My husband recently had a long stay in Sante of Chandler. So many positives with caring and dedicated staff. The first contact we had was Marci (cna) , she met us with a smile that day and every day after . Our personal angel was Kathy F his physical therapist ( totally worked miracles!) Athena ( nurse) was always available to us with a smile and answers to our questions. Cna's Linnea ( who will be a wonderful nurse someday!) and Christina ( tiny but mighty!) Yessie accommodated with quick and tasty meals. Janessa ( ot) and Sara who found his voice. So many to thank. Call lights were slow is the only negative comment I can think of. It's funny that I can't leave a review on Google. I am highly disappointed in this facility. My aunt calls to the kitchen to order food, they don't answer. Her occupational therapist orders for her, they never bring it. Middle of the day, and my aunt calls for the nurse, and ten minutes later they haven't come, I open the door, and the nurse is right across from the door on a personal call and watching Facebook videos. They do not care about their patients. I would find somewhere else for your loved ones. read more

This was the second time my husband was at Sante of Chandler. Except for a few exceptions, I found the level of care to be substandard. Care staff was not caring or compassionate. They acted like doing there job was a chore. Long waits when call light was pressed, sometimes over an hour. Food was terrible. I feed my dog better meals. My husband was there after brain surgery and a partial hip replacement. Forced hi. To sit in his wheelchair and go to the dining room for his meals. After a week he became agitated and aggressive which is not his normal behavior. He fell out of bed twice in one week and landed back in the hospital. I was told they couldn't keep an eye on him all the time. It was suggested that I hire a caregiver on my own to watch him at night. This place may be good for basic rehab, but for skilled nursing capabilities, I suggest you go elsewhere.​​ read more

The physical therapy, occupational therapy and speech therapy teams were a 10 out of 10 for my husband. Kathy, Norma, Sierra, Janisse, Albert, and Desiree were just exceptional. They took such a personal interest in Kent Recovery. Their patience in putting up with Kent's frustration and anxiety at times and helping him work through those issues while rehabbing made a huge difference Physically and mentally. Also, Marco, one of the caregivers was extremely instrumental and helping Kent deal with anxiety as well as physically moving him. My husband is unable to walk, after a lifetime of being an athlete and very active. The only reason I give three stars to Sante of Chandler is that I believe they are short staffed when it comes to caregivers. Their response time is very slow, which was particularly frustrating to my husband. Also, communications between the dayshift and the night shift. we're not always great. read more

I was in Sante of Chandler for about a week after back surgery. Overall, it was a good experience. There were some great staff people that went well over the standard care. I'm not sure I have the correct names, but here are some of them - Amy, Jacqueline, Kathy, Maggie, Janessa, Yessie, Maria, Lacy, Omar and Darion. These people were nursing, housekeeping, therapy, and food services. There were few area of things that could have been better. I realize that many places are low on staff. I had to wait a while for help. There were instances when I didn't get any response from the nurses, so I got up on my own. I was lucky that I was physically able to get up and walk with a walker. The beds are not changed unless requested, and they do not help with showering unless you ask. The weirdest experience was when I asked them to change my bedding and put on a new roll of toilet paper. The nurse showed me where the toilet paper rolls and sheets were. I asked her if I was supposed to make my bed on my own after back surgery and having a fracture finger in a brace, she looked at me with an irritated look and said she would come back and help me. We both left the room. When I came back the bed was made, and the new toilet paper roll was sitting on the old roll in the holder. I guess she couldn't figure out how to put it on. The last thing was one of my last days there, I never saw a nurse. There was someone there to give me my meds, but nothing else. This place is absolutely heartbreaking. I wouldn't wish for anyone to go through what my grandma experienced here. She was admitted to this facility to relearn how to walk, something that should've been met with patience, encouragement, and care. But instead, what we found was a staff that treated her, and others, with little to no compassion at all. It was already hard enough trusting strangers with someone we love so dearly. But we believed she'd be safe, supported, and well taken care of. That trust was completely shattered when I got a call that my grandma had been taken to the hospital. She needed nine stitches because the staff at this facility neglected to help her go to the restroom. Because they wouldn't take the time to help her (something so basic) she had to try to get up by herself. And she fell. She fell so hard that she busted her face open and injured her arm. She was in pain, embarrassed, and alone when she should have had someone there to help her, to treat her with the dignity and care every human being deserves, especially our elderly. It's honestly terrifying to think that this place is still operating when things like this are happening. This isn't just one mistake, it reflects a culture of neglect and indifference. These patients are here because they need help, and yet the very people responsible for their care are letting them down in such serious, harmful ways. My family and I are heartbroken and angry. They can't even schedule a transport correctly. "Oh we thought it was tomorrow, sorry." Even when the paper they signed had todays date. The staff is all wonderful. The owners and executives of this facility should be ashamed of themselves. Apparently there was a labor dispute recently that resulted in the restaurant staff all quitting at once. I don't know or care what they were demanding, but if wages are so low that your entire staff is happy to quit, then you've grossly mismanaged your business. The result is that families painfully sharing their loved ones' final moments don't have a place to grab a bite to eat for themselves. So, executives, you've lost the labor dispute, and you're willing to let your patients and their families pay the price. Your management is a combination of incompetence, selfishness, and cruelty. I'm giving one star only because I have to. Let's see, what can I say about Sante of Chandler. When my mom first was sent there after leg surgery I thought they were pretty good. That lasted about a week and then the incompetence showed up. Between the attitude with staff to the lack of care I could go on and on. Case managers that were total imbeciles and couldn't care less about patients or their care. She was there two months and she still can hardly walk. But hey, they said she hit a plateau and basically told me on a Friday of Memorial Day weekend that they were discharging her and basically good luck. The dr.'s are crap too. I will say a couple of the Aids were fabulous. The cleaning people were nice and their food was not bad. Don't send your loved ones here because they are only in it for the money. Funny when a home health nurse came to look at my mother he was disgusted with the quality of care that was given to my mother. Including her lack of progress and the treatment of her wounds that she got there. Time for maybe the state to get involved in these places and maybe do their job and check them and the patients out a little bit better. There is no reason to talk to anyone at Sante of chandler. I've seen all the reviews and Stephanie always seems to tell people to call and discuss. If she actually did anything about peoples complaints there would t be so many. My mother had terrible care at this facility. If I hadn't sent my cousin who is a Nurse Practitioner to intervene, they would have sent her home unable to even get out of bed after 3 weeks of almost no treatment. This place doesn't even deserve one star! Ismael Colinas treated my friend and her family in a horrific manner during such a sensitive time in their lives! Her poor father is suffering even more now because of it, not to mention the emotional toll it has taken on the entire family! This establishment is supposed to be a loving and caring environment for patients but sadly they had no care or consideration for my friend's dad! They practically kicked him out a day before the 4th of July, leaving him alone with no doctors to help or tend to him , since everyone was off! He ended up in the hospital!! My 92 year old father was admitted for rehab. No one told us we needed to order his meals so he missed meals the first few days. He was there over 2 weeks and continued to decline and no one was concerned and no updates were provided. 2 days before his planned discharge, he had a significant decline and was showing signs that he was nearing end of life due to poor appetite. He was a DNR and didn't want any major medical intervention, just to be comfortable. No one at facility noticed his decline even though he was agitated and hallucinating. They refused to give him any sedatives. They all insisted he was fine. The next morning staff attempted to get him out of bed and he fell and cracked his head open. The ER told us he was in kidney failure and had many critical labs. He died 3 days later. read more

My one star for this place is because the incompetence and rudeness of Ismael Colinas. Mr. Colinas treated my sister poorly and showed no humanity on the situation that she have to endure at the moment, they even make my dad (the patience) sign paperwork saying that he refused treatment taking advantage of him!!! You would think that if you are on life`s inevitable phase of taking care of you old folks at least life won`t also add the impish to the shuffle. Sadly that`s not the truth and LPN Colinas will be there to stop you on you tracks and let you into the secret that they are everywhere and here is one of them. Looks are deceiving!! Nice facility but care and communication is severely lacking. Every time we tried to speak to Luisa the case manager, she was on the phone. We finally pulled our family member out of the facility and before doing so, tried to talk to Luisa. After knocking on her office door, we were told by another case worker she was on the phone. After letting this case manager know we were taking our family member home without being discharged Luisa didn't even bother to come to the room before we left to speak with us to determine why we decided to take her home before the facility officially discharged her. As a matter of fact, I was taking clothes to the car and passed by Luisa talking with someone right outside her office door. She had time to talk to this person but couldn't make time for us. The lack of communication between the Dr. and nursing staff with our family was nonexistent . I would not recommend this facility and totally regret putting a family member in it. To make matters worse, nobody from the facility has called to ask us why we took our family member home without being discharged, that speaks volumes!! read more

My mom has been in this facility 4 times. 2 of the times they were amazing and on the ball with cna staff attending to patient in timely manner , nursing staff listening to the patient , but this last time and on another visit they just didn't seem to care. My mom would be sitting on the commode for an hour before someone came back to help her up, not listening to our concerns about her mental state, and sadly discharged her when I had concerns and my mom was unable to get up on her own they let her go home and both visits she ended up in hospital within 11 hrs of leaving Sante because they refused to listen to the family and the patient. But all to quick to collect your money! I have mixed feelings about Sante as there have been 2 other visits where they were just amazing in care and their concerns... this stay however was the worst!.. and when I called to express my concerns by talking to Ismael he didn't seem to much care. read more

I wanted to take a moment to share the experience we had at Sante-Chandler in May 2023. It's taken me a while to post a review, but I wanted to think carefully and not in anger when I placed this review. My father-in-law was transferred to Sante after spending a few days in the hospital. I have had experiences with Sante before, just not this location. Our family thought the facility was nice, clean, and for the most part friendly. A PT schedule was set up twice a day, which seemed like a hit and miss. My father-in-law was in a frail state, so perhaps PT twice a day wasn't practical in our situation. During daytime hours, we kept a close watch on my father-in-law. Dad constantly complained that his meals arrived cold, and we were told we needed to pre-fill the menu for the week so it would arrive hot, so that's what we did. Their menu ran from Sunday through Saturday. On Mother's Day, having filled out the menu for the week, we noticed Dad did not receive his meal. I personally handed in the menu for the week at the nurse's station the day before. When inquiring about why Dad had not been served lunch, the staff said we had not turned in the menu for the upcoming week, no one had it, and that because it was Mother's Day, the standard lunch menu would not be served - that there was a special menu for that day only and we had to choose from those selections. No one advised us about this special menu. After visiting hours, things got a lot worse. My father could not move from his bed, so when his evening meal was brought into his room, if he were sleeping, they would place the meal on a table across the room. Once he woke up, he would buzz the nurse's station to have them come in to move the meal onto his bed table. His room was equipped with a microwave oven. When the nurse would come in, he would ask her to warm up the meal, and he was told that the nurse had no idea how to operate the microwave oven and she would immediately leave the room after moving his cold meal to his table to eat, which he didn't because it was cold. When you are in a rehab facility, you tend to sleep quite a bit, so you're not quite on a regular schedule to sleep through the night. When Dad would wake up, he would page the nurse for something he wanted, perhaps water, perhaps his Ipad. The night nurse would come into his room on a few occasions and yell at Dad that he should be sleeping and that this was the last time she was going to go into his room until morning. Can you just imagine how that must have felt? To be fair, we addressed all these issues with Sante's case worker, and she was horrified when I told her what was going on. There were also several times that the light would go on outside Dad's door, signaling for assistance, and it was ignored. After two weeks, we found an assisted living home to transfer Dad to. When we left that facility, he was even more frail, and had a gray pallor to his face. When he arrived at the new facility, the first thing they did was examine him to be sure they were aware of all the conditions he had. To be fair, we were advised by Sante that Dad had one bed sore that they were treating him for. When the staff examined Dad at the assisted living center, he actually had two huge bed sores that needed immediate medical treatment. Since he has been at the new facility, which I can't recommend enough, Grace Assisted Living in Chandler, he has flourished and he's doing much better. I understand that Sante-Chandler is a large, beautiful place, but there was, in our opinion, alot of neglect for Dad's care. Finally, on the day of Dad's discharge, I did speak with the Case Worker and expressed how disappointed we were about our experience. She apologized and said she would look into it. If we don't take the time to express to staff what was happening, we shouldn't hold them accountable, and shame on us for not doing that. The elderly need our help to be sure his needs are being met. Anyone thinking of this facility, be careful, if we had to do it again, we never would have gone there. read more

Since my last review, I've had the misfortune of visiting other health care facilities! I now know that Sante of Chandler is one of the best in the area! Please check these places out for yourselves. You or your loved ones should be aware of what facilities are best for you! Good luck! Making sure there is enough staff is very important. If you or your loved ones are recovering from a major injury, such as a stroke or brain injury, please consider avoiding Sante of Chandler unless you have an expert level of knowledge of patient care and insurance/medicare regulations from the very start. Likely due to staffing issues, patients requiring extra care could be severely neglected, such as leaving bedridden patients in urine & feces all throughout the night, causing sores and skin breakdown. We spoke with other families having similar issues but I will only speak from our experience. Even after addressing these issues multiple times and turning on hourly call lights to remind the staff to physically check and turn our family member, the staff would simply turn the light off when it was not convenient, specifically at night. During our final incident of neglect from the night staff at Sante of Chandler over a weekend, we mistakenly said out loud that we were "documenting the incident this time", but were again ensured it would never happen again by the Assistant Nursing Director. Note that in the daytime we had at least 1 family member in the room during all visiting hours, 7 days a week. We know mistakes happen and tried to be positive over many weeks because the Therapists (PT, OT & ST) were all very helpful. However, the morning following that final incident we received a call that our family member was not thriving with PT and due to medicare requirements they will begin an immediate discharge. This was surprising given the Physical Therapist was just telling us that morning how well our family member was progressing. Not knowing the medicare rules, we asked for clarification and options from Deb N. and Kathryn L., the case managers, but they refused any further explanation yet were happy to arrange transport. They placed the blame on medicare so I personally called the state, and was told that Sante of Chandler is required by law to provide us with the appeal process in writing. It turns out Deb N. slipped the appeal information into a folder of papers in the room and wrote on the form that she "explained the appeal options with family over the phone". Because we did not record the discharge conversation and the form was found in the room, it was her word against ours. Lesson learned, but it gets worse. By lying about our family members failure to thrive with therapy, it meant that we couldn't even consider transferring to a different facility because medicare would deny the claim. The house was also still under construction for handicap and accessibility modifications. We did file the appeal and it was initially denied with no notification. The appeal provider, Livanta, told us that Sante was supposed to provide us with a copy of the appeal decision. When asked why we were not informed, Deb N. said that they never provide this notification to patients, and it was up to us to call Livanta. While initiating the next level appeal, we discovered that they had sent over weeks worth of ST & OT notes, but only 2 days of Physical Therapy (PT) notes! One of those days happened to be when OT & PT staff spent the entire time changing a feces and urine soaked bed rather than focusing on proper therapy that day. Also, NO instance of any of the neglect was ever documented. When we requested they send more than 2 days of notes for PT to show the continued progress, we were told "no, that's all [they] are required to send". FYI, technically they are required to provide 3 days, but one day was a Saturday with no therapy. By that time it was too late, the deadlines had passed, and the appeal was denied based on all the information provided by Sante. Deb N. and Kathryn L. did an amazing job covering for Sante of Chandler, using our family's ignorance of the medicare regulations to their benefit, and executing a quick discharge by choosing to limit the PT notes provided to the medicare appeal to the absolute minimum required by law. Legally, it would appear they operated within the rules & regulations of medicare, but make no mistake, they are NOT advocates for you or your family. They only advocate for their employer and have little to no ethical standards. They know how to play the game so you better know before choosing this facility. read more

One star is being generous. My MIL was at this facility following a massive stroke. She also had a bone flap (part of her skull was removed for swelling) and had to wear a helmet if out of bed. She also needed someone to stand by her while she ate to make sure she didn't choke. Food mainly breakfast was given to her (we were at bedside from the start to end of visitor's hours) alone. Not to mention they would shut her door. Her call light was almost always out of reach. There were several occasions when I would find her insulin needle left uncapped in her bed, on her affected side. The aids would lift her into her wheelchair without putting her helmet on to protect her brain. She was left several times overnight with out being turned or changed. After talking to the ADON they said they would make sure nights did better, but the very next day they tried to claim she wasn't meeting PT/OT requirements and tried to kick her out. She developed pressure ulcers. The wound RN took a two second glance and left. I had to point out other blisters/rash elsewhere to be treated. Even with a family member being with her for the whole and sometimes longer then visiting hours she still has very poor care. Being that I am a healthcare worker and know that staff is short this does NOT excuse her not being changed and being left to sit in a wet/dirty bed for hours overnight. This does not excuse RN's not helping the aids. Long story short if you have a loved one don't send them here if you want them to live. Many patients calling for help while aids/RN's/LPN's sat and talked. Also, the case management did not tell us all our rights and hid information inside a desk drawer saying it was given to the patient, full well knowing they should have contact led my FIL directly. read more

My mom was not provided with any meals throughout the entire day @ SANTÉ SKILLED NURSING OF CHANDLER AZ 825 S 94th street Chandler, AZ 85224 My 93-year-old mother recently developed colitis and was hospitalized for five days before being transferred to SANTÉ SKILLED NURSING in Chandler, Arizona for short-term rehabilitation, which was started from November 28th to December 11th 2023. On December 6th, my mom did not receive any meals for an entire day: no breakfast, no lunch, and no dinner was brought to my mom's room. I was shocked to see this horrible negligence of my mom. I could not believe a 93-year-old, sick lady, stayed in her room without food for a whole day. We brought this matter to the manager of the facility, and she told us she was not on site and was out with friends having dinner. We had to reach out to the nurses who work at night. Later, we found out the weekly meal order list which I filled out on December 3rd for my mom had disappeared. And it was not the end, on December 10th, for a whole day, my mom only received one breakfast at noon. Such negligence is ethically unacceptable and poses serious risks to the well-being of vulnerable individuals. The failure to provide meals to my mom shows significant neglect and lack of quality and respect to the elderly of this rehab center. read more

Your first impression of this facility is it appears to have a spa like quality, and the assumption of a place providing professional and compassionate care. After a bumpy start he settled in and found first impressions appear to be accurate although like all things in life you take the bad along with the good. A facility with so many exceptional staffers a few stand out because of their willingness to provide care outside their scope of responsibilities. One star, only because I can't give zero. I'll start by saying my mother was there for about 3 weeks. When I called to speak to my mother, her room phone was not working. I had to call the nurses station and was told "there is nobody here by that name". I was curious since I had just seen her 2 days before. So I called back and was told again that there was no one there by that name. I asked if she was discharged or moved and they told me they couldn't say. I called a third time and was promptly hung up on. One more call and I was told I needed to speak with the power of attorney. Now I'm thinking my mother died! At that point I called the nurse supervisor, Stacy, and had to leave a message. When she finally returned my call, I was advised that she was there, and that Stacy herself would go to my mother's room and make sure she got my phone call. I was able to speak with her that day. A couple days later, I go to call and one of the nurses at the turquoise station put me on hold and left me there for 15 min. I hung up, called again and demanded to be put through to my mother's room. She told me to hang on, went to the room, the phone was picked up, but never given to my mother, then it was hung up! I tried one more time and was told she was sleeping. My mother would have wanted to speak to me. Is this normal treatment of families or is it just me?? My mother was just released to go back to her assisted living facility and I couldn't be happier. Except for the fact that the food was good, and Stacy was trying to be accommodating, I would recommend you keep your loved ones out of this place. I want to give Sante of Chandler and the Staff a outstanding Review. The Care and Kindness from all the staff was incredible. (While my husband was recovering from Triple A surgery, and a horrific infection received from being in another Center). He arrived at Sante of Chandler on Feb. 22, to recover from 2 more surgeries caused by the infection, along with a wound vac. I was with him everyday all day and saw first hand what a wonderful staff is in place here, and how they interact with the patients, and families that came to visit. The nursing staff and wound care staff have gone above and beyond to make his stay and recovery productive and beneficial . Each shift change knew exactly what care he needed and was always responsive to his requests or Mine. Also, he was transported to several doctor appointments by Brian the driver. Brian always made sure that my husband was safe and comfortable while in transport and I was invited to ride along. (the other Center I was not allowed to) I want to say a SPECIAL Thank You to Ian, Sara, Carmalita, Linda, Carolyn, Barb, Lauren, Silvia, Ericka (House Keeping) George, Jim, all the Staff from PT, all the Staff from OT, all the Staff from Speech, and Kitchen staff for always being helpful, (and great food). I know I missed some names but know how much we appreciated every thing you did. Further more, another special thank you to Case Manager Deb, for not only the coordination of care, but for being a Advocate on his behalf with Insurance and Medicare. After his release on March 31, he was able to come home with confidence, and continue to recover and build strength. The excellent services from the PT and OT staff will make it easy to shift to the Home Health Care professionals that are sent to our home now. I salute the hiring process in place. To select qualified persons able to give the quality of care that this Sante has. "Not all Sante's are managed like this one."! I highly recommend this Care Facility for yourself or your loved ones. My mother is currently here, she loves the food and the majority of the staff. Her room is spacious and quiet- includes a fridge and microwave. She is improving here thanks to PT and OT. The front desk has always been courteous and quick to address any of the needs I have had with my mother. Admissions was easy to work with prior to transferring my mother from an acute rehab facility. read more

My dad stayed here for 4 weeks and if a family member was not checking on everything, I'm sure he would have just deteriorated there. Please listen to me and other 1 star reviews on here. They all speak of the same neglect. He did need more assistance than some and his memory was affected a bit after infection, but that is no excuse for the lack of care. I met some nice staff and helpful people so there were moments of hope, but overall everything was lacking. I am assuming the management is not good and they are understaffed, but not completely sure where the core issue was. My siblings and I rotated checking in on my dad, spending at least a few hours every day during key times. Getting answers from any case manager involved many knocks on their door over and over and phone voice mails left unanswered. Multiple check-ins/questions became the only way even with the CNAs. And we really didn't ask much or anything out of the ordinary. My dad waited at least an hour two times (that I know of) for someone to respond to his call button. He remained on a liquid diet without any question till I asked 2 days later and on a drain from surgery (for 2 weeks) till I asked and forced them to assess. He needed help setting up his tray of food, yet his food was left next to him at times and taken away without noticing he hadn't eaten anything. Plus a ring stored in his closet disappeared and hasn't been found. His glasses were lost also and only found by repeated phone calls and bugging them to search. The worst is he was discharged with blood in urine that we were told is hemorrhoids. However after 2 weeks, this UTI (which it really was) became worse and he went back into hospital in a bad state. Not sure who miss-diagnosed this, and what was not done to prevent this. My brother knew Sante to be a better rehab years ago, but was also annoyed to come visit dad and see staff on their phones and chatting/laughing while dad was needing help and not prepared for therapy or to be discharged. Find somewhere else. Don't let the nice building and spacious rooms fool you. read more
