I would strongly recommend looking elsewhere for hospice care. Continuum Hospice added stress to an already stressful situation--my mom passing. Care was inconsistent from the start. After enrolling on a Saturday, no nurse visited until Friday, leaving us on our own for five days. The nurse who did come called (with poor reception) to suggest stopping all medications, then never followed up--I had to track down the physician myself for guidance. I wasn't given the blue book--"Gone From my Sight" that explains the dying experience until the day before she died. Communication and clinical judgment were unreliable. When she had a severe reaction to morphine, it was dismissed by a nurse, and I had to escalate to the physician to get her medication changed. The day before my mother passed, I was told she was not transitioning. She died the next day. There were also clear signs of understaffing. They repeatedly offered help with bathing, but no one showed up for weeks--too late to be appropriate. After her death, the nurse who arrived to declare her death had her name wrong and lacked basic information we had already provided. So I was having to correct the spelling of her name and get my phone to give her the name and phone number of the mortuary. I had given this information to the social worker so it should have been in their system. A small thing one could argue. But did not make me feel cared for. Compassion for my mom passing was absent. It was totally transactional. I regret not changing providers earlier. I tried to initiate a change, but the paperwork was not sent in a timely manner. If you are not happy with a Hospice change at the first sign of trouble as transfers once someone is close to passing are more difficult. I should have been able to focus on my mom and dad during these difficult final weeks--not manage gaps in care. read more

My mother received hospice care in Vallejo with Continuum Care North Bay. The nurse (Allison) at Vallejo Cogir and I talked to the head nurse (Angela) at Continuum on the phone twice - once at the hospital and once when my mother was brought to her apartment at Cogir. Angela agreed to provide servicing of my mother's medication throughout the night every four hours. We even discussed the hours that they would provide care during the night. Angela put none of this information in writing. This organization and head nurse cannot be trusted to do what they say they will do. Continuum says we did not call for help throughout the night. Of course we didn't because we were told by Angela they would cover us. read more

My friend recently lost her mother, and she was very disappointed by Continuum. The caregiver they sent out would show up late, then talk about not wanting to be there at all. They ordered numerous items (like hand cream) that my friend did not want or need, then sent a large bill to Medicare. There was very little compassion either to her and her father, or to her mother who was dying. Not recommended. read more

Hospice In Home Care was provided for my husband, I called and spoke to Angela, she listened to what was going on with my husband, accessed his condition and had an Intake person-Sybil come to my home that night. Sybil went above and beyond to get him medicines needed asap, she spoke with him and helped felt comfortable by explaining what they were going to do to help him and it really helped him relax. Siraaj the Chaplain came by the next day along with Angela to show support and provide things he would need. Sybil really supported the family throughout each day and even after he passed. They were very compassionate, helpful, caring, knowledgeable with helping know what was going to happen next. If I had any concern I could call and Nurse John was able to give insight, assure me what was happening and how to deal with it. Unfortunately, he was in his last days of life and they really helped out to make sure we had what he needed even after his passing with support for me, his wife and 3 kids. I highly recommend using Continuum Hospice & Palliative Care. read more

Continuum Hospice took care of my Aunt while she was in her last weeks of life in Corte Madera, CA. I was very impressed with the compassion, kindness, and comfort provided my Aunt and myself. Social worker Lisa, Nurse Dana, Health Aide Blanca and Chaplain Blane went out of their way to be of service to my Aunt and our family. I was impressed with their knowledge, expertise, and attentiveness. While I have been dealing with my own fears about death Chaplain Blane listened attentively, spoke gently and gave me a sense of assurance I truly needed. My Aunt responded well and was most grateful for the care provided her by Blanca and Dana. Lisa went out of her way to explain to us how to deal with the many details involved with last details and finding a respectful mortuary. Our family will be ever grateful. read more

Everything they did well to care for my father, was wiped out with a single act. I was NOT notified of his death, for 2 full days! Not by hospice. Not by his caregiver. It was the folks at UCSF who told me, when updating information for the death certificate. I thought they were being proactive, since his body was willed to UCSF for research. When called, to tell both sides "Hey, why wasn't I told?" I got - finger pointing. she said she would call / they said they would call. They BOTH should have called! 5 stars for the care. Continuum Hospice may be the most important review I have ever written. I cannot express the love I have for those that were involved with taking care of my mother during her final days. I went into this extremely stressful time in my life with the idea of tackling everything as a caregiver by myself. That was the wrong assumption of course as Continuum convinced me they were there for me and my family 24/7. First and foremost, Deb our amazing RN from Continuum, but more importantly our newly added family member at this point who was there when I needed it the most to help take care of my mother and make sure it was a stress free environment. Deb not only was very knowledgeable, extremely kind, and caring, but my mother felt at peace when she was there assisting in care and the emotional positive vibe that tagged along with her. I will never be able to thank her enough for everything, but made a promise to try my best to do so! Along with Deb came our amazing Social Worker (Kay), and aide Susanna, who both were just as important in the entire process. Obviously another stressful part was the medicine and equipment, which I was told up front to not worry and that my mother would be supplied with what she needed. They not only followed through with that promise, but exceeded it! I never had to plead my case for anything that we needed, it was either delivered the day of or overnight, which was amazing. Another huge aspect is the fact that they have a 24/7 line even during holidays with a multitude of nurses to assist whenever. They stuck by that, as we had called numerous times during the wee hours of the morning or very late at night and a nurse arrived within a 30 minute to hour window (Bravo!). Obviously I can go on and on about the wonderful job Continuum performed, but mostly I want to reiterate to those hopefully looking at this review for the purposes of hospice care that Continuum is the real deal. Wow! Laurie Mckay (RN) came out to admit my sister to hospice today after a long battle with cancer. When she arrived at 9 AM, my sister had just passed away. Since she had passed, Laurie couldn't admit her to hospice but she never let on any of that and did all the paperwork and phone calls necessary to notify the appropriate agencies of the death and arranged everything. She was there for almost 3 hours. We are so thankful for her kindness. I worked as a hospice nurse myself a few years ago and I know excellent care when I see it. 5 stars-well deserved! read more

My husband began a decline requiring way more care than I was equipped to handle. Continuum Hospice to the rescue! From the moment we met her, nurse educator Laurie McKay ushered us into Continuum's sanctuary of support. By the end of that first meeting day, we had a hospital bed, much needed medical supplies and a 24/7 triage nurse at the ready. She updated the POLST, delivered new meds and basically turned my husband's bedroom into a hospital room. The nurses, all six of them, were amazing, each one offering guidance and wisdom. Tutorials on nebulizers, oxygen machines, administration of various medications and taking vitals were patiently explained. Over the course of the next few weeks, I called the 24 hour care line several times receiving exactly the guidance needed while scared in the dead of night. The social worker, Sari Markowitz, interviewed us as a couple helping us unite, bond and create a customized vision for the short future ahead. As my husband faded, Continuum expanded its help. Every day someone visited to offer medical, emotional and practical support. Virtual reality goggles, music therapy and even a choir were available to lift spirits. When he passed, a nurse promptly arrived facilitating the final steps. Our family was well-equipped to peacefully transition with him. We are so grateful to Continuum Hospice. read more

We cannot thank Rena Robinson, Director of Community Relations, and nurses Francine Crenshaw and Mariah enough for the wonderful care they provided for our mother during her final days. They were caring, compassionate and completely devoted to making her comfortable so that she could pass in peace. They provided comfort and solace not only to her but to our family as well. They firmly guided her care through the sometimes backlogged schedule of the nursing facility and ensured that her comfort care remained uninterrupted and at top priority. We felt that we had someone on our side advocating for our mother through the entire process. These three ladies are absolutely the best at what they do. Thank you so much. read more
