    My therapy has been amazing. Punctual, knowledgeable and consistent. PT comes to my house weekly to help me recover safely from surgery.

Unfortunately, due to lack of precautions, he fell out bed and broke his hip and had to have surgery which had him back in this rehab and ended up being there for 2 months. Physical therapy was almost nonexistent for the last month and a half, so he stayed in bed most of the time. He was often left soiled and needing to be changed and wasn't helped out of bed. We had family visit him every day and had we not, I can't imagine how bad the care would have been. My dad is legally blind and needs help eating, or be served finger foods., I came for breakfast one morning and he was sitting alone, in bed with a tray in front of him that just had a cup of coffee dumped all over the meal. He was unable to eat one to help him eat. He developed 2 bedsores - one a stage 3 - because he was not moved frequently enough. Most of the staff was annoyed to be asked to help. The wound care nurse and one younger male CNA were great, everyone else was not. The office staff also said that insurance had denied claims and had us pay $3000, only to find out insurance never denied the claim and actually paid it. Over a month later I'm still waiting on refund. Terrible on almost all fronts!

I would never recommend this place to anyone. The staff and management should be ashamed for how badly they treat these people to the point it's inhumane.

    This review is for the skilled nursing facility for short-term rehab…read more If you are expecting exceptional care, skilled care, compassion, assistance with ANY IADL/ADL assistance in this exceptional luxurious facility, then this place IS NOT for you or your loved one. They do a a wonderful job admitting you then it's quickly downhill from there. They have adequate staff around the clock, just not adequate enough to tend to or provide you sufficient care for you to get better to return home. They kept my MIL's door closed almost at all times. That's attentive, alright.... They took longer than 30+ minutes every single time to tend to her. This included accidents that could've have been prevented. The professionalism and demeanor is insulting considering the field they are in. This includes the management staff. I hope DOH steps in and requires the company re-trains the whole skilled nursing staff and team. Please discuss with your doctors/loved ones before agreeing to transfer here from the hospital post surgery or in need of wound care.

However, the…read morereality seems to me that from my experience of having been here numerous times in the past and even to the present, this facility is quite disorganized, especially regarding communications among the management and the staff. As far as I am concerned, the "Activities" section of this facility needs to improve. Stimulating "Activities" for the residents are a vital part of assisted care facilities. Yet, after providing innumerable service in this regard, I have had extreme difficulty in getting compensation for my services rendered here, i.e., providing necessary entertainment as requested by many of the residents. Non-payment of services rendered is a serious matter. The "Human Resources" of this facility should be rated the worst in Rio Rancho. Unless they change the present system of compensation for those who provide entertainment to the residents, I will never ever perform at this over-rated "care" facility.

I got there at 2:30 and a CNA came in, put a clipboard down and left...no instructions on what I should do. I didn't see anyone until 5:30 when they brought dinner in.I didn't get water until 10pm. I ask for a wheelchair and they said I couldn't have one. At midnight the pain level was a 10 and they didn't have orders for pain meds. I ask for ice bags for the pain and they said they didn't have orders for that either. Common sense has gone out the window at this facility!!! I stayed awake all night watching QVC trying to keep my mind off the pain. At 8am, I called my son and asked him if he had time for a "pity party." I was crying and told him what had happened and that I had to get out of there. Of course then I had several people come in and tell me that things would get better and the night nurse had had a bad week. I doubt if her week was any worse than mine with surgery for a severly broken ankle. I left there at 3pm just thankful to put The Rio At Cabezon or Rio at Rio Vista, as it is now called. My advise is: if you really care about your loved ones DON'T GO THERE!

All starches and no protein no fresh vegetables. One time it was spoiled. Fruit is always from the can. Ask the residence, what they want to drink? Instead of always serving Kool-Aid! The staff shouldn't have to apologies about the bad food they delivered every day and every meal! The room is depressing looking. The furnishings are old and dark. They can make it more cheerful by hanging some pictures and make the room look more welcoming. The staff are nice. Some of them are more attentive than others.
