The SNF nurses and administrative staff were exceptional in their calls and overall communication with me (a daughter of a former resident there). Staffing levels and food quality were at par with other nursing homes. There are large spacious rooms such as the rehab gym and dining room plus a courtyard with live greenery and even a few cats. Dr. Smith, the SNF MD, is exceptional - a great listener, very practical, with strong and solid medical advice and care. He shared excellent - research-based rationales for his recommendations. The reason for the 4 stars is that the facility is in need of a refresh such as bright & clean interior painting, new curtains, and updated closets & cabinets - as they've seen better days... Nevertheless, Stockton Nursing Center has wonderful nurses and nursing assistants and is a good facility.

My mother has recently been discharged from the hospital following a severe urinary tract infection…read more(UTI). A contributing factor appears to be the failure to change her promptly during shifts--sometimes not at all for an entire shift or even two. Since her return, I have been present and have observed the same issue recurring. If this were the cause of her hospitalization, one would expect the Certified Nursing Assistants (CNAs) to consistently maintain her hygiene to prevent a recurrence of the infection. Additionally, I find it inappropriate that CNA Paul informed my mother that his discomfort in changing her was due to her not wearing two diapers to make his job easier. This comment caused her significant distress, leading her to tears. Furthermore, today my mother was not changed throughout the entire day. When Paul began his shift, he was instructed to change her immediately. He responded that he would seek assistance to do so. After approximately thirty minutes, I checked to see if help had arrived. Upon asking him if he intended to change my mother, he inquired whether she wished to be changed now, despite having been instructed to do so promptly at the start of his shift. Such actions contribute to the neglect and continued risk of UTIs and infections among nursing home residents. It is only because of dedicated professionals such as nurses Lola and Edward, who genuinely care for their patients, that my mother has not been entirely neglected. Prior to this incident, I contacted Edward about a week earlier to report that my mother had again not been changed during Paul's shift, and her alert light had been on for over an hour. Edward promptly notified Lola, who took corrective action to ensure my mother was attended to. I urge the facility to implement strict protocols for regular patient care and staff accountability to prevent neglect and infections. I ask you to consider how you would feel if your mother were treated in this manner. It is imperative that measures are taken; without prompt action, I may be compelled to escalate this matter publicly. I believe that The Oak Grove Post Acute in Stockton, CA, could be a better environment--particularly if staffed by compassionate and dedicated individuals like Edward, Lola, EJ, Trishell, Araceli, Jezzryl, Ashley, and others who genuinely care for their patients. My mother and I can discern the difference between staff members who perform their duties merely for a paycheck and those who genuinely care about their patients and find fulfillment in their work. I am requesting that this situation be addressed and rectified to ensure that residents like my mother experience more moments of happiness, feel safe, and receive the appropriate level of care in a facility that should be their sanctuary, not a place of neglect. Lola and Edward are saints in my book and could do no wrong. I don't want to, but I will continue to write these reviews and raise concerns until they are taken seriously, and measures are taken to keep my mother and other patients safe, taken care of, and not neglected. I can't even imagine how the patients who cannot speak up for themselves have been treated and neglected. My heart goes out for those patients. The new company is named Oak Grove Post Acute, not Stockton Nursing Center anymore.

This is his FOURTH stay here at this location. He has vascular dementia, is blind, and has Alzheimer's as well as a few other medical issues. Each time he ends up in the hospital and then we request for him to come here to this facility until he is strong enough once again to come back home so my husband and myself can take care of him. My FIL has been gradually declining in health, mainly in his mind and getting more difficult to care for and a lot of times makes it hard to want to care for him but everyone here at the facility is very good with him and have so much patience with him. He is not always easy to take care of and regardless if Don is having a good moment or bad someone is always here to lend a hand. I know some people would say it's "their job and what they get paid to do" however we can see these people care about their patient's needs. The center has always been clean and nurses have always been available anytime we have questions to ask whether it be lab results, doctor appts and getting the transport assigned for them to take Don to these appointments outside the facility. We have recommended this facility to many of our family and friends and will continue to do so. My MIL was also here at one point while my FIL was here as well. We have been a few places, and this by far has been the one place where we can leave from visiting our dad and feel safe knowing when we aren't here, he is getting cared for and we need not worry. I myself have been in the medical field for years and see these cna's ,nurses, PT/OT, doctors concerned about everyone and treat them as people and not just numbers. Even down to admissions, kitchen staff, and janitorial workers, everyone has been caring and helpful! The food here has been pretty good too. Also they have daily/weekly activities for everyone here as well and celebrate holidays for those that are here and can't be home with their loved ones. There are patios to sit outside and get fresh air as well as a large room in front of the facility where if we have a few visitors at once we will take our friends and family into for more private visits. This is also the room that is used for celebrations and pot lucks. The rehabilitation room is also decent size and has everything to assist someone to build up strength etc. (See pictures included with this post) Don will be coming back home soon. However, we know he will need care like this again at some point (unfortunately these visits are getting closer together as his disease progresses) and both Jason and I find extreme comfort in knowing we can call Lodi Skilled Nursing & Rehab again for our father's needs when the time comes.

The physical and occupational therapy was on point, they help me build my strength and taught me how to navigate stairs so I could go back home. The Director of Nursing, Hansen Khan, RN has a great team of RN's, LVN and CNA's that work at station 1. They took pride in the care they given to their patients. Mr. Haneff made sure he let the patients know they could come to him if they have any concerns with the nursing staff. I want to mention the womb care staff, the kitchen staff also was on point. I really liked how the patients can get their hair,and nails done, i love the daily activities they have. I thought it was cool how they included all the patients in celebration by walking through the halls and in patients rooms like Halloween for example they dressed up.My issues was the food was not great and certain times of the day it gets cold. Overall the care I received was nice. Would I go back if needed, yes I would.

My Mom had surgery at St Joes and was discharged to Clearwater Healthcare Center. She was sent there to help her infection clear, maintain dialysis treatments and keep her diabetes in check. Here's out it went: - They repeatedly failed to give her meds. The first time this happened, she was sent BACK to St Joes with a blood sugar reading OVER 400 and blood pressure reading over 200. St Joes managed to get her back on track, and she was returned to Clearwater for medicine management and recuperation. - Our second go-around with Clearwater, and both my sister and I explained the need for Mom to get her meds (insulin, BP meds and dialysis) at each and every prescribed time. Again, they failed to give Mom her meds. I called both at 5pm when she arrived and they were "working on the orders". I called at 5am the next day and she had no insulin whatsoever. - My Mom calls me and explains that a man is vomiting and screaming across from her room and asking for help, and no one from the facility is helping this man. I can hear him screaming on the phone. - Someone tried to climb into my Mom's bed in the middle of the night. Literally, I am without words. - I called on the fourth day to ASK THEM to do their job and administer meds, they left me on hold for THIRTY MINUTES. They came back on the phone and said, "We explained everything to another family member, we're not going to explain again, call them." And they HUNG UP ON ME. Terrible service. Terrible response. You literally have one job, and it's taking care of people - and they end up BACK in the hospital because you won't administer the meds that are prescribed. WHERE DO THOSE MEDS GO?

Nursing staff at night won't give breathing treatment or oxygen. Nurse told my family member they would finally be released if they stop calling ambulance to come out. This place ONLY CARES ABOUT MONEY NOT THEY PT. When insurance stops paying Clearwater. Clearwater brings patient to hospital to be admitted for 10 mins and returned back to Clearwater to started billing insurance again.

I decided to come in in person to try and make an appointment and the woman at the front desk said they didn't currently have a technician at this location. I asked to schedule an appointment and she actually said "I just told you we don't have anyone so who you scheduling an appointment with?" and rolled her eyes at me. I was actually shocked because I had never been spoken to like that in a professional medical office before. I explained that I meant to make an appointment for when they did have someone working, thinking there was a miscommunication and she replied "And how would I know when that will be?" Typically I would ask for a referral somewhere else but I just left. I was actually shaken when I got to my car and called my doctors office from the parking lot. They were able to locate another office an hour away and I drove there instead. They continued to give me referrals here for imaging work over the years because it was the closest place and a few years ago was the first time I was actually able to get through on the phone to someone to make an appointment. I scheduled a mammogram and I was anxious coming in that day because of the way I had been treated previously. The same woman was behind the counter and she checked me in but the technician that did my exam was great. She was fast and efficient and when the lady from the front desk actually banged on the door while I was topless and in the middle of getting my imaging done to ask her something, the technician was very blunt with her and apologized to me for how unprofessional that was. I got my results on time and I was really hoping things would continue that way. I was torn on the rating to give in my review because the staff here that are rude and unhelpful and just never answer the phone deserve a 0 but the technician was 5 stars all that way. Last year I got another referral for a mammogram and I called for weeks, every day with no answer. Well, technically an answering service picks up and puts you on hold to connect you to the office but after waiting 30 or 40 minutes I would always disconnect. I eventually explained to the woman transferring me that no one ever picks up but she stated she was not on site and could not do anything about that. I even spoke with a manger. About a month later I had an OBGYN appointment and told them I couldn't get ahold of anyone and asked if they could help. The office staff said they were ALSO on hold for my entire visit and could not get an answer. They said I would have to go in person, so I just skipped my appointment altogether. This year I needed a CT and to my amazement, someone friendly answered right away, made me an appointment for the following week and I was in and out of my appointment in 20 minutes with a fast, competent technician. However when I called a few weeks later for a different appointment, thinking things were looking up, it was difficult to get an answer and when I did they said I would have to drive to another town for my imaging. It has been a rollercoaster with this location. Some very VERY bad interactions and a few good so know that you are rolling the dice coming here and that you could have a good experience or a terrible one. My best advice is that if you call and have a ridiculous wait time or can't get ahold of someone, don't keep trying. Just ask your doctor to be referred somewhere else and make the drive because it will be worth it in the end.
