I needed PT really bad, my initial assessment from PT was several days after I moved in then I never saw anybody else for about two weeks, when I went down there to inquire as to why I was told their records show OT has seen me everyday. I filed a complaint with Medicare for fraud investigation. I was also shocked how little equipment they had, I was told their other facility had all the equipment, so we had to wait in line and there were some severely disabled people there. Absenteeism was a big problem which resulted in over an hour response time minimum, one of my CNAs spoke no English at all I would ask her to do something she would nod, leave and I would never see her again, it got to be where I would yell "don't act like you understand me when you don't!" Finally I went down to the nurse's desk to complain and the whole staff came out to gang up on me, how dare I complain about someone who speaks no English, the nurse finally told me that I am receiving good care and I should be happy to receive the care I'm getting. I complained to the administrator and got an apology, after that when I asked that CNA for something she'd bring in somebody else to interpret (a waste of staff don't you think?). My roommate contracted COVID and they wound up quarantine us both no medicine. I asked the doctor for something like paxlovid any kind of prescription, he turned around and walked out. Was it effective no they wound up quarantine the whole Wing. All the physical progress I made with my PT over the weeks went right out the window and I had to start over again. This used up the time to get covered by Medicare which I felt they should cover as they didn't properly treat me. Speaking of the doctor he played footloose with my medication (they just can't leave it alone), when I showed my PCP what he did, he was livid why go to the trouble of assigning and tweaking medicine only to have somebody change them willy nilly without consulting as to why? I consider that arrogant! I had a talk with the dietitian and was assured everyone was getting their nutritional needs considered only to find out all the patients got cheap sugary Kool aid to drink Food was horrible I lived on sandwiches and take out Good luck getting somebody to answer the phone. No I will not go back.

When my Dr. sent me to vibrantcare I had a pinched nerve in my neck. I couldn't move my left arm or…read moremove my neck and I was in excruciating pain. The PT didn't fully evaluate the location of my pain and gave me exercises to move my arm which I couldn't do. I was almost crying in pain and she showed no compassion of what I was feeling. After 4 sessions (1 was the initial evaluation) I told them I couldn't do PT. I started going to a Chiropractor and massage therapy . That helped eliminate some of my pain. A few months later, my Dr. sent me there after I tore my left tendon in my shoulder. I left 4 messages to make an appointment. No response. My daughter happened to have an appointment for her back, I asked her to ask them to call me so I can get an appointment. What the heck! Vibrantcare told her they got my messages but they said they can't work with me because the neck and shoulder pain are connected. So why couldn't they call me and let me know if they got my messages! How horrible is that service! Don't go to this uncaring facility!! I'm getting therapy at Laguna PT now and they are wonderful to me and are helping me tremendously!

I walked into this facility and know what met me, almost all the staff were smiling and busy with…read moretaking care of their residents, no smells or coverup smells , a nice wide open lobby with residents smiling and interacting with one another and the staff in a wonderful manner, as I took a tour around the smiles abounded and hardworking staff were busy attending to their residents needs, this a a large facility and i was amazed no call lights were going off, no one was screaming or calling for help and the facility has a tracking system on each station that displays the times a residents call light had been on and off the 2 nursing stations I walked past no one was calling for more than 5 minutes. I grew up around nursing homes and my family owned and operated 2 of them back on the east coast, in my years of work in healthcare I have been in numerous facilities and I would say this is in my top five especially for its location, its close to everything. She lived in Merced with my dad (married 55 years) and he suddenly passed away. We had to hurriedly find her a senior residence closer to me or my sister with the 24 hour front desk, pull cord in the bathroom and pendant to wear around her neck to push a button for emergencies. Eskaton has all that and more! My sister also lives very close by so the location was perfect. Let's start with Joey, the first person I spoke to on the phone who runs the entrance area/front desk - he had no idea who I was or what we'd been through and he was just the friendliest kindest voice ever. He needed to transfer me to Latoya to inquire about apartments, but I already had a great feeling about it just because of how Joey treated me in that first interaction. Then Latoya was also extremely friendly, empathetic and more than happy to take the time to go over in detail how Eskaton works, which apts were available, pricing, and emailed me floor plans and the Eskaton calendars with meals and daily activities. She was glad to reserve the apt mom wanted even before we could get paperwork/deposit back to her, being out of town and at this time still dealing with my dad's passing, his service, packing to move...they both made a terrible situation into an exciting and welcoming new start. Tristin was next, helping to formalize paperwork and just the sweetest kindest person. They answered all my million questions and Latoya took a video and pictures of the apartment so mom could plan and feel more comfortable about the move. My mom LOVES it here. The place is classy and decor is beautiful. I'm jealous of her adorable apartment! The residents make a point to chat and say hello in the hallway, elevator, dining room. She loves all the activities - seated exercise classes, Ted talk viewings, live music, libraries on 2 floors. Most of all she feels very, very safe and welcomed. The food is delicious and we eat with my mom all the time. They also encourage (if people want to) to sit with others in the dining area. My mom right away started off with a nice group of women including her neighbor across the hall. She enjoys the social aspect of the meals. Often she is very busy with activities and doesn't have time to call us! Helen in the dining room is wonderfully sweet and so fun to talk to. She seats you and comes to every table to say hi and chat for a moment, which we love and I think for those who choose to eat solo it's nice to still interact. At Eskaton you're never really alone, there are friendly staff and residents everywhere. Toni in housekeeping is also friendly and kind and does an amazing job, my mom loves talking with her and seeing her weekly. My mom's apartment is always super clean and fresh thanks to Toni! Tom and Roberto who do all the repairs/hook up cable/hang pictures are so sweet and funny...my mom always talks about how they helped her decorate and they even put together a new walker for her so she didn't have to wait to use it! Recently, believe it or not, my sister actually started having stroke in their lobby!! My mom called it and yelled to Joey they needed an ambulance, he called right away and this is the whole entire reason my sister will make a full and fast recovery. The hospital said it's miracle-it is!! I want to also call out Matthew, Ellen, Daniel, Minayah, Arsel as just so sweet and wonderful. They have helped us out multiple times with literally everything. The building is super secure, the residents are watched over carefully, it's just perfect. I will also share that my mom has only been there 1.5 months and already this is our impression.

(They have performed here before and it was a great success.) The coordinator never returns calls or messages. I never received word back, even after leaving my information and contact number and a voicemail several times (at the desk as well). It is really disappointing to not be acknowledged, have no communication whatsoever and a disappointment for my students not to have an opportunity to play at this time. The former director from last year was very prompt and welcoming.

I was shocked to receive a $600 bill, with $400 of the bill being for a 30-minute visit with the provider. The other $200 was because they gave me a rubber band tied to a hand towel and briefly showed me exercises to do with it. All other offices I've worked with have informed me beforehand if a substantial amount of their appointment would not be covered by insurance. Please be advised to obtain an estimate before your appointment, as they will not inform you if you're not covered, even in my case, where they are in-network with my insurance.

She really does strive to help her patients if they are recovering from an injury, or have had some sort of physical change etc. All of the assistant therapists are just as spectacular, and it's obvious that they love their jobs, and love helping their patients. Weeks before our son's final early-intervention meeting with Regional Center, our speech therapist…read moretold us during a session that "if children aren't making progress at this stage, they may never improve and it may be other things." After discussing it further, she stated, "I'm just gonna say it--autism!" This came as a shock given our son's documented progress and increasing vocabulary. During that same session, the therapist began pushing to reduce our son's therapy frequency due to alleged lack of progress, even though her own reports showed he had made significantly more gains during that period than the previous one, when services remained unchanged. Naturally, we were confused. No evidence or clinical reasoning was provided to explain how reducing therapy would be in his best interest. So we asked how fewer sessions could support his progress better without ever having received fewer services. And we were dismissively told, "It doesn't matter." After this interaction, the tone of care changed. Sessions felt increasingly pressured and focused on whether our son was "improving enough," rather than meeting him where he was developmentally. Thankfully, our Regional Center coordinator allowed therapy frequency to remain in place pending an ABA evaluation. Despite this, the therapist continued advocating for a reduction in services. We were then told that if we did not agree to reduce services, our son would be discharged. The therapist then abruptly left the session 20 minutes early and dropped our son 10 minutes before his next scheduled session while we were considering our options. After providing services for a year, he was discharged without a proper transition plan, and without sharing reports with his next provider, resulting in a lapse in services and disruption to his care. When the Regional Center months later requested mediation with an administrative law judge, the therapist submitted a report stating the justification for reducing services was that our son was "meeting and exceeding goals" -- the opposite of what we had previously been told when services were being reduced due to alleged lack of progress. For a family navigating complex medical and developmental needs, this lack of transparency, professionalism, and collaboration was deeply disappointing. As parents, we rely on the professionals working with our son and expected guidance grounded in evidence and compassion. Instead, we experienced pressure, contradictory communication, and an abrupt termination that disrupted our son's continuity of care and made an already complex Regional Center process even more difficult for our family to navigate. We hope this feedback encourages Connected Speech to strengthen communication practices, uphold ethical standards, and ensure proper transition planning so other families do not experience similar disruptions.

My wonderful mom is 92 years old and has Alzheimer's. She has been a resident at The Meadows at…read moreCountry Place for six years and has received excellent and compassionate care throughout those six years. As her condition has shifted and worsened, the care has adjusted and continued at the same high level. The owner, Rangi Giner, an active and on-site owner, treats her employees and all residents like family. And I've observed nothing but respect, kindness and patience for my mom and the other residents in all my visits throughout the years. There is very little staff turnover. So it's been easy to develop relationships with those caring for Mom. And it's easy to see how consistency in staffing has translated into consistency of care. I have felt like Rangi and her staff have not just been looking out for Mom's welfare, but mine, too, during these challenging six years. They do the basics well. They keep the place very clean. They get the residents up and groomed, out and about, and moving. They provide good, basic and well-balanced meals. There is music and activities. And they keep me informed with any changes in Mom's behavior and health. They do the intangibles well. They treat each resident as an important individual, so they adjust care and food and schedule, etc, based on each resident's needs. The staff always have smiles on their faces -- always. They have cheerful voices. There is kindness in the air. And it's pretty remarkable that it has been consistent for six years. The staff works together, helping each other, so things don't fall through the cracks. Perfect? No. There is no perfect. Just compassionate people working hard and doing their best. I feel very fortunate that I chose The Meadows for my mom's care and well-being. It's a special place.

My dad was worsened at this facility. He was admitted in January 2026 from a seizure and remained…read moretill beginning of March 2026. Because we could not go every day due to 3-4 hours distance, we had to call. Communications was terrible especially with the social worker. We asked the social worker to provide us with hospice options in our town. It took a long time to get a document signed after my dad was released. We saw the physical therapist force my dad to sit up seeing fully well that he was in pain. When my dad was sent home, we did not have much support. The social worker just requested a nurse to visit instead of requesting hospice which anyone could clearly see he needed hospice instead. It was the nurse who immediately saw that my dad needed hospice and he told me which hospice to call. Something the social worker along with the facility nurses, physical therapist and doctors could have done in January 2026 when my dad was first admitted at their facility. After my dad was released from their facility, the social worker would not answer my messages and phone. After 2 weeks, I reached out directly to her manager to request the appropriate doctor document to be sent to the local hospice to help my dad. Such a waste of time waiting to get her to provide something simple while my dad was suffering more and more. My dad passed away end of March 2026, and I really feel this facility made him worse.
