I would HIGHLY recommend Courtyard Care Center. I spent 1 month at Courtyard and was very pleased with the care they provided. Everyone I came in contact with was friendly, professional, and above all, caring. Shout out to CNA's, Olga, Jose, Carlos and Crystal. The Physical and Occupational Therapists were upbeat and full of motivation and praise. Shout out to Shelly, Celena, Mandy and Alex. Also Kim and Kat (PT Manager and Asst. Manager). Courtyard is very clean! All rooms were swept daily and deep cleaned regularly. In the rehab room, all equipment is wiped down with disinfected after each use. Food was what you would expect... (No Michelin Stars) but, the staff made sure you were satisfied and good about offering alternatives. There were a few mix-ups with medications when I first arrived, but those were straightened out immediately. All in all, very happy with our choice. read more

I cannot recommend the Courtyard Care Center highly enough. My husband had to have unexpected surgery at Long Beach MemorialCare when we were on a trip to California and I was referred to the facility by the hospital. My husband stayed for two months at the Courtyard facility. What makes this place unique is the extraordinary employees. They are wonderful - knowledgeable, helpful, professional, responsive and always with a positive attitude. I especially want to mention nurses Denise Pukini, Lena, Gabi and Erika, who were were so kind and thorough, and did everything they could to make my husband comfortable. They monitored him closely and always contacted me if they had concerns. The rehabilitation team is outstanding - Director Kim oversaw everything with professionalism, kindness and humor, and therapists Alex and Shelly were caring, engaging and great to work with. The CNAs (Carlos and Jose) took wonderful care of him. The office and support staff are responsive and always quick to address any questions; the social worker, Maria Perez, went above and beyond in checking to ensure we made it safely back to DC. A special shout out to the receptionists (especially Cecilia) for always making me feel welcome when I walked in. The facility very clean, with an outside courtyard that is pleasant. The building was more dated than other skilled nursing facilities I've visited (I'd especially like to see the staff contact button system updated), but the management team let me know that renovations are in the works. Overall, this is a wonderful facility and I highly recommend it for anyone who needs skilled nursing In the LA area. read more

Unfortunately I had a health incident that required me to spend a significant amount of time under skilled care. We were directed to courtyard care and I am so glad we were. There are many people there I would like to thank including the entire rehab department. I came a long way under their care and the CNAs were wonderful. A big thank you is owed there as well. There is a lovely neighborhood setting, a really beautiful courtyard to hang out in, and activities to keep things lively and allow you to meet other people. I needed a great deal of rehab and they made sure that happened for me and that I was safe but also progressing. To me it is just the right size of an establishment, large enough to have access to equipment and things but small enough to be personal. I just can't say enough about the care I got and the way the CNAs and nurses and therapists made sure I had what I needed. From top to bottom what a great group of caring people! read more

No one wants to need the services of a skilled nursing facility. But if you find yourself in a situation where you do, I highly recommend Courtyard Care. My sister who had spinal meningitis needed skilled nursing and physical therapy to aid in her recovery. The care she has received has been outstanding. What differentiates one facility from another is the people. Without naming individuals out of fear of accidentally missing someone, I can say that these are some of the finest people I have ever met. They are true professionals who are skilled at their jobs, have a fantastic attitude, and will provide the best possible outcome under difficult circumstances. The staff is extremely encouraging and takes a personal interest in their patients. The facility is very clean and well maintained. All of the office staff and support personnel do an excellent job and have been very helpful. Because I've had an especially close relationship with the physical therapy department, I can attest to the exceptional job they do. I know my sister has advanced as far as she has, due to the care she has received at Courtyard Care. We thank them very much. read more

Since I have been at Courtyard Care Center, a couple of nurses come to mind, for going beyond the call for patient care. One is a CNA of the name Alejandra treat those she comes in contact with, with love, concern for the patients, and respectful to those she comes in contact with. She is a very hard worker, and deserves more recognition than she receives, working evening and and night shift. She is a very hard worker, caring, and a joy to be around. This was a horrible experience not only for the patient, but for family. They would not return calls to us on patients status nor were we able to visit due to covid. The phones were broken in the patients room so we could not call directly. The patient was there for a month and went downhill due to lack of care. The only time this facility called is to get their money. I would NEVER reccommend. read more

We're so grateful to the management & staff at Courtyard Care Center for taking such wonderful care of our mother. She's been living there as a long term care resident for over 6 years and she's doing great. In fact she just celebrated her 100th birthday and Courtyard put on an amazing celebration for her! All our family attended and it was a lovely party with fancy table settings, balloons & they even arranged for a proclamation from the White House as well as the mayor of Signal Hill being a special guest! There are always great activities, outings, special lunches & dinners for all occasions and our mom's favorite Bingo! Courtyard is rated 5 stars by medical/medicare. However, do not let that rating assure you that you should rest your love ones in complete care of a nursing facility. This is in general for all medical entities. It is important to follow up on your love ones, visits, make observations, and ask questions. So far, my dad have been there a month and I am grateful for all of courtyard staffs for their hard work in helping my dad like the nurse, admin, dietitian, physical therapy, lvn, cna, volunteers and etc. There are 2 over achiever nurses Michelle and Reggie. They are very meticulous, observant, diligent, persistent in reaching the doctors, sympathetic, caring, have excellent reasoning skills, and have very good and genuine aura. Which is why my dad feels like he's in good hands when they are around. Well...i'm not going to lie to you, some things do go wrong like food. My dad is a renal patient and is on a renal diet however I see milk, chocolate ice cream, choco cookies, wheat bread that are high is phosphorous and ensure when it was supposed to be nepro. I believe he may not be getting enough rotations because I saw a sore on his butt. My dad has been complaining about feeling sick recently and concluded that it may be the meds he's getting. Turned out 1st shift nurse gave norco @2pm and then 2nd shift nurse gave norco at 5pm. @ this rate my dad's liver is going downhill. Acetaminophen damage the liver if the nurses did not know this.... So...that's my report. Just as the staffs have their assign duties, we also have our duties for our loves one. Make the communication and follow up with the staffs. But, I have to say...by far, this is the first facility that I feel comfortable for my dad to stay at. And it's a plus since they have a few Cambodian staff. It's hard that there's a language barrier, so this help alleviate some stress for my dad knowing that there are someone around to translate and help him understand what is going on around him. read more

Our family trusted this care facility with my sister's care after her first knee operation and other than the poor quality of the food we were not disappointed. That was three years ago. Two months ago, my sister checked back in after her second knee operation, and she had a very poor experience. The rehabilitation staff and the custodial staff were extremely nice and competent, but unfortunately a CNA (Certified Nursing Assistants) was not very kind. My sister was on medication that gave her a strong urge to urinate frequently. Since she had just received a new knee, she was under directions not to go to the restroom without assistance from the staff. The first night started off fine, but unfortunately it didn't end fine. My sister who is bilingual spoke to a housekeeper in Spanish and informed her she needed assistance to use the facilities. The lady was very obliging and went out into the hallway and spoke to a CNA about my sister's request. The CNA did not know that my sister spoke Spanish and said "No one is going to help her!" We complained to the administrative staff twice and they were very apologetic but unfortunately this one CNA ruined the whole experience wth her uncaring and disrespectful attitude. No one in our family will ever use this facility again, and the food still is very poor and bad tasting. read more

The best skilled nursing facility "hands down"! I had not heard of Courtyard before my dad's stay there but I did a little research and am so glad that I selected it for my father. The facility is clean, the nurses are very attentive and go over and beyond for their patients and their loved ones. They truly treat you like family at Courtyard. There are great activities going on all day to keep the patients engaged, the PT is awesome; I just can't say enough about how great this place is. They are also one of the rare facilities that have single rooms....an added bonus for people who prefer privacy like my dad. The administrator, Julie Javier, goes out of her way to accommodate you. Rona Moore, the director of nursing, will always greet you with a smile, pleasant conversation and is sincerely committed to her staff providing the best care possible, which they do. You can't go wrong if you choose Courtyard and believe me, if you do, you will be glad you did. I was very pleased, that is why I chose it for my dad twice! Courtyard Care Center is worth every star if not more! If I could give them 10 stars I would. In February, my mom was admitted to the care center and I was newly thrown into an unknown world. I wanted the best for my mom and I researched and visited various care centers before picking Courtyard. I always read reviews and do my own research before I make any decision. Courtyard was and still is the BEST choice! They shine and their staff members are excellent and caring. Particularly, I would like to highlight Maria. This woman deserves a medal! She is the social worker at the care center and went above and beyond for my mom and I. She informed us of the many programs/options that were available for my mom and helped to ensure she had a pleasant stay during her time at Courtyard. I think we spoke at least 3-4 times daily just coordinating all of the paperwork and getting my mom signed up for the correct programs that could benefit her in the future. She is an ACE and deserves so much more than a thank you. She knows her stuff and without her I don't think I would have been able to get assisted living for my mom and have her in such a nice place that is closer to me and have her set for the future. My mom stayed at Courtyard Care Center for four months. I was always greeted with a smile and received updates on my mom's progress and next steps. The nurses and staff were kind, attentive and catered to my every need and request. In the rehab department, Kim, Michelle and Shelly were all top notch! They helped my mom progress and break barriers so she could improve. They were all so friendly and took their job seriously to gain the best results. During the four months my mom was there, I visited 2-3 times weekly so I know the care center well. Once COVID hit, they handled the situation with the utmost caution and kept me updated on everything via personal phone calls, text messages and emails. I called my mom everyday and they created unique and innovative ways for us to stay connected during this time. I am an only child and having a parent being thrust into this unlikely situation can be scary and difficult. I am so happy I made the right choice with Courtyard Care Center. They have been nothing but remarkable from beginning to end! I wouldn't hesitate to recommend them if you are looking for a loving, caring place to send your loved one. My elderly brother spent 5 weeks there for rehab, and we found the staff, from top to bottom, to be most genuine, caring, and attentive. The patients there have been very kind to me every time. I am treated no different than another patient and I love their customer service. The nursing staff is amazing but there's this one nurse was really rude and disrespectful, Sometime I can hear her arguing with the CNA. Its very unprofessional. My aunt and uncle both ended up at Courtyard Care on two separate occasions. Both times Julie made accommodations so that they could be together (as they have been for 60 years.) My uncle can be difficult, but the staff were patient and treated him compassionately. The physical and occupational therapists were great. The facility is very clean, and both my aunt and uncle enjoyed the food. The first time my aunt and uncle were discharged they returned home, and the second time, to a board and care facility. Julie provided invaluable assistance both times to make the transition as smooth as possible. read more

I was accompanying a friend who recently relocated to Long Beach area, looking for a Skilled Nursing Facility to place her mother who had a hip replacement for a short term care and rehab services. As we were entering the building,It smells good , a nurse station was in site and A nurse approached us and was immediately attended to our Inquiries and toured around the facility. The name itself brings a homey feeling , Courtyard Care Center, the patio, garden or whatever they call it beside the General dining area is very beautiful. Facility is clean, Private rooms ,nice ambiance. My friend fell in love, so did I, my friends Mom stayed for about 2 months until discharged. I would highly recommend this place. Nurses and Nursing Assistants are very attentive to the patients needs, they go beyond with everything. Kudos , all of you nurses in Courtyard Care, even on holidays ,instead of being with family, you're all there for our families too. The only thing I've observed, not in a bad way, the room mate of my friend's mom , has been holding her call light ,like it's the only thing she has. If it was up to me, I'd take it away because nurses comes knocking , and sometimes the lady doesn't even need anything but company , and the husband comes and demands attention as if the nurses isn't doing anything , from my point, as I always go with my friend , You're all the best and 5 stars. This person isn't just satisfied, wherever he goes , he will be in the same situation, maybe he should take his wife home if he doesn't fully trust the nurses . Our mom is at Courtyard and has been for about a month, following a serious infection. She will stay another two to three weeks. We are extremely impressed with the facility, both rehab and nursing care. They do, indeed, have a courtyard where you can sit with your family and friends, and it is beautiful. The neighborhood is a quiet, residential, "pride of ownership" area. The employees all seem to be fully committed to what they are doing and to want the best for their patients. We schedule "family meetings" every couple of weeks to determine where Mom is on the recovery scale and continue the with the plan, and everyone is very receptive to that, in fact encourages it. Their physical therapy department is state of the art, occupational therapy has been very helpful and bladder retraining was a success. Julie, the director, works long and hard hours and seems to be very much on top of things. Courtyard schedules an abundance of activities for the people who stay there, and family and friends are invited to attend some of them. They received five out of five stars for their Medicare rating, and cleanliness was one of the criteria. The place is spotless ( Mom reports her room is cleaned twice daily). The only reasons why I didn't give them 5 stars are: The food could be a little more "diabetic-friendly"; Like a lot of facilities, there is a bit too much starchy food. On the flip side, though, all the physical activity that Mom gets has made her blood sugars normal without any insulin at all. The other reason, as the previous reviewer noted, is that sometimes things get lost in the wash--you have to mark them carefully. We have only had a hairbrush and a pair of plaid sweats go missing, though. Finally, it would be nice if all residents had phones in their rooms. You either have to wait for them to get the cordless phone to your loved one or get a prepaid cell phone for them. Overall, I'd say do consider Courtyard for your loved one! read more

Courtyard is nestled in a nice neighborhood in Signal Hill. They don't have many beds, so the feel is very cozy and people know each other. The team is friendly and my experience with them have been super positive. I would trust a loved one to stay here for short term rehab. read more

The rehab staff is great. The nursing care not so hot. Mom was in for 6 weeks with a broken hip. Lost her phone, lost her clothes, released her prematurely. And, took xrays of the WRONG hip for her orthopedic doctor to review!!! Find your loved one a better place to recover. read more

Absolutely the best skilled nursing care facility around, without question. The staff has been very kind and caring for my brother during his entire stay. The physical therapists and occupational therapists were all incredible. If you visit the facility, you will see a great deal of activity going on with everyone doing their job; not sitting around ignoring the patients. I appreciate the personal attention that the administrator, Julie, provides to make sure everyone receives the care they need. My first impression of this place was negative because of how it looked outside. However, the inside is nice. So my negative feelings disappeared. My grandma was admitted to Courtyard from Community Hospital (1st time) for a broken hip from falling and had a mini surgery that rendered her on bedrest. The CNAS here do not turn the patients, I would say I was there for 5 hours and the CNA comes in 1 time to check to see if my grandma needs to be changed (which she didn't because I had already changed her). I am a registered nurse and repositioning should be done every 2 hours as well as rounds for safety. Also these rooms are poorly ventilated and hot, when I asked for a fan they were all in use, and was told that the maintenance was told to fix the temperature. When asked if they have air conditioning their response was we do and it is in the hallway. Well that is nice, but how does that help the patient's who are uncomfortable and the visitors as well. When I followed up for the temperature check by maintenance, anothrer person said they do not work on the weekends so the original person I told had lied to me. Anyway, my grandma stayed there for 5 days and was transferred to community of Long Beach again for low BP, which she had a GI bleed and was given 3 units of blood. After was sent back (2nd time). My grandma barely ate there (loss 10 lbs in a course of 2 weeks) and fell at their facility while trying to go to the restroom, NO BED ALARM, NO FLOOR MAT was in place when this happened. And even after the fall, still no floor mat. After a couple days, she went back again to Long Beach community again for low BP, given more blood. And found the bleed and fixed it, they tried to send them back to this place. This time I refused, I rather have my grandma at home where we can watch her and feed her. She now gained 5 lbs and is eating well and starting to stand on her own. I would stay away from this place if you could, bring your family to a place who actually cares and will make sure your family member is safe. I'm very thankful she didn't break her hip again, for her sake and for this place. read more

My Mom is about to complete her stay at this care center. I have to say although there is no place like home; our overall experience has been very good. My mom broke her hip and needed rehab and care until she was back on her "feet". She is just about to complete the program and will be released soon; she actually said if ever needed rehab like this again she would come back to Courtyard. She has formed friendships with other patients and the staff that helped her stay positive and focused on getting healthy. When our family is in places like this we are so apprehensive and the facility has to work hard to gain our confidence and create a positive experience. Courtyard through their very happy nursing staff, their concerned rehab, and very clean facility all lead by the diligent and kind Julie has done a good job. I would recommend this center to my friends and family if they needed care for a loved one. read more

My husband went from LB Memorial to South Bay Acute Care, we were just exhausted by the lack of care. It was so disturbing. The Nurses and Doctors just didn't care. We contacted Courtyard, they were so helpful, thoughtful and caring. We made arrangements for my husband to transfer there. The day before the transfer the administrator, Julie Javier, called us and asked how she can make the transfer easy for us. She thought of every need my husband had. My god, it was so amazing to be treated like a person. She even had the PT call to see how he could help my husband. We got there everything was ready. Clean, bright and cheerful. The really major thing was everyone had a smile on the face and they CARED! 3 weeks of being ignored, coming to the courtyard was like a new beginning. Thank you to each and every staff member. You made us feel like family. The last place we were at almost made us lose hope. Thank you Julie and your team, you gave us Hope. And I know my husband Will with your help come home soon. I really didn't want to put my name and frustration with what occurred out here but things got so bad I felt I had to. My husband arrived there at a time when many staff were new or in new positions, including the Administrator, Director of Nursing and Admissions. As things progressed I told the admin that he had very little time to fix things before the staff caused very serious problems. Papers that should have been filled out upon admission were filled out 13 days later. As far as care, there were several lovely nurses, including Debbie (now on Maternity leave) and Denise. Others however were awful and care, in general was absent. My hubby was there following brain surgery and twice the surgeon asked for CT scans and they were not done. After a followup visit to the surgeon, antibiotics were ordered and not started for 30 hours. The healthcare NP came instead of a doctor and stated hubby was end of care needed and on they day they were kicking him out he actually was lethargic and barely able to move. For the first time in the more than 2 weeks he was there they actually took his temperature after I made them call his surgeon. With a temp of 101.1 the surgeons instructions had been to get an MD there to assess him or get him to the emergency room. With those instructions Courtyard kept trying to reach the NP, not the MD and she wouldn't answer on a Saturday. I finally insisted that they call an ambulance and he was taken to the hospital. There he had a temp eventually of 102.5 and a brain infection that has kept him hospitalized for 7 more days and will require IV antibiotics for 11 additional days. Other medical sins or omissions were to not bathe him for 13 days, run out of his meds at least twice, not give meds properly several times, not respond to the bed alarm they required him to have for when he went to the men's room, not assist feeding his roommate and took away uneaten food and not assist roommate's family when roommate had vomited and was lying in a bed covered with it. I personally watched the cleaning person put his bare hands in the trash can to collect the small amount of trash and move it to a larger can and then go to the next patients can and repeat the process. They also printed out medical record information for unauthorized family members, very much against HIPAA regulations. If your family member is there, visit often and stay long. Courtyard may be good for a loved one who just needs a clean place to live with other people but not if they need serious care. read more
