When the time came to consider an assisted living facility for my wife's parents, we toured several…read moredifferent facilities. We found some we liked and some we didn't. We wanted someplace that was new-looking, as opposed to showing its age badly, had lots of natural light, and seemed to care about the needs of the residents. We also wanted a place that had onsite memory care facilities. We looked at Sage Valley Senior Living twice and immediately liked that it was a new facility. We loved that it had lots of natural light and a different menu daily with a chef who cared about creating the best eating experience for the residents. We even had a chance to eat there during a subsequent visit with my in-laws. We found the food to be far better than expected. Even the cheeseburger was very tasty with a flame-broiled taste. Since moving them into Sage Valley, my mother-in-law has flourished and made wonderful friends. The staff has been nothing short of fantastic in their care for them. They call us and notify us of problems they discover, and they show their level of caring every day. The staff has been very friendly whenever we see them, the onsite medical staff has been fantastic, the nurses and aides have worked hard to make them feel special. During my father-in-law's final days, the staff was very helpful and always had a caring word for my mother-in-law and family. After his death, the staff rallied around my mother-in-law and greeted her with open arms and hugs when they saw her. I know every place has its challenges, and I came into this facility with eyes wide open, but my wife and I have been very happy with our decision to move them into Sage Valley.

We made the decision to move him out urgently due to multiple failures in care, safety concerns, and misleading information provided to us prior to admission. We were referred to this facility by a hospice nurse, and during our initial conversations, Norman, nurse, assured us that my father would receive daily showers. He also stated that although they did not offer respite or trial stays, we would only be charged for the days he was there, plus a $1,500 community fee. This directly contradicted the information on their own website, which clearly advertised respite stays. The website also stated a 1:4 caregiver to resident ratio; once we questioned this, Layne, the acting owner, removed that claim from the site. During my father's six days there, the level of care was far below what was promised. The facility had only one staff member on duty responsible for cooking, baking, laundry, caregiving, and medication administration. A caregiver openly expressed feeling overwhelmed. My father received only two showers during his entire stay, both provided by the hospice nurse--not by facility staff. On June 9th, when we arrived to check on him, he was still wearing the same clothes from the previous day. My mother had to request a washcloth from Norman just to clean him up. That same evening, we discovered he had been given his medication at the wrong time (8 p.m.), which is known to cause insomnia. When my mother requested his anxiety medication, she was told it would have to wait until Norman could be reached--yet both Norman and Layne had already left for the day, as they typically do in the late afternoon. The following day, we made the decision to remove my father from the facility. When we asked Norman for his medicine while packing him up, Norman handed us another's patients' medication. Once we got home, we realized we were missing his anxiety medication( a controlled substance), we went back and the caregiver couldn't find it. We mentioned to her the incident with Norman mixing us the 2 patients medication boxes, she looked in the other patients' box and found my father's medication. This was extremely alarming and raised serious concerns about medication safety and compliance. Despite these significant failures in care, Layne has refused to refund any portion of the $7,800 my mother paid for six days. She insists that a contract was signed, while ignoring the fact that the facility did not meet the care standards they promised. It is clear that they are prioritizing sales over resident safety and well being. Layne has only been in charge for a few months, and while her father previously had a strong reputation, the current management is not operating at that standard. There is also another recent review describing a similar experience. Our Hospice nurse told us this facility had the appropriate care, she was going on previous visits when Don was in charge. Everything has changed since then. Based on what we witnessed, I strongly caution families to be aware of these issues. My father is at the end of his life, and the care he received at this facility was unacceptable and unsafe.

After a very extensive search of the available facilities in the Leander and North Austin area, with visits to many of them, it was clear to us that Riva Ridge was by far the best choice. Given its small size, at the time of our interest there were no available rooms, so we asked to be put on the waiting list. The wait was well worth it. After moving there, our mother in a short period of time became accustomed to her new surroundings and the homey environment of the place, which is in stark contrast to the crowded-hospital feel of many facilities. Ken Missler is a hands-on owner of the place, and he and his staff are caring and compassionate while maintaining their professionalism in the day-to-day care. The range of residents' issues and capabilities is wide, and the staff engage the residents in games and art, etc., to the extent of the residents' capabilities. Visitation is essentially unlimited, though it was limited for a time during the pandemic in accordance with statewide COVID restrictions. The availability of both assisted living and memory care, performed in adjacent buildings, along with the availability of hospice care, are useful combined features of the facility that are not commonly found elsewhere. My very favorably view of Riva Ridge has been corroborated by others. My wife, as part of her church's visitation outreach to home-bound seniors, has visited many assisted living places in Leander and Austin. None has come close to the homey and comfortable environment of Riva Ridge. Once, when I was visiting my mother at Riva Ridge, I met a former nurse who was looking for a facility for her disabled husband, after removing him from another assisted living place that was not adequately caring for him. Having experience in these matters, given her occupation, and having looked at several possibilities, she told me she was very impressed with Riva Ridge and wanted to place her husband there as soon as possible. Our mother was at Riva Ridge for two and a half years as her health declined, the last six months in hospice care before she passed away peacefully in her sleep. She was taken care of, looked after, engaged with friendly and caring people. I thank Ken and his staff for taking such good care of her, and I unreservedly recommend Riva Ridge to others who need to find a place for their loved ones that require compassionate, professional care.

I knew two other families who had family members there previously and we liked the smaller, more homelike setting with private rooms and a maximum of ten residents. Our mother received the kind of loving care we wanted for her and every staff member treated her sweetly and gently. We visited daily at different times and the home was ALWAYS clean and well kept. I grew attached to both the girls caring for mom and the other residents. When Mom went on hospice the director, Nu, worked well with the scheduling and medication management. Nu seemed to be available to her staff at all hours for questions and decisions. The owner, Sindhu, is very nice and caring as well. I have nothing bad to say about Sedro. When Mom passed away, they were with us every step of the way. Mom was cared for and kept safe and we will be forever grateful that homes like this help families like us when it all becomes more than we can do by ourselves.

While there are a couple of very caring employees, they do not (and cannot) provide the majority of the care in a 24/7 environment. Most employees here are inexperienced and the turnover rate is high (even observed within the brief time that I visited). High turnover rates are almost always due to unfair treatment from business owners and it's not a stretch to then imagine how the residents are treated. If you want your loved one to get quality attention and care, look for a facility that is classified as a "skilled nursing" facility; they must meet higher educational and qualification standards than an "assisted living" facility. Also, if you decide this is the place for your loved one and you come to an agreement with the owner about anything, get it in writing. Although she admitted she made an agreement, she said she wasn't going to honor it and didn't 'have to' because she didn't sign anything. That behavior wouldn't be surprising from some business owners, but it's particularly appalling in this line of work. Resources abound on the Internet; I wish I had had more time to research this properly. There are numerous lists on the web of questions to ask when researching living facilities; to them, I'd add: 1) ask questions about the staff, i.e. their education, certification, professional experience, longevity with the facility, etc. - these are the people who will engage with your loved one on a daily basis, so you'll want to know how they are qualified 2) visit unannounced and ask for a tour and 3) look for a visitor and ask them what they like/dislike about the care their loved one is getting.

His room, original bed was broken, ceiling was caving, peeled paint on walls, AC duct had blue painters tape all across it. His prescription of pain meds was supposed to be administered every 4 hrs with food. They did not feed him & did not know he hadn't been fed - and he had to call for meds every time. We were told they didn't have the capability of scheduled meds. What? NO supervision was given for him getting out of bed. NO shower in room. NO Physical therapy. Equipment ( Blood pressure & Pulse) did not work, he had to take it for them on his watch. The "staff" other than front desk and head nurse spoke very little English. We got him out of there as soon as possible. NOT a safe place.

But staff was very cordial to me upon entering facility always. Unfortunately, after my visit , my father developed a bed sore on the side he needed a position change. I was told that wound care was taking care of it in mid January only to find my father being rushed to the hospital on Feb 1st with malnutrition, dehydration, septic shock and upon evaluation at hospital was diagnosed with gangrene in 3 different places a few sores ready to burst and contusions in several places. Hence my father died from gangrene and the bacteria had traveled throughout his bloodstream and taken over all organs leaving my father in pain and to suffer for 8 days until he passed from the flesh eating bacteria. Please if your loved one cannot talk to you and tell you what is going on, please keep them at home safe where they can be monitored with security cameras. As most nursing homes don't have them as I found out .

Their physical therapy staff is always available in assisting all of their residents that are in need of physical therapy care and support services!!! The SPJST nursing home has nutritious meals for all of their residents to enjoy!!! Their residents always have a variety activities to enjoy and keep them busy during their stay at the SPJST home!!! The entire staff at the SPJST Nursing Home is always very welcoming to their guests and helpful in assisting them during their visits!!! Some…read moreof the people there were wonderful, but they are managed by Spring Hills and they only care about the bottom dollar. On many occasions they did not give my father his medication. They did not shower him as often as they said they would. His skin was always in horrible shape. My mother and I were there every other day, sometimes more and I think that's the only reason he got the level of care that he did. They never shaved him. They were understaffed all the time. They don't pour the water to them, so that was the start of my father's decline. When I confronted the asst manager about seeing my father's medical records after his death...it miraculously took 2 hours for him to produce that. We were told by a woman we trusted that the man was in the back editing it to show he had been given all his meds. The first huge red flag was they didn't give Jim his meds the first night there and my mother and I sat in the lobby for an hour trying to get someone to help us get dad his medication. Please don't go to this place or any Spring Hill facility, they don't care about your loved one, only the bottom dollar. You will constantly have staff turnover and never any answers! Oh, and the literal nail in my father's coffin... they didn't call me when they sent him out (his final ride on EMS) to the ER. A nurse that loves Dad texted me asking how he was and I said "I don't know, you're with him". She said "no they took him out last night". I asked where and THEY HAD NO RECORD OF WHICH HOSPITAL!!! So, my father with dementia was brought in with no medical records, no nothing and had been sitting not able to breath and on a CPAP for 8 hrs alone when I was only 30 minutes away! I got there as soon as I found out. We spent the next 30 hours at his beside with him as he died from pneumonia, sepsis, and a heart attack. And we never heard from the director of the facility so much as an "I'm sorry for your loss". We were told we would get our refund in 45 days... it was 4 months later. Awful humans.

The medical team is alert and engaged. Within days, a speech therapist identified swallowing problems and made recommendations to modify her diet. Physical therapists identified diminished core strength and adjusted her care plan accordingly. The nursing team is alert and engaged, bringing us peace and lifting tremendous worry! Gibran Chambers is a "hands-on" Executive Director and he can be seen interacting with both residents and staff throughout the facility! Marvin Smith, Lauren Urbanek, and Kelsey Lang are the reasons we selected Poet's Walk--they responded correctly to the myriad of questions we asked. Further, they went above-and-beyond, working long hours to insure a comfortable and joyful transition between facilities. We can't say enough good things about our experience!
