Very poor experience. My mother was in this rehab a year ago and it was good. However it has definitely gone down hill. The staff is not engaged too busy playing on their phones vs. attending to patients. When my mother was released she did not have her cell phone. I have the find my app and it showed it was still at Care One. I was away until the day after she was transferred only to find that the phone was shut off or died from not being charged so I was unable to go there and follow the signal. I called and asked for it to be located. Twice with messages for the head of nursing. They did not have the respect to return my call. Very disappointing! read more

Friend had a rehab stay at Care One. He received great care, nursing and CNAs work hard to care for their patients. Rehab staff is phenomenal and always professional. Would recommend Care One to anyone. read more

My husband has stayed here more than five times after hospitalization he particularly likes the very qualified PT staff. He enjoys the resident cat Cooper. The lobby is extremely pleasant for family visits. The lobby furniture is updated and attractive, but the furnishings upstairs are shabby. Almost all the nursing staff are very caring and qualified, 1 or 2 were lazy and grouchy,. The biggest downside, is the food service, I conferred with the chef, but the food was terrible: the portions are very small and the food never arrives upstairs hot, it is not tasty and appears unappetizing, and you don't get what you order. Often he was fed meals which were not allowed on his heart diet. This CARE ONE is in a very lovely pastoral setting. You can look at the windows and see the horse farm next-door. Parking near the front is very limited, but there is ample parking down the hill along the side. When I have had some concerns about my husband's health or care, the staff and management has been responsive and helpful. read more

Staff is rude. Staff complains to patients about their jobs there at the facility about too many patients and making patients feel that they are burdened. No attention to detail. Response is very slow when patients call for medication or bathroom assistance. read more

Other than needing additional aides, most were kind & attentive. As with many kinds of businesses, communication between various care groups (nurses, aides, staff) could be improved. read more

My late husband was in Care One during Covid. I had all the vaccines, but not once, during his 3 month stay was I able to even see his room. He had a covid patient in his room but only could get his PT in the room. He came home with almost none of his clothes. He also was so weak- from not getting his PT that required LONG walks, not walking in a circle in a 2 bed room. He came home, but was in the hospital within a month, hospice at home. He had been in other nursing homes, but I could visit everyday and make sure he was being cared for. After reading these recent reviews, I wish I had found a better place for him. I promised my mother that I would review this facility. Here goes: My mom had a stroke. The social worker at Lourdes told me to pick a facility for after-care. I read reviews, and concluded that CareOne was the best. Except one thing- I did not read Yelp! reviews about CareOne. Wish I had. This place was terrible. My mom was very depressed there and begged to leave. I think she even left early; it was that bad. One English nurse was slightly rude. Maybe I could have won her to my side by stating that I had seen "The English Patient". The nurses would not come when summoned. I had to call to have my mom's diaper changed. They did it forty minutes later. When I asked a male nurse to bathe her, as she had not been bathed for several days, he stated that he would try. The nurses would not ensure that a doctor would sign my brother's forms for federal caregiver leave. They hassled my brother. A doctor did it with ease when asked directly. They would not lend out a charger for my mom's phone. At one point, the landline phone in her room did not work. I think that they remedied the problem, but I cannot fully remember. They told me that she was not eating. No one encouraged her. Ironically, I had skimmed Yelp! reviews on PowerBack, where my mom had requested to be transferred to. The reviews were crappy, so I decided against it. My mom loved being at PoweBack. I thought that since CareOne was in Moorestown, which is close to Willingboro and in a "richie" town, that she would receive great care close to home. I'm surprised by the poor reviews, because I don't see it that way at all. My mom is in the Assisted Living portion, not Rehab. They are very kind to her. The activities people are SO kind and caring. We go there all the time and check on her, maybe that helps. Her depression has subsided and she has gained weight. She loves the food. Is it home? No. But she is safe, cared for, and loved. In any facility you must, as a family, visit and check on your loved one. We find this place beautiful, the workers polite and sweet to my mother, and it is set in a relaxing horse farm setting. read more

This is one of the better sub acute rehabs in the area. The PT is GREAT! All of PT is wonderful. They just lack in not giving patients enough therapy. They spend most of the day in bed or wheel chairs. Therapy should come at least 4 days a week, twice a day. Get these people up & it gives them a better chance to recover. Otherwise They are So helpful & they don't take no for an answer. I've had 2 family members in there, both for 100 days. Most of the nurses are really good & caring & some are very lazy. The OT is GOOD. Getting the patients washed in the morning dressed & out of bed is a very good thing & they do it well. The food is just disgusting! UGH! You need to be nourished to heal , get your energy back & they do not watch your diet! If you can or cannot have certain foods, you get them anyway. Cold, rubbery & just plain disgusting. They have to do much better in this area. Families have to bring food when they visit to make sure their loved one have nourishing meals. The place is very clean & overall decent for a rehab. Unfortunately, they too are short staffed so if a patient calls for help, it could be an hour or two before they get it. Causing some to lay in dirty diapers which is nasty & could cause infection. When they do come, they take good care of the patients. It's just getting some of them there! There's one person to rave about. Her name is FUMI & she is amazing! On her game at all times! Like I said, this is one of the better Sub Acute rehabs around so good luck to anyone who needs to be inside.. read more

This place was the worst. They were rude and treated my mother as if she were a child. And even worse. We had her transferred the next day. All of them were rude and cold hearted. My heart goes out to those subjected to the poor customer service and lack of compassion. Power back is the way to go. Caring and felt like home. read more

I left this same review on the Care One website, naturally they deleted it. So my Mom & Grams were admitted in the same room. My Mom spent all day yelling for nurses to check on Grams as she wasn't doing well. They wouldn't come in a timely fashion for meds, bathing, bathroom, physio etc. I mean my Mom's bipolar meds, my Grams BP, pain meds, etc. were HOURS LATE. Well my Grams was having anemia issues after a pacemaker surgery her dr never should have had her get. My Aunt is a nurse and told him to have her do a cardiac study, not a pacemaker. My Grams was influenced to get this, got it, was in rehab at Care One having internal bleeding & instead of them sending her to the hospital again she was told to wait a week when her blood when down to a 8.1 & next to no iron in her body. They took her CBC/ferritin/iron a week later after taking her off her blood thinners cold turkey with no IV aspirin, no iv heprin, just COLD TURKEY. Guess what happened? A week later her blood was down to 7.1, no iron in her body & rushed her to the ER. They should have done that a week ago to get her blood & iron IV. So now she's been OFF BLOOD THINNERS A WEEK OR MORE. So they get her fixed up, she's ready to go back to awful Care One when my Aunt gets a call that says she's died of a blood clot to the lungs. So if these 'redrum'ers had sent her to ER IMMEDIATELY instead of taking her off blood thinners cold turkey, with no heprin drip on IV, and doing "wait and watch" she'd still be alive today. I myself have delt with medical neglect and it's caused me to be disabled. When you have low blood & iron your blood thickens & your body becomes inflammed trying to heal all wounds. Therefore due to the lack of blood thinners, anemia & subsequent inflammation she developed a blood clot and died. Her body rejected the pacemaker making a clot when she needed to be on blood thinners, get blood and iron IV now, not a week later. I'd you know what to them for medical neglect if I could. My Mom is still there yelling HER head off to get the care SHE NEEDS. She can't walk. She had surgery. They expect her to wait four hours past the time her biipolar meds are due, her pain meds are due, they switch her meds on her which is illegal, and they don't explain what, why and how her foot got infected into the bone. They don't like to clean you up or help you when you go to the bathroom, the food is horrific and they don't even give you any snacks. If you order food you're lucky to have it be warm. You're completely neglected. One time she had to poop so she got up to go poop like a dignified person, rather than pooing in a diaper they told her to. To sit in her shit for HOURS before they'd even come and help you. So she said F it. I'm walking rather than holding. And they freaked out yelling "She's getting up, she's getting up. She's trying to leave." What? Is this prison? Is she an inmate trying to escape? They were in the hall gabbing and ignoring her! Literally burn this place to the ground. No one actually cares about their patients, let alone that they killed my Grams with their "wait and watch" nonsense. It's really sad her insurance, and my Grams only cover the one stop shop where you come in but might not get out alive. read more

In a nutshell, this place should be called CareNone or CareLESS One. Too many mishaps, mistakes, unapologetic, unsympathetic, no empathy or compassion. Had to get my loved one out of there ASAP. Was not fed half the time, sat in a chair for hours on end to tire him out, no physical therapy some days and the list goes on. read more

Evasham Facilty looks like a 4 star hotel. Service is very lacking! Discharge planning is woefully inadequate despite numerous discussions ahead of time. Sent home with poop in clothes in suitcase as an added suprise! Also told the 'patient' to pee in his diaper to avoid having to transfer him to the bathroom as needed. Not good at all. Stage 1 sore on bottom not even mentioned. Cushion for wheel chair to prevent the sore, approved by insurance company did not go home with the patient. Did he even get it? No one seems to know. Unless you can be there every moment of every day - don't put a loved one here. There was not even water or juice served with lunch on more than one occassion. Visitors fetched drinks for the entire table. Can't see how they do well with inspections with this type of non service. read more
