They utilized a modern approach by texting me a link to fill-out paperwork on my smartphone or computer, to be completed before my visit. This has always been a pet peeve of mine. I was able to take my time and populate the documentation leisurely at home. This should become a standard at all medical facilities. So, thus far... a Kudos to Creekside Dental for not handing me a clipboard of forms. First impression and good experience.
