CUSTOMERS BEWARE: My dental plan is DeltaCare USA and Smile Dental are in my plan. I had a root canal on 10/13/2025 and I was told by Gaby the office manager that my plan don't cover root canal and in fact they do and I was charge $$611.00 and I should have a co-pay of $113.00. Then on 6/1/2026 my front two teeth needs a resin composite and I was chaged $700.00 and she told me that my plan didn't cover it. With my plan I should have pay $25.00 for each tooth. I already submitted a complaint with DeltaCare USA. I was scammed for $1,148.00. Let me give you a piece of advice before you commit to a procedure get a written estimate and check with your provider for coverage. read more

Horrible, horrible people & quite frankly the worst dentist office I've ever been to. I honestly came out worst then when I went in. This place is an unhygienic, money milking, time wasting, unprofessional office that I would not wish upon anyone and I'm astounded the staff simply, just does not care. ---------------------------------------------------------------------- POOR SERVICE / SCAM: * Upon getting a crown, the dentist couldn't find my 'nerve' to administer the numbing solution and proceeded to stab my gum with his needle multiple times which was SO unnecessarily painful. He ended up injecting 4 syringe stabs for one tooth & my mouth was so swollen & numb after. * After installing a crown, within the same week, my crown cracked 3 times in which I had to go back and get it 'fixed' 3 times. 1 yr & 2 months later, my crown cracks again and this time the office said I had to pay full price because although typical dental crown warranties range from 5-10 years, this office's warranty is only for 1 year. I asked why my crown cracked so many times, and even before they took a look at my cracked tooth or asked any questions, I saw Gabby (their biller) had written down in their books that it cracked due to "patient negligence" and that the office will not be fixing it. I asked why she wrote that down before even looking at my teeth & she said, "it can't be anything else." * In addition to my crown cracking on the left side of my mouth. My fillings ALSO cracked on the right side of my mouth. In my entire life, I have never had so many dental malfunctions from the same practice and when I asked why, the staff was so rude & unprofessional in immediately blaming the patient without even taking a look or asking questions. This even led to scheduling issues because I'd be scheduled for one service, but upon coming in they'd realize I was due for a different service and the doctor wasn't available so I had to wait over an hour before getting treated. * On my way out of the office after getting no work done, the dentist & biller still had the audacity to try and get me to do an XRay. I said No & that because I just got a new dental plan, I'll be getting it fixed elsewhere. They still insisted that it'd be best practices that I get an XRay with them even though they weren't going to do any work on my teeth moving forward and I could easily get Xrays done with the other dentist. * the only reason why I even got a crown was because they said crowns last longer than fillings and they actively took out a 'gray filling' in order to put on a 'white tooth colored crown.' * The dentist recommended I get 'teeth whitening' for my "discolored teeth." I tentatively said I'd consider it. He brought out "natural teeth samples" of varying shades of white for me to choose from. Upon comparison, all the teeth samples were in fact more yellow than my real teeth in which he sheepishly chuckled and said I also had the option to whiten my teeth to an artificial white in which he then brought out a second booklet of 'white artificial teeth.' I declined as the hygienist spoke over the doctor and mentioned whitening to this degree could unnecessarily ruin my enamel. ---------------------------------------------------------------------- UNHYGENIC: * there were several times when staff members would take dental tools from the cupboard and stick them in my mouth without wiping them down. Today the camera extender fell & I watched her try to stick it back in my mouth without cleaning it. * On a general teeth cleaning visit, the hygienist let water spray all over my face & stained my clothes. * the staff does not practice proper cleanliness as they'll scroll on their phones, touch the overhead lights, counters, patient folders, and then use the same contaminated gloves to touch your mouth. ---------------------------------------------------------------------- RUDE/UNPROFESSIONAL: * the staff here is so unprofessional & rude. They bad mouth their own Doctors and say that 'their doctor's don't know what they're doing' but then they themselves proceed to make scheduling errors, blames patients to CYA for any financial issues, and when the staff does make a clerical or scheduling error they never apologize and instead subject you to a painful roundabout response. * When the office is trying to charge for additional superfluous services & I ask to understand the services I'm receiving, everyone uses vague language and awkwardly plays dumb and tells me to talk with Gabby the biller. Example: I explicitly told them I did NOT want to pay or get an XRay here. In return, the staff insisted they had to take a "picture" for my health. They start putting a lead vest on me and pointing a circular device for a "picture" and I had to profusely ask and eventually physically stop them by saying "is that an XRay, will I get charged?" The hygienist then played dumb and said "I don't know, you'll have to talk to our biller I was told to do this." read more

I've been coming here for years and I really love the new management, they are organized and really help get the treatment I need approved by insurance and if not they help make payment arrangements. So far I've gotten some crowns that feel more comfortable than the original teeth I had before. I've stayed at this office due to convenience but honestly I would not recommend it to anyone. I think they are incredibly money motivated - they tried to charge me $40 during a cleaning for fluoride, they scheduled me for a 7 am appointment and the dentist wasn't even at the office yet and did not show up for 20 minutes. The excuse was that traffic was at a stand still - we all live in LA, there is always traffic, if he knew he had an early appointment he should have been there on time. I got a cavity done here and i had to go back after because they couldn't get the bite right. And to top it off, they charge $150 if you miss an appointment yet the dentist was 20 minutes late for our appointment! They also send you a reminder of the appointment literally the afternoon of the day before. Overall just really unprofessional, disorganized, and unsatisfied. I visited this practice last month for a routine cleaning, and unfortunately, I recommend attending with caution. When I first scheduled my appointment, the receptionist warned that my insurance wouldn't cover a deep cleaning, which struck me as odd since I've never needed one. She insisted they would "decide" once I arrived, which felt suspicious. When I saw the hygienist, he seemed extremely concerned by what he claimed was significant bone loss and asked if I had been in an accident or had arthritis--I'm only 23 and have worked admin for a dental office, so this was a huge red flag considering I've never heard such alarm about my bone structure. I was then led to believe I urgently needed a $200+ deep cleaning due to two 3.5mm pockets and a couple of bleeding spots. However, I've since gotten a second hygienist's opinion and learned that only pockets of 5mm or more would require a deep clean. You also need at least 10 bleeding spots to even be diagnosed with gingivitis. I was ultimately upsold on an $85 oral irrigation treatment instead, which I didn't even need. This felt like a pushy sales tactic for the hygienist to earn more commission, rather than genuine care. I will not be returning to this practice. read more

If I could rate this dentist 0 stars I would. I have never had a worse experience as far as service, attitude, and quality of the work done as I did here. I came to this place because it was where I was suggested to go based on my insurance. When I went in for a cleaning they told me that I needed to put a crown on a tooth that had a previous filling. I was hesitant because my old trusted dentist (no longer supported by my insurance) said that the filling was in good shape a couple months before but decided to trust him and the process began. The rude tech came to get my imprints and left me partially lying down causing the molding to go down my throat where I began choking, gagging, and could not breathe at all. She chose to keep holding it in and ignoring that I was flailing in the chair begging for air and crying. When that was over they drilled my tooth and sent me off with my temporary crown. When I returned for the permanent crown the same tech sat me down and went straight in to rip out the temporary, not being gentle or even telling me what she was doing. While I was already traumatized by that, she then took the permanent one and started to just shove it in the spot and cause EXCRUCIATING pain. I began crying and begging for the dentist to come help. She proceeded to go and complain to the entire office that I was crying and for him to help. He came in and realized there was a ton of left over cement from the temporary that needed to be drilled off of my tooth that she so carelessly ignored. Following him fixing it and getting the crown in I told him it was extremely high and I couldn't touch my teeth together, he shaved it down a couple times and said to go home and give it a few days to see how it feels. Over the next 4 months I lived in excruciating pain, with no sleep and constant headaches. I had to return to the dentist office over 8 times over that period of time in effort to have this fixed. Once finally fed up I went back to my old dentist and begged them to help only to find out that the dentist who placed my crown didn't even seal it properly, it was sitting significantly too high and because of the amount of irritation from the improper placement I had to have an emergency root canal. I ended up paying over $5000 out of pocket to fix the damage that smile dental caused. The staff could not be more dismissive and disrespectful if they tried and they ignored my calls when I tried reaching out to get my money back for the terrible work they did on me. I beg everyone to think twice before going here. I have spent the last year in insurmountable pain and endless dentist appointments and now have fear around going to the dentist. The staff looks and acts as if they were just released from jail, nothing or anyone looks particularly clean and you second guess placing anything in your mouth for fear of illness. After a followup visit today with Dr. Jacobsen, I regret my unfavorable review. He took his time, explaining to me with X-rays, photos and graphics, showing me that the implants he provided were the most appropriate for me, in all aspects. Had I understood this clearly, earlier.. I would have realized the possible options, vs. the risks they would incur. I found Dr. Jacobsen wonderful when I became his client, and after today am pleased to continue holding him, and his staff in the highest esteem. I really agree with the negative reviews after I encountered the same rude & conniving dental practice. I was told that my teeth were in BAD condition & I needed all these very expensive procedures. My teeth are extremely white & straight, I get compliments all the time. Yet, I was told that I needed "deep cleaning," which wouldn't be covered by my dental insurance because my teeth were in bad shape. I paid $305 for the deep cleaning which I think was wrong. Basically, they don't do basic routine cleaning ever. Then, I was told that I needed 2 fillings that were not covered by insurance. By now, I was feeling alarmed that my teeth were going to fall out. After this bad news, I was told that my bridge needed to be replaced at the cost of $2,550. Also not covered according to these lying con artists. I called my insurance company twice to get clarification. I was told by 3 different agents that the procedure would be covered even if there were no signs of decay or cavities. I asked two different agents to call Smile Dental while I was on the line but the agents said they got a busy signal. butthey refused to answer their calls. ! I then drove to their offices to prove that my insurance would cover the cost. The reception & staff reluctantly called them. I was right to follow my instincts. My bridge was in great condition, no decay, or cavities. I would have wasted thousands of dollars for fake procedure. I'm out $305 for the "deep cleaning," but consider myself lucky to learn about this bad & unethical dental office. read more

I have to tell you that the change over at his office was the best thing that could've happened to this office. We could start with the front office staff oh my God kind smiley, friendly, helpful knowledgeable all three of them with basically the same personality as the other. if one was smiling the other was smiling to. Made you feel important and they know what they're doing. I left there with a piece of mine that my insurance stuff would actually get done and get done correctly. I hate the dentist but I can't wait to go back and see them. as for the dental assistant and Debbie is phenomenal, kind, and compassionate. she has the sweetest spirit I have ever encountered in my life you know if you're in her chair she's going to listen to you and do whatever she hast to do to make sure you are OK. you matter here and that's the way Debbie makes you feel. Now for the new dentist finally finally a dentist who's not trying to bullshit you and upsell you and trying to make you buy him the house on the hill. He actually listens. The other negative reviews are right, this office tried to scam me today, AFTER taking my X rays. They told me I needed a "deep cleaning" and "extra irrigation" for my swollen gums (check other reviews- they said the SAME thing to other patients!) and that I needed to pay a copay of 85 dollars. My entire cleaning and x rays are covered under my insurance (shoutout to cigna) and I was not supposed to pay any co pay for my visit today. For this reason, I requested the staff to proceed with a regular cleaning without any of the extra stuff because thats what was covered. I told this to the dental assistant, who then said she would check with the doctor and see if this was fine. She didn't get back to me, so I needed to find her and ask her what he said. She then said that I needed a new appointment and that since they were backed up, they couldn't complete my service. What?! I had an appointment and they had already did my x rays. Why couldn't they just clean my teeth? And they said it was because they were backed up- however if I had paid the extra money they would've cleaned my teeth for me then and there. The answer is simple- I didn't want to pay the extra money so they wanted me to have a harder time getting my teeth cleaned, and they probably would've tried to bill me for the new appointment too. For reference, dentists before have told me that my gums were swollen, but simply cleaned my teeth without trying to ask me for extra money. Additionally, I asked that my x rays be sent to my email, and the dental assistant at the front told me she would after finishing up her work, but fast forward hours into the day I did not receive anything. I just called to check on that, and I was told I needed to pay 25 dollars and that a form needed to be filled, but they did not tell me that when I was in the office and asked for my records. They also said they would give me a call back to talk about it, but when I said "Ok you can call me back at this number", they had to ask me for the number because they couldn't see it. If they couldn't see the number how would they have called me back? The answer is that they had no intent to do so. When I called, the person who answered was incredibly short with me and did not apologize for the inconvenience at all. I am going to call again and again until I receive my medical records that is my right to have. This office is a scam, the staff is unprofessional, unorganized, and forgetful. The only person who was positive in the office was the kind lady that took my X rays, and I wish I had her name. She did her best to explain everything to me and my mom. I have filed a complaint claim about this with my insurance, and judging by the other reviews on here that mention the same type of scam techniques, hopefully it ends well. This dentist billed my insurance for $1,500 of work that they never actually did on me. When I say this place is the biggest scam it's an understatement. I promise if you visit here you will leave worse off than when you came in. Psychological trauma, among other things. The plus was we got a same day appointment for 4 people & the office staff was nice. That should have been a red flag When almost every other dentist are booked a month out. Dr. Rad was rude, wanted to refill all my fillings which I didn't want but he had his front desk check pricing and everything he recommended and it came out to $1,000 out of pocket. He also, wanted me to have a deep cleaning. I was like what no thank you just a general cleaning for me and my family and that's it. First off I made an apt with Dr. Forai because I hadn't heard good things about Dr. Rad and when I got there only Dr. Rad was available. I was having gum issues and told him many times I'm going to Mexico in 2 days with my family what's the recovery process. Dr. Rad said it was a simple procedure I would be fine in a day. WRONG. My entire 7 day trip I was in so much pain I didn't eat. Upon returning home I went to a different dentist who asked who did my procedure because it was done completely wrong and the pain was because it wasn't sealed correctly and I would need another procedure. So thank you for the pain for 2 weeks, ruining my vacation, and doing the procedure wrong which I'll now have to pay again to have it re-done.
