Can't say enough great thins about Dr. Taxman! He is the rare kind of doctor who genuinely cares --…read morenot just about your symptoms, but about bringing you into full health. What sets him apart is his ability to balance naturopathic and allopathic approaches in a way that's truly tailored to you as an individual. He doesn't apply a one-size-fits-all protocol -- he listens, assesses, and meets you where you are. Working with him has been a genuinely positive experience, and I would recommend him without hesitation to anyone looking for a thoughtful, dedicated practitioner who takes your whole health seriously.

Edit: I've had to repost this review about 45 times over the last month and half. Taxman will…read more"report" my review and then I go back in and "edit" and the loop repeats. People get away with things because they rely on others being quiet about it and I won't be silenced. Here are some more details I initially left out of the below review because Taxman seemed confused. While I was having a panic attack he held up his wedding ring hand in front of him and waved his hand around showcasing his ring-- I'm not sure why he thought that was appropriate? What does his relationship status have to do with my appointment? Is this standard for all his appointments? He seemed confused, I told him while in the middle of a panic attack I'm not here to date you?? I'll continue to repost this until the end of time, no patient should have to tolerate this and then pay hundreds. My appointment with Dr.Taxman was incredibly disappointing. I had a small panic attack at the beginning of the appointment, when I recovered I looked up and his head was tilted down and eyes looking up glaring at me... he looked angry. We just sat there and stared at eachother for maybe 60 seconds. I wish I could tell you why but it was scary, bizarre, and cruel. I'm the one who broke the stare off and silence... I have year round allergies I asked about and he shrugged and mentioned one drug, that was it. We went over two prescriptions I have, he said wow that's a high dosage when referring to my antidepressant, he did not elaborate. There was a knot on my throat he said he would be willing to see through till the end-- I'm OK, I'll go to another doctor. It's clear he didn't want me in the appointment and he had an attitude the majority of the time. He rolled his eyes twice while I was talking. I read glowing reviews from other patients who he's helped with similar issues before booking. He did help me some and he softened up near the end. There were several other issues but I'll leave it at that, he seemed confused by the purpose of the visit. I overestimated his abilities as a professional and as a doctor.

As I walked in, I noticed how clean and tidy the clinic space is, very earthy, designed to make you feel comfortable. I sat down and filled out my intake form, which is very thorough. Then Sequoia guided me to the treatment room, I was offered a choice of tea and essential oil, I chose eucalyptus! Love eucalyptus, he let me know he's getting a diffuser soon, but for now he incorporated the essential oil into the treatment. After I sat down, he took my pulse and asked a few questions to clarify my responses on the intake form. And inquired any areas of concern I had, I said 'hormonal acne, left side of my neck is tight and right erector muscle' Then he left the room, I undressed to my level of comfort (I'm a massage therapist, so I'm super comfortable in this setting), placed my jewelry in the bowl provided and got on the massage table, under the flat sheet. During the treatment I was gently mesmerized by the music and felt a release in my left levator scapulae ( back neck muscle) as he needled it. He addressed my other concerns too and responded with a thorough answer when I asked what a certain point was for, or when I asked for the music to be a bit quieter he happily obliged. By the end I was drifting in parasympathetic mode aka 'rest and digest'. Very calm and peaceful place for your nervous system to switch into to heal and realign itself. Important, delicious and so necessary! Love when I come away from a treatment feeling this way. I will definitely be going back for regular treatments with Sequoia, he was awesome!

TERRIBLE SERVICE. I was looking for a naturopath for my primary care doctor, and there aren't many…read moreto choose from in Salem. After not seeing a doc for 5 years, I knew I should establish myself somewhere and have an annual exam, PAP and mammogram, but I had also been having frequent headaches which seemed related to my cycle, so I also knew I wanted hormone testing. Well I was never able to address those concerns because we spent my whole visit filling out patient forms, which I actually didn't fully complete until AFTER my visit and turned in when I was walking out the door (??? first red flag). Then they sent me home with a packet full of info about all of their spa services....(2nd red flag). I swear, that is all this doctor is concerned with - promoting their "SPA" next door. There are posters and popups ALL OVER their office pushing liposuction, vaginal rejuvenation, and some sort of sound wave clitoral therapy (?!?!) right next to the glass cabinet filled with OLD DOLLS (WTF??) It's really creepy. Seriously, see attached pics taken inside the waiting room and patient room. As it turned out, my mammogram showed a suspicious mass, and I had to go back for additional imaging. My multiple mammograms and ultrasounds yielded a BI-RADS 5, giving me a 95% chance that the lump was cancerous, and I would need a biopsy to confirm the diagnosis. Surprisingly, this office received all of this info from the imaging center (SRC - which were AWESOME, BTW), yet they failed to follow up with me. I never received a single phone call from them. I went back in 3 months later for my biopsy (after prompting from SRC), and I was told the results would be in that Friday or Monday. Now I was very anxious to get the results, so I called the office that Friday afternoon and was told the doctor had SEEN the results, but had not yet reviewed them (???). I begged them not to make we wait all weekend for the results, and I was assured a callback that day. Well that never happened. So I called Monday morning, and was then told the results had just come in that morning (???) but again, the doctor had not yet reviewed them. I asked for a callback again, and after waiting another 3 hours, I finally drove to the office and parked myself in the lobby until they handed over the reports, which showed clearly that they had been received on Friday, and yes I have cancer. At this point, I am taking all my reports and looking for a new primary care doctor WHO ACTUALLY CARES. Apparently they only care about you here if your vagina is out of whack. Sorry Salem "Wellness" Clinic, but my vagina and clitoris are just fine. Good riddance!

Sangita is the most apathetic practitioner I've ever met. She isn't listed on any medical websites,…read moreso I can't imagine that she's been a PA for very long. The first time I met her, she told me that I likely didn't have a UTI, I was just dehydrated, because I wasn't drinking enough water. I actually drink a ton of water, but I hadn't that day, because my mom was in the hospital, and I hadn't had time. She very obviously didn't believe me. A couple of days later, I got a message that the culture was positive. Big shocker. In addition to that, there have been a couple of times where my blood results showed something fairly serious, and I got a message that was a few words long, several days after the results showed up on my Quest page. She would have had the results days before telling me about them. She also didn't make any suggestions, or say that I should come in, and when I asked a question in response, she never answered me. I guess that wouldn't be too bad if I actually got a call back when calling about that question. Worst of all, though, was when she ignored a very important prior authorization from my insurance company, even though I did all of the legwork for it. She never even bothered to read the letter I'd written, or review any of my previous bloodwork. I had seen her to discuss that very medication, but I could tell that she had a bias against it. I ended up being denied a very important medication. Oh, but if I wanted to pay $500/month for it at the medi spa for off label use, that would have been fine. So now I have to wait two months to see an actual doctor at a new clinic who will take my high A1C and glucose levels seriously. As for office staff, I have had Target fax them refill requests on numerous occasions, and they claimed that they never received them. I expected it to happen after it had happened multiple times, and I called them a week ahead of time just in case. I left a message and never heard back. A called a few days later, never heard back. After I'd actually been out of the med for a couple of days, I told the receptionist that this was getting very serious. She got snippy with me and said that that's why they request refills be sent a week in advance. I about lost it. I called the following day and told them that I needed an emergency appointment, because I was feeling sick from not being on an important thyroid medication for almost 5 days. They finally got through to Sangita, who made me feel like a drug addict and said that she wouldn't refill it after that time without seeing her. That was fine, but when I did see her, she acted like I was insane and said that there were 5 refills available and didn't know what I was talking about. I will never step foot in that clinic again. There are a couple of absolute gems who work there (their mammogram tech and one of the front office people). However, their PA turnover is very high, and I can see why. Since Trini left, I haven't felt like my health was in good hands.

Really great experience! Highly recommend…read more I've had serious sleep issues for as long as I can remember, and felt as if I had tried everything over the years. A smart and lovely local friend recommended Dr. Samantha and from our very first meeting I knew I was in exactly the right place to begin my next chapter of healing. Not only is Dr. Samantha a terrific person to talk to about anything and everything, (I've had quite a few therapists over the years and I would rate Dr. Samantha as 'outstanding'), she really knows her stuff when it comes to naturopathic medicine. I am receiving exactly the help I need, and my sleep, along with many other aspects of my life, is steadily improving :) Everyone who works at this friendly, cozy office is kind and helpful. Angela always greets me with a smile and does a great job coordinating appointments. Kati gives a really wonderful Healing Massage that I would recommend to anyone. Bevin works with LENS therapy (Low Energy Neurofeedback System therapy helps restore brain wave balance helping to alleviate many post-trauma symptoms and is approved by the US Food & Drug Administration (FDA).) I have had 3 sessions so far and there's no question that the different methods of care I am receiving at Deep Roots are contributing to my improved sense of overall happiness, as well as the quality and quantity of my sleep. Last but certainly not least, I have had 2 sessions with Dr. Luke, who comes so highly recommended for helping unlock emotional trauma we have all hidden away for a lifetime (but takes its toll when allowed to linger within.). So far I'm very impressed and look forward to my next appointment with him. I really can't say enough positive things about this lovely team of people who work so well together for the purpose of healing others. In my opinion, every child or adult who walks in the doors of Deep Roots Family Medicine has walked into a safe and caring environment and will only be better off for the experience. Whether you choose one therapist or several you will enjoy your journey :)

I was referred to this office by a current patient of theirs. We work at the same place so I know they take my insurance. They directed me to their website to fill in "new patient" forms. Which I did and over two weeks later they call to say they've "declined" me as a new patient. No explanation as to why when I asked. I called a few times checking up on this and they kept saying to give it another day or so and I was completely understanding of this. But to finally call me back after more than two weeks with "I don't know why the doctor has decided this" as an explanation is infuriating. I was really interested in this office because they didn't seem like the status quo. The idea of having a mental health department was absolutely a plus. I've been trying to find a therapist I really felt comfortable with for a few years. This would have been ideal so the care providers could bounce off each other as to how I should proceed in regards to meds. So needless to say I'm beyond annoyed and now I have to start from scratch yet again.

My only reason for not giving this place 5 stars is because of their treatment of trans patients. It makes the overall experience, even if it was good in nature, exhausting. Don't claim you're a safe space if you aren't going to put in the effort to BE a safe space.
