I've lost over 20 pounds in just two months, thanks to her expert advice and encouragement. She takes the time to listen and always has great suggestions to keep me on track. What really sets this place apart is that it's also a spine and rehab clinic. Dr. Groelz even helped me address some posture issues and an old exercise injury. Dr. Austin has been amazing in treating me. I feel better, stronger, and more confident than I have in a long time. This has been such a fantastic, life-changing experience. It's as much a physical therapy office as it is a location for spinal adjustment. While I do admit the adjustments are only above par (but not excellent), there is much more that is offered here. Disappointed. Hygiene Neglect & Medical Mismanagement…read more We are disappointed in the treatment that our 85-year-old mother received at Pam Heath Rehabilitation Hospital. She is a vulnerable patient with a complex TBI, mobility issues, and incontinence. We trusted this facility to rehabilitate her; instead, they neglected her basic human needs. * Hygiene Neglect: Our mother remained in the same soiled top clothing for an entire weekend. Despite being incontinent, she was also not bathed by staff. The only shower she received during her stay was given by our own private family aide who had to step in during a visit. Unsurprisingly, she was discharged with a UTI. * Medical Oversight: They placed her on 24/7 oxygen and new medications without consulting her family doctor or us. They completely ignored her medical history and her actual respiratory needs. * Therapy Concerns: There is a lot of "fuzzy math" regarding the required 15 hours of therapy. Therapists consistently showed up late and brought her back early, yet these sessions are billed as full hours. It felt less like rehabilitation and more like a money grab. * Lack of Communication: Staff were unable to answer basic questions about her care or progress. Advice to families: If you live out of town or work full-time, stay away. Unless you can sit in the room 24/7 to "babysit" the facility and ensure basic hygiene and a therapy plan is met, your loved one is at risk. We deeply regret sending her here and hope this prevents another family from going through this type of mess.

Their website shows they provided language translator for their patients . We had to repeatedly have to ask for the nurses to use it but they wouldn't. My mom would have to call us at home or at our jobs to translate what the nurses were saying. She was giving the wrong medication at one point . The room had a leak in the ceiling the bed she had would only go down. The communication from doctors was not great either.

I can guarantee you that if anything were to occur in the future, we will never step foot in your establishment again. Please see the letter I wrote to the head honcho, with absolutely no response whatsoever. This was also just the tip of the iceberg. Throughout this stay, there have been a multitude of issues. To be quite honest, it was very disconcerting. That being said, none of it was towards the actual care team. Everyone that I personally encountered were extremely kind and helpful. This is where we are going to get into the details. Your Director of Case Management, Karen Cook, is absolutely wretched at her job. I'm not going to mince words. I believe I have spoken to her, via phone, twice. Please keep in mind that one of these was during the care conference on September **th, two days after admission. The other was prior to admission. She wrote in an email that she tried to call me a few days later, and that she left a voicemail. Surprising, as I received no missed calls, or a voicemail in the time period that she said she called. Not once did she try to call back. As stated, the only time I received a phone call from this woman was within the first two days of my mom being there. The care conference included her, and at least one therapist. Since then, I have received ZERO updates from any therapist of any kind. Not only that, but some of the emails received from Karen came across as condescending and rude. So, that favor may have been returned. She is absolutely not clear about anything. Today, I picked my mom up to return her to *********, where she resides in the memory care facility. I didn't get a care conference of any kind. Was handed papers to sign by the nurse (who was very nice), and basically shoved out the door. I do have all emails from Karen, if you would like me to forward them. I am beyond appalled. NO updates on therapy!? Nothing?! Phenomenal. Now, I will say my mom is a hard headed broad that thinks she doesn't need sort of medical device to help her move around, when she absolutely does. That being said, I received 5-6 phone calls stating that she had fallen. Oon 3 or 4 of those, she was ok. Another one, a portable xray machine was called in. One was when she was transported to the ER for multiple tests, of which a diagnosis of a hematoma was figured out. On Tuesday, I came down to see mom, and curiosity got the best of me. I asked the nurse to verify how many times mom had fallen. She looked it up, and guess what.........only 2-3 (I cannot accurately remember how many she said, because I was so baffled), were actually documented!!!!!!!!!!!!! WHAT!? So she fell a multitude of times, but only thought to document a couple of them? That is asinine. So, to say the least, I am writing this so you can see what is going on in this business. This is absolutely deplorable. These are family members. These are loved ones. Would you be ok with this happening with a member of your family!? Especially an 85 year old woman with dementia/alzheimers!? Yeah, me either. Thank you for your time.

My doctor, affectionately called "Dr. E." is among a small handful of my very favorite physicians in my 80 years of life. The nurses and CNA's were all kind, helpful and quickly responsive to all of my call lights, requests for help and questions. My physical and occupational therapists were wonderful and enjoyable to work with. Far less important but still greatly appreciated is the physical environment. Bright and cheerful, lots of windows. If I ever need the services of another rehab center in the future I will ask I can go back there. I am sure there is no better facility. I recommend the Center at Northridge in Thornton Colorado to anyone that needs a skilled nursing facility to help them heal.

So this is bittersweet because my experience is both positive and negative but very real…read more So I did detox and then stayed and graduated the 30+ day ( length of stay varies on each person and the case manager apparently) Its a decent enough establishment and most of the staff is genuine and caring ( there are a few with serious attitude issues and are very unprofessional as well) the food is pretty good and the chef is awesome. The place is clean and the atmosphere itself is not to bad. I dealt with some issues throughout my stay but the stay itself wasn't the issue. The issue was my discharge. I went into this program to get sober and change my life, however, mainly for the scholarship to get into sober living upon graduating. Let's just fast forward to the end. A week before I get out I met with my case manager and relayed what sober living I wanted to go to. ( it was not any of the ones they partner with, which I think was my problem) I tell my case manager where I wanted to go and she immediately grimaced and things changed from that day on. I picked a sober living I thought looked like my cup of tea and upon my completion and release I show up at this sober living (tribe) and found out they are no longer a "sober living" now had the Foundry really planned on sponsoring me or cared in the first place they would have known this and helped me course correct. I call them and they scrambled and luckily sent me to ASL sober living. They then apologized and ( on speaker phone in front of my wife and Father advocate with Adam's county promised me not only a 1 month scholarship, a second one for the mishap) Fast forward 2 weeks and ASL is already asking me for rent money. A pretty big amount! Confused I told them about the sponsorship from Foundry and after 5 days of calling the foundry ASL, (and the foundry) point blank told me they would not be sponsoring me!! Left high and dry. I've called foundry 11 times, left messages for my case manager, left messages for staff, my therapist pretty much anyone at this point and the foundry completely ignored all of my calls. ASL (not the foundry) sponsored me for 2 weeks during this time I continued to call and call and foundry still ignored me. Fast forward 2 weeks with no response from the foundry ASL puts me on rent restriction and punishes me for lack of payment! Not their fault, they didn't really try to help me either after the 2 weeks but because of the foundrys EXTREMELY misleading, false promises and down right lying to me I'm forced to leave the sober living! Now im homeless! I call yesterday to try to plead with the foundry to help me as they promised but now my phone number is flagged because the lady who answered point blank says "Michael, last time you called it was about sober living funding which was denied!!! Is that why your calling?) ARE YOU FREAKING KIDDING ME!?!?) so I made a phone call last night and spoke with a sober living who actually cares, within the 15 minute phone call gets me a 30 day scholarship and says I can move in monday!! Just like that. All I needed was some help! The foundry used me! Got my medicade $ and got paid for my stay there and lied to me by saying I wouldn't get a scholarship unless I completed and even after I stayed and COMPLETED 35 days, they then cut all contact with me left me out to dry and didn't hold up their end of the bargain. Insane! They are a bunch of liars, manipulators and do not care about you. Well, they sure didn't care about me. I would be very careful going to this establishment! They used me. They got their money for my stay and then upon completion didn't lift a finger to help me transition, even worse they cut me off and ignored me all together. They used me! Be careful!! Do your research and do not let them take advantage of you as well. This place can go to hell.
