My experience is too long to share in detail but in short; my mother is 89 years old. I would visit her 3 days a week after work. My mother was neglected and overly medicated to the point where I thought her Alzheimer's/dementia was progressing and she did not have much time left. Before a nurse recommended that I place my mother on hospice at the facility, my mother was frequently falling while walking; and falling out of bed. When I asked to see her medications, I found out my mother's Seroquel was doubled at bedtime without my knowledge. I told them to reduce the dosage to what we agreed upon immediately. It was not until after I had her transferred to a different facility that I realized she was being given Lorazepam without my knowledge as well. Prior to her move to another facility, my Mom spoke in "word salad" and slept all day. She was walking and talking when she moved in. I later found out after I moved her that this was due to the change increase in Seroquel and the prescription Lorazepam without my knowledge. I also started to see a difference in her care. She was always wet. She was forced to sit in urine-soaked pullups and her bed linen was always wet. I personally changed her linen every visit. Either the linen on her bed was smelly or had been removed and not replaced by the staff. Even her Broda chair reeked of urine. I had to wipe it down with antimicrobial wipes. When they moved her to a different room, the company that supplied the air mattress ordered her a new one because of the condition of the one she had. I was told she needed a new mattress because her old one was saturated with urine. I was unaware of the condition of the mattress, but I was aware of the urine-soaked clothes and linen because I washed her laundry. Although I complained about the subpar care, nothing was ever done about it. Although I washed her laundry, her clothes still came up missing. Every time I went to management about a concern, it was not addressed, I never received a call back with an apology or a plan of care to increase the quality of care, nor was there any accountability at all placed on the staff. In fact, I was avoided by top management and the facility charge nurse. Even after I gave my notice to move my mother, they continued to neglect my mother. One night, my mother was left in her bed without a pullup or pants on, with a throw thrown over her bottom and legs, no sheet on her vinyl mattress/air bed, her head facing the footboard. She had her arms wrapped up in her sweatshirt because she was cold. I was shocked. When she opened her eyes for me, they were blood shot red. I went to get the nurse, and she claimed she was unaware that this had occurred. The Caregiver did not inform her there was a problem! This is clearly neglect. The pad to prevent harm from falls were left on the floor leaving me to believe she was going to be left like that the rest of the evening and night. No one ever called me to explain why this happened and to apologize. They promise weekly outings but that never happened in the eight months we were there. The only ongoing activities, they offer is drinking beverages at a "drink bar" that is made to look like a café while listening and singing to music. I noticed in the last several months since we left on November 2, 2024, they held activities with guests and posted pictures on their FB website for advertisement purposes because they have a lot of empty rooms to fill and are trying to market the place. A lot of residents have left around the time we moved. In the 8 months my mother resided there, the carpet was never cleaned. It was visibly soiled with dirt and food and smelled like urine in the hallways. The walls were scuffed and dirty, and the furniture in the common areas where the Residents sit and watch TV smell like urine. I complained about that as well and told management that I would spray down the furniture and carpet in the hallways with Lysol and Febreze every time I visit. They started painting the week we moved out. Good nurses have left the facility due to the working conditions. Ninety-five percent of the time, they do not have adequate Caregiver staffing on the floor. As a masters-prepared nurse of almost 20 years, I feel obligated as a fellow nurse and Advocate for the elderly/patient population to let the public know my experience at this facility. I absolutely do NOT recommend Charter Senior Living of Hazel Crest. Only 24 hours after my Mom was moved to a different facility, she was walking and speaking coherently. That let me know she was overly medicated. I got my Mom back. She is currently receiving PT/OT and is doing much better. She has to use a WC sometimes due to arthritis in her knees but she is alert and knows her family.

After the tour she stayed awhile to answer general questions about Assisted Living. She is patient, very pleasant and easy to talk with and has a genuine concern for her clients to find the right facility. All fees are paid by the facility that you decide to choose. There is never a feeling of coercion to choose one facility over another. Highly recommend Jennifer's services.

We actually only had one week to get my mother into a facility. Lee Ann spent a lot of time talking with me and accessing my mother's needs and budget. Within a day she had four facilities picked out and tours set up. We were able to find a beautiful facility that met my mothers needs and budget in just two days. They also recommended different agencies that specialize in senior moves and real estate sales. Both Lee Ann and her husband real cared about my mothers well being making sure that we found the perfect fit. A move like this can be overwhelming. Oasis made the transition for my mother and me very easy. I would highly recommend them to anyone looking to find and assisted living facility for a family member.

Mom was welcomed from the moment we arrived with a welcome sign and a lovely plant and staff coming by to greet her and introduce her to others. They made her feel special, like they were waiting for her to arrive. My Mom lived at Sunrise the last few months of her life. It was hard to have her move out of our home, but Sunrise couldn't have been more wonderful to her. It offered her a nice social life, my Mom was always the life of the party! She enjoyed her friends and finally engaged in a sport, Balloon Volleyball (I think the first sport activity she ever did as an adult). My Mom loved to eat out in restaurants and the dining room was run like a restaurant with a multitude of items to order. The food is good and they were accommodating with special requests. The staff is amazingly kind and truly seem to care about the residents. Jen and Stephanie are the main administrators and they lead a wonderful staff who took great care of my Mom.     This is NOT a 'nursing home'!!! It is a senior living facility! There are levels of care that are…read moredifferent for each resident. Some need help with medications, some need help with showering, & some residents don't need any help at all! For food, residents can choose from the main, sit down dining room, or a Bistro, which is more of a grab and go area. There are plenty of seating options & the counter is staffed 'til 3pm with a person making sandwiches, paninis, & pizzas to name just a few of the choices. There is a Memory Wing for special residents and I've not been in there but I do know the staff member in charge of their activities: Caleb. Caleb used to work the reception desk so I saw him more often then. He is one of the most caring & empathetic people who work there. I'm so happy he moved up in the company but I really do miss seeing him when I'm signing in every day!! Truly, it is the people (both the residents AND the staff!!) here that make this place amazing! Starting with the reception staff: Paula, Emily, Patricia, & Noya are SO nice and they're always willing to help if I have a question! The staff that cares for my father is OUTSTANDING! The dining room staff know all the residents by name, and a few of the wait staff have come up to my father's room to visit with him & see his cat! The staff members that assist the wheelchair-bound residents are wonderful people too! I'm forgetting some of them but Bree(sp?), Valerie, & Eleanor, make every day a better day - always with a smile on their faces and a cheerful disposition! The 2nd floor has a small workout room that is usually staffed with physical therapist but open for residents too. There is a salon, a library area, & a "great room" with a big screen tv and lots of seating. There is another room for exercise classes and poker tables, and a room for crafts and Bingo! (Bingo got so busy they had to change it to 2 sessions!!) There is a theater room & they show a movie/TV series every evening, and a matinee every Friday! All of this stuff is coordinated by the Activities Director, Giovanni, and his assistant, Darlene. My father ADORES Giovanni. Gio is the heart and soul of this place!! He is the one who got my father to play Bingo even though my Dad didn't play bingo! Gio got my Dad to attend the Disney matinee even though my 94 year old father doesn't like "cartoons". (In Gio's defense, he is currently showing all the Avenger movies right now - NOT "cartoons"!) Giovanni checks on my dad every single day before going home at the end of the day. Definitely above and beyond the scope of his 'duties'!!! Gio has formed a special bond with my father, and probably with every other resident too! He's just that kind of person. I call him an 'old soul'. He has TONS of vinyl records and he now has a weekly 'musical memories' hour in the theater room. It's in a 'name that tune' format. Music has the power to transport people to a different time and place - but NOT to a different multiverse!!! Storypoint has a small van that fits the residents walkers & wheelchairs, and a Flex SUV- they just took the residents to the zoo the other day! And they have a couple of grocery store trips each week and many other group outings: restaurants, museums, etc. The main person who drives the residents on these trips is Jeff - also a SUPERB person! One resident told me yesterday how Jeff is a great driver and makes everyone feel relaxed, even when coming home from the zoo in rush hour traffic took almost 2 hours! To sum this place up: it's the people here that make the residents lives more enriched during the sunset of their lives. They warm my heart

Dealing with an elderly parent, that requires 24/7 care due to mid-stage dementia and early Alzheimer's disease is not easy for anyone.
