Second visit earlier this afternoon and I'm happy to say that I'm just as satisfied! Made the appointment last week, which was surprisingly easy even though my schedule is all over the place, but the front desk was willing to stay patient with me and trying to find a time. Came in and I don't think I waited more than 5 minutes to be taken back to my room. Rachel was working again and she helped me sign in and figure out my account so that I could use my points for the best savings. This doctor is SO quick and painless! It really helps that they have another nurse in the room to rub your arm and distract from any discomfort. Again, he still wasn't pushy or made me feel insecure or as if I needed a ton of work done like other doctors I've seen in the past. I think he truly cares to improve, but not over-do it. HE DOES IT AGAIN! Dr. Pilest and his team ARE THE BEST THERE IS. shout out this time to Jessica for being by my superwoman cheerleader and Ana and Sandy for being amazing like always. I'm the BIGGEST baby with needles and I basically pass out every time I get a vaccine or piercing. Dr. Pilest always goes super slow and his nurses do absolutely EVERYTHING to make sure I'm comfortable and ok with whatever is going on. I got lip fillers for the first time yesterday and fainted for the first time during my injections. Jess applied topical numbing cream before Dr. Pilest started, and I could barely feel anything. My favorite doctor in the whole wide world :) This is my second time seeing Dr. Pilest, and again I'm super happy. First of all this guy is a pro. He knows exactly what I'm taking about and what bothers me without me further explaining. He also had to fix the juviderm another doctor did wrong under my eyes, that caused a noticeable bump!!! I'm shipping myself off to Hawaii in October, so I scheduled some much needed facial maintenance - hence I'm back to see Dr. Pilest and Sandy. Today was Juvederm XC, Restylane Lyft, and a wee bit of botox for my marionette lines - the results are immediate, and Dr.Pilest fixed my problem areas in one near pain-free sitting. No bruising, bleeding, or noticeable swelling; my face was left with several miniscule pin pricks where the needle went in, and a rejuvenated younger looking mug. I'll be going back in 3 weeks for maybe a slight touch-up since I had some filler leftover, but we'll play it by ear and see what the he says. His word is the equivalent of the law as far as I'm concerned. We all know how I feel about him, he leaves me in awe. Why am I writing yet another review for TD? I wanted to give a shout-out to my girl crush/nurse extraordinaire, Sandy. Sandy is truly a thoughtful, beautiful individual. I see her 3 to 6 months at a time, but when I there, she puts me at total ease. She's super informative and patient with my questions, generously numbs and ice me, gives me the kind of support I would get from a legit friend I've known for years. She's genuine, a bundle of vivacious vibes, and being around her makes you feel good about yourself. All this, and I didn't even touch on her extensive medical experience. Please clone this woman already. Since I come in regularly, they suggested I sign up for this "Brilliant Distinctions" Program. Apparently I can earn points for my future visits for Juviderm or botox, so naturally I signed up once I got home. Wow - I am so relieved for this experience because I have been terrified of having a needle in my lips again. My 1st & only experience with a needle involving my lips was last Nov, and it was unbelievably painful, and felt like I was trapped in a living nightmare that I couldn't escape from - like something out of a Stephen King novel. Being stabbed over and over again by someone with a needle...and worse was that my bleeding blue bruised lips eventually deflated to look like I had had absolutely no filler put into them at all. What a waste of money and pain that experience was...but I wanted to try one more attempt before throwing in the towel... This time I researched various board certified Derm Dr's in the Orange, CA area, read many yelp comments, watched you tube videos (including Dr Pilests), read realself comments, and eventually felt I would consult with Dr Pilest based on a few reasons: many other patients positive comments online, on his actual years of experience with injectibles, and his standing within his industry. Upon meeting him for a free consult I was unsure about him on a personal level because my Stephen King experience flooded back to me... I have to be frank, I initially thought he seemed a little bit aloof - especially after reading some previous comments from others about his cold/indifferent tone. But I have come to respect him as someone who is very professional, and yet still compassionate. I think his cool tone is misunderstood and he is just a gentle man in his manner. He was very warm and made every effort to maintain my comfort informing me of what he was doing and what to expect. Something I appreciate very much. "Being in the dark is never fun when Stephen King comes to mind..." And his proper training shines through, but medical professional should inform the patient of next steps to put them at ease. So he checked a big box for me with that. Very NB with my dentists too! Dr Pilest and his nurses (esp Maria) also put my nerves at ease, and the pain level was so manageable that I can barely believe it. I cannot say you won't feel a thing because you are still allowing a sharp instrument to penetrate through skin - however this experience was absolutely 80% better than my first attempt at lip enhancement, because Dr Pilest ensures it will be with numbing cream and numbing injections. I barely flinched because of his pre-injection preparation. Hallelulah. Yes, I did pray before the shots as well. Something that was also important to me is I wanted to look like "myself, and not some strange alien from Hollywood. I wanted my lips to look natural, and showed him a photo of my lips from a few years ago when they had more plumpness and fat. Dr Pilest studied the photo before injecting me. I really appreciated that as I have heard so many horror stories of drs. who just apply the same technique cookie cutter application style, without actually listening to the client who is paying for the service, and what SHE wants. Don't these Dr.'s realize you won't be back if they do want they want and you don't like the end result?... All the office staff I encountered were friendly and answered all my questions, and so based on my experience I will be back to get more treatments. It has only been a few hours but I am pleased with my decision to entrust Dr Pilest with my lips, and I am happy with the shape although I'm told swelling will be expected for a few days. I'm pretty shocked at the bad reviews...enough to take time out of my day to sign-in here and leave my own. I can only speak of my experiences with Dr. Pilest years ago. Hopefully his practice has not changed but let me just say this... I was a patient of his, as I said, years ago and had my then-boyfriend seeing him as well (he was also very pleased with him). It is not my normal practice to think of a doctor so fondly (for years), but he is the only one I have thought so highly of. My visits with him were so positive that I've never forgotten him. His 'bedside manner' was exceptional, he always seemed interested in what procedure we were doing and explained it fully and always listened to me. I think my favourite trait of his was his professional skill and the quickness and ease at which he accomplished it (my ex also liked that, too!). My only reason for discontinuing my visits were that I moved over an hour away. Since then, I have had two consultations with local dermatologists. They were incredibly unprofessional, rude, rushed me through my consultation and acted bothered for having a question. Needless to say, I never went back. Now, the hour-plus drive to see Dr. Pilest again seems a small inconvenience for exceptional service and treatment (yes, I plan on re-visiting...I'll leave a review after my visit!). Mind you, I was a patient of his when he was in (I believe) Fountain Valley and barely starting the office in Irvine. I'm wondering if the bad reviews come from having such a larger clientele (poor customer service, scheduling, etc.) ? I know that office staff can impact someone's decision to come back - I hope his staff is trained to be as friendly and exceptional as I remembered in the past...that could really put me off that hour-plus drive...wish me luck! read more

I have been to see Dr. Pilest a handful of times now, I personally think he's great and has definitely helped with I came in for. I disagree with other reviews saying he doesn't listen to you or let you get a word in, I've found him to be a good listener to my questions and concerns and also explains really well. I haven't had him do anything extreme really but he has taken care of some minor things. I first went to him by recommendation of a friend that is an esthetician, I had what looked like a very deep black head on my shoulder and it was making a bump on my skin, it ended up being a cyst that he dug out and removed. He froze off a wart from my finger, which he did say would take a couple treatments but that was fine, it was pretty much gone after 4. He did something to lighten my scar tissue, saying it would take several treatments, which did work after one and it lightened and looked a little smaller but I have not had it done again. I also brought in my husband for his skin and he recommended some products that have been helping him quite a bit. I generally love the staff, really nice girls, very friendly. Lety typically assists Dr. Pilest for my appointments, I think she is really sweet and helpful. The office is very nice, I've never had too long of a wait for my appointment when I arrive on time. read more

I have had severe cystic acne since I was in high school. I have deep disfiguring scars all over my face resulting from the breakouts. I am very self conscious about them. I knew I could never treat them because it's expensive and I'm low income due to going to school and not working much. My grandpa offered to pay to treat them to help me look normal again. I was so grateful and excited to finally get treatment! He wanted me to get the best treatment. I found Total Dermatology and it seemed like they were the real deal. So I went for it. They treated my scars with fraxel laser and since I would breakout sometimes they treated my acne with another laser. Here are my experiences: Since I kept breaking out, Dr. Pilest wanted to put me on spironolactone. Him and the staff told me my body is probably producing too much testosterone which is causing the breakouts. They told me the testosterone would bind to the med and it would get eliminated through the urine. I agreed to it. I took spiro and my face was clear! It was so nice to be acne free for once. But then I noticed some weight gain, but I assumed maybe I was stressed or just ate a bit too much. But then the weight kept on packing on. I thought it was really weird, I never have gained this much weight rapidly before. In a matter of 6-8 months I have gained 40 lbs! I was freaking out. I finally decided to look up info on spiro and I saw all these forums about rapid stubborn wt gain. I immediately stopped taking the med. The wt gain stopped. I told Dr. Pilest and staff what happened. They told me that was weird and that it's not known for it to cause wt gain. They were telling me it's doesn't mess with hormones. But at the beginning, they told me otherwise. Next, they treated my scars with fraxel lasers. That was fine, but I had 6-7 treatments. I had some improvement after several of them but the improvements plateaued. I wish they kept track of when it stopped improving so we could have tried another form of treatment that would treat the scars differently. I did some research and saw that sometimes you need different treatments in order to treat the scars better. So I told them I want to try another treatment. As they looked into my chart they're all like wow thats a lot of fraxel you had there. If that was a lot they should have told me and do a different treatment! Reflecting upon this, it just seems like they wanted my money and didn't care if my scars were improving or not. I finally realized that they're just money hungry and don't care about your results or your situation. They just care about getting paid. I hope this review helps other people that have acne and/or scars decide if they want to come here and risk it. This place is shady. I will never set foot in here again. STAY FAR FAR AWAY!!!! I decided to write this review because of a horrible experience with Dr. Pilest in 2009. He injected one vile of Radiesse fillers under my eyes, which is not FDA approved for under eyes. He left me with severe BLACK raccoon eyes for 3 years and I looked like a drug addict, which I never looked like before he used this horrible filler and charging me $1500 WHAT A RIPOFF!! (Do note; I never asked for that filler only restylane but he said I have a filler that would last a long time and thats what you need!! I stated Im not comfortable with any other fillers but insisted its great for the eyes and before I knew it he was injecting this awful filler product under my eyes)!!! After a couple weeks later, called the office to let him know my eyes are still black and its not from bruising which he totally ignored taking my call. I disputed thru my credit card but he had time to write a lengthy letter to my credit card company stating he had my consent if anything goes wrong he is not responsible, so He wins the dispute case. He is a money hungry incompetent dermatologist only cares about $$$. Why am I writing this review now is because i am still suffering from the fillers left behind .. Radiesse usually last 10-14 yrs under the eyes before the entire filler completely goes away.. NO Doctor in his right mind would have used Radiesse under any patients hallow eyes because of the complications it causes in that thin area... i wish they would pull his license so he would not do this to any one else. I know others that had bad experience with this doctor as well. I am now being treated by an amazing eye surgeon that is correcting his incompetent work after 10 years of spending thousands of my hard earned money to get my eyes fixed. Even the eye surgeon cannot believe he used Radiesse under my eyes which he said has never heard of.... Save your $$$ and go elsewhere ... read more

It's been 3 yrs since I started treatments for my acne. In that time my face has cleared up and we did treatments for the scarring with PRP to also help brighten my skin. I am so very happy with the results and always get compliments on my skin! People always say that they would never know I have bad acne which makes me feel so good because I have struggled with acne since I was 12 years old. And the $100 "deposit" is more like "ransom" I was told on the phone that I could use this "deposit" for "product in the office (I.e. face creams, etc.,) and then when I read the registration form that the office visit was $250, the receptionist said I could apply that $100 towards that days visit. Dr. Pilest and his staff are true professionals in every sense of the word. I have readily communicated my expectations to him and his nurses, all of whom have responded with candor. I have had consultations from other doctors and always have the same experience....they seem to oversell a product or service that does not address my needs or readily resolve the issue as originally communicated. I have been coming to Total Dermatology at least once a year for over 15 years for various skin treatments and in this whole time Dr. Pilest and his staff (love all of them!) have been nothing but kind, respectful and very informative. If there is ever a concern of mine or if I have questions, they will happily and quickly respond. They may be a little more expensive than other places but Dr. Pilest and his staff are professionals and stay up-to-date on all of the latest procedures, always adding new technology and procedures to their portfolio. I've also never been pressured into doing anything I didn't want to do or billed more than what they originally quoted. When he evaluates you and makes recommendations, its just that... a recommendation. You don't have to do it all and have to decide based on what you think will work for you financially and cosmetically. Please do your research before or even google while there. I don't think I could have asked for a better experience.They have a lovely office, warm and competent staff and Dr. Pilest provided as comprehensive a consultation as I've ever had with any doctor. I've been to the office twice and both times it only reinforced that I made the right choice. Following his recommendations for getting my skin in shape with some light laser, a vbeam for redness and products that worked very quickly, I can say my skin has not looked this good in a long long time, and it's looking better every day. The best part is that I feel so much better about how I look and I feel more confident because of that. I could not have asked for a better outcome. Only downside? Don't give them your credit card! Called and spoke to a pushy sales consultant who assisted me with my appointment for botox, eMatrix and possibly a filler. But, I told her I would go over everything with the doctor. I have never done any sort of peel on my face before and also wanted discuss different fillers for lip wrinkles. She mentioned there was one opening the following day. I took it. I told her from the beginning that I can't everything tomorrow. I will for sure get botox though. She said she had to take a $100 deposit to reserve my appt umm.. this is when I should of canceled everything. I've never had any respectable doctor, dermatologist ask me this before. But, I naively told her ok. Well, the next day I called and told her I will not be doing the eMatrix for sure but I may after the New Year. Since my friend who had it done before told me that I would not be able to go out after for at least a few days.(NYE was in one day) I just wanted to give her a "heads up". She sounded really angry on the phone and said that I will lose my $100 deposit and it's non-refundable. But I still wanted to come in for Botox I said. She was then fine with that. So, I arrived at the doctor office, filled out paperwork, and the woman whom I've been speaking with on the phone (I think, Office Manager or Sales Consultant), assisted me into one of the doctors rooms. Her and I discussed eMatrix, Fillers and different lasers of sorts. Side Note: She said she has had 3 eMatrix done last year and had another one recently AND her skin looked awful! ugh! Immediately I thought I defiantly don't want eMatrix if this is what it looks like after 4 eMatrix. Her skin was very rough, uneven, course and blotchy. Not what I was looking to achieve. The next part is what really ticked me off. She had already ran my credit card for $1,600 without my knowledge? WTH? She wanted me to sign it immediately. Even before I met the Doctor or Nurse or No Procedure? I haven't even discussed anything with the doctor yet and I even told her beforehand that I was not going to get eMatrix and I have no idea how she added $750 for Juverderm on the bill? I asked her about fillers for the lips and I said I would like to get filler but I want to go over different fillers with the doctor. We never discussed $750 Juverderm treatment? She just assumed I was getting it and added it freely on my credit card charge. When I started to question why she charged my CC $1600 without my permission and what did the $1600 include. She then got super angry and said "this is not going to work out". She ran out of the room, door was still open and she was starring at me, LOL. She said "she you need to get out of the room!" Jesus!! Beyond RUDE!!! I actually probably would of spent way more than that $1600 but I didn't like how this place operated. I don't know if it was just her or the rules of Dr. Pilest. Since I've never met Dr. Pilest, I would have to assume it's the aggressive angry, sales manager / consultant woman in the office. Long story short.... She refunded me my $1600. I happily left. Called another dermatologist in Newport Beach who happen to have cancelation that day. Yes! I went in, everyone was super nice, no pushy, no RUDE sales people and got my botox and my filler - LOVE! And now I'm looking forward to my next appointment for CO2 laser after the New Year. Yeah! :) read more

I went to Total Dermatology a few years back for a mole check and help with my hormonal acne. After Dr. Pilest offered to have me come in once a month for acne facials and told me my insurance would cover it, he proceeded to bill my insurance for "acne surgery" for exorbitant amounts in which they denied. I was only 20 years old and in college at the time and Total Dermatology sent me to collections!! He also told me that one of my moles came up questionable after scraping it and sending it to the lab and wanted to take more tissue. He left me in a room with his nurse and a scalpel and she went to town on my arm! She removed a 2 1/2" by 1" section of my skin. After several other doctors looked at the massive keloid scar it made on my forearm, they said she cut the tissue at the wrong angle and I now have to deal with people asking me all the time what happened to my arm. They never offered to fix my scar and for years my credit was ruined. I would never go back! read more

I've seen him 3 times, each time he didn't know why I was visiting (with my chart in hand). I was there for a follow up, per his instruction. Finally, I saw him for less than 10 mins total in 3 visits. He didn't ask questions, didn't stick around for MY questions, only shuffled me out to spend more money on products which didn't work. I went here in 2004 with pretty bad acne. I was becoming very desperate and decided to take the next step which was laser treatment. I actually had heard one positive recommendation for the treatment and at that point it was enough for me to try it. Fast forward three horrible treatments later and I have terrible scarring now. I will admit the acne is gone but I think I would have rather waited it out than had a lifetime of scars that I didn't have before. The treatments themselves were brutal. First they beat the shit out of your face with a blackhead remover. I am pretty sure this was the root of the scarring. And then they proceed to do the laser treatment. I admit they told me before hand how intense it was but let me just tell those of you considering this. I was going through college at the time and it nearly cost me a semester. I couldn't go outside for about a week and a half after each treatment. The ensnarement aside it was just physically draining having your face fall off. I am not over exaggerate when I say your face falls off. The treatment I had done was an intense sun burn of sorts that allows all of the exposed skin to peel off. All in all I am writing this to say seriously think about it. It did stop further acne... but I have intensive scarring I did not have before. A trade off I was unaware of and wouldn't have taken had I know. I do NOT recommend this doctor if you are looking to see a dermatologist for medical reasons. He specializes in cosmetic procedures and as far as my experience can attest he is very rusty on the other stuff. The doctor seemed rushed and cold. Her removed two miles and both have scarred horribly. I've had four others removed at various times with very little scarring. He hardly numbed them at all. It was just a horrible experience and he is the only doctor I have ever given one star to on Yelp. read more
