I was sent from the hospital for a max. stay Of 42 days for therapy I was held against my will for 105 Days They registered me into long care nursing, pushed me in a room where the call buttonsrarely worked, the "doctor" treated me without looking at me. The place is filthy, the aides run like dogs from patient to patient often taking 45 min to answer a call), my room was infested with ants, water seeped up from the floor at base toilet, the food inedible (certainly NOT heart smart), the therapists uninterested,and the administrators working hard to charge Medicare & Medicaid every dime they can . The Nurses, the CNA's, Occupational Therapists and Physical Therapists was just an A+++ experiences many memorable Staff, I do wish I could name all of them! I pray I can find the list to let the DON, Dietary, Recreation, and my sweet Housekeeper's boss know how they all worked, played and put up with my silliness for 14 days! There is a new administrator. The old one would listen to the concerns of staff and residents, not so much the new one, so the old one is preferred. The building has been remodeled for the umpteenth time, but it still does help since housekeeping is horrible. My floor is always sticky from them moping it. My room has not been dusted in 3 years. I have throwup under my bed that has been there for months. Now to the issue of an amazing chef. To be known as a chef, you must use fresh ingredients. So no, this place does not have an amazing chef in any regards, just a subpar cook with anger management issues. The facility uses instant potatoes, processed cheese, margarine, not real butter, the lowest quality of bread, and very little fresh fruit or vegetables to cook with and feed us.The food is awful, and could be improved just by using the above ingredients as fresh and healthy options. The so called chef has yelled at me and several cnas where I could over hear what he was yelling just because I asked for something extra or for fruit. We are to never require extra work by him by really anybody in the facility. Residents are not to be treated as a human. This is the message the administrator, DON and unfornunately a lot of nurses portray and seem to live by. The head cooks loves to berate and control. The patient to staff ratio is high at this facility. During the day they lower the ratio by including mgmt, PT, OT, housekeeping, along with rns and cnas to bring ratios down. I have experience many night shifts when one cna is assigned 40 to 50 residents. And the nurses are not much better. How is this safe? The facility doctor was just fired so I have hope the medical care will improve. All it takes to burn out and get rid of good employees is to keep bad employees. This has been the motto here for years. They do not get rid of the bad employees. State is called and the place gets fined, but nothing improves because they keep the bad around. The staffing issue is not because there is a nurse shortage, it's a retention problem. Again...the have no problem hiring nurses or CNAs. They have a problem retaining because of the toxic work environment. This toxic work environment completely overflows into resident care. Before placing family here, please actually talk to residents or CNAs, not mgmt or nurses or the salesperson. Also visit late between 7 and 10 pm when admin is gone but the staffing is low. See how stressed the staff are and how it affects the type of care your family member receives. Also visit on a Saturday. Saturday staffing is always terrible as well. Look in the supply closet. The supplies have been an issues for months. The new chucks are thin and do not protect the bed. I have had many cnas who because they are always in a hurry only change you and your chuck and then leave you smelling urine on your sheets. The CNAS are what keeps me here, yet they are so mistreated. Most CNAs are kind and try to personable with us even though they get instructed by nurses and administration to not be personable. Being personable means they are spending too much time with us. It's surprising how many of us residents are suffering from depression from our physical illnesses and then the facilities tries to limited our positive interactions with cnas. Our depression and acting out behaviors get worse and they can't understand why. I have finally adjusted my expectations to be treated like I'm a "less than" human and I have finally started improving my depression. I literally had to tell myself, I am their prisoner and nothing is going to change. I deserve this treatment for getting sick. As soon as I accepted this, no longer was their gaslighting and horrible care an issue for me. I've resigned myself to stay in my own world of music and a child like tent over my bed. I have found staff who felt it was okay for them to be verbally and emotionally abusive to me down minimize their interactions because of the tent. It is a protective barrier for me. The nice friendly cnas and nurses think it's fun to look under the tent and help me. They get a smile and a laugh and they need those as much as I do. PS, your family member is going to need creative coping skills to stay upbeat in this environment made by corporate greed and lousy communications skills by nurses and administrators. This facility is TOXIC, be warned. You will have to fight like hell for basic needs. CNA's are overworked and unfortunately even the CNA's with good intentions are burnt out and in the constant habit of throwing everybody else under the bus. The housekeeping head is gone which has led to my mother going without toilet paper and paper towels at times, so I bring it up to lead CNA's and they claim they will take care of it but nothing happens. I bring up the issue to nurses and they get confused and provide me random rolls in a closet. Everybody acts like they don't know how to get answers or help to any basic question. Right now there is an issue with my mothers food and the food in the 500 hall always arriving cold especially at dinner when the CNA's are anxious for the 6pm shift change. So the cart sits in the hall for at times over an hour doing nothing because the CNA's will disperse the 400 hall trays and take their sweet time doing it so they can push the 500 hall cart to the next shift. They will blame the nurses for not having blood sugars done and claim they are doing it as fast as they can but they don't need 3 people to stop and wait... split up as CNAs and get to the 500 cart, grab the beverage cart and put it at the T intersection of the 400 and 500 halls and simply get 500 hall started. The residents that are on soft diets and following various IDDSI guidelines are getting completely ignored because see when the food gets cold, it gets HARD,... then a resident on a softer diet can't eat it. Then the other residents in 500 hall when the food gets delivered cold many wont eat it and will put in special requests which then create more stress on an already overly extended kitchen. The 500 cart issue needs to be creatively addressed. I get it when you are a CNA working a 12 hour shift, on the 11th hour YOU WANT TO BE DONE!!... Get help in that case, get creative and request your admin team do something to help alleviate your problems. Simply letting food carts sit for 30+ minutes to over an hour is ridiculous and uncalled for just so a couple of the CNAs that are notorious for this can push it to the next shift!! I have told this facility I am going to the ombudsmen in moments of extreme frustration(happens a lot at this facility) and will get confronted by staff and maintenance people with threats and challenges. I got a better idea, how about you staff up better and eliminate the leaders and managers that are in a constant habit of blaming other departments. The CNA's blame the nurses, The nurses wipe their hands of all things CNA and tell you go to the CNA manager, the managers take nothing seriously and basically roll their eyes at you. You got some managers that are lingering around with an attitude of wanting to be their employees weekend party buddy rather than their manager, so they won't discipline their team. Catering to the bad CNA's only creates more problems for the good ones... you are in a lose lose situation as the bad ones do eventually leave but then they ruin it for the good ones and they leave too. For what insurance and medicaid compensates this facility, it is outrageous that basic needs of dignity and care are completely ignored and issues regarding such needs are laughed at. If you want a nice peaceful place to go on the weekend, GO HERE. The weekends are the worst, all the regulars are partying, the admin team is gone, nurses are no shows etc,... the weekends need to get 1 admin on site at least just to keep the laziness and chaos to a minimum. read more

I have been verbally and emotionally abused and medically gaslit by nurses, the current DON and the doctors. My voice or my family's voice means nothing here. The physical and occupational therapists are amazing and actually listen and try to help but when they are the only ones in the building doing so it gets nowhere. The food is absolutely disgusting except when one cook is in the kitchen. Know that when you ask for something else you will be told no it isn't available on a regular. I ate better food when I worked in the prison system than here and the prisons are on a tighter budget and the pay is worse. Go figure that one out. The snack cart is not replenished several times a week and the options never ever change. When the head cook works you really need to know that you will get poor food except his barbecue and the cnas are scared of him because of his constant yelling and tryrads so you will not get your juice, salad dressing or whatever else you asked for, and it will be your fault. Housekeeping is a joke. Since returning from the flood my room has not been dusted, my bed has not been raised to clean under it, and my dresser and beds have not been wiped down. Currently my bed, table and dresser are so gross from throwing up on them and recently I finally got one of the older housekeepers, who is never in my area but is good, to mop under my bed. Her mop kept sticking to the floor because of how gross it was from the throwing up. There is no point in complaining to anyone from administration down because everyone just blames someone else and they do nothing to help you or worse they then make up some excuse that it is your fault. You need to know that medical doctors are the same doctors at every facility in Southern Utah so your care will be poor no matter where you go. The cnas are what make this place bearable but unfortunately they are working in a toxic environment where they are blamed for everything and expected to work short staffed and extra hours. Their burnout is fast and eventually they begin treating you as bad as the nurses and administration until they quit. Just know you or your family will not have a voice or be a human again once you enter this system. You will be a diagnosis whether it is accurate or not. Remember you are dumb, have no rights, and can not prove anything because once again you are dumb and not right. If you just shut up, lose your identity, and agree you are good, you will be fine. read more

I had my Mother at St. George Care and Rehab for 3 1/2 years until here passing. Our experience as very pleasant. The care ctr was very clean and did not have the typical nursing home smell. The staff there was the best they took great care of my mom, in fact she became part of their family. I never had to worry about how my mother was being cared for because I knew she was in loving / caring hands. She enjoyed the food and the activities that were provided. Her religious needs were also met while she was there. The rehablitation staff were totally awesome, she absolutly loved them and they were able to help her remain mobil even at 93. No one ever wnats to place a loved on in a care center and it was hard for us to have to do that, but If it becomes a necessity then this is the place to use. I would highly recommend St. George Care and Rehab to anyone needs such a service. read more

I've lived here a year so far! The food is always cooked fresh! The therapy team works with each resident and customizes as needed! Every staff member wears a radio ear piece from CNA to kitchen staff to administration staff so each need can be addressed quickly! And activity director has scheduled something fun each week! (Activities are daily) and religious services can be requested if your preferences are currently not being provided! read more

DO NOT SEND ANYONE HERE! This facility is horrendous. The rooms are so dirty that walking barefoot leaves your feet black. The machinery has food stains and blood stains from months ago still on them. The drawers are so filthy that storing anything in them is a health hazard. I also believe the staff is stealing patients medications such as painkillers and anti-anxiety pills. A family member of mine was bleeding for 4 hours before any nurse noticed. When a nurse noticed, they were more worried about what my family member wanted for food. If you care even a shred about whoever you're thinking of sending here, I BEG YOU PLEASE DON'T! read more
