After a hip replacement, Aspire Home health sent Lauren P.T. to help me with in home physical therapy and get me to be able to attend physical therapy as an outpatient at a facility, a couple of times a week. There are not enough words in the dictionary to help me express my lifetime gratitude to this wonderful lady. Lauren is the most patient, positive, and kind person there could ever be. Calin, the owner, is hands on and very dedicated to the care of all the residents. All the caregivers are warm, caring and very detail oriented which matters in this situation. The facility is a remodeled house so it bypasses the hospital, or quasi-condo feeling. Outlook feels like a home. There is a communal room with a TV, a very nice dining and living room and best of all, a fantastic backyard with benches, fruit trees and plants. Mom's room is extremely comfortable with her own bathroom and TV and this was the room her late husband stayed in so it was familiar to her. The room is fully prepped for handicap movement, bathing and toilet. Not every room has these amenities but it was important to me that Mom had them. The meals are all hand prepared and healty. The home is very clean and all the laundry is done by the caregivers. I couldn't be happier with the location Mom is staying at right now.

One star is generous! This is a letter I wrote to the administrators of Pavilion at El Dorado…read moreHills. My father has been a resident at Pavilion at El Dorado Hills since March 2023. As his Power of Attorney and oldest child, I am obligated to make sure that the living conditions for him are top notch and worth the $9280 he currently pays. Although I appreciate the efforts the nurse, med techs, and many of the caregivers have made to make my dad comfortable at Pavilion, I have come to the conclusion that he is not being served in a manner conducive to the care he deserves and the amount he pays. I have brought the condition of his room, primarily the dirty floor and full wastebaskets, to the attention of multiple people, yet consistent cleaning has not become routine. When a resident is incontinent and their soiled briefs are left in the room, the entire apartment smells foul. The apartments should be cleaned daily, and wastebaskets emptied immediately after assisting a resident with toileting. Many items of clothing, sheets, and towels have gone missing or were ruined in the laundering process. I brought this issue up first with the previous director and also with the caregivers. The laundry system is in need of revamping and some training is necessary to improve this situation. The biggest issue we have had is the food provided to the residents. The food is substandard at best and has been served on paper plates with plastic utensils for several weeks now. My husband and I recently dropped in during dinner and were appalled at the meal. My dad's plate contained 4-5 french fries, overcooked pasta that had lost its shape, red sauce, a few pieces of hot dog, and four slices of cucumber. Nutritionally this is a train wreck, and not something I would expect to see in a facility of the status Pavilion seems to portray. Last night, he complained that the food was cold and looked like dog food with french fries. Meals are the one thing he looks forward to and are always disappointing. (Side note: my dad is not a memory care patient, he requires assisted living for mobility and we were promised there would be other residents with similar needs) As you know, the residents are in the final stages of their lives and dining is their one remaining pleasure. They deserve healthy, nutritious meals served on real plates with real utensils. Just because they are Memory Care residents, does not mean they should be fed poorly. I feel sad that many of the residents do not have someone to advocate for them and are being fed atrocious substandard food on disposable plates, yet paying thousands of dollars. Pavilion can and should "Do Better." For the above reasons, I will be moving my dad out of Pavilion by November 30, 2023. Please do not use his departure as a reason to ignore these complaints, but use it as a reminder that your facilities should be serving the residents with the proper care they pay for and deserve.

I will never return to Dr. Nelsen's office. I had been a patient there for about 1 year when I…read morereceived a bill for $103 relating to a routine office visit 8 months earlier. I had made my co-pay at the time of the appointment and did not know what the additional charge was for but assumed there had been a billing error with my insurance. I immediately called the third party billing company and left a voicemail including all of my personal and insurance information. I also left a message for Dr. Nelsen's office manager and never received a return call from anyone. About 30 days later I received a letter stating that Dr. Nelsen, "...will be unable to provide any further services until payment is received. This includes office visits, phone calls, prescription renewals, etc." I found this note to be outrageous. I had only received 1 bill prior and my messages were not returned. I called and left a message this time stating that I needed to cancel my upcoming well exam because I had received a letter that Dr. Nelsen was no longer willing to provide me medical care. One of the ladies from the front office called me back and was quite hostile and defensive and told me how wrong I was (e.g., I had not left any messages because she said their office manager always returns her phone calls and that Dr. Nelsen does not treat her patients poorly even though I felt this letter was just such an example). She said she would call the third party biller to get more information and before I could say thank you, she slammed the phone down. The third party biller called me a few minutes later and confirmed that my insurance had requested additional information regarding the appointment but no one had followed up. She said they would follow up now and no amounts were owed by me. Duh!! This was just an awful customer service experience. If you are going to suspend someone from getting medical care, you better be sure you have your facts straight. And if a patient calls and is upset about receiving such an upsetting letter, you need to be kind, listen and try to help, not escalate the situation by pointing the finger back at the patient and accuse them of lying.

Friendly staff, you get to know the whole office really and they know who you are when you call. Staff has never been short with me or at all made me feel like I waste their time.
