I was sent out pricing details prior to my visits and knew exactly what to expect. From there it all went downhill: 1. I had a crown put on and the permanent fell off less than two days later (which I was told was my fault because of too much saliva) I have had a few crowns put on in my day and never have they fallen out. 2. I paid over $2k out of pocket for the work I had done and was told that my teeth cleaning would now be out of pocket (totally fine, but the teeth cleaning was really mediocre overall and done in two seconds) 3. Received a surprise invoice for $450 dollar 5-months after I had my last crown done (mind you I was already quoted for the work done and paid my share) so this came as a total shocker! When I asked about what had happened the person acted as though this was the first time Wellspring Dental had dealt with an insurance provider before. The explanation for the additional $450 dollars is still a mystery to me. I have paid the invoice and will NEVER use Wellspring Dental again. There are way better options from a dentist perspective here in Harlem. I would truly consider going elsewhere!

His office told me I needed fillings and I had them do the work. A day and a half later the filling fell out and took part of my tooth and left a hole both behind my tooth and a visible hole in the side of my tooth. This is one of my front teeth!! I returned to office and was told they would repair and to come back in. At this point, I needed to repair the tooth right away before the now hollow tooth broke off. I went back to the office and they would not repair. When I argued about what they did and how they cannot leave it like this, Dr Benlevi told his receptionist to call the police. He did not want any patients in the office to hear what happened. His office assistants all kept their heads down, looking uncomfortable with his behavior. I am sure they go through this regularly with clients. I immediately reported Dr Benlevi to the NYS Dental Board Office of Professional Discipline. I have reported Dr Benlevi to my insurance and to the Manhattan Regional Office to file a complaint. Just looking at all the complaints against Dr Benlevi and his office .. I cannot believe Dr. Benlevi and his office are still operating and that Dr Benlevi has not lost his license. Something needs to be done!
