A one page form took them two months to correctly complete…read more 5 visits, one lost form, one incorrect blood draw request, one lost blood vial, one incorrectly submitted form, 15+ phone calls to Bronson and insurance and on and on. Who knew a one page form with *6 questions* multiple choice for my PA Devin Clemence would be so difficult? I had to get a health assessment completed in order to get better coverage from my insurance plan so I figured I would try the new Bronson office in town. There was a 90 day deadline to have this done so I figured I would have plenty of time. It started well enough, I handed my PA Devin the form explained what it was and he started asking me the questions on it and ordered a blood draw. He asked if I thought I ate well and I answered honestly and said no. One of the questions on the form was whether I'd commit to a weight management program. Devin NEVER asked me this and checked "No" on the form despite not asking me. Don't know about anyone else but saying you don't eat well and saying you don't commit to a weight management program are NOT the same thing.  I wanted to sign up for the program. Then Devin asked me if I'd get a blood test for smoking.  I asked him if he had my blood test from one year ago when I had him complete the same form at his previous practice. He left the room and came back and said he did have the previous test, it said I wasn't a smoker, and that I didn't need to take that specific test again. He did tell me I'd have to come back and fast for blood tests for A1C and cholesterol. I have a phobia getting blood drawn and have passed out before so I wasn't thrilled but knew my insurance required it. So I left and planned to come back the following week. I looked online and it said you DON'T have to fast for these tests so I drive back to Bronson that day and had my blood drawn. A minor annoyance but I was willing to let it go. Then I got a call from Devin's MA Christy that Bronson had LOST my blood and I'd have to get my blood drawn all over again. Keep in mind I have a phobia so this was quite stressful for me. So I went back and got my blood drawn again the following week. A week after that I called Bronson to check on my form because my insurance said they didn't get it and I was told they LOST the form and asked if I could bring it to them. Luckily I did keep a copy and noticed Devin had checked that I didn't commit to a weight loss program so I brought it in and told the front desk that he never asked me and asked her to have him correct it and resubmit it. Another week goes by and insurance says they still do not have the form. I call Bronson and tell them and they say they'll send it in. I asked they make sure the form is corrected before sending it. A few days later I get a call from MA Christy asking me to come in for a urine test. I told her Devin said I didn't need to because he had my blood test and she said that was wrong and I had to come back anyway. So I went to Bronson yet again. Another week goes by and insurance says they STILL do not have the form. I call the front desk and tell them and they say they will have Devin complete it and send it in. My insurance gets it a few days later but says I'm disqualified because the form says I won't commit to a weight loss program. Remember I've specifically asked multiple times to get this fixed. I drive to Bronson again and tell them I don't know how to get them to send in this form correctly, I'm not mad but I don't know what to do because I'm running out of time to reach the deadline. I call my insurance 3 times that week and still no corrected form. Finally I call and leave Devin's MA Christy a voicemail and explain all of this again and tell them I am close to running out of time for the form's deadline and that my insurance cost will go up and I'll have less coverage. She calls me back the next day and says she'll submit it. At last the insurance says they have what they need. Why was this so difficult?  I should have gone twice at most and made no phone calls to insurance or Bronson. I was simply floored at how much effort it took on my part to get a one page form submitted to my insurance correctly. I truly hope this was a multiple trip, multiple phone call anomaly that other patients don't have to go through. Better luck to everyone else.
