    My mother was admitted to Post Acute Ukiah for rehabilitation after suffering a fall at her…read moreapartment. During the approximately six months she stayed there, she experienced three separate falls while under their care. After the second fall, she suffered significant bruising across the left side of her face that lasted for weeks, and our family was deeply concerned about the seriousness of her injuries. Following her third fall, her condition declined rapidly, and she passed away three days later. Our family is devastated and heartbroken by what happened. We entrusted this facility with her care and safety during a vulnerable time in her life. We believe there were serious concerns regarding fall prevention, supervision, and communication with our family throughout her stay. I am sharing our experience because families deserve to ask questions, stay involved, and carefully monitor the care their loved ones receive in rehabilitation and nursing facilities. I hope the facility takes concerns like these seriously and thoroughly reviews its policies and procedures to help prevent other families from experiencing a similar tragedy. To the staff and administration: I urge you to examine what happened during my mother's care and make resident safety your highest priority. I write this as my personal experience and opinion based on what my family witnessed.

The food is well under hospital grade food no fresh green vegetables, no nice protein and carbohydrates just the bare minimum . The owner sorely needs to hire a good chef and staff. The physical therapy is good however they should be able to have more equipment and moderize the equipment it is bare bones and under any facility I have ever been to. Social services are good. They should have a therapist come to see the patients. There should be a doctor seeing and. checking on patients weekly not monthly. There's a nearby pool that the patients could really use for rehabilitation but the owner never put in the paperwork. I was sad for my family member having to be in such a subpar facility and this is definitely a review the owner should act on.

We hadn't even been there 5 minutes before we witnessed a staff member being extremely nasty to a patient. She was unhappy with the patient calling her nick instead of Nicki. She was extremely threatening and rude. My husband had to return the next day because of an issue with the doctor release and his experience was even worse than the day before. When he said he was there to pick her up the staff member was extremely rude. She said the woman had changed her mind and was really mean about it. He said she was whispering things to the woman and my husband felt like she was trying to convince the woman not to leave with him. Once it was sorted out and my husband was getting her in the car she made it very clear that they wouldn't help him get her in the car she was his problem now as she put it. She even yelled at the other staff members when they tried to help him get her in the car. I'm disgusted with this place and how badly my husband was treated. He said the other staff was very friendly and helpful but this one woman was horrible. I can't understand how someone can be so mean to someone they just met who is just there trying to do their job.

The staff was always very attentive to their needs, and our own family concerns and kept us informed of any changes that occurred. Their rehab staff is top notch, as was their nursing staff in fact my mother literally did not want to come home. The food was always visually appealing, high quality, and tasted good which is not an easy thing to do when serving food in an institutionalized setting. The activities program were always fun, my parent's became avid bingo fanatics. Social services were kind and receptive to our families needs. I truly can not say enough good things, and would not hesitate to recommend this facility to anyone.

Thankfully the family was able to move him to Red Bluff Healthcare. The difference with the care and rehab at RBHC was like night and day and he immediately began to gain strength and make progress! I think the friendly, happy and caring atmosphere there makes all the difference as we noticed the improvement in dad's attitude and motivation right away. The whole staff worked as a team to help dad reach his goals and celebrated his successes along the way. By the time he completed his rehab and was released to come home, we felt like the staff had become extended family! RBHC is a great facility with a team of folks who genuinely care . We are so thankful we were able to have dad here while he recovered.

    My mother was a resident at this facility in November of last year, where she ultimately passed…read moreaway due to dementia and other serious health conditions. During her stay, the quality of care was inconsistent. While a few nurses went above and beyond in caring for my mother, and for that my family is truly grateful, many others appeared unwilling to do more than the bare minimum required of their job duties. Thankfully, my siblings and I were able to step in and help fill the gaps in care that we felt were lacking. Toward the end of my mother's life, there were times when it felt as though she was being ignored rather than compassionately cared for. What was most upsetting was the way our family was treated after my mother passed away. We were not given adequate time, privacy, or respect to grieve before her body was removed from the facility. That experience added unnecessary pain to an already heartbreaking moment for our family. In addition, Deborah, the Director of the facility, was consistently rude, unprofessional, and argumentative in our interactions with her. There was also an incident involving another patient, where decisions were made before the full situation was properly investigated, resulting in my mother being moved to another side of the facility. Based on my family's experience, I would not recommend this facility to others seeking compassionate and respectful care for their loved ones.

    I was quite cautious about having my mother go to Napa Valley Care Center after she fell and broke…read moreher pelvis because one of the recent State surveys was not glowing. However, I was very pleasantly surprised. While I always think it is important to have family advocates visiting and interceding for their family members, I found the staff attentive in a myriad of ways. Even though, this is a large facility, all the nursing staff seemed aware of my mother's fears, issues and likes and dislikes and were quite helpful. They genuinely seemed to appreciate her sweet personality. The physical therapy and occupational therapy department worked well with her and got her up to speed quickly. The facility itself has been recently remodeled and is pleasant to the eye. The facility did not smell like a "nursing home". The nursing aids were friendly although pulling extra shifts, due to being short staffed. The social worker and the head of nursing were very approachable. I think the recreational/dining area could use some work, and the food was often on the cold side, but my mother always ate everything and seemed to like it. All and all, my mother healed quickly and was not traumatized by being away from home during the Christmas holidays for a solid month. My family will be forever grateful to Napa Valley Care Center.
