The admin team was always organized, communicative, and compassionate they made the entire admission and care coordination process smooth and stress-free. The nurses and CNAs treated my aunt with such kindness and dignity, always attentive to her needs and quick to offer a smile or comforting words. The rehab and therapy staff were equally amazing, helping her regain strength and independence while encouraging her every step of the way. You can truly tell that everyone here cares deeply about their patients' recovery and well-being. The facility was always clean, the atmosphere positive, and the staff made sure family members were kept informed and involved. This place is terrible and I speak from experience. My mother was here for about twenty days late last year and it was a nightmare. I am confident that my mom would not be at home right now, getting better every day, if she came back to Cerritos Vista instead of going to a different, far better, facility. Many of the nurses were rude and acted like we were irritating them when we asked them to do something basic for my mom. The kitchen was shut down for days because of a cockroach infestation that went unchecked for weeks (don't believe me? Read the December 2025 inspection report). The facility wasn't very clean. The food was not good. You never knew when the physical trainers were going to show up and they didn't stay long when they did come. You could wait for hours for your loved one to be changed if they can't go to the bathroom themselves. The gym rarely had anyone in it and that's not an accident. If your loved one needs significant physical therapy to recuperate, do not send them here. They will improve slowly at best and will likely deteriorate due to lack of movement. The doctor that the facility assigned to my mother (who recommended this awful place for reasons I can only speculate about) was unreachable and never visited. Some people who worked there were well-meaning, but they had terrible managers, and were not properly trained. They were also understaffed. I could write a book about everything wrong with this place but the fact that people don't want to work here should tell you everything. I'll end this review with one final insult my family endured because of this place. My mom, who is now safely at home, was hospitalized again after coming to Cerritos Vista. The managers at Vista told us that my mom's room would be held for seven days. About five days later, having decided Cerritos Vista was not fit for human habitation, I went back to get her things from the room. All of her things had been bagged and put into a locked office I could not get access to. There was a new resident in her room, contrary to what they promised. On my way out, some employee overheard what I said to a nurse about it, and found a way to get inside the room with her belongings and gave me some of them. We didn't get a lot of her things back (we lost a chair, some seat cushions, two pairs of shoes, etc.) but I didn't want to return to get them. That's how much I dislike this place. I wouldn't put someone I hated here. Again, if you don't believe me, don't take my word for it. Just read all of the information and inspection reports about this place on ProPublica and Medicare.gov.

I did nothing to sway her in believing how long it had been since I was last showered. but did not need to for her to give me a shower she just did. This would place her in rush mode for the rest of the night.but shower me anyway helping me even after it was not my regular scheduled shower day. It passed and i wad not able to get a shower She was asked by the supervisor and working with me during the time that I needed you could smell that I needed a shower and it was not my shower day. she was very professional and helpful in every way she did the whole job by herself with no assistance except a little that I could have given her.. I would like to also think her supervisor who at the time of hire must have known that she was a hard worker and the excellent worker that she was in her interview That too should be congratulated.. I have spent all of my working life and the Personnel Department of most Fortune 500 companies last one Revlon cosmetics and I too know a good and proud detail oriented worker when I see one. My…read morepoor mother was there for a few weeks and had terrible bed sores when she left there for another facility. She couldn't feed herself and needed someone to feed her but they would never take the time to really try. I was so disappointed I thought they were a five on Yelp. When you get into the facility there's a sign that says their are three. I think that's being generous. For 3 weeks the TV's didn't work. What else do old people have but you watch television? A couple days before she left there they fix the TVs. Cable box was so old and antiquated that it couldn't give him a clear picture or even sometimes anything at all. I don't usually talk about businesses that I'm disappointed but this was special. Be very careful where you send your loved ones!

Christine Q. was my Occupational therapist for my right hand/wrist. She was familiar with my treatment plan, & actually cared about my recovery. She carefully explains each exercise, what she's doing & how it can help. I loved how she managed to give me her full attention as she was working with other patients. She has worked very hard each time I was here for my therapy visit. And again, I cannot thank her enough. My right hand/wrist has made a lot of progress & I am ready to return back to work. I am very grateful that my primary doctor referred me to this wonderful place. And above all, every staff member was compassionate with treating their patients with care. If I ever had a concern or issue, the front office staff (Denise Ortega) and Debra (social worker) were very responsive and helpful; I have so much appreciation for them! The nurses tended to him on a regular basis by changing his sheets and cleaning his room, giving him sponge baths, changing his diapers, and changing his clothes that I left hanging in his closet. The nurses who had a positive impact on my father were Molina, Monse, Cristina, and a male nurse from Jordan who trimmed my dad's beard. When my dad was discharged, he missed them all so much! The nursing staff gave him medicine on time and Dr. Sharma did a good job of monitoring his symptoms and labs. My dad loves food and not once did he complain about their food, so that was telling for me! The cafeteria staff were always friendly. The physical and occupational therapists were friendly, supportive, and positive with my father; thank you, Claire, Araceli, and Carlos! They were very helpful to him gaining some of his strength back. They had a kitchen, bathroom, and a car in their PT/OT space that allowed my father to practice doing certain things when he returned home.

    My father was recently transferred to Southland Care Center following major surgery, and our family…read morehas been deeply disappointed and concerned by the overall care environment and safety issues we experienced during his stay. My father suffers from dementia/confusion following surgery and is considered a high fall risk. Shortly after arrival, we became extremely concerned about the level of supervision, responsiveness, and overall safety precautions in place for vulnerable patients. We had to request additional safety measures ourselves, including bed rails and a floor mattress, because we did not feel comfortable leaving him unattended overnight. I ultimately stayed overnight at the facility myself to help monitor his condition and ensure his safety. What made the situation even more concerning was learning about the facility's recent survey deficiencies while simultaneously witnessing several troubling issues firsthand. The overnight staffing situation appeared overwhelmed, and the environment did not feel appropriate for someone recovering from surgery with cognitive impairment and complex medical needs. Our family has since begun requesting a transfer to another facility because we no longer feel confident this is a safe or appropriate environment for his recovery. I understand healthcare workers are often under difficult conditions, and I appreciate the individual staff members who tried to help, but overall this experience has been heartbreaking and extremely stressful for our family during an already vulnerable time.

He needed the medical care I couldn't provide for him right away. I needed to weekend to prep my home and make calls for nursing care. In the meantime in the hospital the social worker connected me with Administration rep SAM who helped ease my worries and assured me my dad would be well cared for. The weekend my dad was there the nurses helped my dad with medication ,food and his bathing in bed. I was able to drop off clothing necessary and the nurses would take them back for me while visiting hours were over. The issues I ran into were: scheduling appointments were not communicated well amongst the staff. I came in a Saturday and the young receptionist as well as another young lady said I couldn't have had an appointment as I was not on the list. However I was just there Friday and they said I could come in the next day at a certain time. I won't spend much time on this as Sam helped fix this issue and I was very frustrated with these ladies, but given they only go by what their told I get it. Aside from that my dads needs were met.

    I stayed at the Country Villa Bay Vista Healthcare facility from December 14, 2022 to January 14,…read more2023 for physical and occupational therapy following a fall down a flight of steps at my apartment complex that initially rendered me unable to stand and walk due to severe injuries to my knees and quadriceps (thankfully nothing was broken). When I arrived at BV around 10:30pm that first night (my transport from the hospital was very late in picking me up), the staff was very welcoming and helpful, which helped to ease some of my overall uncertainty being in a situation where just a few days prior I was mobile and living my life, to now being immobile and unsure of what was going to happen next (I'm 52 years young). The two CNAs who tended to me that night were soooo kind, caring, helpful, generous, and understanding, I started feeling more comfortable in my new environment and situation. And such was the care that I received overall during my stay at Bay Vista--the nursing care including the medication nurses, the physical and occupational therapists, the patient advocate and social worker, the activities director, the kitchen director, the administrator--everyone offered such wonderful and friendly care and service! And the housekeeping staff were great at keeping the facility and my room clean. During my time there I went from being unable to stand and walk to being able to do both (with walker and wheelchair support) thanks to the amazing occupational and physical therapists! They were so supportive and helpful (and such sweet individuals) and were the perfect team to build the foundation for my recovery journey (and I continue getting stronger at home). When I was having challenges with my insurance company, the patient advocate kept me updated and went to battle on my behalf so I could focus on my healing. She even took the time to comfort me during a particularly stressful experience I had with them. And huge props to the CNAs--the sheroes and heroes who do the actual day-to-day, moment-to-moment work of intimately caring for the patients--who were excellent. Admittedly some were better than others (like with any job), but they were all good, and many went that extra mile to ensure your comfort. I speak not only from the care I received, but the care I observed with my roommates as each of us had different levels of needed care. I could share many instances where the staff exhibited such care, professionalism, support, and generosity with us (including with some of the more demanding [and impatient] patients :-). And although I was happy to finally go home after my stay, leaving was a tad bittersweet *only* because I was going to miss those staff with whom I had developed relationships, especially my therapists. But I look forward to going back--just to visit and say hi to everyone after I have fully recovered! I would definitely recommend Bay Vista to those needing nursing and PT/OT care.

    I attended Gratitude Lodge for 30 days at the Long Beach house…read more Elise was a great house manager and made me feel very welcome. The house was also decorated nice for the holidays. The rest of the staff were great. Since most were in recovery, they understood what we were going through and were always available to help. Staff is available 24/7. The houses I stayed at were nice and clean and full of food (you could even request food you liked and they'd get it from the store for you). They have a tele dr and psychiatrist on call. The first week you are on detox so can't leave the house for AA/NA meetings or outings but after that you can go to the gym etc. They took us to lunch after a meeting and shopping around the holidays, which was nice. We'd go on walks, they'd have people in to teach yoga, hold sound baths, do art therapy, music therapy with a cat and educational classes about drugs effects. There was a jacuzzi tub in the one room which was nice :) I'd recommend a stay at Gratitude Lodge. But it's important to continue with your sobriety. I had to end up doing 6 months in residential and intensive outpatient due to legal issues and I do see the importance of longer stays (I know not feasible for most who work). I'm currently in recovery bridge housing (sober housing) and enjoying my outpatient program but I really enjoyed my time at Gratitude Lodge. If I would've still had insurance, I would've picked this over the Medi Cal facilities.
