OakView skilled nursing facility is a top-notch location for acute rehab. From the moment you drive up to the facility, walk into the building and go to your assigned room, you can see the building is very well taken care of and nicely decorated. This is important as it shows their attention to detail. In the room, there is a lovely armoire for every patient to use for their belongings along with a nightstand, hospital bed and tray. The armoire is much nicer and larger than the usual small closet you find in most facilities. The true star of the facility are the employees. The CNA's are always quick to respond and provide you with the service you need. The LVN's who dispense the medication are equally quick to help and all of them have kind words along with words of encouragement for each patient. Unfortunately, I have been in numerous facilities and non of them compare. OakView is fine tuned and runs very smoothly. Administrative staff checks on the patients frequently to make sure all needs are met. Treatment plans are prepared by medical staff. And the social worker is very kind and extremely helpful in taking care of the patient while on the premises but also after they leave OakView. She even called me two days after my release from OakView to check on me and to see if there was any additional needs I had. I was extremely impressed by this as I had never had this happen before. Never was there a social worker that did a third of the work this woman did let alone follow through and show she cared every step of the way. OakView feels like a resort with medical assistance, not a skilled nursing facility. The food is excellent and if it's not to your liking, the chef is willing to prepare something that meets your desires. No other facility I know of does that. If and when I need to be in a skilled nursing facility again, I hope I am blessed to be able to find a room at OakView. I highly recommend it. P.S. I recommend OakView whole heartedly. After happily living in University Village for several years, My grandma moved to oak view moving from - and between - Assisted Living, Memory Care, and eventually Skilled Nursing. The staff was is knowledgable and treated her with genuine respect and tenderness, and they were consistently thoughtful with our family as well. The facility was clean, well-kept, and organized, and communication with us was steady and reliable. I've even told my husband that this is exactly the kind of place I'd hope for myself one day. During my visits with my grandma, I saw how happy and well-cared-for the other residents were--the daily meals, activities, and interactions all felt warm, structured, and intentional. My mother moved through Assisted Living to Memory Care to Skilled Nursing and every step of the way she was cared for with great skill and loving personal attention. The care was exceptional and the facility was clean and well decorated. The communication with family was also reliable. She was treated with respect and tenderness and the staff accommodated our visits graciously. This is the absolute worst place to put your loved one. I went there to visit a friend who was having such a hard time and she complained about all the problems that were hidden and ignored They have a weak management and many problems here. They charge over $10,000 a month just to rent a small room There is never any management on at night and the staff run willy-nilly and do as they please They just take advantage of loved ones By overcharging them for every little thing. Thank you so much for the help, care, and love that you gave to my father. It meant so much to him, and to me. He came to you almost 3 years ago, during some of your, and his most trying times, yet you thrived and made him feel part of the family and cared for. Despite the difficult times, the thought of my father being in the hands of a loving and caring staff brought us comfort and peace. READ THIS. THIS COULD HAPPEN TO YOU. . For an entire month I had no idea where my mom was, if she was ok , if she was happy. Her old phone was disconnected, her cell was canceled. We spoke on the Sunday she was to move ( we spoke nearly every dat) Monday and then it would be one full month before I spoke to her again. I would call Oakview every day to leave a message at the end pleading with them to be sure she actually had a phone when she received my message. Her memory is quite bad so none must be sure she has the tools to return the call. I received no call from her. I received the number to the director and left 2 messages with her in detail.and quite desperate and worried and she didn't even have the decent t to return my call . I never received any call back. An entire month went by but my mom finally called. That place let me worry to death and could have returned a call. They were cold and callous completely separated from any emotion and lacked empathy. I will not forgive that month of worry. If I had my way she would have never moved into your "Lux Living Facility" I am sure you will find everyone very warm and caring before you sign the rest of your lives in an agreement you will never afford to back out of if you change your mind (if you have fallen for the "University Village" Luxury sceme). They will control your if after you move that much is true. Look up the places legal woes and you will see that much. I see ypu respond to every review here. My dad was ready to be discharged from the hospital but we did not have time to prepare his assisted living apartment. We were told we were lucky to get a spot at Oakview, and after our experience, I understand why. We were only there for a week but I could tell my dad was extremely well taken care of, well groomed and literally catered to. He had physical therapy every day. The team worked hard to ensure he was both physically ready for the next move and they bent over backwards to help us get all the required paperwork and tests done. My mom has been there for about a week. Staff is FRIENDLY, with beautiful accommodations. Contrary to another reviewer, I ate lunch with my mom today and the food was great! The food charge for guests was $7.50 for fish (cod), soup, salad, veggies and apple pie! PT and OT have been fantastic, getting mom ready to return to her own home. We were greeted upon our initial walkthrough (unannounced) and got a tour of the facility without hesitation. Clean was 1st impression, BEAUTIFUL and friendly - 2nd and 3rd... All patients appear happy and well taken care of. I asked a long term patient how she liked it there, and she says she loves it, even after 10 years! read more

I really have to question all these "one star" reviews. I did not see nor experience any of the complaints lodged here in Yelp and I can be very vocal, yet calmly stating my concerns as you'll see in my other reviews. It really makes me think these other reviews are "moles." Oakview is well run by caring and highly trained professional people. Staffers have earpieces on and are in constant communication which each other. The nurses, PT, OT, nutrition and the overall experience is one of the best. I could not find anything to mention and have nothing but compliments to give with gratitude. The food is amazing. My husband and I dined here and noted my mothers meals as being absolutely dialed in towards her diabetic and kidney issues/needs. We had salmon that was off the hook with a beautiful Beurre blanc sauce. No pun intended. It is not noisy at all. There might be more noise in memory care from clients, but that is to be expected as they sadly decline, but truthfully we did not hear anything even from that area. The activities are robust and they bring in events that the patients enjoyed. The activities give semblance and reflect life outside the facility. Music, armchair travels...my mom loved it and she is one to not want to venture out her room. We just had an aftercare meeting and a staff leader from each department showed up and answered our many questions and did not feel rushed. They generously gave us 1.5 hours of their time. When I needed labs for a doctors appt I thought I would have to take my mother out for the morning to a lab, but they offered to do it in house which helped me so much as like many family members who care take are exhausted and need all the help we can get. Trust your loved one here, you will not be disappointed. The experience is so good at skilled nursing/rehab that we would like to get my mother into their assisted living program, but the waiting list is so long (because they are THAT GOOD) that we will most likely look to Sunrise in WLV. read more

I am so sorry that I had to bring my mom here. It is noisier than a circus. People screaming for the police, bingo right outside the room, roommate blasting TV. So disgusting. If you love your parent and they appreciate the peace of silence, reconsider where you take your loved one. I figured after telling staff about the noise, I would come back and visit. They are always short handed on CNA's, so your loved one can wait for a long time to be assisted which can be dangerous and when someone shows up they are in a rush. All they need to do is hire more people. One good thing is that the place is clean Skilled nursing residents needs a lot of help that's why they are there, so hire more help, that's all ! read more

My Mother-in-law spent seven months receiving outstanding skilled nursing care at Oakview. The staff was attentive, caring, and encouraging. The progress she made with the help of the therapists and staff was incredible. They set Mom up for a successful return home. We are so grateful, and Mom is ecstatic to be home. Each day, staff would cheerfully help Mom make multiple FaceTime calls so she could stay in touch with her loved ones during the Covid-19 crisis when we weren't allowed to visit due to state regulations. They even went so far as to frequently polish Mom's nails and style her hair, making her feel beautiful and important. We received regular updates on Mom's progress, and our phone calls were returned promptly when we had questions. We felt supported and informed, and truly felt like the staff at Oakview loved Mom and treated her like family. I have already recommended Oakview to friends who find themselves in the position of needing skilled nursing care, and will continue to recommend them. The team at Oakview is amazing! read more

My father spent a month at Oakview following a difficult surgery. I don't have enough kind words to share about what a wonderful experience it was. The nurses and CNAs were pleasant and helpful. The cafeteria had its work cut out for my picky eater and were always happy to make adjustments for him. The administrative staff was informative and available whenever we had questions. He received PT and OT. They were encouraging and kind as he attempted to regain his strength and return home. I haven't stopped telling my friends about how lucky we were to get him a spot at Oakview. They made a stressful time much easier to get through. read more

My mother has been at Oakview since Fall 2015 when it became apparent that she needed skilled nursing services. For three years now she has been surrounded by angels who entertain her, exercise her, feed her delicious and nutritious food, clothe and bathe her, wash her clothes, provide her needed medication, support her family members, navigate her delusions, calm her confusion, and provide loving and warm reassurances all day (and sometimes all night) long. Given the number of people it takes, it is incomprehensible that even a handful of loving family members could come close to this gentle high-quality support. At her Quarterly Assessment meetings, the department heads sit with eager minds to learn what more they can do to improve on any of the services they provide and to give suggestions on making every day the absolute best for her. I do pop in visits regularly and very often stay for meals. I have never observed anything questionable. The folks who run Oakview are pros. I would suggest that anyone who complains about noisy patients and unflavorful food and other objections that I have read is projecting themselves in the situation. I have seen it in my own family. Try living in the skins of the vulnerable patients. It is easy to look at their plight from healthy vital eyes and find the life-style repugnant -- something to be avoided. Good luck with that! We don't aspire to these years, but it nonetheless happens. Hands down I would want to be in a supervised setting when my time comes. And I hope that angels Oakview will be there for me. read more

During Covid our father who is almost 80 needed to go to a skilled nursing rehab for multiple reasons. This was a very stressful decision and I must say, we hit the jackpot with the staff at Oakview. Maria T. the Director of Nursing and her staff, Manny, JP, Sarah, Michelle, Patty, Deborah and so many others took such wonderful and loving care of our father we highly recommend sending your loved one there. They all treated our dad like he was their own family member and they also made us feel that our family was loved and cared for throughout this traumatic journey with our dad. I always heard and known about the great reputation of Oak view for skilled nursing. Had to put it to test unfortunately due to my moms femur break and needing 8 weeks of skilled nursing. I could not have been more pleased and happy with the excellent care and compassion given to my mom (age 85) ! (and while my dad (96 years old) went everyday too to visit her, he was shown the kindness too).My mom was sad to leave after the great care, and kindness, and professionalism. Their admissions process was great and thorough, and I felt so confident with my mom being there. Nurses and PT were fantastic, admission, social services, activities, hair salon. My mom felt like a superstar and seen and heard. Their food was delicious and terrific too! In choosing the right care you want to feel good and confident in your choice and I could not have been more pleased! Oh and cleaniness of the areas, rooms and faciltiies were spot on, and the staff, nurses, etc. all wear masks so no issues. The communication between nurses, staff, and my family as I needed was good for all the extras I was needing for her care. They took care of various things that helped ease the stress off myself. The outside patios were wonderful too and rooms were good in size so no issues. Bottom line easily a five star rating and I can now affirming attest to the great reputation for great reason. read more

My father spent 3 weeks in the skilled nursing facility at Oakview. The facility is beautiful, clean and efficiently run. The entire team of professionals involved with my fathers care was exceptional. It was a relief to know that he was in such good hands. My father, who can be rather picky, had only good things to say about his care, his therapists and his dining experience. Kelly, the Social Services Director was so genuine and kind. She was extremely helpful in navigating paperwork, doctors and all the business that needed to be addressed. I could not be more grateful. This facility is a gem! read more

I have had dined many times with my mother at the Oakview Assisted Living dining room and finally felt compelled to comment on just how horrible the food is. My last meal there was a ham and cheese panini that was supposed to have a mustard aioli condiment. It was two slices of bread (one of which was the heel of the loaf), two wafer thin slices of ham and processed cheese slices, the kind that come in individually wrapped packets. How is that nutritional? There's so much salt and preservatives in that type of cheese and it really isn't cheese anyway, is it? The aioli was nothing more than yellow mustard slathered on. Just because the people who live there are elderly doesn't mean that they don't enjoy or appreciate good food. For what they are paying per month the quality and choices should be far better than what they are. Most of the residents don't have a lot to look forward to on a daily basis other than mealtime - for some it is the highlight of their day. I will say, however, that the serving staff has been wonderful. They are very kind and extremely patient. It's a shame that our elderly in this country aren't more respected and cared for and take higher priority over the almighty dollar. read more

My mother had been a resident of both the Assisted Living and the Skilled Nursing Facility at Oakview, following 11 wonderful years of independent living at University Village Thousand Oaks. I cannot find adequate words to describe how phenomenal Oakview is in every way. It is a beautiful, homelike atmosphere, and both the leadership and the staff are the most compassionate, loving, talented, and capable group of people I have ever encountered. I would like to name each of the terrific people who have lovingly helped my mom and supported my family, but I would basically have to name every person on the Oakview team. Not only do I fully trust Oakview to give my beloved mom the very best care, but I can't imagine a better place for anyone who needs either an assisted living or nursing home setting. read more

If you want your elderly relative to be in danger, this IS the place! This place is supposed to be a top notch nursing home (SNF). I shudder to think what the others are like? I have heard so many stories of horror...yet these elderly people, nor do their family take the time to contact an ombudsman or state representative, so people know what this place is really like! Many of these elderly people have no ability to make phone calls, or even write! Upon arrival, we gave nurse Amber some meds that we were told would be put into bubble packs. This never happened. The meds (full RX) were lost and supposedly were trashed, as nurses are not supposed to take meds from patients. Their medicine system in the drawer is crazy! I bet mistakes happen all the time. They switched times my mom was supposed to get meds on a few of her meds. Some were AM and were given in the PM and vice versa. We complained DAILY to get her meds refilled. Never happened. We begged for them to order an eye drop she needed, never happened. We aren't supposed to bring in patients meds and let patients give them to themselves....BETTER to be blind and dead, I guess. She went to the eye doc and sure enough her #'s increased, due to them never getting eye drops and us not being informed until several days later. They told US to call the doctor! Screaming patients are handled by closing their door?!?! Nurses were way too slow to answer call buttons. They tell patients to wait...but many just get up to use the restroom, as no one comes or wet the bed waiting. The food is DISGUSTING. Again, isn't this supposed to be a top notch facility? We were not told we could write items in...but then we discovered alternative choices on the back of the menu. There was not much room to write in choices. We did in red, with a yellow highlighter...but they NEVER got things straightened out. What if someone had serious diabetes? What if someone was anaphylactic to an ingredient? They got items wrong constantly, either the wrong item or they just left off items. My mother is "with-it"...many there are not! Talk about a law suit! The Foods they serve have chemicals and are cheap! One day she got 2 trays...one, with nothing she ordered and one with some of the items she ordered. Yellow highlighter ignored AGAIN. Vegetables are severely over cooked, losing nutrients. They were mush...my mother has no eating issues. This brings up another thing. Oak View ordered Speech Therapy...not my mother's physician (doc had no clue it was ordered). We never did find out why. Could it be just a way to bill Medicare? That is my guess! Bathroom trash was rarely emptied. Few nice things. The place did not smell. The rooms are very nice and do NOT look institutional. There were sitting areas in the room for visitors and pets were allowed to visit, very nice! They had some activities...but most of the people attending were not "with it", so my mom rarely went. It would have been nice if she could play games, etc. with people of her mental caliber. Luckily, we visited often. A few employees WERE GREAT: Franklin, Ricardo and Alyssa. Even one of them was shocked at how things were run at Skilled nursing. The guys mentioned above went out of their way to see if my mom needed anything. They made the bed and helped do some straightening up. Very sweet, friendly and attentive. The receptionists were all great! I saw Lisa the most. She was AMAZING, sweet, kind and helpful. LeeAnne, the OT aide was also great. Most therapists, my mom said were very nice and helpful. Oh...another thing. We asked for bed rails, just grip bars for 3 days...FINALLY had to talk to the Asst. head of nursing and she got them installed right away. The final straw.....my mom had a new med prescribed, so we didn't have a prescription. We were promised upon exiting the facility that Amber would order more meds, as mom was down to 2 pills, which are needed twice a day. Amber said she would be sure my mom got the meds. NOPE! Didn't happen. We kept asking for them. I called the nursing station and got no answer. Finally, today, on Christmas I asked for the head of nursing. I called at 11:30am the first time. Talked to Maria at noon. She said it probably couldn't be done and it was not their protocol to order meds and deliver them to be taken at home!??!?! HUH?? Maria said she would make phone calls, but was doubtful the meds could be obtained. I yelled LAW SUIT...several times. No word from Maria. I left Maria a message on her voice mail at 12:45pm. I got the front desk to help. She tried. No luck. I waited, thinking Maria was at lunch. Waited...Waited...Finally I called AGAIN. It was now 2:15pm. I was told Maria had left for the day. EXCUSE ME!????? I called the front desk AGAIN for help. Not taking this new medicine for my mom's new medical condition could cause a stroke or death! I called again. FINALLY Kristin at 2:35pm said the meds had arrived and my mom would get them. No one bothered to even tell me. read more

Yes, it is expensive, but all of these facilities are, thanks to our country's sorry health care system. Bottom line, we couldn't find another place that compared to Oakview. I stayed there twice after knee replacement surgeries. I was there for rehab primarily, MediCare coverage w/secondary Total time in the facility was about 4 weeks. I feel the operation was very clean, food was fine for an institution, good and friendly staff, well run and was most satisfactory except for the lousy TV-system the had the rooms equipped with. Although it is nice to have the assisted living available for my mother. I would like to address some of the things I have witnessed here. I will give some pluses to this facility though; one being the serving staff. I have come many times to visit my mother during breakfast, lunch and dinner. Breakfast in the assisted living is interesting. They have one server serving all the residents which must be overwhelming for them, but service is always great. I would like to point out two of my favorite servers who my mother and I both seem to love; Marcelo and Natalie. I have only seen Marcelo during dinner but if you have the privilege of being served by him you are in good hands, and with Natalie she has given my family exceptional service although I normally do not see her working anymore which is unfortunate. Some of the things I have witnessed here are the treatments of staff. Some of the so-called "supervisors" treat their staff terribly. I have witnessed the servers supervisor belittle and yell at them in front of residents and other co workers, mostly for things that the servers have no control over. That in my eyes is not the right way to talk to people and am afraid this supervisor talks to residents like that when family is not around. Another plus about this facility is some of the caregivers and the one very tall man that delivers medicine to the residents. They have all given exceptional service to my family. Overall, I am giving this facility a 3 stars because most of the staff has great service, except for a few of them, but losing 2 stars because I would love to see management treat their staff better in front of residents and other coworkers. read more
