We were assigned Emily as our case manager and she was great to work with. However, we interacted the most with Grace who was on-call at the time we engaged them and helped my Dad a good bit during his last couple of days. Grace was super patient with us when we asked lots of questions about his care and hospice in general. LifeChoice also provided a rabbi for us and I think my father really appreciated that the rabbi visited with him for over an hour. I think they provided my father a lot of peace and comfort during his final days. My only regret was not engaging them sooner. Their whole demeanor and attitude is that of angels, compassion, empathy, love..When he was in the hospital, he was not receiving the proper pain management, they kept delaying the hospitals hospice program while my dad was dying in pain. I was desperate to get him home. Alena, the administrator, moved mountains to make that happen. They got my dad home in less than 18 hours, they were by my father's bedside within two hours of me calling desperate for help, which felt like a miracle during such a difficult time. Daniel, the nurse who came to the hospital, was so attentive and caring such a such a genuine person. And once we were home, nurse Julie and nurse Grace took over and truly went above and beyond. They made sure the bed oxygen tanks supplies and meds was delivered before my dad even got home.They made sure my dad was comfortable, out of pain, and surrounded by dignity and respect--something he was not receiving in the hospital. there was also a Sweet CNA that came to help as well, Because of them, my dad was able to pass peacefully at home, just as he deserved. That means everything to me my father and our family. All of my father's friends and loyal family were able to come visit with him before he passed the next morning.. I highly recommend this hospice agency to anyone going through end-of-life care for a loved one--or even for themselves. Don't wait until the last minute. Have this hospice team set up ahead of time--you will not regret it. They are truly angels doing God's work. Shannon Johnson-parson

    If I could give this place no stars, I would. My father was a WWII vet and died in pain because…read morethey refused to give him more medication. Also the nurse who should have been coming out every day when she knew he was close to dying decided that wasn't necessary. This place was never good to begin with. Nurses were incompetent (overworked too which wasn't there fault.). VA paid this place an tremendous amount of money each day but you'd never know it by the level of care my father received. We'd ask for supplies and we were told they were sent when they actually weren't. If the VA recommends this place, ask for another one. No compassion whatsoever from an organization who is in the business of caring for people. The CEO and nursing staff should be ashamed of themselves. I have also filed a complaint with IDPH.

    First-time reviewer, I try to see the positivity in every situation, but this is something that…read moreneeds to be said; especially for families encountering this sensitive and very difficult period in their lives. Apex Hospice has exhibited an extreme lack of communication and total loss of empathy. In my encounter with Apex, my friend who had requested and filed forms to avoid another hospital stay was transported to the ER because the on-call nurse for Apex "was 60 miles away, so what do you want me to do". After bringing this to Apex's attention we had hoped for better care; we were let down. My friend was taken off of vitamins that he asked to continue on because "are they really necessary right now"? He also had his caregiver turned away from buying groceries because their nurse hid his wallet and did not want to tell him where it was; like it was a game to her. My friend had oxygen delivered to his home, to increase his current amount, their nurse did not hook him up to the appropriate (and prescribed) oxygen concentrator for two full days because she did not feel it mattered. Further, after the request of my friend and his family to NOT be removed from his home, Apex tried diligently to place him in a rehab center so that they could allow themselves time to get a nurse; they contract their nurses from other agencies. It is appalling to me that in a profession such as Hospice Care, a company could treat someone who is actively dying with a lack of respect, dignity, or compassion. As previously stated, we did try to bring this to the attention of their Director as well as the nurses and Case Managers that we encountered. However, we saw no improvement. Lastly, when I called to see if the nurse had been made aware that he passed, and to see what the next steps were, this was the response I received: "I know that he expired, I am going to be there when I can get there.(then hung up)" I hope, sincerely, that these individuals receive better care should they ever need it.
