I've seen the daily routine of care and you can see the aids are in experienced and seem careless at times leaving her for hours of unattended checks on her for bathroom visits. This leads to her room smelling of foul smells. This is the reason local family members for not wanting to see their grandmother. I'd give five stars to of activities department. Because of the one great RN named Dawn, she should get five stars. So, that's a total of two stars for the rest of the place. For the price we pay, this place should have better aids and provide more attention vice leaving them to exist in their rooms to sit in filth.

If my aging parents lived in this state, there's nothing that would give me greater happiness and peace of mind than seeing them retire to FVD. Imagine a super cool college campus with great people, fun activities and awesome food - but NO job, responsibilities or stress whatsoever. That's what retirement living at FVD is like. You move in young, healthy and spry in your 60s and basically have the time of your life. As you get older, additional care and support is there when you need it. The new expansion they're doing, while obviously causing a bit of construction noise/mess now, is going to be AMAZING once it's complete. The Health Center (nursing home) side is wonderful too, once the time comes for that level of care. The people are great - both residents and staff. I love this place and I'm so glad I get to work with the team and residents as part of my day job!

We needed a spot immediately, and I feel the price reflected that need, it was very expensive, maybe unfairly so. I feel they put her in a price point higher and had assessed more care than what she truly needed, and getting that downgraded was more difficult than it needed to be. I feel part of the reason for this was some the forum was in the transition of new ownership which makes things bumpy. When she first arrived was the forum at Knightsbridge and when she left, it was the estates at Knightsbridge? To be honest, I'm not sure what the name of the place actually evolved into everybody still calls at the forum. I have since moved her out of the facility I can say that when she was in their care, her experience was good. She never was treated roughly or had any incidences that I needed to be aware of. The food seems to have gone downhill and I do not like that the assisted living across the street where she lived is not as nice of the public spaces as it is across the street for the independent care that is my biggest ripe of this place the people who need more care and pay higher cost have a lesser aesthetic in their common space and that did irritate me. I'll be up. It irritated me more than my mom. We moved her to a different company/facility not because of care or quality of care but more due to location. I do like that the forum has memory care and skilled nursing. It was a nice place for my mom, but I would explore pricing of other places when you're looking at this place, this place is good but also is expensive comparatively.

We also switched to a facility in Upper Arlington for a 6 or 7 month period during that time. The people at The Forum cared for my Mom in so many ways. They went from being friends to being caretakers during her transition. I could not recommend The Forum highly enough for how they took care of Mom. We moved Mom to the facility in Upper Arlington after she broke her hip. Great physical therapy team and great facilities. But the one thing they lacked was humanity. They didn't care about Mom, just the checks my sisters and I were writing. Back to The Forum. Not the gold standard for facilities, but certainly good enough and they are upgrading constantly, but more than the Gold Standard for the assistance we received and the help from the entire staff. Mom passed a couple of weeks ago. The nurses and hospice people that took care of Mom were in tears. Genuinely. Love this place and you'll be blessed if a family member has to be there.

Skilled nursing rehab is in a separate building. The expectation was that my mom would be there a few weeks to build her strength so she could use her walker again. My mom was in a room with a roommate who sat at a table the entire time my mom was there. My mom was taking pain meds every 3 hours. She missed her meds and after an hour she was told the order was just placed with the pharmacist. It wasn't placed when she arrived. My mother, father, and brother thought the room was small and not clean. My mom discharged herself within 3 hours. When I mentioned this experience to someone at my book club she said she was there for a short time. She agreed the room was small for two people and said her roommate kept waking up at 4am and walked around the room. She did not say anything negative or positive about the care she received.

I had to tell several people that my dad is not verbal and does not know how to use the call button a few times he wasn't even checked on. There were a few good people there but again they are overworked. The food is horrible, never warm. It's never what they say it's going to be on the calendar. It's very dirty and often smelled of urine. We made a couple attempts to let them know how upset we were but management never followed up. I would LOVE for cooperate to leave their loved ones here and they would see how they like it.

Willow Brook smells warm and inviting. There are sitting areas to visit in so that you aren't stuck crowding into one of the rooms. The staff that I interacted with were kind and helpful, and they all knew my friend by name. I would definitely recommend this place to anyone who is unable to live alone anymore.

My Mother and Father moved to DAL in February 2023. We were very excited and hopeful based on what…read morewe were told about the care my parents would receive, all of the social activities they would be involved with, assistance to meals, assistance with showers 2x/week, laundry 2x/week, bed linen change 2x/week, minimum number of RNs/LPNs and aids per resident levels, etc. Very quickly, however, it became evident that none of this was going to happen as we were told. My parents were stuck in their room for days, meals were brought to their room on trays (not their choice of food items). They had no table in their room so they were trying to figure out where to eat in the room. Physician orders for meds consistently were not retrieved; they went days without critical meds. Showers were nearly non-existent and haphazard. I had to go DAL and hunt down aides (who I usually found sitting on their own, often seemingly hiding) to make sure they provided needed and proper care. I had to put my parents' laundry basket in the hall and inform workers of the need for them to do laundry. Bed was not changed regularly as in care plan. And speaking of the care plan, none of the workers followed it. I had to call daily to ask the facility to take my parents to activities that were pre-selected for them, and to be taken to the dining room for their meals. My Mom fell often due to safety standards not being followed. Aides would pull her walker such that she had to hurry behind it. My Mom would be left in her wheelchair without the brakes on and without her walker within reach, resulting in struggles to move and in many falls. Workers allowed her to fall by not helping her and then would not report the falls. We even have video of a fall and how the aid reacted to it. When we notified this fall to the facility, the aid and the facility denied the fall even happened, and it had not been reported. Sadly, after being at this facility for 15 months, my Mom passed away. My Dad was not checked on for 14 hours nightly from 6pm to 8am; he was not toileted during this time, and there certainly were not "eyes" on him every 2 hours as required. I went to the Nurse Director who stated that she was aware of poor care at night and was going to look into it. Nothing changed. I had my Dad checked for UTI, which was positive. I had PT ordered, and it was PT that found and reported a bed sore to me, not the aides or nurses who should have found the bed sore. Next my Dad's UTI went into pneumonia, sepsis and death Keep your loved ones out of this facility, which sadly seems to be how all of these facilities are operated. They tell you wonderful things that they will do for your family member, but they do not do what they say; rather they say what you want to hear and then totally fail to provide loving, caring, beneficial care. You have to maintain constant oversight and be a constant reminder to the facility and staff by calling, visiting and checking on them daily. Highly recommend you put Ring (or similar) video cameras in your family member's rooms, as it is the only way to help you see what really is happening (and not happening). You have to advocate for them 24/7 or they will be left to manage on their own. This facility does not provide true assisted living or proper human care. We could share a lot more, in detail, of our experiences, but we will leave it to the above for now.

ZERO STARS. DO NOT MOVE YOUR LOVED ONE HERE - ESPECIALLY MEMORY CARE!…read more 1. The repeated lack of integrity and non-existent accountability to execute basic safety requirements, while a family member repeatedly reached out to an entire organization, including upper and executive management with cries for help and documented proof/pictures is appalling. Our mother fell 5 times in 4 days under Senior Star's "care." By the time we saw our mom at the hospital her face, head, and knees were covered in bruises and she had lost her ability to walk. 2. The incompetence of the management team at the Dublin location is staggering. They lack proactiveness, empathy, a desire to provide a positive customer experience, and basic organizational skills. They are so disorganized that they let a stranger walk into our mother's room, dismantle her hospice bed, and drive away with it. For many months in 2022, they completely forgot to give our father (with stroke history) his cholesterol medication. Within 12 months, we reached out to the local ombudsman twice for help. 3. They lean towards medication to solve problems. Staff encouraged more medication for our mother to "improve" behaviors which triggered a downward spiral. After her doctor prescribed medication the third-party company they work with performed an assessment, which recommended MORE medication. Our mother was so overmedicated on all the wrong medications (according to the local geriatric psychiatrist) that she went into psychosis and at 83 years of age had to stay in a psychiatric hospital for two weeks to remove all the medications from her body. 4. They repeatedly told us one thing, in terms of medical care and safety protocols for a high fall-risk resident and did not execute what they said they would do. After a repeated history of this, we decided to install a video camera which showed us a fall mat was not always placed by the bed, no-slip socks were not always worn, the telephone was kept out of reach and our mother crawled to her phone, the bedroom door was kept closed at night, they repeatedly forgot to flatten the hospice bed for proper sleep, kept the TV volume blasting through the night and left the room lights on until a family member would call them at 1 AM asking to turn things off. This went on for months. ADVICE: Do not move your loved one here. Find other places and do your research (especially on the source of those positive Google reviews). Put a video camera in the room and check the credentials of the employees giving medical advice. Periodically scan your loved one's medication list to confirm accurate medication distribution. Just because Senior Star is located in an affluent community, Dublin, Ohio, do not assume they'll offer the best care.
