is ridiculous! So, why do we keep Mom there? Partly because she's 90 but mostly because she has built community with some of staff and residents and is willing to take the bad with the good!

I am writing this review based on my personal experience with the Gardens of Riverside, where my…read moremother lived for the last four years of her life. Initially, everything seemed fine, and my mother was happy and active. However, things took a turn for the worse after Gracie Garcia took over as Director. I noticed that some caregivers seemed unqualified to care for Alzheimer's patients. In June 2024, I started noticing marks and bruises on my mother, but Gracie and Joanna refused to explain why, citing that I was not the power of attorney. Her power of attorney did not visit. My mother's cognitive state began to decline rapidly. I stopped receiving updates and photos of her activities, and during my weekly visits, I saw many residents, including my mother, left unattended in wheelchairs with no stimulation. In August, my mother was left standing unattended and fell, hitting her head and causing a brain bleed. Despite my persistence, the facility delayed sending her to the hospital for five hours. Gracie claimed it wasn't an emergency because she didn't see a bump on my mother's head, demonstrating her lack of qualifications. My mother's doctors' notes indicated that she needed follow-up care, but both the facility and her power of attorney failed to provide it. My mother passed away two months after her fall, and her death certificate listed a subdural hematoma as the cause of death. If my mother had received proper care, she might still be alive today, as she was not in the final stages of Alzheimer's disease. I am sharing this review to warn other families about the Gardens of Riverside and the leadership of Gracie Garcia. I've documented my observations of my mother through photographs from June to November. Although I was currently involved in a lawsuit with her power of attorney, her passing ultimately brought relief from my daily concerns about her well-being.

I had visited before and had placed him on a waiting list. Staff had always been very nice and called me immediately if they had any concern. If I had any issues, Gracie took care of them immediately, but those issues never involved the care my cousin was receiving. At one point they called and said there was an opening in a smaller building, and they wanted to move him there so he could have more eyes on him, since he was becoming unsteady on his feet. This did not increase his rent. It is a memory care facility, so keep in mind that residents like to go "shopping". So, make sure all items are marked permanently. I used a washable tag and snapped them on with a tool from Amazon. Do not take anything of value, like jewelry or anything you would regret going missing. Do not take more items than they really need. The staff can give you a good number of items they really need. You really have to be realistic when placing a loved one in any facility. It's not being cold hearted; it's a fact of life. My cousin had very thin skin and bruised easy. That happens as we age and when he had bruises on his arms, I could tell they were from the way he leaned into his wheelchair. Two people at a time also had to pick him up to move him into his wheelchair, a recliner, the shower chair and he gave no assistance at all. A six-foot-tall man. I never worried while he was there, and this was a great relief. The Gardens called me as soon as they saw a pink spot to let me know they were contacting his health care provider. I was also comforted to have a conversation with a visiting nurse, and he had stated that this was one of the best facilities in the area and the staff was doing a really good job on treating a bed sore. My cousin no longer walked, nor stood and I was able to view the log on how many times he was turned each day. My cousin had starting to refuse food and would throw it and spit it out. The staff kept me informed and worked with his healthcare provider to try anything they could to give him nourishment. My cousin passed away on Good Friday 2025 from complications due to Alzheimer's. The Gardens staff was very comforting, helpful and considerate. He started to transition three days earlier and was on hospice. I would especially like to thank Gracie, Lynn, Vickie, Amanda, Jojo, Claudia and the maintenance man for their help during my cousins stay and during his transition. There were many others for which I don't know their names. They all showed so much compassion and understanding that was truly heartfelt.
