We took a tour here and decided to put my mother here a year ago, to this day she is happy and is well-taken care of. If you are in the market deff schedule a tour with David (he is great). My mother is in the dementia care unit which is run by Megan. She is always available for a call and sends us videos and pictures of my mother on her phone almost daily. It has now been a couple of months and she still loves everything about the place. The rooms are spacious, great activities, awesome nursing staff, and everyone else that makes the place room smoothly are wonderful. The care she receives is exceptional, the rooms are spacious and clean, the food is healthy and nutritious, and the other residents couldn't be sweeter. I could not have found a better place for my Mom and would recommend it to everyone.

After I filed a complaint, the Florida Agency for Health Care Administration (AHCA) issued a 48-page Statement of Deficiencies citing failures including not following physician orders and medication-related deficiencies. These findings are public record. During those 41 days there were three Executive Directors (DJ, Tammy, and Heather). The Head of Maintenance (Luis) also left. When I moved my parents out, I confirmed a zero balance. Our sales agent had represented one free month. After move-out, Katherine Schachter (Regional Director of Operations) and Mark McBride (Chief Operating Officer) provided written assurances regarding credits and refunds. However, full billing statements were not provided until February 26, 2026 -- five months later -- After my parents had already moved out, we received two phone calls delivering the same recorded message from Heather (Executive Director) identifying by first and last name an Independent Living resident who had died by suicide. I reported this as a potential privacy concern. Based on our documented experience, I do not recommend HarborChase of Dr. Phillips Assisted Living. The response to my post is to contact HarborChase. No I gave you 5 months to correct your mistakes. Give my parents their money back!! You violated both the medical and financial part of the contract!

    Briefly, my mother lived independently until July 2nd, when it was discovered, she had a heart…read moreattack and later diagnosed with Multi Focal Arterial Tacti Cardia. As a precaution she was sent to a facility for rehabilitation and strengthening. It was definitely against my better judgement to use this facility, but I was not given the choice by the male Hospital Case manager at Health Central. That's a different issue which we had to address with Medicare (go to medicare.gov to file your complaint). Nonetheless, her first few days at Solaris at Lake Bennett, my mother was mobile and taking herself to the toilet and even walking to her Physical Therapy sessions. I began to have some major concerns, one of them was the emotional and physical abuse she received from an LPN because she complained about her care. My mother was so frightened she called me crying. She was to afraid to say who the LPN was for fear of retaliation. A real perception when you rely totally on others for care. Mother finally revealed who the LPN was that threatened her after we left Solaris at Lake Bennett. At Solaris at Lake Bennett, when she was sleeping, they would leave her food tray on her side table until the next food tray was to be delivered. I know this because on random occasions my sister would visit after lunch and stay until dinner and had to call to get someone to take the cold food away. Another time the tray was left on the farthest edge of the foot of the bad where mother could see it, smell it but could not reach it. Was not until we arrived in the late evening that we saw the tray closed, cold and still on the edge of the bed. Another incident was the withholding her meds or just forgetting to treat her glaucoma with her prescribed medicines. Further, she once sat in her wheelchair for 5 hours needing to urinate and eventually had to relieve herself in the wheelchair and still sat in the urine for a further hour plus. When my mum's weight was taken, she had lost 10 pounds in 3 weeks. I asked the LPN and Nurse Manager and Dietician why she had lost the so much weight and they said it was water weight. I asked if the dietitian had visited her in 2 weeks to inquire why her food trays were being returned 100% uneaten, she gave an excuse that she was busy. And apparently, they were giving her hot water instead of coffee, telling my mother that it was indeed coffee. Imagine my surprise when I randomly tasted the "coffee" and discovered it was warm water. Truth be told the evidence of her care was revealed in her blood results. I attempted to speak with the Director of Nursing - Debbie I think her name was- but she NEVER made an appearance. I was then directed to speak with someone who claimed to be "Assistant Director of Nursing". She took lots of notes but did nothing about the situation. Sadly, the Director of Nursing NEVER called me about my concerns norm offer an explanation, she just HID in her office because she is very aware of the inadequacies of the staff and general responsibilities.I spoke with the Physical Therapist Director who had no idea of my mother's health and PT had already labeled her as "maximum assist" when upon arrival she was taking herself to the toilet. Within 3 weeks she had deteriorated so badly that she DID need maximum assist which the staff was not capable of providing that care she then required. The individual who administered her IV for an aggressive antibiotic therapy did not wear a mask and when my sister and I expressed our concern, he threatened to abort the procedure. 3 days later she rushed to Health Central AMS and EVENTUALLY diagnosed with COVID. My sister was on a video call with her PCP and myself when the LPN declared Mother as "AMS", EMS Transport refused to document "AMS" after they did a series of tests, and Hospital records show she was NOT "AMS". It was a Friday and apparently, it's a known fact that when the SNF are understaffed on a Friday- they send the patients to the hospital. In the ER I discovered she had dark deep bruising on the rib area and abdomen and a diagnosed pulled muscle in the left thigh, from being "manhandled". Last Year my father-in-law died at Solaris. Had the CNA/LPN checked on him regularly they would have noticed he stopped breathing and unfortunately rescue was not called in time. As a family we were traumatized by her very low level of care, which lead to the sudden decline with Solaris. If you love and care about your senior person, go elsewhere. Solaris is unprepared to care for your sick and weak senior. Solaris at Lake Bennett is just a "meat market" it is understaffed, with over-worked spiteful, unprofessional staff, and worst, untrained in geriatric care.

    I had a conversation with Alisha, the admissions director. She told me there was a bed for my mom…read morebut since it's Saturday we could drop her off on Monday. She verified her rehab days and that everything was fine. On Sunday she told the hospital she didn't want my mom there since she didn't like me. She is an unethical person in my opinion, forcing us to find a new facility on a Sunday as my mom is already discharged from the hospital. Alisha, would not discuss with me and does not care about patients or thier care in my opinion.
