I went to Freeform Chiropractic about three months ago, and honestly, I was getting close to considering surgery. The tips of my middle and ring fingers on both hands tingled 24 hours a day. If I did any kind of work with my hands -- trimming trees, working in the yard, even trying to play golf -- my hands would go completely numb. I actually had to stop playing golf altogether because I couldn't even finish nine holes before my hands went dead. On top of that, I had a burning sensation around C2-C3, and an MRI a couple of years ago showed six bulging discs and a pinched nerve. I was wearing wrist braces every night because if I didn't, my hands would go numb and hurt horribly. My neck range of motion was terrible -- I could barely turn my head far enough to check for traffic when changing lanes. Fast-forward three months of going to Freeform Chiropractic three times a week, and the difference is unbelievable. My neck mobility has improved dramatically -- I now have full range of motion. Bait and Switch Front desk lied stating they take insurance. My insurance covers x number of chiropractic visits and x number of PT days I know for a fact because I called my insurance. I went to the trouble of gathering all of my spine imaging studies and reports for them. When I scheduled my visit fully expecting to be adjusted and asked the front desk girl again if my insurance would cover the visit, she said "yes". At my visit, one of the 2 chiropractors came in and said he looked at my reports and had a "game plan" together and it was going to cost me $2000+ and that they "don't do pay per visits". Needless to say I didn't get an adjustment in spite of being in significant pain. Go elsewhere if you have insurance. Otherwise, you'll be finding their upscale facility with their club music going on. read more

Past experiences have made me avoid chiropractors at all costs. However, I am pregnant and my back pain started to intensify added by hip pain that interrupted my sleep. I kept hearing "go to a chiropractor". So, I did my research and decided to give Free Form Chiropractic a chance because they had really good reviews especially from pregnant women. First, let me tell you the front office by far has the best customer service that I have EVER seen. Charity and Diandra are friendly, always greet you by name, make you feel welcomed, part of the family and never skip a beat. They have recently added a couple new faces that are fitting right in. The doctors at Free Form Chiropractic are all well educated, informed and attentive. Dr. Jacobs has been great. He addressed all my concerns and made me feel comfortable and confident in my decision to give chiropractic another shot. He has answered any questions not only directly related to chiropractic related topics, but health related as well. Dr. Worthington has been great. He has been the other doctor adjusting me also making sure that I feel comfortable and answering all my questions. Even if he is not the one adjusting me for that visit, he always makes sure to stop by and say hi. Dr. Thomas has not adjusted me, but he has gone over my reviews, done muscle work and also been very attentive to any questions or concerns. There are also a few interns that are in the office. For me they mostly have just done the muscle work, taught me some stretches or moves on the TRX bands, or hand me my clip board that I forgot at the last station. (Which is a lot, thanks guys!) They are always friendly and welcoming. Being pregnant hasn't been the easiest thing I have ever done in my life, but the relief that I get from the visits makes it a little easier. Waking up from extreme hip pain has decreased TREMENDOUSLY, and my back pain has decreased significantly as well. I highly recommend Free Form Chiropractic. First visit they took x-rays and asked me questions. Second visit they went over my x-rays and then started listing my treatment plan. He went over 9 things and wrote down some initials. He then says it would be a 3-month treatment schedule and I can pay $1276 up front, or $1398 over 3 payments (extra $122 to cover the extra credit card transaction fees which is nonsense). I could not get any treatment that day unless I purchased 3 months. It felt super used car salesman, super hard sell. When I asked for more details on the various treatments (brochures, pamphlets, flyers, printouts), he said he had no written information on any of it. I was just supposed to hand over $1300 based on a bunch of abbreviations. (Attaching the paper). That's absurd so I left. They wasted my time and money, and it's sad they do that to people seeking medical treatment. read more

I have always been a bit skeptical about going to see chiropractors because I never thought that they did anything but just crack your back. BOY was I wrong! I used to get headaches at least 3-4 which meant I took Tylenol all the time. I used to get anxiety not having a bottle of it in my bag or in the car, just in case. I started going to freeform when they held a mini clinic at my work months ago. I am 36 and at the time had neck pain and my knee was hurting on a daily basis, enough so that I was limping throughout the day. I came into freeform and was evaluated and they put me in a program to address the issues that I had. Well, it was one of the better decisions I have made for my health in a long time. In two months my neck had a better range of motion, was no longer stiff with pain and overall my body felt much better in general. The largest improvement was my knee. Misleading Facebook advertising. Advertised $37 initial appointment, X-rays, and follow up appointment. They clarified this was a cash price and no insurance was required. Upon arrival for my appointment, the receptionist asks for my insurance "just in case." They billed my insurance over $300 for appointment and x-rays. I never even returned for follow-up appointment because, even though they were friendly, it felt like they were trying to sell me a car. They wanted to me to come back to review xrays with my husband and family like they were about to tell me I had cancer or something. My guess is they wanted to use high pressure sales to get me on some sort of payment plan and not be able to use the excuse that I need to "talk to my husband first." Do. Not. Waste. Your. Time. This place is a joke. I am currently pregnant and experiencing a lot of hip and lower back pain. After reading all of the glowing reviews online I made an appointment with Freeform. I went to the first appointment for a consultation where I was put in a room and sat down and discussed my pertinent medical history with one doctor. Then he said they do a "team approach" to their treatment plans and another doctor would be coming in to follow up with me. So I go through all the same history again with the second doctor. They get me on the table to look at my back and hips and say they've identified the problem areas. They're very good with the slick talking, very rehearsed, sales pitches they give you along the way. Then they make it seem like I'm a top priority and get me in the next day, for what I thought would be the first adjustment and the start of some kind of treatment. The next day I come back and instead of going to the tables where they do the adjustments and work, I'm taken back to another room. Where I wait for over 20 minutes. Then one of the doctors comes in and gets a scratch sheet of paper out and goes over a 90 treatment plan that is going to cost $1700. He insists this is paid all up front. There are no other options presented. At that point I wasn't sure if I was signing up for a gym membership or if I was still trying to get some medical help and relief for the pain I was in. After I express hesitation to commit to a 90 plan with my due date right around the corner, he then says I can pay 1/3 today and then the rest after I have my baby. It was a very pushy and uncomfortable sales pitch that was completely focused on the $$ that he had quoted and had little to no interest in my health and well being. I left feeling extremely disappointed and frustrated. It was a complete waste of time and I'm still in pain! This is not a medical practice that cares about patients quality of life or actually helping their patients in any way. This is a business whose sole focus is making a profit and they will run you through there like a mill. It felt more like I was dealing with a sleazy used car salesman than a medical professional who should be interested in helping. And sadly I'm sure there's a lot of people that have and will cave to the pressure of their pushy sales pitches and scare tactics. Places like this are the reason why a lot of people think chiropractors are a bunch of quacks and not actual medical professionals. read more

This place is a bit of a "mill". You don't really have 1 on 1 with the chiropractor because you sort of get hearded through your visit, which for the majority of the time you do your own exercises. Aside from that, my biggest issues their questionable accounting practices. I asked for my insurance not to be billed but they billed them anyways and I found out by accident. When my insurance paid, they said I will get reimbursed at the end of my therapy incase I want to put it towards more therapy. At the end, it took some work to get it back but not until after I was told that I received some extra services that they are going to charge me for so I won't be getting the full amount. The services were discussed before start of treatment and were included in the money that I already pre-paid so that was unacceptable and I told them as much. They returned the full amount because I called them out on it. So, when you go just make sure you don't get ripped off. After I told them that I won't be signing up for their monthly services, I was rushed through my last visit and shown the door. My back still hurts. I won't be back. read more

I am a ball of mess with back issues, sciatica, migraines, arthritis/tendinitis knee, shoulder issue, and Trochanteric Bursitis in my hip. I had finished Physical therapy for my shoulder, back, and knee a couple years ago. I knew I needed to try something more. Dr. Jacob came to my job with a presentation and a great deal on consultation. Since I've been going, I have not had a migraine, not even a headache as often as I use to. My TB is better, from hurting consistently to only 40% of the time. Which helps my workouts so I used this opportunity to change my eating habits as well and have lost 12 lbs in 5 weeks! My back is not hurting 110% of the time. More like every so often. Such a relief to be able to do housework without Ibuprofen. To be honest I was not expecting the results so fast, the 1st two sessions helped my digestive issues (and I'm only half way through). They not only help align you but they work with strengthening your muscles, which is one of the primary reasons I decided to stay.
