I've had the luck of being a resident twice at the Laurelhurst Village acute care facility. I'm on the "younger side" as SNFs go (49 the first time, 50 the 2nd) but both times I was highly dependent for care. The facility works hard to keep you with the same staff for continuity, which I think is great. There is some shuffling (someone else may answer a bell or you may rotate to a different therapist due to scheduling) but they are all very kind and open to conversation based on your particular challenges/needs. I ate in my room as I had little appetite so I tended to graze. I have a plethora of food allergies so that was often a challenge, but I was supplemented by outside. I was always very aware of the respect the care staff had for my dignity and my privacy. It is much appreciated. I admire how much they work to make the residents feel they are involved in their own care and help keep them socialized and represented within the community. Activity is encouraged throughout-and often residents practice their improving skills through the hallways. I love that it smells clean and not like body fluids or even medicinal. I never saw anyone abandoned in hallways or even in the cafeteria. That being said, I couldn't give the facility 5 stars. There needs to be more RN support. I was told I couldn't see my primary doctor while I was In the facility because I was under the care of the facility's doctor-yet in over two weeks I never met the doctor. I think the CNAs need more credentialed support. read more

All the nurses will care for interest and your safety but when they as in fill in temporary they are not so great services. The food was not great but the nurses are great and staff too however they are short handed when you needed to go bathroom and needed assistance they do take long time to come to assist you. They are good listeners as in your needs like adding sugar to coffee or creamer ect. read more

I highly suggest never putting your loved one's in this place the care is horrible they do not care for your personal needs and the staff are all lazy, the food is atrocious and they ignore valid complaints and concerns. More concerning they do not get medications right, if my mind was gone I would have taken another patient's med's and they are disruptive during the night telling me that my diaper need's changing & I am able to go to the bathroom on my own. Granted the rehab is in another building, but this is what you're greeted with first. Half the building is used for long term care, the other half abandoned. Walked into rehab building, doors not locked, no one at the from desk or any of the offices. I wandered the halls on my own.... Sleeping patients, open doors, no security, I could have walked into any room and taken whatever and no one would have stopped me. 45 minutes later someone finally showed up in the lobby. read more

I wanted to check it out before going there for rehab. I called at 3 pm for 1/2 hr (repeatedly dialed bc the call times out after 5 mins??). No one ever answered. I'm leery of a care facility that doesn't answer the phone during business hours at least. read more

My mom felt well-cared for here while she rehabbed from a broken shoulder. Nurse Laura, and amazing CNA Willow helped her in her journey to wellness. Plus, us family members always felt welcome like it was an extension of our kitchen or living room. read more

This place is a giant filthy mess. First experience was noting feces on "recently cleaned" equipment in a patient restroom. Second was a broken support on a toileting device. I showed the staff and they didn't seem particularly concerned that the device could collapse and cause injury. Third is they do not have a working general number and no way to access voicemails. They've been "working on this" for months. Staff call and provide this non-working number for family to call back. With regard to medication issues, staff and their upper management stated "it's cool" when they mislabeled life sustaining medication and another staff member laughed when this happened. It took them over five days to get important medications from the pharmacy and it was only after repeated calls and family hounding the nursing director that they finally contacted the pharmacy. At least two falls occurred during the ten day stay specifically due to staff not responding to call lights. When my relative was transported back to the hospital, the nursing director never bothered to call our family. Personal belongings left in the patient room mysteriously disappeared and still have not been accounted for. If you want your relative to be left alone without proper care, this is the place for you. If you prefer to have people who actually care about their patients, go somewhere else. Finally, when reviewing the most recent state assessment of this facility with the nursing director and asking specific questions about the report, she was combative and mildly hostile. I toured Laurelhurst Village with my step-father this week. He's disabled and has decided to move into an assisted living place. I was pleasantly surprised by how lovely this place is. The staff is super kind, it's very clean, and the residents seem very happy to live there. They offer short-term and long-term care depending on your needs. It's next to a beautiful park, and there are shops and restaurants around. My step-dad really liked the place and it's definitely at the top of our list. read more

In September 2020 my sister-in-law was in the rehab unit at Laurelhurst for the last 2 weeks of her life. The care given to her by all of the staff and Hospice was outstanding. Because this was obviously an end of life situation close family was allowed to visit with her at anytime. Beverages and snacks were available for family every day and someone was always available to answer questions or give advice. The private room was clean and well appointed. The public meeting space for family and social workers was pleasant and comfortable. We were pleasantly surprised! read more

After looking around at other nursing homes, we found that this was the best one. It's very clean and bright and so far everyone has been really nice and helpful. My grandm is only here for physical and occupational therapy until she can get back up and moving after a broken hip. I was wavering between 3 and 4 stars because yesterday, my grandma told me when they went to take her to her physical therapy, they weren't ready for her yet, so they just left her in the middle of the room in her wheelchair with a newspaper. Being a 90 year old woman who is very week right now, how was she suppose to read her paper without a table in front of her to lean it on? They left her for 2 hours with nothing to do! I know that everyone is really busy here, but they shouldn't take residents from their rooms until they are ready for them. As far as the food goes, it doesn't look very good and my grandma isn't a big fan. They do allow you to bring in food foe them and even have a refrigerator that they can keep it in, so I thought that was a good touch. Like I said before, we looked at a few different places for her to go, and if your loved one has to go somewhere, I would pick Laurelhurst Village. No home is going to be perfect, but this one is clean and has a friendly staff. read more

I was a resident in the rehab center 7/25 to 8/9, 2015. DO NOT Use this facility! Even though rehab team is great, the rest of the care is horrible. I am a celiac, diabetic and had a broken femur. They did not have enough walkers so I had to ask a friend to buy one for me. Before I was able to help myself, I frequently was left waiting for 20 to 40 minutes. This is terrible is you are in pain and left on a bedside commode or having a low blood sugar! The Certified Nursing Assistants were too busy or just did not care. I heard them gossiping outside my door when I had pressed the call light for assistance. I was served gluten on my second day there and was sick as a result. It was so hard to fight the staff to get the correct diet that I contacted friends to bring in juice for my low blood sugars and food I could eat. After I left, I emailed the director a list of events that had occurred. He made one attempt to call, but was not available when I called back. Later, my comments with suggestions were met with excuses. Conversation was frustrating because I was told that some of these events had happened before! Shame on Kaiser Permanente for requiring their NE Portland patients to go here. read more

I would give this place negitive 5 if possible. My father has been to this place twice now. The first time was short term and went okay. I think they switched management because this time was a nightmare. My father is a stroke patient with a paralyzed left side. He has cognitive issues. He also is a dyalisis patient and has both his kidneys removed. He fell and broke his femur bone. He has a external cast which is made of two metal rods which hold his leg straight. This place reached out to me and told me my father wasn't making any progress so they wanted to discharge him. I expected them to realize that he would not be able to return home do to the fact that he has a broken leg. It was a no brainer to me that my dad can't make progress until his cast is removed and healed. I thought they would let him stay and then once cast was off they would do physical therapy to gain his strength back in the leg. Well, that read more

My husband was transferred there from OHSU because of a brain bleed. The rehab portion of this place is a joke. They were supposed to spend 60 to 75 hours a day with him and that didn't happen. Instead of giving him rehab to get mobile, they never did the same activity consistently so he was confused as to what he was to do when. The therapists were always looking at their watch and I never did figure out why. There was a 50% turnover in upper level staff; nurses, head nurse, social worker, etc. The only non turnover were the CNAs. They made transportation arrangements for the day of discharge, but had to cancel his ride because they couldn't find his prescriptions that were to go with him. I would NEVER recommend this place. read more

My father entered this facility to receive TPN (nutrition through a PICC line in hopes to improve his health. We were trained to do this at home but it was to complicated so he was admitted here. This was all new to us and it felt promising. I called the facility and asked that they let us know when my father arrived. The woman on the phone was very helpful so I thought this could be good. I NEVER received a phone call. After calling them back and finally figuring out what room my father was in I went to visit. There was no one at the front desk to help us so we just went in. The place looked clean, but I could not find staff to give us directions. I was told on the phone that there would be a wheelchair there to borrow so that I could bring my mother in to visit. When I inquired they said we don't do that. The next day we were there for my dads TPN treatment and while watching the nurse hook up the food bag to the PICC line, there were not alcohol wipes. When I inquired, he said there was not need for them. Well, when we were trained at home we were told that was the most important thing was to keep it clean because it was food and would attract bacteria. No alcohol wipes. The next days that we would visit my dad would complain that he was left on the TPN for 14 hours and when I check with the nurses, they said that they were busy and sometimes they could not get to him in time. There was not communication with these people unless I asked. My father complained that he was left in the room alone and was told nothing about his care. As we spent more time there I realized that the nurses and doctors didn't even know what he was admitted for and didn't talk to each other or us. When I would go to the nurses desk most of the time they were joking around with each other and I had to stand there an wait. One day there were joking about sending the new person to do the "digital exam" on someone. I'm pretty sure you can guess what that is. The attitudes of the staff in this place was basically the same as an entitled person. Every time I went to inquire I was told something different. I was told that my dad was a difficult patient so they just ignored him. And by the way wouldn't call me or my mother to let us know anything. We had to learn that my had had end stage liver disease from a doctor that that just assumed we knew. The last straw was when I got a call from some director that I've never met explaining that they will arrange transportation to get my father home and that the hospice nurse will meet us there. The next thing I knew was my mother calling me and telling me that Laurelhurst sent my father home in a cab in his socks (no shoes) and made him carry his own stuff out to the cab. Had I known, I would have picked him up. It was an unsafe act to send a patient home by himself in a cab and not let anyone know that this is happening. This place should be fined for an unsafe act and they should seriously consider changing their management. I'm sure people have had good experiences here maybe. But my suggestion is to check out the place thoroughly before you let a loved one stay here. read more

This place should be shut down. I work in skilled nursing and this is one of the worst nursing homes I've seen. My grandma had to be here a couple of times because she had Kaiser and there wasn't another facility with a bed open that takes Kaiser. The physical therapists are borderline incompetent. My grandma was partial weight bearing on her one leg and they tried to make her go up and down stairs because she had stairs to enter her home. They just wanted to kick her out even though she lives alone and can't even get in her house! You can't walk up stairs AND be partial weight bearing. Her doctors orders stated no doing stairs, but they didn't maintain any of her MD orders. The rehab manager is rude and unprofessional. The social worker/discharge planner was also rude and had no desire to speak with family members or coordinate care. The food came cold EVERY DAY. She couldn't go from sitting to laying down by herself, but after helping her to the bathroom, the aids would leave her sitting on the edge of the bed and then leave room! The aids rarely put her call light within reach or the bed side table with water near her. When she needed help, she couldn't call the nurse. When she was thirsty, she couldn't get to her water. When she did have the call light near her, and she called the nurse for help to the bathroom, the average wait time for someone to answer the call light was 45 minutes. (these are all violations of State law, by the way.) I could go on and on. This place is horrible. The therapy department is useless. The care from the nursing staff is negligent. The management is rude and even down right mean. If you care for your loved one, don't let them be admitted here. read more

Laurelhurst Village is a beautiful campus, the admission staff is very friendly and inviting. The therapy does a great job. Big private rooms and bathrooms were a big plus. read more

My mother was admitted there today. She left the hospital at 3:45 and I have called twice in the past two hours with NO call back. They have a phone machine at the nurses station and it says to leave a message and that they will call you back. Now, I don't know how long is too long, but I really have no idea if my mother has even arrived or if she is okay. I tried to get to the facility but the roads were not okay and my car does not do well in the icy weather. So, does anyone there think that it might be a good idea to really return a call to a family member who is concerned about how their mother is? More on this later. I am not happy with their lack of communications and we will see how their care is. By the way, I am a nurse and have worked in and been in nursing management positions in nursing homes. I have been a nurse for over 30 years. I would not tolerate this of my staff. So, you bet I will be writing on how this goes with mom's rehab. read more

My brother recently moved into Laurelhurst Terrace, the Assisted Living section of The Village. He is on the younger end of the spectrum of residents there. After 4 months I can honestly say he loves living there. It's a real community and the staff who work there are fabulous. They have the right number of activities and beautiful gardens, which the residents get to work in. All in all, I could not be happier for him. We are so lucky to have places in our city like Laurelhurst Terrace. read more

The staff at Laurelhurst Assisted Living--The Terrace--has been loving, responsive and conscientious. Father John came to visit my mother and encouraged her to make the move which was so helpful for us. There was a change of ownership and 3 changes in managers in the 2 years we were there but everything was seamless. When my mother's dementia increased, they helped me find a Memory Care Facility which at this time they don't have. Our family is grateful to everyone at Laurelhurst Village! read more

Great staff, they work hard and the care is great. Food tastes good and the building is clean. I highly recommend Laurelhurst Village.

I'd like to say I have a parent in a caring, detail oriented care facility but it is not. CNAs range from excellent to rude, or just stupid lazy louts bothered by resident seeking care. They forget they are in the customer service business. Meals are forgotten to bed confined residents. No follow through on concerns other than a lot of "Balh, blah". Often short of staff with some CNAs working double, sometimes, triple shifts. Is this legal? Lack of professionalism. Half the nurses are skilled really good, detailed care services who take initiative, others could not care less and are only there collecting an undeserved paycheck. One actually whispered in resident ear after returning from the hospital, "your friends have died". Changing times range from 10 - 45 minutes, lack of details (like a really hot room without changing gown after resident requests, led to yeast infection), an outside flobotomist when service refused by patient insisted on duplicate blood draw causing documented massive injury. I wonder if the health department ever checks in. Mgt is quick to cover up. Still I hear this place is better than most. Kudos to the CNAs who are saints and the therapists, you know who you are. read more
