2-Weeks Post-Op

I'm only two weeks out from a deep plane facelift, neck lift, temporal brow lift, and upper lip lift with Dr. Sadati, and I'm already ecstatic with the results. I have almost no scars, very little swelling, and everything looks incredibly natural. Dr. Sadati is a true artist -- skilled, precise, and caring. His team was just as wonderful. I couldn't recommend him more highly!

I am 71 years old and wanted to regain a more youthful appearance. Looking online at Dr. Sadati's before and after photos and watching his videos on social media I decided to contact his office to inquire about having a preservation neck and face lift. After meeting with Dr. Sadati and some of his staff, I decided to go with him with no reservations. My aesthetic journey from beginning to end with Dr. Sadati, and the staff I met... Tim, Janet, Jocelyn, Daisy, Katie and Rhonda was exceptional. They made me feel valued and respected. Having aesthetic surgery elsewhere in the past, the contrast between Dr. Sadati and his staff was nothing short of remarkable in comparison to the other. Dr. Sadati is not only a gifted surgeon but he is a shrewd businessman. His facility is first class. Organizational aspects of the business reflect great organization, communication and instructions. And finally and more importantly, Dr. Sadati is genuine. A man with profound humility and kindness. Something one does not see often. A true gentleman in every sense of the word.

I had an excellent experience with Dr. Sadati and his team. I had a face and neck lift. At 62, I was looking worn out, haggard, droopy and my fleshy neck was plain awful. Now it is 3 months after my surgery and I feel wonderful. I particularly did not want general anesthetic because of the side effects. The twilight sedation that Dr. Sadati uses was a breeze. After surgery I had some bruising but not a huge amount. I took it real easy for 8 days then had my stitches out. That part I was kinda dreading but again, no pain at all. During the first few days I mostly took tylenol and pain meds a couple of times. I ate clean and drank lots of green tea and followed the aftercare healing protocol to a T. A friend who had the identical surgery with a highly regarded local doc here in Scottsdale AZ had a horrible recovery and now needs a revision. Even though I had to travel out of state to CA, I chose Dr.Sadati based on his extensive experience and the documented thousands of natural results of surgeries he has performed. I look better than I did 10 years ago! I wouldn't hesitate to given Dr.Sadati and his staff the highest recommendation.

Dr. Sadati performed a deep plane lower face and neck lift on my husband in the Spring of 2022. My husband had a hereditarilly bad neck. It was so bad it impacted not only his entire face but certainly how he felt about himself. I watched him transform from a man embarrassed about his looks to a man full of confidence and excitement about his future. When I saw all of this taking place before my eyes, I wanted to feel the same way. I am 66 years old with Irish-Ancestory skin and the ability to wrinkle earlier than others my age. My neck was close to a turkey neck, amongst other things that just made me look even older than I already was. So in January of this year (2024) I had the same surgery as my husband, along with a lip lift and a full CO2 laser on my face. Now I look 15+ years younger and feel like my husband feels. Dr. Sadati is the EXPERT in making you look yourself, just younger. And he is so warm, caring and humble it amazes me. His staff is equally special in their professionalism and treatment. He is THE SURGEON to do this work. No wonder he is considered the best in not only Orange County, but one of the best in the country. I have already referred several of my friends to him.

Went to see Dr Sadati to perform a rhinoplasty and chin implant (the latter recommended by him) back in 2015. He ended up botching both procedures. He scooped out the bridge of my nose, turned it up my tip too high and my left nostril had major issues. The chin implant was too big and looked disproportionate. I had a very costly and involved revision only a year later to correct what Sadati did. He never accepted responsibility and provided only excuses. Had the audacity to disregard the medical opinions of other respected surgeons. I'm writing this review so that I may help someone else from falling victim to the same fate. The following photos represent the before, the after with a Sadati and the present day after my revision. Also including the surgeon's notes and invoice. The facts speak for themselves.

This is going to be long. Beware. INTRO. 08/2020, had a consult for a septorhinoplasty w/ Dr. Sadati. He rushed into the room like Myrtle the Ghost darting in for a quick haunting, glared as I explained my concerns about deviated septum & bulbous tip. He said he'd "make my nose better" while doodling a joke rendition of my 'nose' w/ a blue pen on a notepad. No proper simulations. Seriously, my 5-year-old niece & the neighborhood drunk have better drawings. Still, I was naive & uninformed, thinking this was normal for a surgeon. I was wrong. SURGERY. 9/10/20. No complaints. I went home fine. On cast removal day, Sadati didn't seem to care about actually talking to me. I recall him walking in w/ his Chad social-media-guy to record an IG video, asking me to act surprised while telling everyone "how my breathing is better & how happy I am." I felt pushed to do this so I agreed. I was only 1 week out..how could I know my results, let alone honestly tell people I was happy?! Irresponsible to ask patients for this 1 week post-op. RECOVERY. Over the last 1.5 years, I emailed, called, and/or saw Dr. Sadati & employees a few times about concerns for my nose. They seem to believe that I didn't reach out, but I DID & have proof. I expressed concerns about a strange deformity adjacent to my bridge, large supratip leading into columella, & a collapsed nostril. Each time, it was insisted that these problems were "swelling". I naively believed it. At one point, in his office for my collapsing nostril, Dr. Sadati accused me of "forcing my nostril in" when I breathed in! Like, what. How is that even possible? How do you independently contract ONE nostril while you're inhaling, while keeping the other open w/o touching your face!? TRUTH. Last week I got so fed up of my nose still looking deformed, that I basically begged Dr. Sadati for a steroid injection for the deformity adjacent to the bridge. There has been NO improvement. I still have ALL of the same deformities that he called "swelling." So THIS week, I consulted w/ 10 double board-certified, & 4 overseas, world-renowned surgeons, all who said I appear to have these deformities following my rhinoplasty: pollybeak, hanging columella, collapsed left nostril, & bulbous tip still. Revision takes up to 8 hours & will need to harvest my own rib. REFUND REQUEST. I sent Dr. Sadati's office an email yesterday, requesting refund for the cosmetic part (since the septo part DID help my INTERNAL breathing issues & was covered by insurance). The response was from the "Director of Business Affairs" ( probably should be more appropriately changed to "Damsel of Bizarre Assertions"), Janet. 1st, she ignored my refund request, asserting they "did not bill my insurance" for the septo part of my surgery. Very funny Janet, because I have an email dated 8/27/20, from YOU to ME, w/ a cost breakdown & the words "billed to insurance" next to the septoplasty expenses, in WRITING. But let's say it's true for your sake; it's still irrelevant to my refund request for the cosmetic portion. Deflection at its finest. 2nd, Janet asserted that I didn't mention my concerns at my recent visit..umm literally my visit was FOR a concern. But ok. I'll give you the benefit of the doubt for that one. Maybe you meant I didn't mention my other* concerns - which is true. Because I mentioned them in prior complaints/visits!! This recent visit was ONE of numerous complaints! Make it make sense, Janet! Look at your emails & notes! You will see my concerns over 1.5 years. 3rd, she said "SHE" reviewed my photos; & my nose has "significant improvement". Wow Janet, you're a better plastic surgeon than the surgeons I spoke to, who trained at world-class institutions for life-long careers with verifiably happy clients. I'm honestly impressed by your surgical skills. Why are you working in business admin? DISCUSSION. Dr. Sadati prob still thinks my surgery w/ him was a gift to God's green earth. He'll prob reply saying my review is angry/cruel, ignoring the real and RAW emotion after $15,000 thrown in the trash. Or maybe he'll write a snarky reply like he seems to do to others (most bad reviews get deleted though I believe), trying to invalidate them. Saying things like they were a no-show to an appt, or that they said they were satisfied before & he doesn't get it, yada yada. Who knows what he'll say for mine. God knows. I feel like these people would summon a demon straight from hell if they could, to distract from the real issues. Proceed with caution. I don't believe these ppl care an OUNCE about the human beings that walk through their door. Pics of my nose & video of my nostril collapse below. I cropped the pics to focus on the nose. You don't need my full face to see this tragedy. Patients, msg if you need someone to talk to. This could've had MUCH more anger. But I tried to keep it as descriptive as possible, especially if it means saving someone's face.

I look way better in my before pictures (no track mark). Hopefully this review prevents something like this from happening to other people.

The surgeon was a triple board-certified plastic surgeon I had known for over 30 years. I trusted him and believed I was in excellent hands. The result was terrible. My breathing worsened and my nose looked unnatural. I lived with that outcome for over two years, trying to rationalize it, but I couldn't. This was my face. It affects how I breathe, how I sleep, and how I feel every day. So I did serious research. What I learned changed everything. Rhinoplasty is not just cosmetic. The nose must function. We are meant to breathe through our noses. When the structure is weakened or obstructed, airflow is compromised. Air must move freely when you inhale and exhale. The internal support has to remain stable so the airway does not collapse. Without that support, you mouth breathe, wake up tired, and constantly feel like you're not getting enough air. I experienced all of it. My research led me to Dr. Aaron Kosins in Newport Beach. Because mine was a revision case -- after my nasal bones had been broken in the first surgery -- preservation rhinoplasty was no longer available to me. I had missed the boat. Preservation rhinoplasty reshapes and repositions the natural structures of the nose instead of aggressively "hammering down" the bridge and rebuilding it from scratch. Dr. Kosins is one of the pioneers in using ultrasonic piezo instrumentation to precisely sculpt nasal bone rather than manually breaking it. This allows refinement of what a patient is born with while improving both appearance and function. Due to the prior damage, significant reconstruction was required, including cartilage taken from my own rib to rebuild and properly support my nose. If I could give one piece of advice: only do a piezo surgery! Dr. Kosins is widely regarded by his peers as a world expert in rhinoplasty, including preservation techniques and complex structural revision cases. Many established surgeons seek out his workshops and cadaver labs to refine advanced techniques. He has the rare ability to communicate directly, thoughtfully, and sincerely. From the first meeting, I knew I was finally in the right hands. And then came the result. It was a knockout. My breathing is excellent. Air moves freely through both sides of my nose when I inhale and exhale. There is no resistance, no collapse, no struggle to get enough air in or out. For the first time in years, breathing feels effortless. Cosmetically, my nose looks natural and balanced -- not artificial or overdone. I also want to acknowledge his team. Risa, the practice manager, was accessible and organized throughout the process. Judy was BEYOND amazing!!! After surgery she called and texted us multiple times a day, every single day, making sure I was doing well. We also had direct access to Dr. Kosins whenever we needed him. I never felt alone or uncertain. Choosing Dr. Aaron Kosins and his team made a major difference in my life, and I am extremely grateful.

I waited until the one-year mark to leave this review because I wanted to give my body enough time…read moreto fully heal and settle before speaking on my final results. Unfortunately, I am very disappointed with the outcome of my breast augmentation and with the level of follow-up support afterward. I had surgery on May 5, 2025. Around 4-5 months post-op, I began noticing significant asymmetry developing. By the time I returned for my follow-up appointment in September, the issue had already become very noticeable. At this point, I developed capsular contracture on my left side, causing the breast to feel hard, tight, and unnatural. On the right side, the implant dropped significantly lower than the left, creating visible asymmetry and uneven nipple positioning. I also experienced poor scar healing around my right areola, with the scar remaining thick, raised, and persistently itchy nearly a year later. To be fair, Dr. Kosins did acknowledge the asymmetry during my September appointment, discussed revision options with me, and his office provided information regarding the Allergan warranty coverage. I understand capsular contracture is a known risk of breast augmentation surgery and not necessarily the result of negligence. However, what left me disappointed was the lack of continued follow-up and support afterward. After the initial discussion regarding revision options and warranty information, there was no further outreach, follow-up phone call, scheduling assistance for next steps, or even scheduling of my one-year post-op appointment/check-in. As a patient navigating an upsetting complication less than a year after surgery, that absence of communication left me feeling unsupported and alone in the process. I never expected perfection, and I understand complications can happen with any surgery. But when complications do occur, communication, reassurance, and aftercare matter just as much as the surgery itself. Patients deserve to know not only how successful outcomes are handled, but also how they are supported when results do not go as planned.

That surgeon left me with a large, uncomfortable, and useless butterfly graft in my nose, a drastically altered nose shape, and did not address my deviated septum, which was the whole point to begin with. Dr. Sepehr was extremely patient and diligent in reviewing my medical history, operative reports, before/after photos, and goal outcome photos with me. Dr. Sepehr was not only able to address ALL of my aesthetic concerns, but he also corrected my deviated septum. I could not be more thrilled with the results of my surgery with him.
