Review date: January 31, 2026 Ballard Rehabilitation Center demonstrated a level of neglect that placed my father's life in immediate danger. My father was discharged from the hospital to this facility for recovery. Within less than two days, due to lack of care and staff negligence, he was rushed to the emergency room in significantly worse condition. My father is non-ambulatory and dependent on staff for basic care. During his stay, his assist/call button was repeatedly ignored, and he was checked on only once over the course of an entire day. Staffing was clearly inadequate, and there was no effective oversight or responsiveness. When I attempted to reach the nursing station, phone calls went unanswered repeatedly. It took seven attempts to reach anyone at the front desk, where staff were rude, dismissive, and focused on explaining "process" rather than addressing an urgent medical concern. I had to demand immediate escalation after my father sounded like he was struggling to breathe and then lost consciousness while I was on the phone with him. Despite these alarming signs, he was left unattended. Only after I insisted on direct action did staff finally check on him. Earlier that day, my father contacted his home caregiver stating he felt unsafe, was not receiving care, and believed he was being prevented from leaving despite requesting discharge. After staff acknowledged he could not be held, he was still left without supervision or assistance. Had I not intervened persistently and demanded action, my father would not have survived to receive the emergency care he is currently undergoing. This was not a misunderstanding, nor an isolated delay -- it was systemic neglect, poor management, and a failure to provide even the most basic standard of care for a vulnerable, dependent adult. We had read prior reviews and hoped this experience would be different. Unfortunately, our experience mirrored the most serious and concerning complaints already documented. I do not recommend this facility. If you care about the safety and well-being of your loved one, take these warnings seriously and advocate aggressively -- or choose another facility entirely. read more

I was extremely disappointed in the room they provided for my Mom. There was unfinished spackling on the wall. A completely worn out corner of the wall by the bathroom door. The toilet was filthy, crap stuck to the toilet bowl. Absolutely unacceptable. I am going to get her out of there ASAP! read more

The overall care that the residents receive is atrocious. Men and women in hallways screaming for help will haunt me,

By ANY means, do not have a loved one placed at this facility. The nurses and staff are extremely neglectful and provide little to NO care for their patients. My mother cried for hours to be ignored resulting in a 911 call that landed her at another hospital with a blood clot in her heart. I would never wish having a loved one at this place. No idea how these people are employed. They have no idea who they're calling or informing ever. They have zero communication between each other so everything is a different story. Even the paramedic warned us you will care for yourself while here, no help whatsoever. Shut this place down. read more

Absolutely horrible in this place, I have been long term for 7 years, the staff is disrespectful and disgruntled and demeaning, a lot of them are bullies and antagonistic, we have too many dementia residents that do not belong in this kind of setting and the admissions office have a bad habit of mixing them with the normal patients, a lot are heavy care with a lot of screaming from them, they need to have a memory care ward, the staff is unable to perform the simplest of tasks like getting us ice water, they act like it bothers them, and a lot of us have our call lights on for 2 hours or longer, I would not put a loved one in here read more

I had a CNA today by the name of Ashley that was outstanding she spent at least an hour and a half helping me and my roommate and then after lunch I had the worst experience here with medication I'm not sure what they do with it but they sure aren't giving it to the patientI read more

My dad had to go to this place because he had crappy insurance. When he needed something, he would hit a button which should signal an attendant to come in and see what he needs. Attendants would never respond, he finally stopped ringing the button. He was at this place for about a month and was only given a bath about once a week. The sheets on the bed were so thin, there was holes from being too worn out. He was supposed to receive PT five times a week, he received it randomly. There was no selection of food and they would serve heavy carb/refined carb foods. The one positive were the social workers, they seem like they cared. read more

Trying to get ahold of someone is like pulling teeth, and when you do finally reach reception and get transferred to who you want to speak with, they never answer and don't return phone calls. Yeah, I get the whole Covid thing, that is why my Dad is there after all, but even he can't get someone to help him! This has been pure hell dealing with this place. read more

My wife's disabled brother was admitted to hospital, and was told by Ballard Center his room could not be held. Fair enough. However, after telling the family they would box up and store his possessions -- ten years' worth, some personal and irreplaceable, but others with real value -- we are now being told his things were thrown out or possibly stolen, and too bad so sad, there's nothing they will do. Any attempt to get further information is met with either the runaround or by a smug manager claiming their policy is to throw out personal possessions after 30 days, without notice, whether it is of value or not. We're filing a police report and investigating other options. Pass on this place. read more

I was sent here from a hospital following a heart attack because there were so few beds available for rehab in Seattle. This place is horrible! I left after 11/2 days of non-treatment, poor food, inattentive care, loud noise and screaming from both staff and mentally disturbed patients. Happily, I was able to escape and am back home, thanks to friends and finances. This place is like a living hell. If you can afford it financially, stay away from this Bedlam! read more

My mom caught a suspicious case of pneumonia here right before the covid-19 outbreak. Staff on each floor were almost non-existent. don't send your loved ones here unless you can't afford to go anywhere else. read more

very professional staff! that go out of their way to personally help residents with all of our needs! CNA care is better here than, the other 2 buildings I have been in while in Seattle! would recommend to everyone who is looking for Physical Therapy, or a long term place if needed for Daily care that is needed! oh ya the duck pond is beautiful! read more

This Rehab Center deserves a better rating than some of those on other review sites. The facility offers pleasant suroundings, cleanliness, and a staff from doctor to nursing assistents that will aim at fulfilling the patient's need. Thank you all. read more

Ballard Center rehabilitation and nursing is an exceptional facility. The care that I received was outstanding. The nursing and nursing aids were the most caring and helpful. The PT and OT worked effectively to help assist me to regain my mobility. I enjoyed the treatment plan they installed. Food and dietary needs were addressed and any cravings i had the staff satisfied to my delight. All meals were delicious and more than satisfying. During my stay if I had an outside appointment that was nessacary , they had personal to schedule transportation to and from appointments. They had all my needs addressed. This place is gross and it's employees are very loud and inconsiderate with carrying on during late hours outside. I've been woken up almost every night. The worst part is at the crack of dawn every following morning an employee drives up to the back entrance with music blasting, exits the car to go inside while leaving the music on full blast for everyone else's 6am "enjoyment". A couple minutes later the employee returns to the car and drives off. I have complained to the establishment several times about the disruptions but it never changes. read more

Horrible Place, they Lie ABOUT EVERYTHING AND BREAK LAWS STATE & FEDERAL EVERYDAY! The little girl working as a pretend social worker there LIES, if the state only knew what she was doing. There is a big and continued misappropriation of government funds and residents funds, there paperwork doesn't add up ever unless they make it up on the spot which is their specialty thank the girl that claims she has experience to be doing accounting...yikes. They continually neglect and steal from their residents. They continue to lie to the state, the police department, the fire fighters it's sad that they won't be investigated... people have been dying there because of the neglect nothing to do with COVID. They continually inappropriately hire uneducated inexperienced people from anywhere and the patients suffer because of it. Good luck trying to reach your loved ones if they answer. This place has no ACCOUNTABILITY nor do they TAKE RESPONSIBILITY for ANYTHING! Their hiring is beyond neglectful. i have been at ballard genesis rehab for almost two years. there are not enough worker they each get 12 or more people to care for. the food is crap. all meals are cold . the meat is always dry and hard to cut. you dont get what you order. when you complain it doesnt help. have talked to everyone. no good. they are always running out of things. read more
