Dealing with Virgina (older Filipino lady) to do an intake is the biggest waste of time. She is so unprofessional and sloppy, it is very frustrating corresponding back and forth with her when she goes out of town so often for "family emergencies", and is the main point of contact for intake. My mother is an Air Force vet that has frontotemporal dementia. She is 63, which is a little early to be experiencing this. I went on a tour of this facility back in January, so this has been a very slow process. I told Virgina my mothers age the day of the tour where she wrote it down, as well as other details such as the current facility my mother is at. I was concerned when I would call back to check on the process that she would ask me the same questions over and over, as if Virgina is suffering from dementia herself. She has been making false promises since February when she verbally told me my mother was approved to move in the first week of march. The first week rolled around and I didn't hear anything from Virgina, so i called her direct cell where i have been getting sent to voicemail ever since because she apparently has been having back to back family emergencies or events where she goes out of town (lol). She will send you to voicemail, then ask to call back later through text. I guess this facility was rieed of my checking on the status of the placement because they finally determined today after months of deliberation that my mother is suddenly "too young". This was already after Virginia said she would be moving within a week. They knew my mother's age months ago, it seems as if they are just lazy and want to pick and choose who to care for. Glad they showed their true colors before I sent the person I love the most in the world here... if anyone can suggest facilities that are diverse and have caring staff (as my mother is black), I would be totally open to suggestions because I am starting from square one again. read more

Jasmine Terrace was Mom's home for the last 10 years. We could not have asked for a better home for Mom. She had good and bad days, and the Jasmine staff insured she had the proper care for all her needs. We first met with Lori who exudes calm sensitivity. The care staff led by Rocio and Carla stayed. on top of her medical needs and guided us through. The kitchen insured Mom received her favorites. This review is merely on thee working conditions at Jasmine Terrace at El Molino. I started as a caregiver at Jasmine Terrace, I normally was responsible for 10 residents. Being a caregiver here can be challenging because if somebody "calls off" the number of residents can go to 15+ per caregiver. It would be challenging because even if somebody was to call off for their shift you would not receive aid by the supervisors, I get they are always busy but sometimes they had the ability to help the caregivers when short handed but they never did. The PM shift(caregivers) does a MAGNIFICENT job, it's amazing that even when they are short handed they always get the "job" done with no aid by the supervisors. Within 3 weeks of working at Jasmine Terrace I was promoted to "Med Tech". It was truly challenging considering I had one week to learn and remind over 70 faces and names of residents I was responsible to give their medicine to. The only aid I received by the supervisors was recognition of residents. There is no proper management at Jasmine Terrace, considering I have retail management experience I saw MANY discrepancies that were never dealt with. Residents at Jasmine Terrace do manage to escape the facility, residents do get in carnal situations, residents do spend hours alone with nobody interacting with them while they are in the activity room, residents do have mattresses that are old and appear to be infested, residents are not fed on time or even at all at times. I hope this place can soon provide the care the residents deserve and need. read more

My father is doing great at Jasmine Terrace! My father needed an assisted living/memory care facility as he has Alzheimers and it was no longer possible for him to be cared for at home. Luckily, I found Jasmine Terrace! From the moment I started looking into facilities I felt very comfortable at Jasmine Terrace. My first encounter was with Lori, who gave me a tour and explained everything that the facility had to offer. Lori was so helpful with all my questions and concerns. She was so caring not just for my father but for me and making the decision of moving my father into a facility. To this day Lori has a genuine concern for my father's well being. My father has been at Jasmine Terrace for over two months now, and he is doing so well, so much that his doctor agreed to lower his medications. Every time I visit, he is clean & shaven and his room is always clean. His day nurse "Glenda" is great with him! The night staff and all the aides are so caring & comforting. I visit him a lot and I watch how they care for all the patients. I know with all the different levels of patients, their job can't be easy, but they make it look so effortless. They really know their patients, and they have really gotten to know my father too. When my father and I walk around the property, every staff member knows him by name and always say hello and get him to smile. I was worried about my father wandering or trying to "escape" from a facility, but this has never happened. I attribute this to the level of care and great staff that watch over my father. Although it is a memory care facility, most patients have the ability to walk around the entire facility & courtyard - this was perfect for my father who loves to walk constantly and loves being outdoors in the courtyard. It's all about money, not patient care at Jasmin Terrace. Totally substandard care at this place. Check with the State of California -- they have been investigated and fined more than once due to inadequate staffing. That is to say, it is not clear that have enough qualified staff on duty much of the time (i.e., high ratio of patients to staff; staff mostly CNAs, not RNs or even LVNs). The result? Patients yell incessantly and are not adequately taken care of, alarm clocks go off for hours in unoccupied rooms, etc. The management of pain and dementia demands more qualified staff. At first glance it looks nice -- the location, landscaping, fountain, pool, etc. But if your loved one needs kind and competent care, look elsewhere. read more

My family chose to place my mom at this location for a few months. We were absolutely amazed at how professional, quick, and efficient the staff and administration was in transitioning my mother in. The nursing staff was helpful and my mother spoke fondly of them. The staff was amazing, and I noticed how much they cared for her and all of the other residents. The food was good, the holiday gatherings were amazing. Lori, who we worked with for the transition, was professional, caring, and a strong leader. The only reason we moved my mom out was because she needed a space with different services. We are forever grateful to the wonderful team of Jasmine Terrace. They are miracle workers. read more

My father spent his last 3 years at Jasmin Terrace and I will forever be grateful for the outstanding administration there, and top-notch, wonderful caregivers and staff. I can't say enough about the consistently wonderful staff I encountered throughout my dad's stay there, in addition to their excellent professionalism and competence. They were also problem-solvers beyond any expectations when various issues, some quite complex, came up. With all the responsibilities they were handling, they still went way above and beyond. We found Jasmine Terrace in a panic when we needed to move my mother-in-law quickly from another facility because of her deteriorated medical situation and am so glad we did. Virginia, the Exec Director, was so welcoming and made the intake process simple and easy for us. Have found the service, care and facility to be excellent - from Denise (and everyone) at the front desk to wonderful Lori who's head of patient care and the primary family liaison. The facility is beautiful and clean, and my MIL's room was spacious. They have daily entertainment and the atmosphere is cheerful despite many of the resident's diminished mental capacities. Their philosophy is to get the residents up and downstairs in the community rooms, every day instead of having them lay in bed. Because my MIL is immobile/unresponsive, this is a huge hassle for them but they do it with such grace. I feel like they put her needs first rather than their convenience. Most importantly, the people who work here are absolute saints. I've never had one occasion to find fault with the service or attitude of a single staff member. I pop in at odd hours several times a week and my MIL is being patiently hand-fed and changed. I am very grateful to Jasmine Terrace and highly recommend. read more

I have seen some snake hole skilled nursing facilities, but this one is not one of them. It is much better compared to the ones I have been to. I was only compelled to write this in response to the one star someone posted. I thought I would put in my two cents since I have seen many of these types of places. At least they have a Board Certified Psychiatrist that comes to see the patients once a month for those that have Psychiatric issues. It is clean, and though you don't have as many RN's, there is a good system in place to address most concerns, and it is well within the standard of care. This DISGRACEFUL facility sent my mother to the hospital for several days last month and NEVER CONTACTED ME. They told me that 'the family was informed' which is a outright LIE. They currently have my mother SLEEPING ON A MATTRESS on the hardwood floor because she fell out of bed and injured her head, which once again, no one bothered to call me. In September my mother was punched repeatedly in the head by a resident so I have reason to believe she was brutalized once more and Jasmine Terrace is covering it up. When I spoke to the 'Executive Director' Virginia I was told my mother said something TO PROVOKE THE ATTACK. My family is paying thousands of dollars a month to have my mother sleep on the floor and be physically abused. Jasmine Terrace refuses to move my mother to a carpeted room, nor will they provide a bed with protective railings or any satisfactory explanations. The 'Executive Director' (Virginia) also refuses to return my calls. I still have no ideas why my mother was hospitalized. STAY AWAY from this God-awful place. I am filing a complaint with the State Department of Public Health. read more

Believe me, this was my mom I was placing, and I did my research. Senior Care Network at Huntington Hospital recommended this Memory Care facility, my mom's internist said that she works with a gerontologist who thinks Jasmine Terrace is one of the better facilities, and one of my mom's physicians at Huntington Hospital when she had a stay there said that Jasmine Terrace was one of the good ones, and we all know that not all of them are. Plus, what I found there on my own were a pair of administrators in Virginia and Lori who were honest, kind and caring and went out of their way to make my mom and our family happy. The facility is clean and the staff are friendly. My mom's caregiver "decorated" her regularly with big silky floral hair clips and sparkly headbands. There were an immense amount activities for the residents. My mom was happy there. Unfortunately, she started to need around the clock care and we decided she needed skilled nursing, so she is no longer there. I wish she still could be. read more

I really like Jasmin Terrace. My 97 year-old mom has been a resident for two years and I am so grateful for this place. A Dementia/Alzheimer's diagnosis is a heartbreak. While researching the best place for my mother I stumbled upon Jasmin. Thank God I did. Family-owned and run, with a dedicated and consistent support staff. Some of the residents do at times become agitated. That is part of their illness. Instead of sedating them they try to find other measures to deal with this. Recently, my mother had become a "screamer." Rather than giving her more powerful psychotropic medication the visiting M.D., Dr. Moore,discovered she had a urinary infection. She was given antibiotics and is back to her "new" old self. The staffing shortages were there when she first started but have been resolved for at least 18 months. The social services director Lori Lackey is so responsive and caring. Every night of the week there is different live entertainment. Every holiday they hold a fun party where all family is invited. A music therapist comes every week. My mom may not remember much but when she participates she experiences true joy. We love the diversity of backgrounds of the residents and I have become friends with other daughters who visit a lot, many driving from as far as Santa Monica or the San Fernando Valley to have their parents here. I can see why. My mother has had many hospitalizations and they always call me right away. Her follow-up care is excellent. There are fancier places, of course, but none are warmer and more caring. read more
