This place is horrible, their staff. It's so rude. They act like you're bothering them if you need any kind of assistance when you page them. They don't even come until hours later. They verbally abuse you. When you put in egrievance, they do nothing about it. They are unkind. They have bugs in their food that they serve you. This place is would give a negative star if I could. I guess since they are funded mostly by medicaid and medicare, the state doesn't regulate it.I guess. This place is disgusting along with the people working in it. My father has been a resident for 6 weeks now, and from the top of the building (administrator) all the way down, everyone has been helpful, cheerful and always open for a conversation. As a retired physical therapist who has worked in many nursing homes, this one stands way above any other I have worked in or visited in the last 40 years. My dad has been happy with his care, the food, and the respect shown him on a daily basis. And we are thrilled with the staff's care and their ability to answer any questions quickly and professionally. Other nursing homes need to visit the Suites to learn how they should be providing care. read more

UPDATED 5/31 FOR MAXIMUM TRANSPARENCY/ACCURACY (ORIGINALLY 5/29). CMS cited The Suites for 84 deficiencies from May '22-Nov '24, including 16 in its latest report, and fined it nearly $210K. Eduro, its corporate owner, has over $2.19M in fines; among orgs with 30+ SNFs, only five have a worse abuse icon rate. As of 5/31, I can confirm additional CMS action post-Nov '24. Based on complaints/grievances filed on 1/2/25, 1/3/25, and 1/16/25, CMS cited two more serious deficiencies: F0609 - Failure to report suspected abuse/neglect or theft and to report investigation results: CMS found The Suites failed to report multiple serious incidents--including a medication error and alleged sexual misconduct--to the state, in violation of federal law. F0610 - Failure to respond appropriately to alleged violations: The facility failed to conduct proper investigations and didn't submit findings to the state within 5 working days. These were issued following a Feb 2025 complaint survey. CMS rates The Suites "Below Average." If The Suites/Eduro had any evidence of defamation, they would have issued a formal takedown notice. They haven't. I'm very close to retaining counsel to file complaints with CMS, NM DOH, licensing boards, and HHS OIG, and possibly to pursue a False Claims Act against Eduro/Suites. I'm in contact with journalists In my mother's case, Dr. Odinaka J. Nnanna "manufactured" diagnoses of acute respiratory failure, severe malnutrition, diabetes, acute-on-chronic systolic heart failure, and multiple lung diseases solely to maximize Medicare reimbursement and inflate the facility's CMS star rating. These false codes inflated the appearance of clinical complexity, enabling The Suites to bill for higher levels of care that were never needed or provided. This is fraudulent misconduct and a manipulation of public reporting systems meant to protect families: deception for profit by Nnanna. The Suites fabricated medication reconciliation. My mother entered with no pressure ulcers and left with a Stage 2 ulcer. According to HHS, this should never have occurred. Patricia Olguin, NP, claimed six visits. Her notes and those of Ebube Njemanze, MD, used identical templated language (e.g., "5/5 muscle strength" daily) while also diagnosing M62.81 (muscle weakness). NO REAL EXAMS EVER OCCURRED. All 3 providers are linked to UKA Healthcare, a locked, non-operational "medical practice." Olguin is also listed under Daiya Healthcare; Holly Lorenzem, NP, under PARP--both using the same WA address/suite. Lorenzem's billing address is Canyon Transitional in NM. These affiliations suggest ghost billing/shell practices. At the care meeting, my sister demanded discharge; staff admitted my mother wasn't improving. Yet the EHR falsely claims discharge was due to "sufficient improvement." Olguin never attended the meeting, despite her lies. THIS MEETING WAS RECORDED. Try to weasel your way out. I dare you. Olguin billed Medicare for 30 minutes of Advance Care Planning during daytime of 3/18. Her note was fabricated, citing vitals from nighttime. The "MOST" form confirms no discussion with patient. I witnessed Olguin's 3/12 visit. She stayed 2 minutes, admitted she hadn't reviewed the hospital chart, yet later documented a 50-minute visit using vitals not yet charted: a pathological liar. Lorenzem's notes are copy-pasted and contain fraudulent falsehoods: James Dallas, LPN, falsely claimed to be DON on 2/13. On 3/28, new DON Tamer Osman also lied, stating Dallas had been the acting DON throughout my mother's stay. Osman's signature appears only once--days post-discharge. On 3/14, the day after I raised legal concerns, Jessica Lucero, Med Rec Dir, had my legally blind/mobility-impaired mother e-sign five legal docs twice each (once as herself, once as her own legal rep) within 90 seconds. Metadata shows Lucero signed hours earlier. All were flagged invalid, including a NOMNC uploaded 4/17; an Arbitration Agreement uploaded 3/27. 15 FORGED SIGNATURES; 5 VACUOUS LEGAL DOCUMENTS. Lucero escalated a valid HIPAA request to Legal. Eduro's Gen. Counsel Dustin Monroe sent a "Legal Chart"- not the full medical record. It was packed with bulk orders and retroactive entries. Notably, all PT/OT notes were missing - to conceal fraudulent documentation! I recently called The Suites: it doesn't have a medical director. I asked for the DON--the phone rang endlessly: two serious illegalities. Documents show Nnanna (covertly) functions as Medical Director-indefensible/downright sleazy The Suites' website shows outdated staff listings. So much for transparency. The moment we walked in, the staff made us feel welcome and supported. The nurses and aides were attentive, kind, and always took the time to explain things clearly. The facility is clean, well-maintained, and has a peaceful atmosphere that helped ease our worries. It's never easy to make the decision to move a loved one into a nursing home, but The Suites Rio Vista made the transition smooth and reassuring. They killed my brother, there is no Doctor at this place only nurses. He died of septic shock even though he had cancer. second cause of death of bacteria in peusdomomas. I had to call them to give my brother morphine, which they kept. He oncologist at Rust said to get him to the ER asap, he died two hours later. They need to lose their accreditation and I will see it happen. Never gave him physical therapy, he stayed there, no phone in room, tv was broken, he died within five days. read more

On 3 October 2023, I fell, broke my hip . On the fourth I believe I had a total hip replacement at UNM hospital. was in the hospital for a couple weeks, at least a week and then sent to a rehab facility and I chose suites of Rio Rancho Vista's this place looks nice but don't let it deceive you !!! I've been here two weeks and then the two weeks they have not got me my medicines on time once.also my nurses name was Halley she was the only one ,nurse that could handle things and get things done. read more

My wife was in The Suites for 10 days as part of a rehabilitation program after she was released from the hospital. The leaders and staff treated her (and me) with caring, kindness, respect and friendliness at every interaction. I was impressed with this in part because I was at the facility every morning for 3-4 hours, every afternoon for 2-3 hours and every evening for 2-3 hours. They were always friendly and helpful. (and never acted like they were sick of me). The OT and PT staff were friendly, respectful and prepared with a full complement of activities and tests to help insure a successful recovery for my wife. They included us both in the planning, scheduling and implementation of the OT and PT work. While I have read a couple of reviews that were negative, that was not our experience. We would recommend this facility to a family member. After reading some of the reviews, we would suggest to them and to any one with a family member that needs a Rehab program or nursing facility that they be at the facility every day for an extended period of time getting to know the staff, asking questions and being as helpful as possible to the staff as well as your family member. If you think/feel the staff are not including the patient and family in the ongoing treatment and planning & scheduling of future treatments that you should initiate communication with them and if needed make (time certain) appointments with them to go over past and present plans as well as what the future treatments, goals, plans and timelines are. In many cases you will be the one to manage and work out the long term successful care of your family member. read more

My therapy has been amazing. Punctual, knowledgeable and consistent. PT comes to my house weekly to help me recover safely from surgery.

As a ex employee I would not recommend this place at all staff is rude no communication no friendly faces nothing to show they care about their patients. Loved ones are only seen as numbers and dollars to this facility. Administrators here only care about making sure they are getting paid versus ensuring that our loved one's care is properly met. My father was here for 3 weeks, and I guarantee only one nurse (Paulette) cared for my father. Tanya Walters runs the facility with a pompous attitude while doing a horrible job at making sure the patients' needs are met. Staff members will continuously point fingers at one another and never try to make the patient feel comfortable. I personally witnessed multiple levels of neglect, lack of response times to the nurse bell, hours passing without anyone checking on residents. They even rescheduled my father's medical appointments without family's consent and refused to allow family to take my father to his doctor appointments. I had shoulder surgery last week and, I think, it went well. I spent 2 - 3 days in the hospital then was sent to Rehab, the Suites in Rio Rancho. The Rehab. Hospital was the worst of the worst. I demanded I be given a discharge on Monday. These are some of the things that happened to me: I have been taking 3600 mg of "Gabapentin" a day for nerve pain for almost 20 years. The Suites decided it was too much for a human being and cut my dosage in half which was causing me a great deal of pain in my hands and arms. I begged them to increase the dosage to what I always have taken. I was starting to have symptoms of withdrawal and extreme pain. Just before discharge the "doctor" said she looked it up and now knew what Peripheral Neuropathy was. She then said she would increase my dosage. I said "no, I want out of here". I was given a "AMA" discharge. The first night I went to the bathroom in a wheelchair. I got out of the chair and couldn't lift myself onto the bed and the buzzer was on the other side of the bed. I slept on the floor the whole night waking up twice. My roommate finally buzzed someone to help me in the morning. I cannot believe I slept on the floor of a hospital. I guess they don't make rounds. As I left Lovelace they gave me the ice machine that I was hooked up to reduce swelling and promote healing. They NEVER connected me to it. They were supposed to. This past Monday or Tuesday the Occupational Therapist came for me early in in the afternoon. As she was wheeling me to the therapy room someone said to her I wanted a shower. So she gave me a shower then was starting to leave the room. I then asked she if she was going to take me to therapy. She said the shower the therapy and the allotted time is 30 minutes. So it is either a shower or therapy. There are a few more major and minor incidents. These are unacceptable! No human being should be treated like this! I have a call into the doctor to have me switched to Home Health Care. I hope they can help me with my shoulder. The police came to do a well check today, 12/11. Apparently the discharge must have triggered it. I should have researched rehabs more thoroughly since this place has severe violations. This place should not be in business. This Dr. Gardner is a "Quack'. She knows mothing and is very arrogant. My roommate has a condition I knew about from volunteering. She is making a wrong diagnosis. My mom stayed there following a hospitalization (that had greatly reduced her mobility) and up until her death from accumulated health problems. Most of the staff were very friendly, and all were responsive when I made requests regarding her care. I am giving four stars instead of five because I never heard from a doctor directly; however it was usually easy to communicate with the nurses and other staff for updates. Mom told me that she was satisfied with the food and with her care -- and that the people were nice. Further, I found them to be very compassionate (and respectful of her advance directive) when she entered end-of-life care; they focused on her comfort, played her favorite music for her, and worked with our family to facilitate a safe end-of-life visit. read more

They are trained to say all the right things and it looks nice but this is not where you want to send a loved one to get better. The quality of care is poor and while at this facility my Father fell and hurt himself because he could not get anyone to help him to the bathroom. When they respond to you they agree to do things and then there is never any follow up. Poor communication among all the staff. I wish we would have looked date their yelp ratings before sending my Father here. If I could rate them a 0 that would be more reflective. If a family brings up issues they rush your loved one out the door with not proper discharge plan in place. The discharge services are horrible and not within standards of good practice. I am giving one star because you can't give a zero rating!!My mom was there for a week and two days and after being released from Lovelace Westside...she declined immensely!! The doctor, Dr. Gardner is a quack and incompetent...he saw her for the first time a week after her admission! He was trying to blame my Mom for not doing what the doc recommended....how could she when she was asleep all the time and near death! We demanded a release and he really wasn't on board. After a week of stay they finally did bloodwork and her kidneys were failing and he basically knew he needed to CYA so he did release her to Sandoval Regional where she has gotten exceptional care. They were sooo shocked that nothing was done for her at Rio Vista! DO NOT take your loved ones there! One doctor for a large facility is ludicrious. The staff are amazing and they work with what they got! This facility has changed its name several times...hmm....and has had alot of infractions reported! This place has really turned around. My grandmother has been a long term patient there for almost 2 years now, and before the current company took over she received terrible care. However, in just a few months we've noticed a significant difference. The place is cleaner, and the staff more eager to assist. We can now all sleep better at night knowing that she is receiving the best possible care. read more
