Luckily they had an opening and I moved heaven and earth to get him in there because I could see right away how caring they were. The place was spotless and lovely and they made beautiful homemade healthy meals. Tom was there 5 years.I am convinced he couldn't have received better care anywhere else. I am eternally grateful. Now I just spoke to the owner who shares my birthday..she called me to wish me a happy birthday. She told me they currently have 1 opening. If you are looking for this kind of care for a loved one, don't hesitate to call them. They usually have a long waiting list. It is a difficult decision but when I put my husband there I knew it was for the best. I simply could not have provided him with the specialized care he received at Gentle Home Care.

Visits were unpredictable, calls sporadic and orders for supplies a complete nightmare. There was always the feeling that staff would drop in for a few minutes to be able to bill Medicare. It was very apparent that this was a FOR PROFIT outfit. The worst part was how they totally let us down when my dad was actively dying. The nurse (not his regular one) had to leave because her shift was over. No one else was sent to help us deal with this difficult experience. Afterwards we heard NOTHING from Heartland management even after making our feelings known to the regular nurse. CHOSE ANOTHER COMPANY FOR HOSPICE.

Kaiser must give you a referral to all hospice groups in your area. Ten minutes later I get a call from an Heartland agent who wants to come immediately and sign my husband and I up for Heartland Hospice. I ask what services will be provided and he states an RN will come every two weeks. I ask about bathing and he states they will teach me how to do it. After a few more questions with even more unsatisfactory answers, I actually say "pardon me, but what benefit is Hospice to me if I continue to try to do it all myself?". His reply is they "manage pain", not an issue in our case. No thanks. Weeks go by and we continue to provide daily care as best we can as a family until a crisis puts us in the ER. They hear our unsatisfactory connection with Heartland and suggest Hospice by the Bay, stating it is imperative we get Hospice help to assist in this process. Hospice by the Bay came immediately, delivered drugs to the house that night, provided equipment to ease his care and both an RN, social worker and, even a health aide to bathe him, all showed up the next day. Turns out Heartland is a FOR PROFIT hospice run be a national health group. They less they do the more they get to keep from the Medicare daily stipend for hospice patients. Hospice by the Bay is a non profit, aided by volunteers and truly encapsulates the hospice experience. Run for the hills far, far away from Heartland. I can not stress this enough. PS: I notice no bad reviews on Yelp so let's see how long this review stays up. There is also no option to notate a Comments section of their website. Just Google profit vs non profit hospice and you will be amazed and alarmed. He had been quite resistant to moving from his independent living facility in the south bay, but it was clear he was not getting the care he needed there, even though we were paying for extra services. After an illness and 10 day hospital stay, we knew he couldn't go back there and are delighted and grateful that Creekwood had an opening. They were flexible about his recovery needs and the transition went very smoothly. He is very content and calm and is actually showing signs of cognitive improvement now that he has all of his needs met at Creekwood. The facility is always clean, and the kind staff is wonderful! Everything is included in the monthly rate: help with showering, dressing, shaving, bathroom help, medication administration, incontinence supplies, so you don't have to piecemeal individual service costs, which makes things so much easier. The best part is I have the utmost confidence that all of his needs are being met and he truly feels at home now.

In late October of 2025, our family was scrambling to find a hospice care provider for my mother in law. Finding a reliable hospice provider for a loved one is a daunting task often made under stressful circumstances. Our family, after extensive searching and disappointing recommendations from other sources, found Sonata Hospice through positive online reviews on Yelp. This review comes from a genuine family who utilized their services for a critical 10-day period. The level of care provided by the entire Sonata team to both our loved one and our family was amazing and far exceeded our expectations. We felt completely confident that our loved one was in excellent hands, and we as a family felt fully supported throughout this difficult process. Several team members deserve specific commendation for their outstanding professionalism and genuine compassion: Rachael (Nurse/Case Manager): Rachael was outstanding. Her work is a true blessing and a calling. She provided expert clinical care while simultaneously offering immense comfort to our family. The attentiveness she provided our loved one was consistent with the level of care one gives their own family. Her clear communication, explaining procedures, answering questions, and involving us in the care plan was a model of professional and empathetic care giving. Marsha (Home Health Aide): Marsha treated our loved one with dignity and affection, like she was caring for a member of her family. Her hands-on care was superb. Maria (Chaplain): Maria brought much-needed peace, love, and spiritual support during a challenging time, helping us navigate the emotional and spiritual aspects of the process and providing our loved one with that extra level of care. The Office and After-Hours staff were consistently responsive and efficient in meeting all our immediate needs. The support they extended through the difficult moment when our loved one passed, provided ease and compassion during our time of grief. And our hearts were warmed when at her funeral we saw Rachael and Maria present and still supporting us. Deciding on end-of-life care is perhaps one of the most significant decisions a family can make. Concerns about the quality of care, trustworthiness, and prioritization of patient well-being over financial considerations are paramount. Sonata Hospice put those fears to rest. They are a trustworthy, compassionate, and highly professional organization that we wholeheartedly recommend to any family navigating the hospice journey. Training was comprehensive and organize, and I never felt like I had an unanswered question. And for what's hard to teach on paper--how to experience and share compassion for others--is embodied by everyone on the team and taught by example. It's hard to find a team of caregivers and supportive staff to exhibit all this while being fun and professional to be around, and it benefits both myself as a volunteer and even moreso the patients and their families that we serve at our best. I'm truly grateful to be a part of this team and look forward to continuing serving with them.

    I would strongly recommend looking elsewhere for hospice care. Continuum Hospice added stress to an…read morealready stressful situation--my mom passing. Care was inconsistent from the start. After enrolling on a Saturday, no nurse visited until Friday, leaving us on our own for five days. The nurse who did come called (with poor reception) to suggest stopping all medications, then never followed up--I had to track down the physician myself for guidance. I wasn't given the blue book--"Gone From my Sight" that explains the dying experience until the day before she died. Communication and clinical judgment were unreliable. When she had a severe reaction to morphine, it was dismissed by a nurse, and I had to escalate to the physician to get her medication changed. The day before my mother passed, I was told she was not transitioning. She died the next day. There were also clear signs of understaffing. They repeatedly offered help with bathing, but no one showed up for weeks--too late to be appropriate. After her death, the nurse who arrived to declare her death had her name wrong and lacked basic information we had already provided. So I was having to correct the spelling of her name and get my phone to give her the name and phone number of the mortuary. I had given this information to the social worker so it should have been in their system. A small thing one could argue. But did not make me feel cared for. Compassion for my mom passing was absent. It was totally transactional. I regret not changing providers earlier. I tried to initiate a change, but the paperwork was not sent in a timely manner. If you are not happy with a Hospice change at the first sign of trouble as transfers once someone is close to passing are more difficult. I should have been able to focus on my mom and dad during these difficult final weeks--not manage gaps in care.

The nurse (Allison) at…read moreVallejo Cogir and I talked to the head nurse (Angela) at Continuum on the phone twice - once at the hospital and once when my mother was brought to her apartment at Cogir. Angela agreed to provide servicing of my mother's medication throughout the night every four hours. We even discussed the hours that they would provide care during the night. Angela put none of this information in writing. This organization and head nurse cannot be trusted to do what they say they will do. Continuum says we did not call for help throughout the night. Of course we didn't because we were told by Angela they would cover us.
