    My husband was evaluated by Dr. Zahra Shahim on January 16, 2026, after developing rapidly…read moreprogressive bilateral leg weakness and losing his ability to walk over just a few days. Despite this presentation, the referral from Dr. Shahim to Neurology was classified as "standard" rather than urgent, with the initial neurology appointment scheduled for March 5. Although the appointment was later moved to January 29 after follow-up, it still did not reflect the urgency of his rapidly progressive neurologic decline. On January 20, 2026, I returned specifically because Steve's rapid loss of mobility over only four days deeply concerned me. I explained that this degree of deterioration did not seem normal and asked whether his referral should be escalated or whether additional action was needed. I was advised that the current plan remained appropriate and that he should go to the Emergency Department only if his condition had worsened since the January 16 visit. Because I remained concerned, I immediately took Steve to the Emergency Department. He was evaluated and unfortunately was discharged without admission, neurology consultation, or neuromuscular workup, and was instructed to keep the neurology appointment already scheduled for January 29. At that appointment, Dr. Frank diagnosed Guillain-Barré syndrome the same day based on his examination and EMG testing. Dr. Frank commented that he wished he had seen Steve sooner. Rapidly progressive bilateral weakness with loss of ambulation should prompt URGENT neurologic evaluation regardless of the eventual diagnosis. In his case, the diagnosis was Guillain-Barré syndrome--a neurologic emergency that can progress rapidly and, in severe cases, lead to respiratory failure requiring mechanical ventilation. Early recognition and timely initiation of treatment are associated with better functional outcomes. He ultimately received IVIG treatment. More than six months later, he continues intensive neurological rehabilitation, still requires assistive devices for walking, and continues to experience high level of pain. At a follow-up visit after Steve's hospitalization, Dr. Shahim commented that she was glad the Guillain-Barré syndrome had not progressed to involve his lungs, noting that some patients require mechanical ventilation. We couldn't believe what we were hearing and I responded that this was precisely why I had returned on January 20 asking that his rapidly progressive neurologic symptoms be reconsidered and his referral be escalated. Instead of being empathetic and apologize, Dr Shahim raised her tone and insisted she acted correctly. We left that discussion feeling that our concerns regarding the urgency of his presentation had still not been acknowledged. We lost our confidence in Dr Shahim.
