They said to fill it out quickly and they *try* to back to me before the weekend. I spent hours being very thoughtful and returned the form within a few days. Then the frustration began. I followed-up several times and got quick responses or no responses. I was told twice that they would *try* to call the next week but they never followed through. If you are too busy to take on new customers I'm perfectly fine with that. But if you're not up front you end up looking incompetent and inconsiderate as you string them along to nowhere.
