My grandmother stayed here for the last few weeks of her life. I truly believe that her lack of care contributed to her demise. There aren't a lot of positives to this place. Their communication is terrible, trying to get them to answer the phones was almost impossible, and room furniture looks like it was picked up from a garage sale. I feel like the staff had no real love for the patients, and their attitudes show it. I would not recommend letting your loved ones stay here. read more

Horrible communication! My mom was released here after surgery because my dad and I were sick and were unable to take care of her. First day she was there, she was given wrong dosages and missed her morning medication. I called the facility to speak with someone and only got voicemails. I called our social worker at the hospital and they were even having a hard time communicating with the staff. After an hour, I physically had to drive to the facility and bring my mom's medication to her. Even then, they didn't give it to her until 3-4 hours later. It took several hours for the facility doctor to 'straighten' things out after several phone calls between my mom's doctor and the facility doctor. The next day my mom was supposed to get labs and not eat before them. My mom's doctor specifically left those instructions. What happens? Well, of course the instructions were ignored and labs were given after my mom ate. Final day, I finally get the clearance from my mom's doctor to pick up my mom. The facility refuses to release her to me until the doctor clears her. I call my mom's social worker and my mom's doctor and again they are having a horrible time trying to get a hold of someone. My overall review is there's an extreme lack of communication at this facility. If my mom didn't have my sister and I advocating for her, I'm scared to think what could have happened to my mom. Oh my goodness where do I begin with this review?! This place is filthy with the worst staff I have ever encountered in a medical situation. My father is a diabetic and they were giving him pure sugar with his dinner plate?! When I asked about all the sweets and improper diet a diabetic shouldn't have, I was told that he needed the sugar for added calories since he wasn't eating well. I'm at loss here? Sugar for added calories? WTH!?? How about simple carbs if that's really the case here? I may not be a dietitian or nurse, but I know what's allowed for a diabetic to eat and it doesn't include whole milk, white bread or pure sugar!? He came in for rehab after having multiple strokes, and they didn't have a bed for him in acute rehab in Vallejo. We landed in this hell hole, I have yet to see a therapist to start any type of rehab! The staff at this place are most interested in gossiping than attending to patients. I've tried to call a nurse when my father was chocking and it took them more 20 minutes to have someone even notice! Insanity is all I can say about this place or hell hole! I wouldn't recommend this place to anyone, unless you want your loved one ignored and treated with no respect! If I could give a zero rating I would... please avoid making the wrong decision for your loved one. I wouldn't leave my pet here let alone a loved one. I visited my cousin while she was admitted to St. Francis Pavilion for two months, she hasn't seen a Dr. or know what their plans to help her are. Not even a physical therapist has done anything for her either. The unit needs to be remodeled and really cleaned, very disappointed in there care and no one seems to care. I would like to take the time to say thank you to this organization for taking such good care of my mother, Chiu at 204C. She spent one year in the facility, but she formed connections that she hadn't in years: she learned English, she made friends, she was taken care of, she was amazed with the cleanliness (she noted that the nurses mopped the floor every morning and changed bed sheets every day.....). Even though the food didn't suit her taste, she still liked the breakfast served around 7 am. She particularly loved the employees: Lanie, Nonma, Fidela and Josephine and the boys (I do not know their names) who delivered her medicine from time to time. It was heart breaking to lose her, but I know she is in a better place and we could only pray for her. Again, I hope the facility can keep up the good work. read more

And when they did answer after trying for an hour I was told to call the 3200 number which I have also been trying with no answer. I hope the care for their patients better than this!!! read more

This place is AWFUL!!! We had no choice as this was the only facility that would accept my husband that was covered by our insurance. My husband was at St Francis for a week and a half before he had to be readmitted to the hospital via the emergency room. The facility didn't provide the proper medication causing his illness to get worse. The food was abominable, call lights don't get answered, and they put everyone in diapers and let them sit in their feces for hours. There was absolutely no compassion from the staff. The attending physician didn't answer calls or return voicemails when we had questions regarding my husbands care and medications. The last time I saw my husband he was on an unmade mattress on the floor because he had fallen out of the bed and was in too much pain to be moved. The next day he was in the hospital. It was appalling!!! I would never send a loved one to this facility ever again!!! read more

I recently had a very bad experience with a "skilled nursing facility," and was very upset to learn that I had to go to one again. As St Francis Pavilion is only 5 miles from my home, I decided I would give it a try, but if it was anything like the first one, I'd just go back home. I was amazed at the difference. The facility is nothing fancy, but sufficient. The food is more than acceptable, it's good. Physical and occupational therapists are friendly, encouraging, and very helpful. Administration is competent, and they make visits to the patients rather than sitting in an office all day. Patients here have varying levels of need but from what I saw, all are treated professionally and respectfully. If you or a family member find yourself in need of such facility, I highly recommend this one. Also, I need to mention some names of some of those who really went the extra mile to make this a positive experience, communicating well and ALWAYS having a warm smile and friendly disposition. I wish I could remember all of the names, because there are three more that I'm not listing. Here's the ones I do remember: Elsie Karla Rayna Crystal Diana Abby Brandon Cristina Dinno I'm not happy that I'm not remembering the name of the therapist who worked with me the most, but she was wonderful, and there were three other nurses whose names have slipped my mind. I have been here since July 3 and what I have seen is a Facility that is totally avoid of any site management. For instance I have been asking for a Covid booster vaccination since I got here. I asked every day and every day I got a smile and OK. The least one would expect is the courtesy of A followup to let me know that the request has been forwarded to the proper person. Well do not hold your breath as a communication is totally nonexistent. The really worst part of this is that management, if there is such a thing allows this type of non-communication to exist. The staff that actually does the work like the CNA's and the RNs are for the most part great but management is not allowing them to The best job that they can. My observations show and understaffed and overworked working environment and I believe that is a result of upper management putting the needs of the stakeholders ahead of the needs of the patients. The problem with facilities like these that are privately owned and are for profit facilities I'm more concerned about money and how they can rip off Medicare. This place has been a disaster for three decades and they have been caught and find many times. if you are looking for a rehab facility for a loved one or friends do not I repeat do not consider this place. This is not a facility for humans. Avoid avoid avoid at all costs. I could write a book about my 17 days here and how ridiculous this place is. For those of you who are old enough to remember, the management or lack of management seems to be run by the three stooges. I can't say enough about the love, excellent care and stellar communication I've experienced throughout the months my mother has been at St. Francis Pavillion. This has been the most challenging period of our lives, and St. Francis has done so much to make sure the residents feel like the incredibly precious human beings they are, and that family members are kept in close communication about all that's happening. Though I can't speak for anybody else, I can say with confidence that my mom has experienced TRUE HEALING here. After other very prominent hospitals and elder care facilities clearly stated that my mother's condition could not be cured nor would it ever really improve, after just a few months at Saint Francis not only has she improved SIGNIFICANTLY, but the main health concern that had been pronounced UNTREATABLE by physicians, surgeons, registered nurses, specialized visiting nurses and home health aides within an entire "healthcare" system has been HEALED COMPLETELY. My mother had to return to one of the hospitals that she had previously been to, where all treatments had completely failed for this serious condition. The attending doctor literally called all available nurses that were present to come view this "miraculous" occurrence because he could not believe his own eyes when he saw the improvement. The "dire" situation had completely RESOLVED thanks to the incredible care at St. Francis!! MANY medical professionals from this other hospital had said it would be her cause of death. That was well over two years ago. It was suggested on many occasions at that time that we should seriously consider hospice. Had we listened to these unfathomably aggressive and certain recommendations, I never would have had the last two years with my mom, which may have been two of the best years we've ever had together. And I say "together" despite being so far apart. Because even throughout COVID we spoke via FaceTime every single day-- throughout the epidemic, which impacted almost all of the residents. And now the residents have RECOVERED from COVID (!!) and most recently all residents have now received both immunizations I am so grateful and in awe of the honesty, kindness and most importantly the GENUINE HEALING CARE of all those who continue to look after my mother at St Francis Pavillion. I believe for the majority of the medical professionals at St Francis their work is a calling, not just a job. read more

I am incredibly pleased with the care that I received from both the physical and occupational therapy departments. However, the CNA's that looked after me were nothing short of exceptional. Irene, Rose, and Sandra specifically made all the difference. Their sense of humor made me laugh and keep my spirits high. These three women in particular go the extra mile everyday. They went above and beyond the call of duty to be helpful to and supportive of me. The joy and energy that they bring to their workplace is infectious, and their attitudes gave me that extra bit of positive energy that I so desperately needed in order to recover. I am eternally grateful to have been placed in their care and my recovery may not have been as quick had it not been for these 3 incredible women. I commend all of the staff members I interacted with fro taking pride in their work and doing such an excellent job caring for their patients every day. Please let Irene, Rose, and Sandra know that I wrote you this letter as a testimony of the great work that they do. They are truly angels here on earth. read more

I'm always finding some sort of skin flakes or feces stained mattresses, and they don't even have bed sheets to put on top of them sometimes. Don't recommend this facility. Although the quality of care was great! The nursing staff member Supervisor Catherine is inappropriate with male patients! Getting confidential information and using it for personal use! Texting inappropriate pictures making sexual advances We reported A staff member there's inappropriate behavior with patients while they are in your care. I provided the text messages where she was coming in to a patient while he was in her care and sending naked pictures of herself to him. Stocking his house. He was very medicated after being in an accident. In a vulnerable state as many patients are there in your facility. I spoke with the the admin there and he assured us she would not bother us and he was having a training with your staff. I can't believe this is appropriate and acceptable behavior for your business. I can only imagine how many other patients she has been inappropriate with. I will see what are the next steps to take to protect the patients from being victims there at your facility. read more

A select few workers here are very nice, but only a very select few. The other workers? All I see is them chatting and joking with each other in the hallways, in their counters, and in the cafeteria. There's always dirty dishes near the sink and almost always no napkins. How can there be no napkins in a cafeteria?? Oh, what prompted me to mention these people's laziness is that they are soooo slow to respond. When my uncle presses the help button, I expect someone to be there within a minute. What if something serious happened and I was not there to physically go get a nurse? Yes, we waited a couple of minutes and still no one came and we had to physically go get someone. This is no way to run a rehab facility for people who need help. read more

My father was released to this place for rehabilitation per insurance this place was a 5 star and location was convenient. I should've looked into it a little more. From the day I stepped foot in this place I knew it was a piece of shit facility. The day my father got transfered was on a Tuesday evening I called the facility to make sure he arrived and no one answered the phones I valued about 10x, the next day I show up and it's a ran down old facility, they had him in a room with 2 other people , old room, no ventilation. I asked to speak to a manager or supervisor and was sent to speak to admissions coordinator (Rachel) I asked for a private room and asked if he could pls have a nurse that spoke Spanish or to use interpreter services, I also expressed the fact that no one was answering the phones. She gave me her cell # and told me to call her if anything. The staff had poor communication skills, didn't speak to social worker who was suppose to give me plan of treatment. That same night I called the facility to check up on my dad and again no answer kept getting a voice mail, texted the admissions coordinator and informed her and finally got thru. My father then tells me he's been calling the nurse for 30 min and no one has responded to him, he's been holding his pee and can't move, I again get in contact with coordinator who says she would get in touch with staff, staff comes gives him a urinal and leaves he is now left with a urinal on the table, he was given medication that he didn't know what he was taking it for because language barrier, he asked to brush his teeth and was told by a individual he could only brush teeth once a day, he had to go number 2 and went on himself because again no one was answering his calls. Not one time did a md come and assist him, I don't even think they have a md . He only received physical therapy once for 30 min which was on day 5. On the same day he looked depressed, pale didn't want to eat and notice a infection on his leg rash on chest , we asked the nurse if they been treating it and she said we did t know he has this , we asked for a ice pack, hand sanitizer and they said they didn't have any "wtf" this was the day I said I'm getting him out I dpoke with the director who was nonchalant told her this place is a dump, they neglected my father, it was negligence and that I was requesting a ambulance to send him to e.r. and thats wat we did!! Upon getting to e.r the doctor said ... let me guess 99 escuela , he said we get patients like this from them all the time you did right by getting him out of their, they keep changing owners to keep operating and the staff are individuals from agency's not certified. If I would've left my father another day I'm sure he would died in that place. It breaks my heart for those who don't have family to advocate for them. This place needs to shut down. This convalescent needs to close down. My Grandmother was abused by a cna that spit in her face . No diversity of staff speak in tagalog.My Grandmother is paralyzed and her food was cold and left on bedside table unable to feed herself. Grandmother had bed sore stage 3 , the colostomy bag broke several times , seizure meds not given causing admit to kaiser blood work detected no seizure medication. The State needs to investigate this facility. The cna was fired for abuse. Please contack the State Licensing , adult protective services. Police dept, ombudsman to report abuse , neglect. This was my second stay at St. Francis Pavilion and I am sorry that I have not reviewed it before. St. Francis is perfectly situated so that friends and family can visit, and it is on a quiet street. The staff is warm, welcoming, and caring. Do not pay attention to the negative reviews. This is a rehab and recovery place, not a spa. The nursing and health aide staff are beyond compare with their commitment to your health and happiness. My grandmother (stroke patient) just passed here recently in the care of these STUPID PRICKS. The dietician who gave a 5 star review needs a BIG reality check: - FOOD: everything you've read in the other reviews about trays just being dumped there are absolutely true. My grandmother suffered a stroke and came to this hell hole, so obviously she can't feed herself. We visited VERY OFTEN and around mealtimes, we would just see her tray sit there at the FOOT of her bed. How was my 80 something year old grandmother who weighed 70 pounds (tiny woman) supposed to even get to the food? This tray would just sit there for the longest time -- 45 minutes to an hour. SHE COULDN'T EVEN TALK. Meanwhile these so-called "hardworking compassionate people" as described by the dietician are just standing around and talking to other patients or talking to each other. VERY common occurrence. "Understaffed" means "not doing much" in this case. Another example: plenty of times I saw a male employee just sitting in his chair looking at his phone while he's supposed to be "watching over" another patient in the same room! - CARE: Poor care. i.e., my uncle went to see my grandmother one afternoon and noticed a puddle by the side of her bed. He pulls back her blankets and finds that she had peed and it soaked through her sheets and leaked onto the floor. The excuse we heard was "oh, she peed a LOT, I just checked her 2 hours ago." DOES THIS MAKE SENSE?? She peed SO MUCH in 2 hours that it leaked onto the floor and created a giant puddle?? Sounds like "I haven't checked up on your grandmother in more than 4 hours and I don't have the balls to own up to my mistake so I'll just lie to you." Can you complain? Of course you can, but will they listen? No, they'll just feed you with BS excuses and empty promises. In the end, they only want money. I'll spare you the other horror stories. If you want your loved ones to starve and sit in their own pee, by all means send them here. Otherwise, I would suggest sending them somewhere else. read more

My mom just had hip surgery at Kaiser in San Francisco two days ago and was transferred to St. Francis for rehabilitation. I am shocked that Kaiser sends its members to a facility that reeks of urine, is severely understaffed, places rehab patients in rooms with longterm nursing home patients, and has emotionally distressed individuals roaming the hallways screaming at the top of their lungs (among my many complaints). Although the predominantly Filipino staff members seem to be nice people, the bottom line is that patients aren't given the attention they need and the space isn't being cleaned properly. All the niceness in the world won't make up for that. I'm doing all I can to get my mom transferred to a more suitable facility but in the meantime I want to get the word out on Yelp to warn others. DO NOT send a loved one here and if you are reading this and Kaiser Permanente still uses St. Francis as a rehab center, DO NOT allow them to send you or anyone you care about here for rehab. The conditions are abominable and the owners and operators of such a deplorable business should be deeply ashamed of themselves. I visited St. Francis for less than an hour and when I left my nostrils and eyes burned (from the lack of fresh air and noxious smell of urine) and I had a sore throat. I don't know how anyone can be expected to recuperate in such a filthy place. I am not sure this is a fair star rating but am up grading it by one star because my previous rating was from several years ago and the facility was under different management at that time. I know nothing about how things are there now. However, I received a personal message from the current management who at least seems to care what families of patients think. If you have a loved one there I recommend you establish a one-on-one relationship with the administration. They can ensure the staff gives your loved one the best care. I renamed this place "Ramshackle Estates." I was there 1 week December 2015, following 3 weeks in Seton for Sepsis. I had to constantly remind the staff when it was time for my meds. The wait for a pitcher of water was an hour. It was difficult to explain medical issues to the nursing staff, who predominantly spoke English as a second language. I was grossly mismatched with a roommate. This is not a rehab facility, it is a nursing/hospice place. I am a vibrant, bordering on immature, 50 year old. My roommate (no disrespect intended) was in her 90's and near deaf. Her TV was always on SO loud. When the nurse wouldn't respond to her call button (which was frequently), she would consistently badger me until I too wrung for the nurse. i am being generous with 2 stars. read more

My dad is sadly a guest there currently. At least it's temporary. He was sent here by Kaiser after having a surgery. This place is dirty, no one seems to care, and I'm not sure how anyone can get some rest here. There have been numerous times that they've forgotten to check my dads sugar and give him his insulin before eating. They've "ran out" of pain medicine many times and once being an entire weekend. Your lucky if you see your doctor once a week. Good luck trying to find a nurse! If you even try to complain or bring something up they just shrug you off. My dad woke up to crap on the floor from his room mate and it took hours for them to clean it up. He's asked that they move him multiple times and yet nothing happens. I am so happy this is a temporary situation and couldn't imagine sending my loved ones here. It's sad for the older guests who are unable to advocate for themselves. read more

My partner has been here for a few months now. I can't emphasize enough how kind, compassionate, and caring their staff members are. Federico and Nick, both CNAs, have taken such amazing care of him. They are gentle, attentive, pleasant, really caring guys, you can see it in their demeanor from the moment you meet them. His nurses, Ara and Gabby always take action immediately when I bring up a concern too. I feel he is being well cared for, and I feel he is safe there. He feels safe with them too. It has been life changing for us to have him stay at Saint Francis, especially after having been at so many places before, were he did not feel cared for, and where I felt our concerns had not been addressed. I did not add the 5th star because there is a socialization component missing for the patients. As nice as the staff is, there is little time to take a patient out on the wheelchair, introduce them to other patients so they can talk to one another, or keep their minds engaged. My partner can not move, but his mind is sharp, and he would benefit from having activities like some other places offer: book readings, music, ice cream days, things like that. Don't get me wrong, the staff taking care of my partner is 5 star, so my review with 4 stars is just there to point out that a little something is missing. Do not let that discourage you from bringing a loved one in, because they will care for them. The place is kept clean, and the food is reasonably good for a place like this. read more

Walking in, this facility will not impress you... BUT: OMG What wonderful and amazing staff. My 82-year-old mom with a broken leg was well cared for and looked after by sweet warm staff. They not only took care of her rehab needs but because she is very social and talkative and everyone took the time to chat with her. An example After coming out of the hospital where there was always something non-veggie on her tray after telling them that she was a veggie she arrived after dinner to rehab and they said 'we have a sandwich on the way for you--its an egg salad sandwich because you are a vegetarian.' read more

I am appalled at how quickly this place became the worst case scenario for a loved one. 2 days after entering, a loved one was already abused and injured by a rough staff. She was shown no dignity and labeled incapable of responding for herself. Instead of keeping her best interest in mind St Francis staff managed to turn the situation into harassment on my part and threatened to call the police on me. I left in tears and in fear that this is where my loved one will pass. Mark my words, I will continue to write reviews and add photos until dignity and respect are restored. The system is crooked and please, please if your love one is sent here and there are no other options, stay present. Check for bedsore, tell your loved one not to fear these staff and to report EVERYTHING to you. St Francis is a death chamber. I would like to say how impressed I am about how St Francis has handled the COVID-19 pandemic. They listened to the CDC right from the beginning. The Whitehouse could take a page out of St Francis book. There have been very few cases at St Francis. Nothing like what you hear on the news about all the deaths in nursing facilities. The update the families every week. And I must say it has been very positive. They test regularly. They are on the ball. There some very nice CNA'S . LUCY,JOSEPHINE, RUDY etc. Shy in social services always makes sure your transportation is taken care of for your appointments. I also like the physical therapist. In this time of COVID 19 I would rather my brother be here than anywhere else because Janell and the administration are doing an excellent job. My aunt stayed here at the St. Francis Pavilion for rehab therapy. The management and staff are so friendly and helpful. The updates and communication during these challenging times of COVID19 have been very calming and reassuring. We appreciate the team at this nursing home. Keep up the great work! read more

My Father is being taken care of by the great staff of St.Francis Pavillion. St.Francis is very clean and friendly. My Fathers nurse (Nick) is a stand up guy, he goes above and beyond to make sure my Father is ok. Nick has been helping my father for almost 8-9 years and we couldn't ask for anyone better read more

My mom has been here for more than 3 years. I like to give review for the first time. Staffs here are friendly and listen to your problem and try to resolve as soon as they can. Although some employees stay short term, a few of them have been working here for more than 10 years. Comparing to other places, this facility is quite clean. They mob the floor every day and clear the garbage can daily. I like to say thanks to a few CNA who take good care of my mom very well. In the morning shift Shirlyn, Nissa, Marrisa; evening shift Florence, Thelma are doing very good job and patient to my mom who has dementia. They know how to entertain her so she will co-operate with them. I deduct one star for dinner meal which is kind of boring same food for many days. I wish I will see more fresh fruit and veggie. read more
