THIS UPDATES MY PREVIOUS REVIEW:…read more WARNING: This facility is currently being "closely monitored by CMS.gov" due to "a documented pattern of poor-quality care for one month between August 2023 and July 2024." And according to several employees I trust, the Center for Medicare and Medicaid Services was here for the whole second half of 2023, too. Sadly, though, this apparently toothless agency, slated to disappear under Trump and Kash Patel, didn't see fit to cut off Medicare and Medicaid funds, which would have effectively shut this miserable place down. Where do I even start? As I write this I'm in bed with COVID, courtesy of the way they mishandled an outbreak that continues to threaten patients and staff alike. In my case this means no visitors for at least another week. The roar from the antiquated plumbing keeps many of us awake all night, while the equally ancient HVAC keeps the air uncomfortably hot and dry 24/7/365, forcing us to guzzle ice water all day, which we of course pee out all night in bed. The food runs the gamut from merely unappetizing to outright disgusting. Worse, they stuff us with white bread, white potatoes, white rice, Froot Loops, Frosted Flakes, Kool-Aid and Hawaiian Punch, then recommend a laxative every morning. Some of us ask visitors to bring us high-fiber carbs and high-protein shakes whenever they come. I should add that there are many skilled and compassionate staffers here, but the stingy, appallingly greedy owners wont let most of them do their best work. So spare yourself or a loved one months or even years of misery and choose Luther Manor, St. Ann's or another nonprofit instead.

I'm not Catholic, I'm giving you a sociological observation, the staff and administrators here exude sincere compassion driven by their faith. I do volunteer entertainment work so any assisted living-skilled nursing facility I review is going to be good at scheduling social activity and cultural stimulation for patients and residents, but St. Anne's seems extra special in this department. The outside isn't so attractive but the insides are clean and comfortable, the chapel is lovely and there are many friendly gathering rooms. Family and friends seem quite welcome but the most remarkable thing about this place is that the food is good, though they did foist some decaffeinated coffee on me.

Unfortunately, they have no staff and they have employees that have different credentials doing the work of absent staff. Really not absent but they have no one and refuses to call agency for assistance. The Executive Director is a pretend leader. RUN FOR THE HILLS!!!! They have no CNA's or Nurses. They have a person who pretends to be the DON, and she has a horrible attitude. I feel so sorry for theses residents that has to endure these unacceptable conditions. It's was said that they were using cut up sheets to wash and clean the residents. A DON that has a solid background of being a leader is necessary. Offering the employees that are there an incentive to work overtime would be a great. Most of the staff that are helping have different credentials. That is definitely commendable. The blind is truly leading the blind. I'm so embarrassed to say that I worked there for a month. It started off with a cup of vegetable soup that, with the addition of just the right amount of crumbled up Ritz crackers, warmed my soul to the core. Next came a deliciously prepared grilled baloney sandwich with melted cheese on a toasted brioche bun. This sandwich took me back to my youth, much the way Ego was transported when he ate the ratatouille dish created by Rene in the movie. The masterpiece was made even more amazing by perfectly seasoned, fried shoestring potatoes. The climax to the meal was a masterful rendition of cake and ice cream presented in the form of a heavenly piece of devil's food chocolate cake topped with the creamiest of peanut butter frosting that would make a Reese's peanut butter cup weep. All of this was paired with a scoop of spumoni. An amalgamation of the most delicious chocolate, pistachio and cherry ice cream. The likes of which I have never experienced before. This experience would not have been possible had it not been for the flawless execution of our guide, Sherry, through this culinary excursion

Our family's experience with Brookdale Brookfield Woelfel Road over the course of nearly four…read moremonths (summer-fall 2025) was mixed. While there were some positive aspects, significant concerns about staffing, communication, and quality of care ultimately led us to move our mother-in-law to another facility. On the positive side, the food was very good. Residents were offered choices and could eat in a restaurant-style setting, which she genuinely enjoyed. Additionally, some staff members were exceptionally kind and went above and beyond in their care. However, there were consistent issues that impacted her well-being. Planned activities were frequently canceled due to staffing shortages. Because she is not mobile and requires assistance to attend activities, she often depended on staff to inform and help her participate. Without regular family advocacy, she was not consistently included. About a month into her stay, she was moved from a shared dining table with other residents to sitting alone--this occurred on her birthday. Given that she was adjusting to a major life transition and grieving the recent loss of her husband, this change was especially difficult. During her stay she was not consistently being taken to meals and lost a lot of weight in a short amount of time. We also observed concerns with staff presence and responsiveness. At times, staff appeared disengaged, sitting on phones or eating in common areas, making it unclear whether they were on break. Staffing levels seemed particularly limited during evenings and weekends. More concerning were issues related to personal care. During her stay, she lost 10 pounds and experienced multiple UTIs. There were several occasions when my husband arrived in the morning to find her still in the previous day's clothes, in wet incontinence garments, and with soiled bedding. While staff sometimes stated she had "refused" care, we were not confident that consistent or sufficient efforts were made. The call button system was also problematic. It required residents to hold the button down for an extended period before activation, and response times were often delayed. Some staff members were unfamiliar with how to properly reset the system so my husband had to show them how to use it. Although we requested the simple old style call button, we were told none were available, despite observing newer residents being given the old style buttons. We also encountered issues with missing personal belongings. A gold chain was replaced with a non-gold one, and a new TV remote went missing. Additionally, our mother-in-law reported that a staff member asked her to write them a personal check, which we immediately reported. Another resident nearby also reported missing cash. As her condition declined due to UTIs, she required the use of a Sara Stedy transfer device. We had to purchase this equipment ourselves and train staff on how to use it, as they had not received prior training. Eventually, we were advised to move her to memory care for more support, but instead we chose to relocate her to another facility. Since moving, her condition has improved significantly. She has regained strength, is walking short distances, maintaining her weight, bathing regularly, and is consistently dressed and cared for. While there were some compassionate individuals on staff at Brookdale, our overall experience did not meet expectations for the level of care being provided, especially since we were paying for the highest level of care. In speaking with long-term residents, we also heard concerns that the quality of care has declined over time.
