On days my Mom did NOT want to shower, they LIED to her saying they called her Daughter (ME) & that I told them to make her shower. My Mom cried all the way down the hall (my mom had a stroke and was immobile). When these issues where brought to up to her Social Worker, they then sent in Psyche to do an eval (which no phone call was made to me about-which by law they are supposed to do), tried making it out that my Mom was SUICIDAL (another lie). This place is SICK! I YANKED MY MOM OUT as soon as that happened. If my hands werent so full, i would of filed a complaint with the State. Do NOT TRUST these people with someone you love. I wont even get into the things we witnessed happening with other patients in this Facility. Their license should be pulled! Im sure Im missing a lot of other things we went through, Im doing this review on a whim. And Nurses WILL drill the Families for info, then later use that against the patient and make it seem like the patient gave them all of the information. They play DIRTY!! Staff RNs were helpful and courteous however the LVNs tended to be more robotic and too busy to introduce themselves and explain to me what they were going to do. It was mostly "here's your meds!" I enjoyed my stay there for the most part. Activity staff were great like Willy, Hillary, and Jennifer. From top administrator Leslie always had time to talk to me AND LISTEN. She always had the brightest smile. Pt rehab program was very efficient as well. I would recommend this facility to anyone that needs the care.

They allow the residents to yell, cuss, & blast rap music as loud and as long as they want. They stole multiple things from my loved one. This place needs to be SHUT DOWN. It's really sad to see the elderly not being taken care of properly and rotting away in their beds. This was overall an awful experience , I don't think it gets any worse than this.

My father entered this facility with documented "good rehab potential." Until only weeks earlier, he had been ambulatory and self-sufficient. Yet almost immediately, Dr. Cha repeatedly dismissed his worsening condition as simply "old age." What we witnessed felt less like compassionate rehabilitation medicine and more like an elderly patient quietly being written off. One physician later intervened with IV hydration and steroids after recognizing a serious decline in my father's condition, and we saw noticeable improvement. Yet shortly afterward, according to staff, the IV was removed despite my father struggling to swallow even water. Communication with the family was deeply inadequate despite staff knowing my father spoke no English and communicated only in the Calabrian dialect of southern Italy. He was therefore heavily dependent upon proper medical oversight, attentive observation, and honest communication with family members advocating on his behalf. The medical records later obtained by our family contained troubling inconsistencies, contradictory notes, omissions, incorrect dates, and even the wrong gender in one report. Most heartbreaking of all was the feeling that my father was no longer being treated as someone worth fighting for. Despite ongoing weakness and COVID symptoms, he was discharged to a skilled nursing facility instead of receiving higher-level hospital care. Two days later, he was dead. After reading numerous prior reviews of San Joaquin Valley Rehabilitation Hospital, I was deeply disturbed by how many experiences closely resembled our own concerns regarding patient care and Dr. Cha's medical oversight. Multiple reviewers described issues involving medication management, poor communication, dismissive behavior toward family advocates, questionable treatment decisions, and inadequate patient care -- concerns that strongly parallel what is documented in our records. My concerns are not speculative. I possess extensive medical documentation, medication records, physician notes, timelines, and related evidence that I believe demonstrate serious negligence involving Dr. Cha and, by extension, San Joaquin Valley Rehabilitation Hospital. What is especially troubling is the consistency between our documented experience and the accounts already publicly described by multiple other families here. Because these matters are supported by substantial documentation, I am willing to privately share relevant records with legitimate investigators, attorneys, licensing authorities, journalists, or families seeking information regarding this facility. If others experienced similar issues and wish to compare records or discuss concerns privately, they may contact me. Patients and families deserve transparency, accountability, and safe medical care. They are the only facility in the Fresno area that would accept someone with his diagnosis. When I removed him from his previous facility that was rated 4 stars by Clovis Community, he was so weak that he could not sit up or stand because he had lost nearly 30 pounds because of their poor care. The receptionist at Sierra Vista was very welcoming. The staff was friendly and I could ask any staff member about his condition. At his previous facility I was only allowed to speak to one person and she never gave me answers to my questions regarding his weight, BP, etc. Sierra Vista's dedicated Physical Therapy staff was able to get him up on his feet and walking again. I am extremely thankful for them doing that! When he was discharged to go home many of the staff hugged him goodbye or gave him a kiss on the cheek. They will be happy to know that he is still up and walking. Thank you Sierra Vista!

I take reviewing Sierra Vista healthcare center very seriously because someone's loved one's life…read moremight depend on it. I first want to say that the staff was very kind and hard-working. My Dad had been to Willow creek 9 months prior and had a better experience- but his health was not as bad as it was so it's hard to compare. Willow Creek and no other facility would accept him except for Sierra Vista because he needed a self catheter every six hours due to foley complications he had at the hospital. I'm very grateful that they accepted him and were willing to do this difficult job. My dad arrived after being in the hospital for two weeks. He was immobile and we were hoping he could gain some strength through physical therapy. Upon arriving, I remember feeling as if things were not set up for a patient to succeed physically. My dad's bed felt very cheap and had no rails for him in case he fell. The TV didn't work and he wouldn't have been able to see it anyway, because the bed could barely prop up. He could barely reach his food due to the height differences of the bed and the table. The food was absolutely terrible, and I repeated to them several times that it took him hours to eat. He needed the food to sit in front of him for hours and that he would eventually eat. After 20 minutes of putting the food in front of his face, they would take it away and toss it. Due to this, his kidneys failed because he was not getting any food. They also restricted his fluids because his sodium levels were not great but it was clear he was completely failing from dehydration and malnutrition. We had some other issues and I felt it was almost impossible to get a hold of the doctor. I think there's one Dr on sight once a week or so... when my dad had an allergic reaction, it took hours and hours for them to get approval to put hydrocortisone on his skin, so I actually went to CVS and got it myself because he was suffering. After four days, my dad declined terribly. He couldn't even speak or understand anything. They had trouble getting the catheter in and his bladder got distended so they had to call emergency. At Emergency we learned that my dad's kidneys were failing. He was in the hospital for another two weeks and ultimately it was because he didn't have fluids or proper nutrition. My dad was put on Hospice and since he has been in assisted-living- it's been a miracle and he has made a complete turnaround. I really think it came down to nutrition and comfort. Sierra Vista has some things to work on like cleanliness, the humidity was almost unbearable, the food was a huge issue... But I do believe the staff did their best. All really good people... physical therapy probably wasn't the best situation to put my dad into, so I can't put all the blame on them... I feel he was neglected a bit, but I also think the staff had so much on their hands with other patients. I think if your family member is declining at any facility like this, it's best to get them somewhere very comfortable because being comfortable and fed well is the key to healing.
