We are extremely grateful that we chose this rehab center for our mom after she was discharged from the hospital following a recent fall. Mom stayed for six weeks. When she first arrived, she was unable to walk. With the rehabilitation and care she received, she improved to the point where she could walk smoothly with a walker. We couldn't be happier with her progress. What impressed us most was the people and the leadership. When we first arrived, managers from different departments came to introduce themselves and genuinely tried to accommodate our requests. Josephine even gave us her cell phone number and told us to call anytime if we needed her help. Laura checked Mom's room every morning and promptly addressed anything we brought to her attention. The speed of resolution was truly amazing. The CNAs, especially Timothy, Brandon, and Theresa, were kind, patient, and very attentive to details. I remember one small moment that really stayed with me. One day, I was walking behind Timothy when we passed an elderly gentleman sitting in a wheelchair. His pant legs were unevenly rolled up. Timothy noticed it, stopped, bent down, and carefully fixed the pant legs for him before continuing on. It was such a small gesture, but it showed me how closely he paid attention to the residents and how much he cared about their comfort and dignity. Brandon and Theresa were also wonderful with Mom. They were caring, patient, and attentive to her needs. We always felt that Mom was being treated with kindness and respect. I want to specifically mention all three of them because their daily care made a real difference in Mom's recovery and in our peace of mind. Nurse Maria was always on top of Mom's medications and care, and she always greeted us with a smile. The admission/administrative staff were also wonderful. They worked with us to find the best arrangements to make Mom's stay as comfortable as possible. Administrator Adam was very friendly and approachable. We could see how his leadership and the management team's commitment made a real difference in the way the center was run. The facility is older, but the warmth and caring of the people make it feel very welcoming. When Mom was discharged, we were happy to see her walking again, but we also realized how much we would miss the staff. Thank you to everyone who took such good care of Mom and helped her regain her ability to walk. We truly appreciate Timothy, Brandon, Theresa, Maria, Josephine, Laura, Adam, and the entire team. Their marketing and admissions team is very responsive, collaborative, and fantastic at keeping communication seamless. The outstanding social services team is always available to assist with care coordination and discharge planning. I had the option to go home with home health after my knee replacement surgery, but I thought it would be easier on my family to go to a post-acute facility instead. I chose Los Altos from a list of facilities I was given and I'm so glad I did! Their rehab team was amazing and helped me reach my goal of going up and down stairs pretty quickly. The nurses and CNAs checked in on me regularly and helped keep me as comfortable as possible, whether that meant giving me my medications, helping me get out of bed, or passing along questions I had for the doctor. Everyone I interacted with was friendly and made the stay a lot easier than I expected. I'm normally a really active person, so I thought I would be bored during my stay, but they actually had a lot of activities and entertainment. I enjoyed things like gardening and movie night. The food was good too, which honestly surprised me. You always hear about hospital or facility food being terrible, but there was a lot of variety. I'm diabetic, so I did have some limitations, but the dietitian worked with me and my preferences so I still had things I liked to eat. I had never stayed in a skilled nursing facility before, so I didn't really know what to expect. I ended up having a really good experience at Los Altos and would recommend it to my family and friends. read more

These guys get a tough reputation and I was a bit skeptical at first but a family friend recommended this location after their father had been there for a good month. Now that my mom is back I can't say enough good things about Los Altos Post Acute and the care they provided for my mother. From the beginning, the entire team was kind, attentive, and genuinely cared about making sure she was comfortable and well taken care of. While there were a few bumps in the road at the beginning everything was fixed promptly and I felt my mother was in good hands. I especially want to recognize the therapy team Macey and Matt were amazing! They did an incredible job working with my mother and were always patient, encouraging, and supportive throughout her recovery. They took the time to work with her at her pace and helped her make progress much faster than we were assuming. It was clear that they truly cared about her success and getting her stronger. Knowing my mother was surrounded by people who cared about her gave our family a lot of peace of mind. We are very grateful to the entire Los Altos Post Acute team for everything they did for her. The great social services team sent my mom back to us with in home services covered by her insurance to assist us in this time of need. It's not easy when your parent gets to that age but they've helped us through every step after her hospitalization. Im giving 2 stars but it should be zero for this hell hole. Its starts in the parking lot with never enough spots for visitors to park and the incredibly rude valet attendant. First after they changed his wound care orders these quacks said his insurance didn't cover that type of wound care and tried to discharge him. My spouse advocated for himself and called his insurance member services to verify what's cover vs not covered. Member services told him the wound care and stay at the facility was fully covered. He has Kaiser medi-cal. After my spouse told case management and the dictator opps I mean doctor what member services They stopped providing the 45 min physical therapy sessions and changed it to 15 min sessions and the therapists stopped working with him to learn how to walk again with his prosthetic. They infact stopped helping him altogether and he just went to exercise on the hand bikes. The whole reason he was there was for PT and wound care. Lets get into the food and portions served. The food lacked adequate nutrition, and they watered down the juice and provided a toddlers portions to a grown man. Not to mention extremely small serving of fruits and veggies. Its like they want to keep people sick and not healthy so they can collect more from insurance. The doctors and nurses failed to enter new orders or end old ones. They never followed his wound care orders . He had huge amputation wounds that were supposed to be changed three times a day; they barely changed them once daily, sometimes twice, and sometimes not at all the entire day. He developed pressure sores that they didn't help with; I had to change his bandages to help them heal properly. They would "forget" to give him his warfarin, which he needs to prevent blood clots. They tried to give him a medication after it was discontinued because he had a severe allergic reaction to it, as no one discontinued the order. Other patients suffered too. They let his roommate sit in dirty diapers for hours . This guy didn't eat on his own, and I never saw anyone feeding him. They let people scream in pain for hours before anyone went to help. They had "favorites" who received better care than others. Staff turnover was egregious; they also seemed to drop everyones hours and made everyone part-time so they wouldn't have to pay benefits. I watched the entire staffing change over the last year and the new owner's cut their wages as well, check They hired people who barely had their licenses and only provided about one or two days of training. Out of 18 Yelp reviews, 13 are 1-star due to the same reasons. The owner is fake AF he responds to yelp reviews saying theyll make changes but does nothing to implement them. The soda machine used to cost $1.75 for drinks and worked properly. They got a new one they fill themselves, and they made the drinks smaller and more expensive at $2.50 for a 16 Oz bottle Vs the old machine at $1.75 for a 20 oz. All the owners care about is profit and driving their luxury cars. The DON is a joke and doesn't even seem like she has ever managed a facility she probably barely got her license and got promoted for doing extra special favors for the owner, She wears heavy make-up and huge fake eyelashes and just sits in her office all day. She will then act annoyed when she has to get up for patients or anything. I watched a few of my spouse's roommates literally starve to death because no one fed them. The only time the facility looks and smells nice is when they are touring investors so the owner can CON more people into investing in his horrible facility. I suggest that if you have a loved one here, check on them as often as possible because the staff are not . If your loved one has trouble eating, please feed them because the staff will not. If they need wound care, check their wounds and take pictures of the progress to see if it's getting better or worse because they will not change the dressing as often as they should. If your loved one developed a bedsore wound while under their care, report them to the Ombudsman and California Department of Public Health. I did . The patient to staff ratio is atrocious because they can't keep staff on board and they won't give people full time hours or benefits. read more

Adam responds to bad reviews about this facility, but nothing seems to change. Just had a family member at the facility and the care was not good. Medication errors, unresponsive to call lights, routine care not provided. Same issues as has been reported numerous times. The patient rooms are not air conditioned and during this hot spell, the room was in the 80's. So took our family member to the hallways where it was cooler. The 1star is for the for cleanliness of the facility. Would never allow a family member to enter this place again. read more

Stopped by to tour the facility. They have valet on site which made it very convenient. Destiny at the front receptionist desk was very nice and made me feel welcomed. I've been to a few other facilities and Los Altos Post Acute felt inviting. I was able to speak with the administrator, Ryan. He reassured me that we would be in good hands. read more

Do NOT allow your family members to stay here if you really care about them. Edna who is the DON is horrible and I'm not sure why she came in this line of care when she allows her personal feelings to affect THE CARE of PATIENTS. Dealt with them for a month now and absolutely nothing has changed but gotten worse. CNA's do not do their jobs properly. Head nurses/nurses are absolutely horrible I don't know why they took an oath. If your family is bed bound do NOT expect them to get them out, clean them properly or regularly, even get FED. All of these BAD reviews are for a REASON. Do NOT leave your family or patient here. Unless you want them to be not fed, missed medication, WRONG medication, no baths missed. I emailed Adam to which He still has not responded but it's really good at responding here on Yelp to get everybody to think that he is actually doing something. So from management to the director of nursing, I promise you they do not care. They do what every other company does if you're a family member or friend or whatever and you show up they will act like everything is OK and all good. But in reality, all they do is hide from the truth fake comment on all their reviews. Send other people out to do their dirty work when you come, but will never address the issue with you personally, and that goes for the patients there or the family members that come to visit. The people here in management everybody that is up there is absolutely pathetic and I pray that they never have to go through this with their own family members. Came here to visit a relative and the place felt pretty professional overall. Staff were responsive and didn't make us feel rushed when we had questions. Therapy seemed focused on actually helping patients improve, which I appreciated. It's a higher expectation area and it feels like they're trying to meet that. read more

My grandmother received the best care in Los Altos. She was treated with such care and love that she bounced back after her stay. The rehabilitation team got her up and better than she was before she got there. We are pleased to say they treated her like family. read more

I emailed the following to the Administrator of Los Altos Post Acute. I did not receive a response. Mr. Follrath, Today was my husband's final day at Los Altos Post Acute. His stay began on January 9. I am submitting this report now because I was genuinely concerned that raising these issues earlier could have resulted in reprisal while he was still under your care. Our goals for his admission were straightforward and reasonable: consistent, meaningful physical therapy--particularly to address left-side weakness following seizures and prolonged immobility; and basic daily care that ensured comfort, nutrition, hydration, dignity, pain management, and appropriate mobility support. Unfortunately, these goals were not met. I personally observed one of his therapy sessions. He was in the therapy room for approximately 20 minutes--far below what we were led to expect and what is appropriate for a skilled nursing facility. During a review of his care plan, his care manager, Geraldine, read documentation indicating that my husband required assistance with eating. Despite this, I repeatedly found his meals served cold, uncut, and untouched. On January 15, I was present when his dinner was delivered. He was served cold tomato soup, a cold grilled cheese sandwich with hardened cheese and an overly hard crust, and a single cauliflower floret consisting mostly of stem. All food was cold and unappetizing. When I brought the tray to staff and asked whether they themselves would consider this an acceptable meal, I was sent to the kitchen. There, there was no accountability--only an offer to substitute food, without acknowledgment of the underlying problem. On January 16, at approximately 3:00 PM, I discovered that my husband's water jug still contained the same paper and debris I had placed in it the previous day. The water had not been refreshed, indicating he had likely gone the entire day without access to clean drinking water. Attempts to address this with staff were ineffective. Throughout his stay, I encountered dismissive and, at times, scolding behavior from a particular CNA. When I requested assistance for my husband, I was told--sternly and in front of others--that she was busy and that I should "make sure to include all requests at once" because she had other patients. This interaction occurred again in front of Geraldine, his care manager, on my final day at the facility. My husband was also repeatedly left with only a single sheet and was consistently cold. I often found myself responsible for obtaining extra blankets, asking staff to reposition him, straighten his bedding, and help move him up in bed to relieve discomfort. He was experiencing pain in an area of concern for a potential bedsore--an issue that should have been proactively addressed. This experience has been deeply upsetting. What we are already going through as a family is incredibly difficult. To be met with carelessness, indifference, and at times outright rudeness only compounded that distress. I understand that this is demanding work, and I see how hard the job can be--but that does not excuse the lack of basic care and compassion I witnessed. My heart went out to the patients who are easiest to ignore simply because they cannot advocate for themselves. The one consistent exception was Geraldine, his care manager, who was attentive, responsive, and professional throughout. She stood out precisely because the standard of care elsewhere was so lacking. I regret bringing my husband to this facility. my mom has been in this facility for over a year now and all i have to say is i wouldn't want her anywhere else. the bonds we've created with the staff lead to a chemistry formed, helping us have an ease of mind knowing my mom is in good hands. The Dr from Kaiser working for my client in this facility is the worse I've ever met! So rude, the tone of her voice, her facial expressions the way she answered questions was harsh and not helpful at all. Some of the nurses are racists they need to improve their attitudes towards the patients and their families. Most CNAs aren't doing their job to the best of their abilities, for example changing diapers not cleaned, rough and leaving fleeced and soiled on bed till we (family) comes in to visit and change it. If we don't remind them the shower they won't shower the patient. Foods is nice, PT is funny if the family member is there they will do their best if no family member presents they just come in and do a 15mins exercise in the room then leave, and then they write on the report the patient can't reach her goals. Ridiculous! Overall, Worse care, worse staff and the management don't come and talk to the patients and check on them like the long paragraphs they describes each patient on their reports. I will never allow any of my family members to go back to that facility ever!!! read more

I have been very reticent about allowing any family member to receive care at a SNF. However, our experience at LAPA was not what I expected. Adam Follrath, the administrator at LAPA went to extraordinary lengths to accommodate and advocate for my brother while he was at the facility for his 7 week stay. My brother was released from the hospital and was placed at Los Altos Post acute. He enjoyed the many activities provided for the patients, and the staff made an extra effort on his behalf. The staff is very friendly and helpful. The facility is a clean and happy environment, which I wasn't expecting. Having been there during the holidays, the staff made it a festive time for the patients and tried to include everyone. The administrator was very responsive and in a timely manner addressed any concerns or issues, coordinating his care team to assist. The free valet service is also very helpful. read more

My mom fractured her hip joint and went through a total replacement surgery. After being discharged from the hospital, she was transferred to Los Altos Post-Acute. Honestly, we had no idea what to expect. Trusting others to care for your loved one is never easy, but walking into this facility changed everything. My mom's 6-week stay exceeded all our expectations, and her recovery was remarkable thanks to the many people here who supported her. The facility is clean and well maintained. The care staff--including CNAs, nurses, and physical therapists--are all kind and responsive. The skilled nursing and physical therapy teams played a major role in helping my mom regain strength and walking ability quickly after her hospital stay; The social services team is one of the best we've ever encountered, and we are especially grateful to Mills, who was always available to help. She was patient, attentive, and supported us with everything we needed throughout my mom's stay. My mom's experience here truly put us at ease and relieved so much of our anxiety. We're grateful to so many people we didn't get the chance to thank in person. We would absolutely choose this facility again and highly recommend it. read more

My father was returning home after a stroke, and I was overwhelmed managing all the follow-up appointments and home setup. Karina coordinated with physical therapy, made sure that social services contacted a home health agency, arranged for needed medical equipment to be delivered . Karina made it possible along with the care team create a safe home environment before discharge. Once my father was home ,I was able to breathe with relief. After fracturing my shoulder I spent 9 days in this facility. The experience was very negative. The only good thing I can mention is PT and OT team. Friendly, dedicated, thorough. Everything else was bad. CNAs were inexperienced and unhelpful (I believe in many cases they were sent from agencies and had no specialized knowledge). Calls for help were sometimes not answered for hours. One night at midnight I asked for pain medication, and it didn't get delivered until 9am (!!!) only after I called the front desk in desperation and filed a formal complaint. Of course I've met good and dedicated CNAs and nurses as well, but overall I absolutely DON'T recommend this facility to your loved ones. read more
