At first, I didn't like Trellis. My dad was there for a month and he has Alzheimer's. Too many facilities are not prepared to handle these types of patients, however with the rise in Alzheimer's patients they better figure it out fast! I found that Trellis staff actually cared and tried to make it the best experience for my dad. After 6 months my dad broke a hip and is now at Skye Canyon. The staff here are horrible. They do not care, they act inconvenienced, etc. So I pick Trellis all day long over Skye Canyon! read more

My 88‑year‑old dad spent 24 days at Trellis Centennial Rehabilitation Facility after a six‑day stay at Centennial Hills Hospital, where he was treated for malnourishment and severe dehydration caused by a brutal case of shingles. Once he was deemed "medically stable," the hospital transferred him here for physical rehabilitation. Like most patients arriving at Trellis, he came in on a gurney--weak, confused, and suffering from "hospital delirium," a condition I had never heard of until then. He had a long road ahead of him. The facility itself was nicer than I expected: large common areas with plenty of seating, big‑screen TVs at both ends, and a pleasant outdoor courtyard. But the real strength of Trellis is the healthcare staff. They were exceptional. My dad's primary CNA, Aleah, was the standout--truly the star of the show. Watching her care for my dad, I would have believed she was caring for one of her own family members. Even when he was ornery or threw her a curveball, she handled it with patience and grace. His occupational therapist, Raymond, and his physical therapist, Badge, were also excellent. And across the board--from nurses to CNAs to PTs, OTs, nutritionists, and housekeeping--the clinical team was skilled, compassionate, and consistently attentive. They are the reason my dad made real progress. Unfortunately, the administrative side of Trellis is a completely different story. From the moment we arrived, we were handed a stack (well, an electronic stack) of forms. I understand the need for paperwork, but the entire process felt designed more to protect the facility than to support my dad's recovery. The biggest issue involved my dad's macular degeneration treatments. He receives eye injections every seven weeks, and one of those appointments fell during his stay. I started trying to coordinate it a week in advance. His doctor's office explained that because he was a resident at Trellis, Trellis--not Medicare--had to be billed. If they billed Medicare, it would be denied. What followed was a frustrating loop of unanswered messages, unclear answers, and administrative runaround. Despite my efforts, I ultimately had to cancel the appointment. As we approached the 20‑day mark--the point at which Medicare stops covering 100% and the patient becomes responsible for $217 per day--the business office suddenly appeared to say my dad was "nearing his baseline." Interestingly, none of his healthcare providers agreed with that assessment. Still, after five additional days, the administrative staff declared he had reached maximum improvement and discharged him. My dad and I were both uneasy about it, though it did allow me to get him to his eye appointment that same day. Now, nearly two months post‑rehab, my dad is doing better than he was before he got sick--despite the admin team insisting he had "baselined" long ago. Go figure. I do want to acknowledge Julie at the front desk, who was consistently pleasant and helpful in navigating the administrative maze. In the end, I owe a tremendous debt of gratitude to the healthcare workers at Trellis. Their skill and compassion got my dad back on his feet. Outstanding Care and Support from the Staff at Trellis Centennial I cannot say enough good things about the incredible team at Trellis Centennial. From the moment we walked through the doors, the care, professionalism, and compassion shown by the entire staff was evident. I want to especially recognize Jasmine, the Business Manager. She went above and beyond to help us navigate the financial and administrative side of care -- something that can feel overwhelming in a stressful time. Jasmine was not only knowledgeable, but also kind, patient, and incredibly clear in her explanations. She made the process smooth and took a huge burden off our shoulders. Her compassion and empathy is one of a kind and Trellis is so lucky to have her. Thank you so so much! Equally impressive was Anicka, one of the nurses on staff. Her warmth, attention to detail, and dedication to her patients stood out every single day. The physical therapy department is top notch in my opinion. I must give fair warning , if you are a diabetic, I would not recommend this facility. In my first review I thought that this was an outstanding facility. However, reality set in. I had items from my personal drawer liberated by my nurse without facility or my authorization. When confronted the nurse said that she was not on shift when she clearly was. The facility does have staff that truly care about their patients. Those individuals are a nurse named Charles charge nurse Karen, a cna named Emily and activities coordinators named Melissa, Josette and Breanna. The therapy department as a whole is one of the best that I have ever had the pleasure to work with. I would list them all but that list would be very long. I still recommend this facility but I would encourage families to visit often and challenge things that either seems off or outright wrong. I hope that this review helps! read more

I feel like patients get ignored if they are bed bound. The deteriorating condition is not noticed by anyone until it is too late. My mom was too weak to feed herself due to a broken wrist and it was only 1 or 2 CNA's that helped her eat. Most of the time she did not eat or drink. She developed pneumonia and nobody took action. You need a serious advocate if you stay here. It is clean but you will get the bare minimum care. read more

My family and I are so thankful for Trellis Centennial rehab and all of its departments. From the moment you step in the door, you feel welcomed and valued. Not only is it a beautifully aesthetic rehab facility, with a lovely sitting area, but you are greeted so beautifully at the front desk too. There is even a little coffee bar, with table and chairs for guests visiting loved ones. My mother had a recent stay at Trellis Centennial for almost two months. Her care was so exceptional and consistent. From dietary needs and physical therapy, to the activities, doctors, nurses and CNAs, one word... "EXCEPTIONAL" My mom's care was exceptional, which helped her thrive and heal. My mom made such wonderful friends at Trellis Centennial, she hated to leave. Kristy, the director was beyond helpful and accommodating, starting with my very first phone call. She answered her phone each and every time I needed to ask her a question, or seek her help. She even took my calls and texts while she was on vacation. Kristy is very professional, courteous, motivated, enthusiastic and most importantly... compassionate. The same is true for her entire staff. You could not ask for a better rehabilitation facility. My mom loved it there so much! If I could leave a negative for this place I most definitely would. I was denied my medication (she also told me that I could ask all I wanted and that she wasn't going to give it to me) I then went asking for a supervisor, my nurse "Jessica" replied, "Go ahead, she's on the other side of the building," This was said viciously, as she knew I wasn't supposed to be walking (reason I was in there after a surgery). A female admin came into the room with an in-training gentlemen, named Eric (whom was very professional) and he was also the person that reported several instances of neglect, abuse and bullying I experienced while there. Later on the female admin came into my room and was raising her voice, then started hitting her chest while telling me ," You reported me to MY supervisor!!". She went into telling me that I was wrong about my medication she spoke so fast, while raising her voice that I could barely understand her. (Funny thing was that I wasn't the one that contacted her supervisor (Adam), it was the gentlemen "Eric" , whom was the Admin in training). This female administrator literally made me feel not safe, between yelling, and later entering my room alone, hitting on her chest and telling me I was wrong. I wouldn't let these people take care of my dogs. First impression the place looks great; make no mistake, it is not! I left the facility the next day because I was afraid of being abused and had no way to defend myself. Im now recovering at home with home health care. Do yourself a favor and don't put any of your loved ones here, just please, pass it by and find another facility. Healthcare workers like the nurse Jessica are a detriment and shouldn't be permitted working in healthcare. (I do want to add that she became very condescendingly accommodating after being spoken to by her supervisor), but I heard afterwards talking about me with another nurse after the fact (it wasn't nice). Ultimately it was a horrible experience... they lied and held my meds, saying that my meds weren't transmitted from the hospital (which wasn't true, per Adam and that I was correct about medication times). There is too much to write (with the exception of unapproved (by my physician/surgeon)physical therapy on my shoulder and arm , which if performed could have done extreme damage. Last words, "Adam" gave us his "personal phone number" (I think to shut us up), we tried calling several times which would go to a filled voicemail box and several texts went unanswered. Covid doesn't prevent them from being able to properly bath and clean after diaper changes, etc. My mother came home with the worst bleeding diaper rash and peeling skin all over her body because of neglect NOT COVID!!!!! My mother was transferred to Trellis back in April of 2024. She had two compression fx of her lower back, along with diabetes, heart disease, mild dementia, and stage three kidney failure. I thought it best to observe the facility for a while, get to know the staff, the physical therapy, and the caseworkers before forming an opinion. The first thing on my pro list was the private rooms. ( the last time she was in a rehab, her room mate was a problem). They were also accommodating, let us bring in her recliner from home, and I learned I could have had a private CNA of my choosing come in to do AM care and PM care.( wished I had asked sooner). Her day time CNA "Camilla" was wonderful. She gave my mother shower, she was patient and did not rush her. According to my mother, she was better at it than me, ( I am a retired LPN with 30 plus yrs of experience) Lol. The physical therapy team did a great job, especially the speech therapist "Tracy" she picked up on a swallowing issues that was causing the pneumonia on and off for months. Tracy was able to get a barium swallow test ordered and it showed silent aspiration. Since we have been adding a "thickener" to the fluids she drinks, no more pneumonia. My mother may have had a totally different impression, she would panic if the CNA/nurses on the night shift did not answers the call bell night right away. She would tell me see waited hrs and hrs. I do not know if that was accurate or not since her confusion was worse while she was there. I did see one CNA put my mothers untouched food tray back in the rack, and tell the RN my mother "ate well"( this CNA was not caring for my mother, just collecting trays) I did pull the RN aside and tell her..... In conclusion, I will say Trellis was so much better than the last rehab she was in, but like everyone else, and every other hospital and health care facilities, they need more CNAs and more housekeepers. Not that housekeeping did a bad job, but seniors can be messy, they spill things, a lot. The social worker was very approachable. My review just touched on things that I look for, everyone is different, and has different expectations. If I had to re-admit to a rehab, I would chose Trellis again. Hope this helps. read more

My mother stayed at this facility for rehab work due to back injury this summer. The staff was very friendly, services such as PT,OT, nursing, and CNA were all good. Some really remarkable caring PT/OT staff at Trellis. Care plan was organized and we were kept informed /good communication. The room and facility were clean and organized. My dad was here for a month after a fall. The staff was very helpful and it was very nice to have a private room. We live out of state and we had many interactions with Julie, Annabelle, and Nicole G. They were very helpful with navigating this challenging process. Thank you, Annabelle, for all your help with Dad's discharge. It was the best planned discharge he has ever experienced. His home health care was contacted and his medications faxed in advance. Annabelle is a rock star! Also, Preet, my Dad's nurse, was excellent, caring and attentive. My one recommendation would be that it would be great if the patient rooms had phones. It's difficult to be out of state and not get to talk to your loved ones. They do have a communal phone that can be shared, but it isn't always available. My Dad said the food was good and again, it was nice to have a private bath in your private room. Also, shout out to Michelle who was very kind to my sister when she called about my dads lost glasses. My dad has had to go to Skilled Nursing Facilities in the past, and they were awful. If you care about your loved one, avoid this facility at all cost. Do not let the new exterior façade and friendly front desk staff fool you. The staff at Trellis are incapable of monitoring and handling their patients. Period. My father stayed at Trellis for just under a week. In that short time, his health deteriorated very quickly. He asked to see a doctor on a Friday morning, but the facility doctor did not come to his aid nor even see him. Friday thru Sunday he went without seeing a doctor, even after numerous requests to the Trellis staff. Throughout the weekend, he developed a serious infection, his lungs filled with fluid, and he was experiencing severe chest pain. On Monday morning, we had to immediately remove him from the facility and rush him to the ER. My father was not monitored at Trellis, the staff had no idea an infection was taking hold of him, nor did they notice when his neck and legs began to swell with fluid. I filed an official complaint with Trellis and have yet to hear back from them. After all this, a Trellis employee called me the evening we rushed my father to the ER and said, "A strange woman took your father and we don't know where he is or who she was." That woman was my sister who begged the Trellis staff to help him and they did nothing. read more

I'm going to start this review by saying I was skeptical at first I truly thought this was to good to be true. Over two weeks ago a dear friend was about to be discharged from the hospital in Henderson with a diagnosis of terminal cancer. We spent days searching facilities in Las Vegas to move her into. After visiting several and being severally disappointed I contacted Kristy at Trellis. From the moment she answered the phone, through today as I write this review over two weeks later our care has been top notch. Those reading this review have undoubtedly seen the others listed here. I am probably one of the most skeptical when it comes to family health. When we arrived at the facility we were met in the parking lot by Kristy, Danielle and Kailey. From that point forward the level of service our family has received has been nothing short of first class. Let's face it as a business we are only as good as our staff. When working with the sick staff too will get sick have we seen short staff days yes, has the staff present stepped up to the task YES!!! Rosie, Peter, and Doris have gone above and beyond. If life isn't bad enough we came in from the hospital with Covid, would have been nice for the hospital to test prior to release. None the less at no point did we feel like an outcast. After two weeks of mindfully watch two of us felt comfortable enough going home for a few days, upon return we weee greeted by name and when asking our family member how the week went we were met with joyful tears about how well she had been treated over the last 4 days. Let's face it none of us ever want to face this situation but if you or a family member do this is the place!! We are driving 35 miles from one end of the valley to the other and could not be happier to do so!! Everyone from the front door to PT, Nursing, to the Housekeeping staff have been amazing and have made this experience one for the record books!! Judging from the other one star reviews, my initial impression is spot on. I called on several occasions to get information about the director of nursing to report a safety violation I witnessed at their facility. The receptionist, Berenice, was extremely rude and proceeded to inform my wife and I that no manager was on duty, in order to evade our complaints. When I entered the building, I was not greeted by the front desk staff. When I proceeded to use the restroom, I noticed an extended area of flooding, beginning from the toilet to the door of the restroom. The pungent. odor of urine and fecal matter permeated through the building. As a state licensed nursing home, the behavior demonstrated is in violation of patient rights and NOT conducive with a caring environment. I wouldn't trust a dead roach in the care of this negligent dump of a facility. Don't let the newer building fool you. I will be reporting this facility to the State Ombudsman for falsifying information in order to evade a complaint. read more

Was there in 2020 for four months and it was wonderful they took care of me and both of my knees, and got me to where I could walk. The food was it was great. The help with the physical therapy was wonderful. They transported me to all my appointments and I was just back there again for a leg with three broken bones and they treated me the same way. If they did the first day I was there I would recommend Nurse to anyone if you want to get better. read more

My mom transferred from Summerlin hospital to Trellis and was there from late night July 30th to August 11, 2022. Ms. Eikes, Admission person was very nice and helpful. She always helped us in what ever she could, or direct me to the right person to talk to. Norma the Activities person is very nice and very helpful! When we got to the facility I spoke to both night nurses about her meds, care and any other info I knew it would be helpful for them to know. I made sure to be there early next morning so I could talk to the day nurse as well. I've experience in the past that youll need explain everything again to the day nurse. Unfortunately, when I got there in the morning the receptionist Julie, was not nice or helpful, I didn't feel welcome at all! I asked who would I speak to about my mom's meds. She told me to check with nurse, I did and the nurse sent me back to Julie the receptionist! No one definitely knows what they're doing. They do not enter information in the patient's files, so you have to keep repeating yourself and hope they get things right!! The first day I didn't know who was the CNA assigned to her, no one came to introduce themselves at all. Three days in a row I went first thing in morning and all 3 days my mom wAs soaked wet in urine!! Even thought I kept telling the doctors about my mom retaining urine and needed to be on a straight catheter or the foley, this was never done so my mom end up in the hospital again! We kept having problems with the medicines and then I found out that they were giving her medicines that we were not aware of! You trust this places to take care of your love ones and you should be able to go on with your normal daily responsibilities. Unfortunately we found we couldn't do that so I was there just about everyday! There's so many things we were unhappy with this facility but it would take me too long time to go into more details. We took my mo out of there before she was discharge by them. There was a different nurse just about everyday. This was our experience, so I wouldn't be able to recommend this facility at all. I honestly hope others have a better experience or Trellis improves their services to their patients. (Sorry for any grammar mistakes) read more

If I could leave ZERO stars I would. This place is an absolute joke. If you are not close to being independent do NOT consider this place for rehab. They have no ability to help anyone who needs assistance. They are left to be in a bed with no help and zero plan to help them improve. Patients will sit in their room with a call light on for hours and no help to ever come. Nurses will not help and their answer to anything is that's the CNA's job. The charge nurses point at the nurses and no one wants to assist. My dad was left multiple times with No Call light near him and left to lay in a wet bed. He left the facility with wounds bigger than he was admitted with and sores from falls they are 100% responsible for. read more

Terrible communication and follow up. Due to covid my mother in law can't have visitors. I will leave politics out of this. My wife has asked 5 times to get on the approved communication list. Hopefully the fifth time works. Over a week there we still don't have information regarding her surgery or pathology reports. Nobody there seems to care about the families. We have no idea what's happening. One time, a staff member said "Why don't you ask your mother what's happening?" This was just days after major surgery and she is 81. She was very confused and didn't know any details. I will not recommend this facility especially during the lockdown. read more

Patient Consult crew is a joke. They did NOT give me all the options available for my father. They refused to accept At-Home Hospice (you do not have to be terminal for Hospice!), even when confronted with facts. Saying they 'were not able to give this as an option'. When I did choose hospice, a doctor to whom had never visited with my father (I was a helicopter daughter) went to my father and told him he should not leave. When I confronted this man, he refused to give his name, only saying he "was a doctor". When my father got home, the amount of sores & open sores on his behind was horribly cruel. He literally was bruised from his knuckles to his shoulders. When they came to get his BP, did they use a cuff machine? Nooooo. They ripped off the bandages that were placed on both arms and proceeded to take his BP to the screams of my father. I inquired to the nurses, as well as the administration why I see staff go from patient to patient or intermingle with patients, nurses, doctors - and NOT wear a mask! I complained that the Chef refused to wear one. When I confronted him, he snapped his mask, which he kept under his chin, and said "I am!". This man often seen hacking as if coughing up a lung, had absolutely no regard for health of others and flaunted this as he was often seen talking with the very administration to whom I complained. My 94 yo father got COVID. He should've had his bed at an incline, to ward off fluid hitting his lungs. This was not done & he got pneumonia. I'm sure there are multiple ways to get COVID as well as get pneumonia. There is no reason why he had to be in constant pain with all the sores on his behind. read more

My brother has been here for two months recovering from a stroke. Everyone here has been wonderful. The food is amazing !! I showed a picture of it to my daughter and she said OMG where did you eat that and I said that's my brothers dinner trey! she wanted to go to the restaurant that had that!! every meal has been over the top, so unusual for these facilities. read more

I went in for help after hip Surgery. I stay at home by myself and had no one to help me. At first this seemed to be very nice facility! But after 6 days of being uncomfortable in a bed that now l have back issues do to them not giving me a bigger bed and only having one recliner.The Dr's ,Occupational and Physical Therapist are Fantastic!! As Well as All the CNA l came in contact with.. But l told a CNA l wanted to be discharged and l heard the Charge Nurse refuse to help me saying it 7:00 soon l am doing nothing!! Wasn't until l ask for a complaint form l got any assistance!! I was going to let it go.. But lt didn't set right with me.. One bad apple ruined entitled Charge Nurses at Night!! And when l asked one Nurse to help me in bed she became irritated and said l gonna have to get somebody else at that point l said don't worry!! And when asking to be discharged l was asked why?? I had a 20-day stay at Trellis Centennial in April, 2021 after hip surgery. During my transportation to Trellis from the hospital, conversation with the attendants led me to believe that this was a top notch facility. A few years back, I had a knee replacement and my rehab was at Silver Hills. The driver told me that Trellis was a step-up. Unfortunately, I don't agree with assessment. I have to rate them even. Although, I met some nice people at Trellis (especially the therapy group) several of my other contacts could use some friendliness training. There were also times, I had to remind CNA's to attend to the necessities of being bed ridden. After a few complaints about keeping room and bathroom clean each day, the message got through. I found the bed very uncomfortable and I would have thought phones would have been provided as well as a means for the patient to turn on a night light if needed. read more
