My father in law was repeatedly telling staff that he could not breathe. His daughter was called and told that staff could not calm him down. By the time she arrived, he had called 911 himself from his cell phone because he was still struggling to breathe. What made the situation worse was that a staff member told his daughter that she had spoken with a director and that he would not be welcome back if he left by ambulance because of the way he was acting and because he had called 911 himself. He was not simply being difficult or agitated. He was transported to the hospital and found to be in acute hypoxic respiratory failure and suffering an NSTEMI. His condition was serious enough that he was intubated and placed on mechanical ventilation in the emergency department and then admitted to the ICU. The hardest part for our family to accept is that he was repeatedly saying he could not breathe, yet the situation appeared to be treated primarily as a problem of calming him down. He ultimately had to call 911 himself. We are also extremely troubled that calling 911 for emergency medical help was reportedly used as a reason to tell his daughter he would not be welcome back. We have submitted a formal complaint regarding what happened. Based on our family's experience, I cannot recommend this facility. read more

I was at Lincoln Meadows for rehab for my right hip replacement. It was such a good experience. From the Nurses to the CNA's and PT, you couldn't ask for anything more. They treated me with respect and kindness. The Facility was clean and smelled pleasant. I highly recommend Lincoln Meadows Care Center. read more

At first, I was skeptical in admitting my mother-in-law here in this facility based on the reviews. She's 87 years old and was admitted for a pelvis fracture and more importantly just to rehabilitate her to learn how to get out of bed and to walk with a walker. I don't generally write reviews, but this place deserves a great recommendation. I must have contacted them at least 5 to 10 times in that week that she was there, and there was not one single time where they were not courteous on the phone and helpful. You can hear the kindness in their voice and the concern. When I visited my mother-in-law, I can see that her area was clean and tidy. Most importantly, my mother-in-law was in good spirit and did not seem unhappy. It took her a day or two to get comfortable with all the noise and surrounding, but they somehow got her comforted and calm because she didn't have any complaints after a day and that's important. Just want to congratulate your staff for doing such a great job for people that are in need, especially Siada. Thank you. read more

I don't like giving one-star reviews, but unfortunately my experience warrants it due to one specific employee. Her name is Saida. She works for Kaiser and is onsite at Lincoln Meadows, and my direct interactions with her were extremely disappointing. She has virtually no bedside manner and made an already stressful situation significantly more difficult for both my loved one and our family. Rather than providing support or guidance, she came across as dismissive and pushed board-and-care options in a way that felt inappropriate and uncomfortable for someone in need of housing and care. Since this experience, I have spoken with multiple people who have had family members at Lincoln Meadows, and a number of them have independently shared similar concerns involving the same individual. This is a small town with only one nursing home -- word travels quickly. I understand she is a Kaiser employee, but knowing that this appears to be a repeated issue makes it shocking that she is still in this role. Her involvement completely overshadowed what otherwise could have been a positive experience. If it weren't for this interaction, I would have rated this location a solid four stars. Unfortunately, her conduct ruined what should have been a supportive process during a very difficult time. read more

My mom had a recent day at Lincoln Meadows for 2 two weeks after having her hip replaced this place has definitely improved since my grandmother was here four years ago and it wasn't as good. Glad to see that they have new ownership and better management. The staff who work over at this location are great. All the CNA's were super friendly, very helpful med tax physical therapy. Everybody took really great care of my mom and made sure that she was receiving the care that she deserved. Make sure she was getting her medicine that she needed when she needed it. They were definitely very on top of it. Definitely a place to look into if your loved one has to go here I would strongly recommend checking out their website and going in and checking out the facility very clean smells very clean very great establishment and they make sure that you have all three meals every day.thank you Lincoln meadows care center you guys are awesome read more

The day shift staff seem to care about the patients but not so much at night. My brother in law tried to get help to use the bathroom facilities but no one EVER showed up. He slept in soiled clothes and bed the entire night. They sent him home after 20 days PERIOD. He was not ready to go home as he still could not get out of bed without serious help. His wife is 83 and unable to help him. You have to ask, does this place only care about making money? I don't recommend this place. read more

The people who work here are kind and caring. Unfortunately also overworked. My husband was here for 2 weeks and I'm very grateful that I had somewhere close by that could care for him during his difficult time. The food left a lot to be desired but I often supplemented. I wish the PT patients were separated a little more from the residents because sometimes the hallways were loud. Bless their hearts. I'm devastated. My father was admitted to Lincoln Meadows care center after being in the ICU at Rideout for kidney failure. I had to listen to the strongest man I've ever known become distraught, inconsolable, and suffering. Not only were both of his legs broken while in their care, but he also passed away, and the hospital refused to inform me, his next of kin and only daughter, that he had died. Every time he called me he was crying. He passed on September 27th, and I did not find out until October 14th. I had to literally call every funeral home and coroner in 3 counties before I could locate my father. I'm heartbroken. Completely destroyed. He was only 67 years old. He was my dad. And he never even got to meet his youngest granddaughter. If you're thinking of sending your loved one here, only do it if you can be there daily to advocate for them. I live so far away, I had no say in where my father was placed and couldn't be there to make sure he was receiving quality care. And now he's gone. And I will forever be haunted by trying for hours to locate his body, without any information on how, when, or where he passed away. Tragic and unforgivable. At least for me. read more

For the amount of patients (92) the care center is lacking support due to California lacking funds.

Following a short hospital visit, my father went there "for one week" for physical therapy. They never worked toward his end goals in physical therapy; they refused to listen. I kept telling them he was in a weaker state than when he arrived. Their "care" was extremely inattentive and neglectful (pee in your bed - we'll come in and clean you up later). I arrived one afternoon to find him sitting in a wheelchair in a wet diaper - he said he'd been ringing the bell for 3 hours because he had to go to the bathroom. They told him they were busy and they'd get there later. I told him to press the button now. After 15 minutes, no one answered and there was now a puddle on the floor. I went to the nurses' station where a nurse was texting on her phone and another nurse was in the office. I stood there for five minutes listening to the alarm from his room beep until finally the nurse in the office saw me and she found someone to send into his room to clean up. This was not an isolated incident. My Dad hurt his back when he reached for a light chain at the head of his bed. They said the x-ray was negative and that he must have pulled a muscle and gave him pain meds/muscle relaxers. He was left in a sedated and cognitively disoriented manner. All the while he was still screaming in pain every time they rolled him in his bed or sat him up. I asked several times to have the doctor examine him, but the best answer I could get was the doctor wasn't there that day (the normal once-a-week visit) but maybe the next day or the day after. Two weeks after his "pulled muscle", he was transferred to the Sutter Medical Center. The ER thought he had a heart attack or a stroke because his BP was elevated, his heart rate was erratic and they thought he possibly had dementia. After four days, I convinced them to figure out what was causing his back pain. Tests revealed he had broken his back! He also has a large area of bed sores on his tailbone and some pretty impressive bruises on one of his legs and one shoulder. The sad thing ... this place has some very nice people. I think they are very short-staffed and need some good training. For the most part, it's a well-kept and clean facility. read more

Initially, I thought this place was decent, only because of one nurse, that took really good care of my grandmother. After reading the reviews I was scared for my Grandmother. But still I gave them the benefit of the doubt.... 1) they " lost " my Grandma's clothes, on multiple occasions 2) while visiting her several times, multi times I observed one nurse roll her eyes every time she came in the room. 3) they kept the prescribed pain killers my Grandmother came in with. 4) they wrote up a summary and gave it to her doctor, total bullsh!t lies. Made her seem like she was totally dependent on the care center & incapable of fending for herself, which is a lie. She couldn't wait to leave, she was counting the seconds. This place is a depressing money pit. Do not send your loved ones here. & Don't try to ask me to reach out to you, because I effing won't. read more

Hi I've been a patient for over five years and currently am a patient. The management is very negligent!! The patient care is outrageously slow and poor! Would never recommend for anyone or any family member period! Kitchen manager is wonderful!! Med Nurses wonderful. House Cleaning wonderful. Activity Dept. Wonderful. Too Now Update 2024 AC not functioning properly for weeks on end!! Covid running rampant!! Constant equipment malfunctions!! Charlie's doing better in the kitchen!! Sherries doing wonderful in activities!! John's firing em left and right!! Things are still coming up missing here and there !! Fireworks show was wonderful!! Our mom received horrible care here. She eventually developed bed sores which became infected and caused horrible pain. It was very difficult to get her medication on time so she suffered in pain. Don't ever let a loved one go to this facility. read more

Less than a star! They are super understaffed , when the residents have to go to the bathroom, they make them sit in their bed and go on a pad so the Residents have to sit in their own poo and pee until a staff member can come along and clean them up. My mother now has a raging UTI and for some reason, the doctors or whoever is supposed to take care of those things is not prescribing her antibiotics. She's had a raging UTI for more than a week now. As a person who is non-ambulatory, it seems that those things should be taken care of immediately. I am absolutely disgusted by this place. read more

THEY KEPT HER CLOTHES and SENT HER HOME IN A HOSPITAL GOWN. They "disappeared", nobody tried to find the clothes. She wore the clothes during her time at Lincoln. My guess is that either they steal patients clothes (and other things) or neglect. Neither is good when you are in taking of patients. On Sunday, staff told Mom to get dressed, getting discharged. When she returned to her room to get dressed, she was told they "needed the room" and transfer to another room. Staff was gathering her contents, placed in a bag and moved her to the new room. From the time between being placed in 2nd room and bed time, she asked multiple staff to get her clothes. Nobody addressed this. "I'll check on it". On Monday, she again asked for her clothes. Again, same thing. She was discharged on Monday, wearing home in hospital gown. I HAVE NEVER SEE A MEDICAL FACILITY DISCHARGE SOME WITH THEIR HOSPITAL GOWN. YOU ALWAYS GO HOME IN YOUR CLOTHES. After getting home, she told he that other small items (but are important to her) have also "disappeared". She says that generally the place was ok, but there may be some neglect on changing sheets, is why there is only a 1 star. read more

As a nurse who's worked at this facility before and has had loved ones be patients at this facility, I highly recommend it. They provide top notch care around the clock and have an excellent rehab department. The administrator and management staff are always available to assist with any needs that arise. The facility is beautiful and spacious and the staff is highly qualified and trained well. You or your loved one will be in great hands at Lincoln Meadows. If I could give this place 0 stars I would. My grandmother was recently here for rehabilitation after surgery and they were absolutely no help. I work in an assisted living facility myself which is understaffed with over 100 residents. Compared to this place which has plenty of staff and no where near as many patients. I know how it is to be busy and not be able to answer to so many calls at once but there was no reason for my grandma to wait over an hour to be assisted to the bathroom. The care is horrible. They left her sitting in a soiled depends constantly, and would not get her up for meals or activities until my mom called and complained. This caused her to develop a bed sore. So if you take anything from this review, let it be that you shouldn't take anyone that you care about to this facility. read more

Toxic place to work at. Extremely short staffed. I feel bad for anyone that works here. Am/Noc Nurses and some CNAs definitely carry this place administration and staffer make this place horrible. They just get patients in knowing they don't have the staff, without the staff we can't give the patients the care they need. read more

The care you get at Lincoln meadow care center in Lincoln California is horrible. I wouldn't even take a dog there, a homeless person gets better treatment on the streets. I feel bad for anyone who has to be in there. I had major back surgery number 4 on 12/4/2023. I was in the hospital for a week, which I thought was a bad hospital treatment too, but they were angels next to this place. I thought I was going to a rehab like the one I was in for a month in las vegas after my 3rd surgery. That was a great place, but they stuck me in a rehab/ nursing home,and they brought me here at 10:30 pm, and took me to where I call the nut house, thet had patients screaming at each other, screaming at the nurses, tge nurses screaming at the patients, I couldn't sleep with the yelling of omg, omg, I can't breathe I can't breathe help me, and nurse wheres my f__ing medicine, help me, the nurses took forever to come into the room when n u call for them. I got there to be rehabilitated, I can't BLT, which is bend, lean or twist, and I'm doing it all cause they don't help. I need help, walking to the commode, getting wiped, getting back in bed, getting washed cause I'm not supposed to bend twist or lean, and I did it alone, even after the pt nurses told the nurses she needs ur help, she can't do it, I had a issue with getting my pain meds, they don't wake u up to eat or take ur meds, this is a place in hell, please don't bring ur parents of any family members here, if I could give it zero stars I would, I have anxiety attacks cause of my pain and nobody to help, if i dont get rehabilitated or if something goes wrong because i do it myself, i will be suing. Don't ever put your loved ones in this snake pit! My mom went in due to she broke her hip. She was in there 5 weeks and came out not able to stand or walk. She lost 15 lbs. in there; they didn't assist her with eating knowingly she has dementia. They never bathed her only sponged bath. Didn't wash her hair for 5 weeks. When I asked the supervisor Mary Jo when is her appointment to see the Orthopedic Surgeon. She flat out lied and said she didn't see that there was an order from the hospital doctor for a follow up. Yet the Friday before my mom was scheduled to leave the facility that Sunday, the supervisor called me and said she has an appointment the next week. What a liar! She should know that a patient who has broken their hip needs to be seen by an Orthopedic surgeon for follow up. She is a RN and knows this! The PT Cameron is also useless. He's the reason my mom can't walk or stand; she's so weak. In addition, first he was making my mom use s walker that was way too tall for her. Then he started using a a shorter walker that was broken. My mom had to go to the restroom and when I put the light on for assistance, no one came for 1/2 hour. I had to find someone to help her. They just let the patients sit in their own waste. The floors are dirty and so are the walls and bathrooms, as well as items missing. They had Covid patients mixed in and sharing bathrooms with non Covid patients. NEVER put your you loved one in this facility. It should be burned to the ground. read more
