I would never refer anyone to Modern Periodontics at 1409 Kingsley Ave Orange Park, FL. I have also shared my experience with my dentist and strongly recommend they also stop referring patients to this office. My dentist referred me specifically because of ONE tooth. During my consultation, however, the periodontist recommended treatment on 5+ teeth. He specifically told me, "If I'm going to be in there, it wouldn't be worth it to only do one tooth" Since I thought that was extreme - I asked that the five most important teeth be included on my treatment plan so I could determine what my out-of-pocket cost would be after insurance. The resulting estimate was $4,596 before insurance. I contacted the office numerous times by both phone and email in an attempt to obtain a revised treatment plan and estimate. It was extremely difficult to get a response or make any progress. When I eventually requested an estimate for treatment on only two teeth, I contacted my insurance company directly because I wanted to better understand my coverage. During that conversation, my insurance representative told me that the office had actually confirmed my coverage with them MORE THAN A MONTH EARLIER, but I had never received that information from the office. Even more concerning, while comparing the estimates, we discovered that the office had increased the pre-insurance price of several of the services between the estimate they provided me and the treatment plan they submitted to my insurance company, despite the services being the same. You should know, I have still NOT received that final estimate. I also learned during this conversation that the provider I saw at the consultation was OUT-OF-NETWORK and also NOT the provider listed on the referral document send from my dentist to Modern Periodontics office, which was what prompted them to call me and schedule an appointment. I learned that since he is out of network it would cost me $1,000 more solely because he was out of network. This could have been avoided if they booked the appointment with the provider ON THE REFERRAL FORM SENT TO THEM!! Overall, this resulted in a very frustrating and costly experience. I am now out $101 for the consultation and have to start the process over with another periodontist. DO NOT GO HERE! If I could give them a "0" I would. read more

I would not recommend this practice. The front office is dysfunctional and incompetent. I was referred to Modern Periodontal by my primary dentist in Fleming Island for a cleaning and exam. After my initial exam they scheduled me for a cleaning. I was told there were no cleaning available at their Orange Park location and I would need to go their location on Baymeadows location. I agreed to go to the other location and because I was in no rush I said I would wait until an appointment was available at their Orange Park location. Which was scheduled almost two months later. I went to their Orange Park location on my scheduled appointment and was told that I was at the wrong location and I should be at their Baymeadows location. I told the front office they made a mistake and the Baymeadows location was too far from my house. As a show of good will they sent me a bill for $100.00 for a missed appointment! On my last visit there I again tried to get this resolved with the front desk and I was told to sent an email. I am self insured and was in the process of spending over $4,500 for laser surgery and cleanings. Not now. read more

Most of the staff was wonderful- mainly the hygentists. The procedure it's self was very painful and they neglected to give me enough meds while undergoing the surgury. The most frustrating part is their cancellation policy. If you need to rescedule an appointment, make sure you leave yourself over 48 hours- and weekends and holidays do not count. read more

My recent visit to Modern Periodontists went as boring as they usually are. I was seen on time, received a wonderful (yes wonderfu), cleaning by my hygienist (please forgive me for not remembering her name as always), scheduled my next appointment, then left. The staff was great and caring as usual but most importantly, my hygienist has a wonderful personality as shown with every visit. It's nice to NOT have a robot taking care of me for once. I was sick with the flu. After giving the office a courtesy call to cancel my appt they charged me $50 for a missed appt. They said it was inside 48 hrs but could waive the charge if I had a doctor's note. I saw my doctor later & provided them with a note but they now state there is no waiver of charge. I have seen this periodontist for over 2 years, show up 15 minutes early, & pay my bills when work is done. This is not how a faithful customer should be treated. I will no longer see this periodontist & I am not recommending this office. read more

This place definitely deserves to be my first bad review ever. Staff is absolutely HORRIBLE and need way more training! Loretta was the worst and disgusting for yelling at my 4 year old son to get his feet off their big chairs even though he's FOUR and can't even keep his feet down because he's short and their chairs are so big. She could of addressed his feet with me, the PARENT, before instantly snapping at my child. Loretta and Brooke were condescending, rude, and nasty when it came to my children. It was bad enough Brooke didn't even have me on the schedule when I got there (as a new patient) and the hoops they made me jump through due to that. The way Brooke made it seem like my 2 kids were such a problem or inconvenience to be at the appointment with me was gross; make it a policy or just say something from the start. Instead they made me fill out an extremely lengthy amount of paperwork and sit in the waiting room for a while before having Loretta come out to berate my child and say they need to reschedule my appointment. That's when I couldn't get out of there fast enough with my babies. I've gone to dozens of dental appointments that included X-rays with my children and no office has ever had a problem with them. They wanted to find an excuse to reschedule me (at the expense of berating my children) due to Brooke's slip up and being hours behind in the first place. Called their office the next day and spoke to a Taylor to report Loretta/my terrible experience and I was told John (an office manager) would be calling me back but wouldn't you know it, NEVER got a call back. Proves even more of this trash company. Should've looked up the reports they have on the BBB before I even went. Save your time, money, CHILDREN, and go anywhere else!!! read more

Absolutely horrible company. Went in a year ago for a procedure that was preauthorized by the insurance company. A year later get a collection notice without ever receiving a bill from them. Called the office and they claimed the claim was denied by the insurance company. Called the insurance company and the claim was filled out incorrectly by the office. Left multiple messages for the office and the insurance company also called several times with zero response from them. Finally got through and they sent my account to collections even though they said they wouldn't and the insurance company would have paid the claim if it had just been filed correctly in the first year instead of sending it straight to collections without resubmitting the inproperly filled out claim or responding back to me or the insurance company. Called today to pay them so I no longer need to deal with this incompetent company and couldn't due to them not knowing if I should pay them or the collection agency. Do not deal with this company unless you want to deal with incompetence and extremely poor customer service. read more

First visited Dr. Kuznia on referral from my kids orthodontist. She was very nice and her staff is super friendly and engaging. They did a good job and made everyone feel comfortable. It was a good enough experience that I went to see her for myself as well. She is very accessible and makes sure that you feel comfortable. They have two offices one in Orange Park and one off Baymeadows in Jacksonville, so it's easy to get there from any part of town. I hope I never have to come back but if I do I will be coming here. read more

Horrible billing practices !!!This office has the most shady billing practices I've ever seen. I scheduled my procedure , got my quote for my portion , paid it and had the procedure thinking the billing was fine. Several months later got a bill for what the Insurance didn't pay. Spoke to my insurance company who informed me the dental office missed adding info that would have supported the cost as well as incorrectly billed units of service. Did a three way call with my insurance and the office ... the office was rude to me as well as my insurance company. Very unwilling to help. I finally found a lady in the office that said she would take care of it ... and she didn't. Communication regarding the billing is nonexistent and no one in that office is willing to lift a finger to help... if you can even get a person to take your call. read more

I have to write this review because I feel that the office is taking advantage of the customers and their insurance. Dr. kuznia is an excellent and professional dentist, however I feel the staff is doing her wrong. On more than one ocation I was quoted a price with my portion and I always keep paying more. I referred my wife to the clinic and she was quoted about $800 with her portion being about $300. The day of the procedure she paid a little over $400, more than expected. Few weeks later we have another bill for $200. When talking to the billing person she said that she was going to talk to her manager and then call me back, still waiting for an answer and it's been 3 months. Now, I think that whoever is in charge of billing should know by now what the insuance company would charge. This is a mayor problem and it's constantly happening. I paid my bill, but got my dental records to find another provider. Will alert my insurance company of the situation. read more

I was referred to Dr. Kuznia earlier this year about a certain concern with my gums. Dr. Kuznia proceeded to tell me that my teeth were going to fall out by the time I was 40 unless I underwent a gum graft procedure that would NOT be covered by insurance. I'm a healthy, non-smoking 20 year old who has always had a clean bill of health with my dentists. I got second opinions from 2 other very qualified periodontists who told me that no surgery was necessary unless my gums receded much farther- something to just check on in 1-2 years. Dr. Kuznia's language had scared me a bit- she described how my gums could literally "unzip" if they were bumped too hard. I understand that in some cases, such scare tactics are necessary, but after hearing from others, it seems as though I was almost pressured to undergo a very expensive, "aggressively preventive" surgery. I encourage others to get second opinions about surgery and weigh their options that way. read more
