Should there be any emergencies or questions, he would not address them in writing. He continued to talk over me. I asked the front desk to help us so we wouldn't have communication barriers. Deaf does not mean we all can read lips. Some can, some cannot. Some can hear better, some cannot hear well. Some can speak well; some cannot. This violated my ADA rights. This tracks the character of the center behave to the people of representation; no compassion for that. Seek elsewhere.

The front desk folks and super friendly, as well as patient and informed. Paperwork doesn't take too long. The radiology technicians are wonderful and respectful, gentle and patient. I will probably always use this business if I need to have radiology work done.

I had to call and initiate the refund. Be sure you keep track of how much these people are charging you and make sure you dont overpay. They will try to steal from you.

Anyway.. he also ordered a chest x-ray as well as a shoulder and neck x-ray since I pulled something at the gym.. and no I didnt pull "it" at the gym in the steam room, jeez! So after waiting for 45 minutes I finally got all the x-rays done. What is up with medical offices that like to keep you waiting?! The cool thing though that the office manger realized I had been waiting for a while and went to find out what the hold up was since there were no one else there waiting. BTW, the cell phone coverage in their is practically nill, I had to keep switching seats to find a good signal. Sit closest to the door if you want to be able to connect with the Yelp app.. At least the place didn't smell like old cleaning rags though the guy that came in as I was leaving, well, I think he was wearing them.

I had a COVID Antigen test at Formula Diagnostics. I got an appointment the same day (good!)…read more Before the test I asked if they take insurance, but said "no, but we will provide everything you need to submit it to your insurance company". So, they did a good job with my test and my results within 20 minutes. When I was sent my results, but it was missing: 1) Type of service/Procedure code. 2) Health care professional name/credentials 3) Health care professional Tax ID #. I called twice, emailed them 2 times with no replies (bad!). Then and went to the facility in Scottsdale and they said they didn't have it (bad!). Bottom line, they provide a good service if you want to pay full price. If you want to submit it to your insurance, don't believe them when they say they'll provide you what's needed to submit it to your insurance.
