    I've had experience with at least 4 local care facilities at this point, when I say I am appalled by this place I'm not judging them by impossible standards. The common areas look nice and new, but the patient rooms are disgusting. They pack 3 beds into rooms that are sized for 2 beds. It's loud and chaotic. There appear to be a lot of staff on duty when you go in, but not a single one of them asked me who I was or what I was doing there. My loved one started having anxiety attacks on the 2nd night there. If you have to send a loved one here, be vigilant.

Call button consistently went unanswered for over 25 minutes. Food trays left out of reach and spouse was diagnosed with malnutrition upon discharge. As the direct result of a 10 day stay here, my husband lost every gain that was achieved at Los Robles East Campus Acute Rehab. Note to East Campus Discharge Planner: concentrate less on your ensembles and more on speaking the truth about T.O.P.A. REPORTED VIOLATIONS- 1) "9:30 p.m. when Good Samaritan observed Resident 2 standing in the middle of a four lane road in front of the facility with a purse wrapped around wrist and a walker...you couldn't see the Resident because it was so dark..." 2) "Yesterday patient was found by the church. A lady walked in the door with Resident 1 and stated she saw Resident's name on her wrist band. The lady also stated, Resident was confused and did not know where (Resident) lived." "Resident was observed to have a urinary catheter...the treatment nurse confirmed there was no documentation for monitoring signs and symptoms of infection for 10/21, 10/25, 10/27, 11/7 and 11/9/21 in Resident's record." "Medication Administration Record indicated: Give one (1) tablet by mouth every four (4) hours as needed for severe pain 7-10/10...A further review indicated staff administered the medication to Resident...for a pain level at zero (0)..." "...the facility failed to ensure a medication cart and narcotic storage boxes were locked, secured, and inaccessible to unauthorized staff, residents, and visitors when one of five medication carts was left unlocked and unattended in a resident hallway." "Review of the past three months Resident Council meeting minutes indicated the following issues: call lights not answered timely, staff turned the call lights off without giving the care needed; wandering resident entered multiple rooms uninvited; missing items; food was cold, no flavor, rice and beans were undercooked, open faced sandwich were served closed and meat was tough."

    Most unprofessional, understaffed facility I've experienced. After my knee replacement on Feb. 2024 the hospital transported me there as I live alone. I was not given a ID band until the 3rd day. Shared a room with another person who was hard of hearing and wouldn't wear their hearing aides and had a lot of visitors any time of the day. There was a "thin" curtain between us. I rarely got my pain meds on time and there is no "in house" pharmacy and the weekend Rx came whenever the "person who handles them" came on their shift. I was on drip antibiotics and the tubing filled with blood several times and they would put wrappers on the foot of my bed. Sometimes it would take l/2 hr to 45 min to have them answer the call button. Because I couldn't walk to the bathroom they put a bedside toilet by my bed (without a cover) and I would have to ask them to please empty it when I had a BM. After a week and a half while I was throwing up I asked them to call 911, and the ambulance took me to Los Robles hospital where they literately saved my life. While in the ER my blood pressure raised over 200 due to my pain. When I asked a friend to pick my belongings, they didn't have to sign in and no one even talked to them, They just went to my room and bagged my things up and brought them to me. Through the help of prayer, the ability to be my own advocate I'm grateful I didn't die. SM 85 year old patient

From the moment you drive…read moreup to the facility, walk into the building and go to your assigned room, you can see the building is very well taken care of and nicely decorated. This is important as it shows their attention to detail. In the room, there is a lovely armoire for every patient to use for their belongings along with a nightstand, hospital bed and tray. The armoire is much nicer and larger than the usual small closet you find in most facilities. The true star of the facility are the employees. The CNA's are always quick to respond and provide you with the service you need. The LVN's who dispense the medication are equally quick to help and all of them have kind words along with words of encouragement for each patient. Unfortunately, I have been in numerous facilities and non of them compare. OakView is fine tuned and runs very smoothly. Administrative staff checks on the patients frequently to make sure all needs are met. Treatment plans are prepared by medical staff. And the social worker is very kind and extremely helpful in taking care of the patient while on the premises but also after they leave OakView. She even called me two days after my release from OakView to check on me and to see if there was any additional needs I had. I was extremely impressed by this as I had never had this happen before. Never was there a social worker that did a third of the work this woman did let alone follow through and show she cared every step of the way. OakView feels like a resort with medical assistance, not a skilled nursing facility. The food is excellent and if it's not to your liking, the chef is willing to prepare something that meets your desires. No other facility I know of does that. If and when I need to be in a skilled nursing facility again, I hope I am blessed to be able to find a room at OakView. I highly recommend it. P.S. They also have an assisted living facility.

    Go here ONLY AS A LAST RESORT!…read more My experiences at the the Center For Advanced Wound Healing were horrible. I was charged $5,462.89 for 5 minutes of nurse time, 5 minutes of doctor time and less than $20 of materials. No specialized equipment was involved. I was treated on two separate dates for the same wound. It was a small crack on my big toe surrounded by a callous. The wound was approximately 3/16 inch long, less than 1/8 inch deep and less than 1/16 inch wide. It was just a small crack in my skin, but I was having difficulty getting it to heal. On both visits, the routine was the same. I removed my shoe and sock and sat in an examination chair. The nurse removed the 1-inch band-aid from my toe and applied a common over-the-counter disinfectant. Dr. Bui arrived and examined my toe. He then took a disposable scalpel and spent about one minute trimming my callous. Then he applied a small amount of wound dressing (about 1/4 x 1/8 inch) and covered it with a common 1-inch band-aid. The doctor and the nurse both spent about 5 minutes with me. The wound dressing is about $17 at Amazon for 4.3 square inches - enough to treat my toe more than one hundred times. I have no complaint about Dr. Bui's treatment. It didn't solve the problem, but I don't know what else he could have done. I do have a huge complaint about the cost. I was charged $5,462.89 per visit. A total of $10,925.96 for the 2 visits. They used less than $20 of materials at each visit. 5 minutes of nurse time and 5 minutes of doctor time at each visit. My podiatrist bills $201 for the exact same treatment. I spent many hours on the phone and writing letters trying to get these charges corrected. I was able to get a charge for $1,294 (described as "dressing wound, 19.1 square inches" - I have no idea where that came from) removed. If you call Los Robles to dispute a charge, plan on spending at least an hour on the phone only to get a promise that "someone will take a look at that". DO NOT go to the Center For Advanced Wound Healing unless you are near death and have no alternative.

As a senior placement consultant, first impressions matter deeply, and this one absolutely set the tone. I was warmly greeted at the door by the Administrator, Tammy, and from there, the tour just got better and better. Tammy is a gem--her love and commitment to her residents radiate from everything she does. She treats them like family, often going above and beyond to navigate the complexities of the healthcare system to secure the right durable medical equipment or services residents may need. That kind of support is not something she has to do--it's something she chooses to do. Tammy also has a strong network of providers and often relieves families of the burden of coordinating care, especially during an already overwhelming time. Her knowledge of innovative, often more effective equipment and care solutions is impressive, and she does it all with the residents' best interest at heart--frequently at no benefit to herself and at no or limited cost to them. On one visit, I arrived to find Tammy in the kitchen preparing a full Irish feast for St. Patrick's Day--corned beef and all the trimmings! The home was buzzing with life, with therapists coming in for different residents, and creative artwork by the residents proudly displayed throughout the home. Even though it's a 6-bed facility (which isn't required to post activities), she has a calendar posted--because she understands how important it is to give residents something to look forward to and to keep everyone meaningfully engaged. The home itself is beautifully remodeled, fresh, and impeccably clean. Residents are encouraged to personalize their rooms with their own belongings, creating a true home-like environment--not a facility-type lifestyle. There's warmth, laughter, great food, and a genuine sense of comfort throughout. The backyard is just as thoughtful--a peaceful gathering area perfect for fresh air and connection. Tammy ensured the recent remodel used only top-quality materials, all while staying fully compliant with assisted living standards. Each time I visit, I look forward to seeing the same uplifting energy and warm atmosphere. We toured 4 homes set up for us by Assisted Living Connections (an amazing local company who helps to place loved ones in long term care). We knew immediately that this was the right place for our mom, but did our due diligence to tour the remaining homes on our list. Even though Cinnamon Gardens home is calm and beautiful, the main reason for choosing this place was the administrator Tammy. Tammy is so kind and put us at such ease, we knew my mom would be well cared for. One year later this is 100% the case. Tammy is organized and keeps me well informed. She is always looking for ways to engage the residents and to do something special for them. Family members are made to feel so welcome when visiting. The place is spotless. There is a nice shaded patio outside to sit and visit. The inside is light and bright with comfortable recliners for the residents. The staff are super kind and attentive and so respectful to the residents. They make families feel at home. I worked with Jan, who covers the Ventura area, and she was an absolute pleasure to work for. She was very friendly and personable, asked me questions about my father's needs, budget, and area of interest. She then prepared a list of 5 different facilities, set up tours, and toured the with me to ensure all of my questions were answered. She also had a wealth of knowledge about each facility, and helped me decide the best fit for my father. After moving him in, she also checked in regularly to see how the move was going, which was a very nice personal touch. I would highly recommend this service for anybody who needs to help a family member move to assisted living. It made the whole process easier and more manageable.

Your daily actions, such as always entering my home with a sweet greeting and a smile. No matter what may have occurred on your drive to work you never let it change your kindness and compassion towards me. Your daily respect for my autonomy and personal choices have been a great relief and blessing to me. Last year, 2025, was enormously difficult for me physically, emotionally and mentally. I was badly injured by a caregiver who really didn't want to be with me. She wanted to run my life instead of letting me make my own decisions. Everything was a struggle. I am still dealing with the pain and suffering from her carelessness almost a year later. You, however, are the complete opposite of that caregiver. You show genuine care, compassion and empathy to me. At the same time you keep a close watch on me for my episodes, for any unusual reactions, for changes in my body or mind that may indicate something physically is about to happen or something that is happening without my knowledge. Your daily care of me is exceptional. Everything from reading my body signs or noticing that I am in the middle of a call, task or just not ready. Yet, you come back or tell me it's getting late for something we have scheduled. This may be as simple as my breakfast or as important as helping me to get ready for my doctor appointments, my bloodwork or therapy. You encourage me to try many things that can help my health, mood or my outlook for the day. Such as, helping me relearn my Spanish while I help you with English, or walking around the house with my gait belt and walker. You keep me on top of my goals. These can be as basic as making my bed every time I get up (in the morning and after my nap). In fact, this helped me create and continue this habit. Another goal of mine is building my strength and stamina enough to walk around outside my home. This has not happened yet, as I have been primarily bed bound since February 19, 2025. You encourage my stretching in bed to keep my body as limber as possible. This helps reduce injury in case of a fall. My personal Internist, Dr. Fondren of Newport Beach certainly appreciates your encouragement and reminders to help me to regain the strength I have lost since February 2025. (when a different caregiver injured me). Once I have regained a little more endurance, then I can start physical therapy. Finally, my family has been at peace with you being my main caregiver and I love how well you and Gabbi work together caring for me. Just knowing how dependable you both are, how you communicate with each other changes, concerns and even my meal planning and cooking. My sister and my son (they both call me almost every day) are amazed that I finally have a team like you and Gabbi. From my first phone call to Bright Star to the aide they provided, I am 100% satisfied. I spoke with Bonnie when I first called and just like the aide provided, they both have a heart for individuals with significant special needs and for the parents of these individuals. When I first called, I felt the love. I was heard, understood, and matched with the perfect aide for my daughter. I was exhausted from dealing with the school district during IEP meetings, speech therapist who didn't care and the list goes on. It was actually healing to speak with Bonnie during my first call to Bright Star. That's only the beginning. They sent me a wonderful aide to work with my severely delayed nonverbal adult daughter who was lashing out and hitting and scratching the other one-to-one at the adult program. She was miserable there and nonverbal cognitively three and had no way to tell me, but I figured everything out from other parents who experiencing issues with their nonverbal adult children at Momentum. Our one to one has a heart for working with my daughter. My daughter was refusing to enter the front door at Momentum and that all changed. When I drop off my daughter, she runs to see her aide and they mutually love each other other. My daughter is no longer aggressive or in tears. The truth is, if I lose this aide for whatever reason, my daughter would just be home on her iPad all day in her room. Because this aide is basically running a mini program inside the building at Momentum is the only reason she still goes there. Unfortunately the staff at Momentum is not trained or well paid and the turnover rate is high, so my daughter has a problem with changing faces and transitions. With Bright Star my daughter now has a one-to-one aide and it's the same face every day and this face loves her and we love our one-to-one. Bright Star provided a one-to-one who is a blessing to our adult daughter and has given us a piece of mind that is God given. Thank you, Bright Star. Thank you to our one-to-one. It gives me hope that there are good people in this world.
