My father had been at this facility since early November and it's been nothing but problems after problems. The first week being their his new clothes (with tags and worth hundreds of dollars) were stolen and administration has yet to reimburse since his property was never recovered. That's just the start of my laundry list of problems I have. I set up a care meeting regarding the neglect of not having a call light and personal belongings that were taken and never returned. I feel like all staff members at that meeting just appeased our complaints and our concerns were never addressed. My father has a service dog and they have been denying us visits because proof of service paperwork for the dog needs to be provided. Even with proper paperwork that's discrimination. Today January 26th 2023 at 2:13pm pst I received the most terrifying phone call from a staff member saying my father escaped and is missing. MISSING! After the "proper" amount of time pasted and all people on his emergency contact list were notified then they proceeded to call the police. My father managed to walk all the way to Foothill Blvd. in 20-30 mph winds, Foothill is 2 miles from the nursing center! Thankfully the police found him and my husband was able to inform me that he was found safe and returned before any staff member called me to inform me that he had been found. As it stands we are currently waiting on a call back from DHS. Just look at his smile. He's happiest with his family! I hate that he even needs to be in skilled nursing. My mom has been in CNC for a little over a month now and our experience so far has been very good. I believe the success of a patient is based on the attitude of all involved and so far I have the highest regard for all of the staff from leadership down to the CNA's in my mom's care. She's pretty stubborn and if they can make her comfortable and reasonably happy, we're good. Obviously, this is not a 5 star hotel so comments such as poor food, cleanliness, treatment have to be taken with a grain of salt but again, I'm pleased with what's been happening so far. My Mother was taken here for P.T. after her hip surgery. She is epileptic and needs anti seizure meds. The very first day she was admitted, she was not given her meds. The next morning she was not given her meds either and my mom went into multiple grand mal epileptic seizures. She had to be rushed to the nearest ER to have her meds given through I.V. and the doctor verbally stated that she had no meds in her system. Upon asking for the meds administration log, a worker HAND PRINTED times on a sheet right in front of me falsifying information to cover their mistake. I removed my mom the very next morning and filed a complaint with the insurance and with the CALIFORNIA DEPARTMENT OF PUBLIC HEALTH. If your loved one is not treated right, please file a complaint against this center. It's really easy and you do not need to go anywhere. They do the investigation and contact you back with the results. read more

I will highly recommend this place. The staff are friendly and if you had a complain the Administrator act right away. The Physical Therapy is highly recommended too. I can't even move my half of my left arm and leg When I was admitted. I can't even stand. Three staff is help to stand and go to bathroom/ wheelchair. Now I can walk with my walker and can stand by myself with the help of the Physical Therapist. Beyond terrible! The people who work here don't care about doing their jobs and even worse, don't care about their patients. My dad has been here for almost a week now and it takes days for anyone to do anything. When you press the call button for assistance, it takes hours for them to respond, if they do at all. An absolute dumb! Very low quality care in everything, very very little hygiene, standards, very unprofessional, staff, etc. The place reeks of urine and feces due to improper disposal of soil diapers, as well as the fact that most patients have to sit for a good while before they can get changed. Due to staff laziness, they literally make patients write their names in their clothing, as if they're in a concentration camp, so that everyone's stuff can be thrown in together and laundered at the same time with the same nasty detergent, no softener, no ironing, impressing, nothing. There are no showers in rooms, patients are forced to shower in locker room style filthy, shared cubicle showers, that they are transported back-and-forth from through the public corridors. There is no emphasis on health food, the place is a desert if you're looking for fresh fruit and veg. All of the food is pre-prepared, they simply serve stuff on a glass plate to give more of an illusion of in-house prepared food. I observe many instances where the staff exploited peoples conditions in various ways, such as telling someone who is in a confused state that they did, or didn't ask for something or that they already did something, etc. They antagonize patients in order to play them against each other to try to get patients fighting with each other. They use the concern of still being afraid of Covid as a way to get out of having to take patients on social outings, even though they have full-time staff who are specifically there as activities coordinators. If cleanliness is important to you, this is not the place for you. They have very very low standards for cleaning, and what they consider the ones monthly deep clean, is not even sufficient for a daily cleaning, according to many people who would have higher cleaning standards. read more

Awful- i was given this place's name as a possible rehabilitation place for my mom post surgery but all i ran into were cons. First they require a covid vaccination card and will make a copy before being allowed inside but there is no full tour. As soon as I walked in, it smelled of stale cigarette smoke all the way from the smoking area to the lobby/front door. Also it was visibly over crowded without enough workers. They didnt offer a full tour, only a a peek/100 feet in past the smoking porch and not the patient rooms. But from what i could see, there were lights on showing the patients weren't getting care or lacked enough staff. The patient area i could peek was lit up light Christmas lights - always a bad sign. We ended up here when our loved one was discharged from Kaiser and was not stable enough to go home safely. He was weak from laying in the hospital, not using his muscles and needed to regain his strength so that he would not fall at home. We preferred to take him home, but we understood that some physical therapy would benefit him so we agreed to have him released to this rehab facility. Had we known the kind of care he would have been getting, we never would have agreed to this facility. When you walk in, the lobby looks clean and well decorated. It does not smell like some other facilities I have toured and the people were friendly enough, so I had hopes that things would go well. At first, I was pleasantly surprised when we were getting assurance about how much they would be doing for our loved one and what the physical therapy involved. I explained what our goals and expectations were and I was told that everything was noted and that everything we requested would be done. Sadly, this was all misrepresentation Due to COVID, visitation is by appointment only. 30 minute visits are allowed, but they are difficult to get, so we were unable to see for ourselves what was going on. After a few phone calls with the patient, we quickly learned that nothing we had been promised was happening. For the first day he was never taken out of bed once. Supposedly a physical therapy "Evaluation" was done so I assumed that meant they had gotten him up to work with him and see what he was capable of. I was wrong - the evaluation involved two people watching him eat and having him push against some things to see his resistance. He was not even taken out of bed for this evaluation. Since our concern was him being able to get in and out of bed without falling and this was well communicated to the staff and supposedly noted in his chart, I would think working with him to get out of bed would be part of his therapy. He did finally get a limited amount of therapy for 20 minutes the next day, but it did not work with any of the concerns we had. We were then told that they have do not do any physical therapy on Sundays. How does a rehab center not offer rehab 7 days a week? So in three days he was scheduled to have 20 minutes of physical therapy....this program is not going to get the patients stronger. But I do not think their goal is to help the patients improve. The goal is to keep them there an bill the insurance. This did not seem to be a short term rehabilitation center. We learned later that there were a number of patients that appeared to be there for mental deficiencies that are long term residents. There was a woman wandering the halls barefoot talking into her remote control like it was a telephone when my sister went by to drop off clothes. I don't think anyone expects the food to be good in these places, but apparently it was so bad and served cold that our loved one requested a protein shake instead of a meal. They said ok, but never came back with it. We had to call from the outside more than once to finally get it delivered hours later. We were originally told that all patients were bathed every other day based on odd/even room numbers. When I asked for the schedule for the room in question I was then told it was not actually every other day, it is 2 days a week based on room number. The care takers are also not gentle. When they do finally get the patients out of bed, they are very rough with them. I am guessing this facility can keep your loves ones alive or they would have lost their license by now. So if your only goal is to keep someone alive in a place where you don't have to take care of them, then this might work out for you. But if you have any hope of someone getting healthier or stronger, this is not the place for you. They will tell you what you want to hear and not follow through. If you have no alternative and someone you care about gets stuck here make sure to be visible. Call repeatedly, visit daily, and check in with your loved one constantly to see what is really going on. read more

The second you walk in it the stench hits you like a brick wall. The faculty is unkept, they are understaffed, the Sprinkling on shift are blatantly rude, and argumentative with patients. After-hours patients are locked in. That HAS GOT to be a fire violation. Medications are not administered timely or properly. Non-ambulatory Patients are left laying in their own feces for extended periods. When they are finally taken to showers they are taken on a Hoyer lift completely naked....no sheet, nothing! Medical diets aren't followed, and the charting is incorrect. My brother was told he had a blood infection and sent to ER only to be told there was absolutely nothing wrong w him. So clearly they aren't treating patients according to the correct charts/ labs. The food mimics that if a prison and the restrooms are atrocious. One nurse emptied a urinal into a plastic bag, threw it on a shared restroom floor where it stayed for more then EIGHT hours. If you love your family, avoid this place, do so at all costs!! Challenge the insurance company even it is medical! The neglect at this facility is an absolutely disgrace! read more

My Dad was transferred to this facility a couple weeks ago. I was a bit nervous after reading some reviews. So far I can say the staff is focused on the care of my dad and all the patients. They shave him, and keep him clean and feed him. The staff has been responsive to our concerns and are welcoming. Keep in mind every facility is understaffed due to covid but the dedication does remain the same. Keep in mind if you come before or after the regular times 9am to 6pm you may need to call to be let in, or knock on the side door to get a nurse to open the front door. Staff is friendly and dedicated. There is roughly 3 patients to a room but that seems to be the norm during these times keep this in mind when visiting. Therapists are also dedicated and seem to care. 5 star rating is well deserved read more

They killed my mom, my mom went to this nursing home to receive physical therapy for her legs. She did not go into this nursing home with COVID-19, they made it so strict to even visit her they said that you would have to be vaccinated and the staff is also vaccinated, they said they tested their patients regular. This place is horrible! The pictures that you see are only on the entrance of the place but, once you go inside it is old. Beds are super old. Nothing like the picture of the room they have online. Do not put your family member there!!! A one because nothing less! It stinks! The staff ignores the patients!! My sister was transported in with a broken leg from Santa Barbara. After twenty minutes of no staff coming in the Attendants apologized and told her they had to get back up north. She waited and called me in tears I could barely hear her from screaming in the room with her. I didn't know what the do called Kaiser the only Care Coordinator was in the ER, I told her the situation and asked what I could do, she could only help if an ER patient and it was after hours so no one was available. I asked how i could get her to ER she said. 911 so I called 911 and 15 minutes after the police officer arrived the facility called to apologize! Their excuse after over an hour of hot even acknowledging she was there was they had another patient check in!!! They can only handle one patient at a time, I guess that was the reason for the screaming patients!! I pray their families see this and check on them and get them to somewhere safe! It took us until the next day to rescue my sister!! Two police visits and $183 transport! Do not put your family member there!!! read more

When I first was admitted to citrus I was frustrated and concern related to my stroke and weakness. Nursing,case manager,social services,therapy,business office manager,dietician, maintenance all departments were cool and treated me fair. Very attentive to my needs.Staff provided friendly astomphere,knowledge,and assistance to all my needs and concerns. I would recommend Citrus to my family and friends. Just don't do it! If you love someone, this is just not the place for them. There are some wonderful staff here and after 2.5 months here, I will miss some of them. But in my, not so humble, opinion, they may have to regroup and do everything over. It's drab, depressing and there's quite a bit of attempted placating ("we will..." "I'm sorry that was your experience " "That's not what we meant when..."). The fact that the website photos are DRASTICALLY different than what you find, once you arrive, proves that they know the facility needs work. I won't get too specific here but there are staff that have been there long enough or that are in the "in group" that don't perform well at their jobs because they know nothing will happen to them. So, they do the bare minimum, just to get by. Unfortunately, that means patients that cannot speak up for themselves, don't get treated as well as they should and the ones who can speak up (and do), get shipped out before their rehabilitation is complete, to keep the apple cart from being upended. The saddest part is that with some care, this could be a better place but it seems they'd rather keep sweeping things under the rug, instead of change. As soon as I spoke up, they made up reasons for me to leave. Why would I want to stay, you might ask? Because most of the therapy staff was excellent and despite the cleanliness, outdated facility and horrible food choices, the therapy was helping me. Again, please don't leave your loved ones here, but if you absolutely MUST, check on them often...without warning so you can see how they're being treated. read more

He is a kaiser member and this is one of the facilities they recommended, the admitting process went smooth and he stayed for 3 weeks to get his physical therapy in and came right back home. Allowed my father to sit unmonitored for a week while his heath detiorated. No one trained enough to chart mental status. No one really cares. Nurse yelled at us for asking her to check on him. Came "highly rated" by va Loma linda staff, but honestly don't see where the 5-star rating came from. They did not inform us of the positive covid patient in the center and my father caught it. We had a close friend of the family's who's been living there for over a year. Her daughter allowed us to go with her on a scheduled visit so her mother could at least see us from behind the plexiglass overlooking the parking lot. We were impressed that the facility followed social distancing guidelines like they're supposed to so that everyone there stays safe, but they still made sure their residents don't feel isolated during this pandemic. I enjoyed my stay at the Citrus Health Center for several reasons, which I have stated below. First, I should say that I spent two weeks at Citrus due to suffering a stroke on my left side (I have never had a stroke before). When I arrived, I had no mobility in my leg and arm. This is the placement my health provider provided, thus we did not have chance to research, it was a true learning experience. Positives: Caring Nurses and CNA's (Certified Nursing Assistants) even during the most difficult times Many bilingual staff Clean basic living and facility areas (beds changed daily) Food is what you would expect, low salt/fat. Appropriate portions Access to management when needed Excellent therapists, very knowledgeable (A+ for me) Excellent Covid-19 rules are in place for all who are in the building Staff always took my wife's phone calls and deliveries Concerns: Time it took for Nurses or CNA's to come and help/answer questions, I had to be patient. (I will add, the average CNA had anywhere between 12-18 patients to attend too at any given time. What I found, some of these patients required constant monitoring, thus other patients where left to wait, this also pertains to the nurses. I highly suggest to the management that more staff is hired due to the type of high-need patients Citrus serves) This facility serves both high and low functioning patients. At times, the needs of the low functioning patients had a negative effect on the ability of the staff to attend to the high functioning patients. Inside gray wall paint is very institutional (dark). Wall pictures of old Fontana...why, who cares! All staff wear gray institutional scrubs (yuk). I viewed this atmosphere as counterintuitive to patients wanting to get healthy. Suggestion: a new paint job with positive colors, scrubs for the staff that are bright, cheerful and place slogans of positivity on the walls and hang pictures of famous people who have overcome tragic health/life events. Make sure to educate all staff about the slogans and pictures so they may share and possibly inspire their patients. Final thought: I have read the other reviews, I see their point of view. What I observed and this is from my point of view, families are dropping off family members and thinking that the Citrus staff has a magic-wand that will automatically make their loved-one well. This is not the case, the patient has to want to try and families need to be involved as best as they can (I understand, Covid rules make this difficult). For me, my goal is to be healthy, self-reliant and get back to what I consider normal. Citrus made a difference in my life... now I'm getting better! I came to this nursing home 3 days ago. Since then I've witnessed and experienced substanard care directly due to lack of help. As it is..... it's extremely difficult when one becomes incontinent, it's degrading to have someone come and clean your bum. You have to accept that it's necessary, because you're helpless in that area. If that wasn't enough I witnessed, a woman in my room left laying in her own feces for 3 hours, because the night shift just didn't change her. As soon as she said she had poop they would come from time to time turning off the call light, but then they kept leaving saying they were coming back with a diaper yada yada. When actually what they were doing was saving it for the next shift. She was crying "why don't you clean me?" "how can you do this? My son brought me here for help" "change my diaper l can't sleep with poopoo" She was continually ringing the nurse button but no one answered, then she began to pound that button until it broke. When the nurses came, she was hollering. " clean me " "clean me " "clean me " they told her they understood she needed to be changed but she didn't have to yell! They still didn't clean her! The nurses said "can you believe she broke the button!" They had it replaced, and still left her laying in her own feces. When the next shift comes on, it takes some time to get organized delaying the process even longer. Sadly the CNA on the new shift had to take the blame for this woman waiting so long. By that time she had been already waiting for 3 hours. I couldn't sleep at all with her yelling. Several hours later , when the morning shift came on, she was still confused, wanting to know how come she hadn't been cleaned , why nobody comes, why had she been left, unfortunately This Crew had no idea what she was talking about! She wanted help , and this time she wanted to call the police! I myself had to wait an hour twice so far. The state of California nurse to Patient ratio for nursing homes is 64 patients to one nurse. That is not the case here as we had one nurse to 79 patients! There are supervisors that would meet the nurse requirement but they're not on the floor treating patients or helping in any way with the patients. In fact a patient wouldn't know they existed unless you repeatly, or nearly demanding to speak to them. The food they serve here is absolutely hideous! I posted breakfast on my Facebook page I had 73 comments on how nasty it looked. I posted a picture of my dinner for people to try to guess what it was because I had no clue. I got things like stuffing sandwich, coleslaw sandwich, what are those noodles floating around in there, and please tell me you didn't eat it. So I rate this place with two stars and that's because much of the staff truly care, they work so hard to keep your loved ones comfortable. They are sweating while they run from patient ,and left just as frustrated as the patients. Because they have no problems letting you bring in the food that is necessary for your loved ones survival. Although management leaves a lot to be desired. read more

The rooms do not look like the pictures at all they are small and he was in a room with 2 other men with a little dinky big back black and white TV. So sorry that we let our dad get transferred there from the hospital for "rehab". They just let him lay in bed 23.5 hours of the day. We called several times a day as we were not able to get in to see him for several days after he was admitted. The staff would tell us what we wanted to hear and made everything sound like it was great. Then we would talk to dad and get a completely different story. He said the food was horrible. Without proper nutrition and exercise, he was never going to get better. After a few days we checked him out against doctor's orders. We can provide better care at home. read more
