With the help of Johann from Home Health, my mother was placed at Medical Center Convalescent Hospital. Before her admission, I read several Yelp reviews and honestly, many of them were not very encouraging. However, this facility was close to both my home and workplace, which allowed me to visit my mom every day, so we decided to give it a chance. We arrived at the facility around 4 PM on a Saturday. To my surprise, Ana, the Director of Nursing, was there waiting for us. What immediately impressed me was that it was the weekend, yet she greeted us with a warm smile and welcomed us like family. She personally showed us to my mom's room. Although the building itself is older, it was very clean, there was no urine smell in the hallways, which was a huge relief. My mom's room was spotless, with freshly changed lines and the housekeeper was even hanging newly washed curtains. Anna, then introduced us to my mom's CNA, nurse, and the entire admitting team, including PT,OT, dietitian and admitting staff etc. Everyone was friendly, smiling, and genuinely welcoming. At that moment, I felt reassured and at peace knowing I was leaving my mother in such caring hands. Around 5PM, my mom was already served dinner with the correct diet. I was truly impressed by how efficient and organized the staff, completely the opposite of what I had read in some reviews. The very next morning, I arrived around 8AM and found Glenda, my mom's CNA, already giving her a bed bath. Shortly after, Christy from PT came in to assess my mom. Again, I was amazed, this was Sunday, and services had already begun. The PT team was extremely patient with my mom, who was initially very scared to walk. She tended to tiptoe instead of placing her feet flat on the ground, and her gait was unsteady. With time, encouragement, and consistency, she improved significantly.Throughout her stay, all the nurses and CNAs were patient, loving, and kind. Every time I visited, my mom had nothing but good things to say. She remembered everyone by name, and even the housekeeping staff became her friends. My mom preferred staying in her room, so the staff would come in, sit with her, chat, and keep her company. That meant so much to us. One thing that truly stood out to me was Ana's leadership. When the facility was short-staffed, Ana didn't just supervise, she worked alongside her staff, even doing skin treatments herself. During the Palisades fire, when patients were transferred to this facility, I personally witnessed how the entire team worked together seamlessly to accommodate everyone. Their teamwork, dedication, and compassion were remarkable. After 11 months, my mom fully recovered and is now able to walk safely with a walker. We decided it was time to bring her home. When the staff found out, they were genuinely sad, the best way. They expressed how much they would miss her. There were so many staff members involved in my mom's care that I can't name everyone, but they truly became her second family. I am honestly teary-eyed while writing this review because I witnessed firsthand how dedicated, hardworking, and compassionate these people are. They loved my mother as if she were their own family. My family is deeply grateful for the care and compassion this nursing home showed my mother. From the very beginning, the staff treated her with dignity, patience, and genuine kindness. They cared for her not just as a patient, but as a person.The nurses, CNAs,housekeepers, and support staff consistently went above and beyond to ensure my mom was comfortable, safe, and well cared for. They listened to her, respected her, and treated her with warmth. Knowing she was in such loving and capable hands gave our family great peace of mind. Kudos to all the CNAs! Joyce became one of my mom's friends-she never missed giving my mom a shower. She moisturized my mom's skin with lotion-I could tell coz she does not have scaly skin! My mother was sent here to strengthen the bacteria balance in her gut. She was forced to wear a diaper and was not permitted to use the toilet, and she is not incontinent. She would tell them I have to go to the bathroom and they'd say well then go in the diaper and I'll come right back and change you, and they wouldn't be back for hours leaving her to sit in her own waste. The food she was served was so bad sometimes she did not know what she was eating, but she had to eat because she was so hungry. I sent her food from Instacart so that she would have some healthy food to eat in her room. They would limit her water so that she would not urinate which resulted in delirium. She called me in the middle night and her mouth was so dry. I could hear her lips sticking together because they were refusing to give her water. They ignore the call lights. They would leave her in her poopy diaper for hours at a time, causing her to get UTIs. The delirium that she acquired at this place from their inadequate care was so bad that my California family thought that she was in her end times and was going to leave her there. When I called the office and kindly asked them to change her diaper for three hours to change one diaper, they hung up on me. I heard the nurses with unkind angry yelling tones yelling at my mother while I was on the phone and they did not know the phone was open. I texted the management of the facility trying to get my mother help and 90% of the time they ignored me. When company is there or they know the phone is open they are not hostile but as soon as there's no one around, they are. I've already called Ombudsman's to check out this facility. It should not exist; It is that bad. I drove 1800 miles to rescue my mom from this facility two weeks ago. The emotional abuse is one of the hardest hurdles to help her over. Do not take any loved ones to this facility. I will be reporting them to anyone who will listen. My son sent this up accurately when he said "that facility is nothing more than a pound for old people."

The only downside I had was being awaken at 4-5a to give me one of my meds that could have been given to me at 6-7a when I got one of my IV bag doses! On a couple of occasions when I visited a family member and a very good friend, I noticed the front office staff are welcoming, the administration seems like they are on top of everything and their level of care, responsiveness, and overall integrity have consistently impressed me. The nursing team is both highly skilled and deeply compassionate. They do more than just administer medications and monitor vitals -- they take time to engage with each resident, understand individual needs, and notice changes. On two occasions I have seen nurses staying extra minutes to comfort residents, proactively address concerns (like pain, hygiene, emotional support), and follow up with families to ensure everyone is informed. Very few facilities care like Rialto Post Acute. The leadership at this facility demonstrates professionalism and responsiveness. When I've raised questions or concerns, the administration responded promptly, openly, and with real care. Policies are transparent; staff seem well‑supported and trained, which reflects in how smoothly operations run and how comfortable residents feel. The facility itself is clean, safe, and well‑maintained. Residents are treated with dignity and respect. Activities for engagement are organized, and the staff's attention to emotional as well as physical well‑being shines through. It's very upsetting to not get through to nursing staff or doctors for updates. My grandma has dementia and we just want to see how she is doing and try to coordinate care/ determine placement based on her individual needs. The bathrooms were gross. Smelled like urine. The facility is not horrible, but it could be cleaner. The care seemed fine, from what I was able to observe while visiting. My grandma ate breakfast, which consisted of eggs, toast, a fruit and oatmeal. It looks like they have scheduled groups/ classes for residents. That is great. The residents were so sweet. My grandma has made some friends, which makes me happy. The facility is not very big. They do have an outdoor area for residents to walk around. That's nice.

I recovered at Redlands Healthcare skilled nursing facility, after an accident that crippled both…read morelegs due two large-bone fractures, needing rods and pins, and an ankle dislocation that needed a plate implant. The other leg, a knee needed reconstruction. I could barely put weight a little on one leg when I got there. The Physical Therapy section of the care was excellent. Even when I could barely move the legs they had me getting my blood circulating with stretch bands in bed and lifts. I felt so inspired to heal. We worked so well together, my team who helped with all kinds of care, from clean ups, bathing, the meal plan, the medications and even some fun activities such as painting, word games, number games and searches. There were special events such as ethnic music and dance, an ice cream social, and birthday celebrations. Staff people, nurses and assistents were cheerful and helpful. Administration: top notch. I considered staying longer, but the care was so efficient at getting me back on my feet again that I got to leave two months early! I even went back to visit some of my friends-- both patients and staff, and felt welcome and almost like home! No wonder this place is always full to capacity with people lined up to get in! Thank you to every person who contributed to such a speedy recovery! They made sure that I got home safely, as well. 10/10 I highly recommend this facility!
